중사각근 이완자세가 일반성인의 경부 회전에 미치는 영향 (The Effects of Scalenus Medius Muscle Relaxed Posture on Head-Neck Rotation of General Adults)

  • Purpose : This research intended to find out the effects of scalenus medius muscle relaxed posture on the increase of head-neck rotation for general adults. Methods : This research was conducted on 30 subjects whose both sides head neck rotation angles are not the same and that agreed to participate in the experiment. In a posture of sitting erectly on a mat, both sides head neck rotation angles were measured with C-ROM equipment, and while supporting arm in direction opposite the side where head neck rotation limited and leaning over the body at 45 degree and neck at vertical condition against ground, head-neck rotation angles were measured each with C-ROM equipment. Results : The Head-Neck rotation angle on the side of limited head-neck rotation demonstrated more increase in scalenus medius relaxed posture than in erect sitting posture, showing significant difference statistically(p<0.05). The head-neck rotation angle on the side of non-limited Head-Neck rotation demonstrated more decrease in scalenus medius relaxed posture than in erect sitting posture, not showing any significant difference statistically (p>0.05). Conclusions : This posture may be used for preventing limit of head-neck rotation caused of scalenus medius muscle tension and increasing head-neck rotation.

Influence of Head-Neck Rotation on Elbow Flexor and Extensor Muscle Activity and Strength in Normal Adults

  • Purpose: This study examined the effects of the directions of neck rotation position on the muscle activity and strength of the elbow flexor and extensor muscle. Methods: Forty-one healthy adults participated in this study. The subjects were asked to their elbow 90° flexion in three different neck rotations (neutral, ipsilateral, and contralateral) in the sitting position. The muscle activities of the biceps and triceps brachii muscle were measured using surface electromyography. And the muscle strength of the elbow flexor was measured using dynamometer. One way repeated measures ANOVA was used to compare the muscle activity and strength of the elbow flexor and extensor depending on the different neck turning directions. Results: There were significant differences between contralateral neck rotation and ipsilateral neck rotation, contralateral neck rotation and neutral position. But, there was no significant difference in the triceps brachii muscle activity in comparison with the neck rotation. There were significant differences between contralateral neck rotation and ipsilateral neck rotation, contralateral neck rotation and neutral position. Conclusion: To summarize this study, the elbow flexor and extensor muscle activity and strength was highest in the contralateral neck rotation position. In other words, it was possible to confirm the effect of Asymmetrical Tonic Neck Reflex in healthy adults whose primitive reflexes were inhibition, and head and neck positions should be considered during clinical evaluation and treatment.

Effect of Kinesio Taping and Proprioception Training on Pain, Neck Disability, Craniovertebral Angle, and Muscle Activity in Forward Head Posture

  • To examine the changes in pain, the severity of the neck disorder, craniovertebral angle, and muscle activity in young adults with forward head posture. 37 "N" University students in their 20s with forward head posture, including both male and female participants. Measurement of pain, NDI (neck disorder index) craniovertebral angle, and muscle activity were taken before and after the 6 week intervention period. The pain was measured using the visual analog scale. The severity of the neck disorder was measured using the NDI The craniovertebral angle was measured by taking a photo. The muscle activity was measured using surface electromyography. Neck posture correction exercises paired with proprioceptive training is the most effective intervention for reducing pain. Both neck posture correction exercises paired with Kinesio taping or proprioceptive training are effective interventions for addressing neck disability, craniocervical angle, and muscle activity. Neck posture correction exercises paired with Kinesio taping or proprioceptive training are more effective at addressing pain, neck disorder, craniocervical angle, and muscle activity than performing the neck posture correction exercises alone.

악설골근 하부까지 연장된 이악하부 표피양 낭종의 구강내 적출 1례 (Intraoral Removal of Submental Epidermoid Cyst Extended Inferior to Mylohyoid Muscle)

  • Epidermoid cysts located in floor of mouth can be easily removed intraorally. The cysts inferior to mylohyoid muscle have been excised transcervically. However, an intraoral removal of a cyst extended inferior to mylohyoid muscle has not been reported yet. A 20-year-old female visited to the hospital with a cystic mass in submental region. Neck computed tomography revealed a 6.0${\times}$4.3cm sized circumscribed cystic mass in midline of submental area. The cyst lied external to the genioglossal and geniohyoid muscle, extending inferior to mylohyoid muscle. The mass was removed successfully by intraoral approach. It was performed under the exposure by the division of genioglossal and geniohyoid muscle, traction of the cystic wall after aspiration of the cyst, and digital compression externally.

랑거씨 액와 아치 1예 (One Case of Langer's Axillary Arch(Axillopectoral Muscle))

  • Langer's axillary arch(axillopectoral muscle) is a rare disease. It is a variant muscular structure of the axilla, which was described as Langer's muscle by Ramsay and Langer. It could evoke chronic ambiguous neck pain. Therefore, it should be considered, when the patient failed to find the origin of neck pain which do not react any treatment. If it were diagnosed, it can be easily managed. However, it is not well known to otorhinololaryngologitis. Here we reports the 53 years old female who developed pain on supraclavicular area as a result of an Langer's axillary arch.

