치료적 방법을 연부조직 손상단계에 따라 사용하기 위한 지침에 관한 연구 (Study on Guidelines for Using Therapeutic Modalities According to Injury Phases of Soft Tissue)

  • This study was performed to understanding on guidelines for using therapeutic modalities according to injury phases of soft tissue. Clinical decisions on how and when therapeutic modalities may be used should be based on recognition of signs and symptoms. as well as some awareness of the time frames associated with the various phases of the Healing process. The physical therapist must have a sound understanding of that process in terms of the sequence of the various process of healing stage. The results of this study are as follows: 1. Once an acute injury has occured, the healing process consists of the imflammatory response phase, the fibroblastic-repair phase, and the maturation-remodeling phase and can impede by various pathologic factors. 2. Modality use in the initial acute injury phase and the inflammatory response phase should be directed toward limiting the amount of swelling and reducing pain. 3. Modality use in the Fibroblastic repair phase may be change from cold to heat. The purpose of heat is to increase circulation to the injured area to promote healing. 4. During the Maturation-Remodeling phase, some type of heating modalities, ultrasound, or short wave and microwave diathermy should be used to increase circulation to the deeper tissue. In this phases, physical therapists must control training and conditioning habits to allow the injury to heal sufficiently.

Immunomodulatory properties of medicinal maggots Lucilia sericata in wound healing process

  • The healing properties of medicinal maggots (larval stage of Lucilia sericata) are widely used in the chirurgical debridement of non-healing wounds including diabetic foot ulcers, venous and pressure ulcers, where classical approaches have failed. Several kinds of wounds are prone to complications coming out of a specific wound bed environment. There are multi-resistant bacterial species present, their pathogenic impact is multiplied by their ability to form a biofilm. Moreover, immunological events in chronic wounds differ from those in acute wounds. Non-healing wounds are cycled in the early inflammation phase with increased levels of inflammation attributes like inflammation cytokines and matrix metalloproteinases produced by inflammation phase cells. Application of larval therapy promotes progress in the healing process to the next stages involving tissue granulation and re-epithelisation. Larval debridement is an effective method of cleaning the wound of cell debris, necrotic tissue and bacterial load. This happens in a mechanical and biological manner, but the whole complex mechanism of the maggot healing activity is still not fully elucidated. Centuries of clinical practice brings noticeable proof of the maggots' beneficial effect in wound healing management. This long history led to the investigation of the bioactive components of the larval body and its extracts in vitro. We introduce a review which describes the immunomodulation impact of maggot body components on the cellular and molecular levels of the wound healing process.

Healing abutment case를 이용한 healing abutment의 효과적인 세척에 관한 연구 (A study on the effective cleaning of healing abutment using healing abutment case)

  • 목적: 본 연구는 Healing abutment case (HA case)를 이용한 Healing abutment (HA)의 효과적인 세척에 대하여 알아보고자 구강 미생물을 위상차현미경으로 관찰하였다. 대상 및 방법: 임플란트를 동일 악에 2개 이상 식립 한 32명의 환자, 총 64개의 HA (실험군 32, 대조군 32)를 선정하여 실험군은 알코올 솜으로 세척 후 HA case에 넣어 0.05% chlorhexidine 소독용액을 담은 초음파세척기에서 5분간 세척하였고, 대조군은 알코올 솜으로 세척하였다. 관찰 시기는 1차, 2차 내원 시 각 군을 세척 전 관찰하였고, 실험군은 1차 내원 시 세척 후 추가로 관찰하였다. 이 때 미생물의 양에 대하여 400배의 위상차현미경으로 관찰하였다. 결과: 1차 내원 시 각군의 구강미생물 양을 관찰한 결과 통계적 유의한 차이는 없었으며, 성별, 상·하악, 협·설측에 따른 차이도 없었다. 치은연상, 연하에 따른 구강미생물 양은 통계적으로 유의한 차이를 보였으며(P < .05), 실험군 세척 전·후 비교에서도 유의한 차이를 보였다(P < .05). 2차 내원 시 각 군별 비교에서 구강미생물 양의 유의한 차이를 보였다(P < .05). 결론: Healing abutment case를 이용한 healing abutment의 세척은 단순 알코올 솜 세척에 비해 효과가 있는 것으로 사료된다.

The Wound Healing Potential of Lignosus rhinocerus and Other Ethno-myco Wound Healing Agents

  • Wound care has become increasingly important over the years. Various synthetic products for wound care treatment have been reported to cause toxic side effects and therefore natural products are in significant demand as they have minimal side effects. The presence of bioactive compounds in medicinal mushrooms contributes to various biological activities which assist in the early inflammatory phase, keratinocyte proliferation, and its migration enhancement which are pertinent to wound rehabilitation. Lignosus rhinocerus (tiger milk mushroom) can reduce the inflammation phase in wound healing by fighting off bacterial infection and modulating pro-inflammatory cytokines expression in the early stage to avoid prolonged inflammation and tissue damage. The antibacterial, immunomodulating, and anti-inflammatory activities exhibited by most macrofungi play a key role in enhancing wound healing. Several antibacterial and antifungal compounds sourced from traditional botanicals/-products may prevent further complications and reoccurrence of injury to a wounded site. Scientific studies are actively underway to ascertain the potential use of macrofungi as a wound healing agent.

시멘트 복합체 표면의 자기치유 박테리아 생장 곡선 (Bacteria's Survival Curve on the Surface of Cement Composite)

  • Bacteria used in self-healing concrete, which arrest the crack, helps increasing the durability is well known. However, the survival and activity of the bacteria are precisely unknown. In this research, to know the bacteria's survival curve on the surface of the cement composite, bacteria's survival curve has been measured by CFU at different curing days. The survival curve of 3 days and 7 days curing does not show the significant differences in their survival tendency. However, the slope of death phase of 7 days curing was steeper than the 3 days of curing. This research was focused on the death phase but for further research, set of interval time will be reduced and observe the lag phase and exponential phase.

하이브리드 자기치유 소재를 혼입한 콘크리트의 치유성능 (Healing Performance of Concrete Containing Hybrid Self-healing Materials)

  • 본 연구에서는 유·무기계 자기치유재료를 기반으로 박테리아 펠렛과 고상캡슐을 각각 혼입하여 사용한 하이브리드 자기치유 콘크리트의 치유성능을 조사하였다. 치유성능 평가방법으로 정수위 투수시험을 적용하였으며, 평가지표로 치유율과 등가균열폭에 의해 산출된 치유된 균열폭을 활용하였다. 정수위 투수시험 결과 초기 균열폭이 0.3 mm일 때 MC-BP와 MC-SC의 치유율은 MC와 비교하여 2.1~3.0 %pt 높았으며, 하이브리드 콘크리트의 치유된 균열폭은 0.017~0.018 mm 증가하였다. 결론적으로, 두 가지 치유소재를 함께 사용하더라도 자기치유 성능은 크게 향상되지 않은 것으로 나타났다.

Feasibility Study of Determining the Healing Phase of Achilles Tendon Rupture in Rats Using Optical Coherence Tomography

  • Optical coherence tomography (OCT) is a noninvasive technique for microscopic investigation of tissue. We thought that the OCT method could be a potential tool for monitoring the healing process of a tendon. In this study we used two rat models, denervated and non-denervated groups, to observe a variety of healing phases of Achilles tendon (AT) injury. We made samples of AT injury lesions, to take OCT images and to make histopathological samples of serial sectional tissue. In an OCT image the denervated rat showed no specific finding, but the non-denervated rat showed a large defect lesion that was scaffolding tissue. OCT findings combined with pathologic findings showed advantages in visualization of tendon microstructure over other imaging modalities such as MRI and US, and OCT is beneficial to making a treatment plan, especially the timing and intensity of rehabilitation. Therefore a multimodal platform using OCT for evaluation of tendon injury may be potentially useful for many applications.

수중운동이 아킬레스건 손상 흰쥐의 보행에 미치는 영향 (The Effect of Gait to Apply Aquatic Exercise on Achilles Tendon injured in Rats)

  • 본 실험은 아킬레스건 손상 후 운동의 적용기간이 회복과정에 미치는 영향을 알아보고자, 흰쥐의 우측 아킬레스건을 손상한 후 수중운동을 적용하여 기능회복과정을 관찰하여 다음과 같은 결과를 얻었다. 1. 건 손상 후 회전봉에서의 보행시간이 대조군은 9일이후, 1일군은 10일 이후, 4일군과 7일군은 8일 이후 손상 전 보행시간데 대해 유의한 차이 없이 회복하였다. 2. 4일군은 육안관찰에서 손상부위를 찾기 어려울 정도로 정상 건에 가깝게 치유된 것을 볼 수 있으며, 대조군에 비해 유착의 정도가 아주 양호한 편이었다. 이상의 결과를 종합해보면 흰쥐 아킬레스건 손상에 대해 염증기를 지난 후 적용하는 수중운동가 건치유 과정을 촉진시킬 수 있을 것으로 사료된다.

맥동성 초음파가 흰쥐 창상치유에 미치는 조직학적 변화 (The Histological Observation of the Effects of Pulsed Ultrasound on Wound Healing of Rats)

  • The purpose of this study was to evaluate the effects of pulsed ultrasound on wound healing and observe during the wound healing process the distribution of mast cells according to histopathologic findings. Eighty Sprague Dawley rats which were divided into 4 groups received full thickness skin wounds on the back. Each of the 5 animals was sacrificed immediately and then sacrificed again 1, 3, 6, and 12 days after injury. Specimens from the wounds were removed during healing and routinely processed with a hematoxylin-eosin stain and a toluidine blue stain. The authors then observed the distribution of mast cells under a light microscope. The results of this study were as follows: The rate of wound healing and the length of the wounds of the pulsed ultrasound group II was significantly faster than group I on day 6 and day 12 (p<.001). Group III showed the most significant effect after12 days (p<.001). Group IV also showed a significant effect at 12 days (p<.01). A low-intensity ultrasound .5 $W/cm^2$ resulted in a fast healing rate. During the wound healing process mast cells had a tendency to decrease in the acute inflammatory phase. During the wound healing process mast cells were thought to contribute to the healing of the wound.

  • Cutaneous wound healing is a highly orchestrated process comprising a number of well-defined temporally overlapping phase. These include the migration of inflammatory cells to the wound site, and re-establishment of an epithelial barrier (re-epithelization), and the deposition of matrix by fibroblast [1]. We attempt to find out that active ceramic resources has effects on cutaneous wound healing. And furthermore, we investigate its related mechanism and expression of cytokines of cutaneous wound healing which is accelerated by active ceramic resources. (omitted)