  • Background: This study was to investigate effects of Correlation Analysis between Cervical-Vertebra Angle and Neck Range of Motion, Muscle Strength, Sternocleidomastoid Thickness of Patients with Forward Head Posture Design: Correlation Analysis. Methods: The subjects of this study were a total of 54 people in the forward head position and their ages were between 30 and 50 years old. The subjects cranio-vertebral angles, neck extension, neck flexion, neck rotation angles, neck flexor strength, neck extensor strength, sternocleidomastoid thickness were evaluated through measuring instruments. The thickness of the sternocleidomastoid muscle was measured using an imaging ultrasound diagnostic device (ultra sound, Versana Premier, GE Medical systems, China). CVA was measured by measuring the side photo of the subject was taken with a camera and evaluated.. neck joint range of motion was measured through digital inclinometer for extension, flexion, and neck rotation. neck muscle strength was measured by measuring the using a digital sthenometer. Data analysis in this study was statistically processed using SPSS version 26.0 (IBM SPSS Inc., USA). Correlation analysis was used and the statistical significance level was set at 0.05. Results: The results neck extension(r= 0.70**), neck flexion(r= 0.67**), neck rotation(r= 0.56**), neck extensor muscle strengt(r= 0.85**), neck flexor muscle strength(r= 0.66**), sternocleidomastoid thicknes(r= -0.81**) It indicates that there is a correlation. Conclusion:These results improve the Cervical-vertebra angle of patients with forward head posture should include a program to improve the thickness of the SCM. In the future, study can be used as an evidentiary material for treatment interventions to improve the Cervical-vertebra angle of patients with forward head posture.

흉쇄유돌근의 회전피판을 이용한 수술 중 발생한 유미루의 치료 1예 (Case Report: Intraoperative Management Using Inferior Based Rotation Flap of Sternocleidomastoid Muscle for Chyle Fistula)

  • Chyle fistula is one of the complications of neck dissections. Although immediate surgical repair seems to be the best choice when chyle leakage is observed during the operation, some operators can be embarrassed when chyle leakage is heavy and not controlled during surgery. In this case, chyle leakage was occurred after extensive resection of lymph nodes in left level IV, and was not controlled in any way. The clavicular head of sternocleidomastoid muscle was dissected and inferior-based muscular flap was rotated to cover the suspected region of fistula orifice. Amount of drainage was checked less than 20 ml per day in the following days, and drain tube was taken out on the 3rd postoperative days. We present the technique using the inferior based sternocleidomastoid muscle flap for intraoperative management of chyle leakage not easily controlled.

Comparison of the muscle activity in the normal and forward head postures based on the pressure level during cranio-cervical flexion exercises

  • Purpose: This paper proposes proper and effective neck exercises by comparing the deep and superficial cervical flexor muscle activities and thickness according to the pressure level during cranio-cervical flexion exercises between a normal posture group and forward head posture group. Methods: A total of 20 subjects (8 males and 12 females) without neck pain and disabilities were selected. The subjects' craniovertebral angles were measured; they were divided into a normal posture and a forward head posture group. During cranio-cervical flexion exercises, the thickness of the deep cervical flexor neck muscle and the activity of the surface neck muscles were measured using ultrasound and EMG. Results: The results showed that the thickening of the deep cervical flexor was increased significantly to 28 and 30 mmHg in the forward head posture group. The sternocleidomastoid muscle activity increased significantly to 24, 26, 28, and 30 mmHg in the forward head posture group. The anterior scalene muscle activity increased significantly to 26, 28, and 30mmHg in the forward head posture group. A significant difference of 26, 28, and 30 mmHg in the sternocleidomastoid and anterior scalene muscles was observed between two groups. Conclusion: To prevent a forward head posture and maintain proper cervical curve alignment, the use of the superficial cervical flexor muscles must be minimized. In addition, to perform a cranio-cervical flexion exercises to effectively activate the deep cervical flexor muscles, 28 and 30 mmHg for normal posture adults and 28 mmHg for adults with forward head postures are recommended.

목 폄 시 탄력성 테이프의 신장력 차이가 전방머리자세 증후군의 근수축 개시시간에 미치는 영향 (The Effect of Different Elastic Tape Expansibilities on the Onset Time of Muscle Contraction during Neck Extension for Forward Head Posture Syndrome)

  • Purpose: Thisstudy aimed to identify the effect of varying the expansibility of elastic tape on the onset time of muscle contraction during neck extension for forward head posture syndrome. Methods: Forty-five young adults with forward head posture syndrome volunteered to participate and were randomly assigned to one of three groups according to the expansibility of the elastic tape (25%, 50%, 75%). The onset time of muscle contraction for the neck extensor during neck extension was measured using an electromyographic system (Free EMG, BTS, Italy). Multivariate analysis of variance was employed to determine the effect of different expansibilities of elastic tape on the onset time of muscle contraction during neck extension for forward head posture syndrome. When there was a statistically significant difference by MANOVA, Scheffe was used as a post-hoc test. The level of significance was set at α=0.05. Results: In the comparison of the onset time of muscle contraction of varying elastic tape expansibilities and measurement times, there was a significant difference between the groups (Lt. UT, Lt. SCM, Rt. SCM) (p < 0.05), but there was no significant difference in the interaction between the measurement time and the group, between the measurement time (p > 0.05). Conclusion: In the neck extension, 75% of the tape extensibility in the Lt. UT and both SCM shortened the muscle contraction onset time.

기관내 삽관으로 발생한 하인두 천공에 기인한 심경부 감염 1례 (A case of deep neck infection resulting from intubation-induced hypopharyngeal injury)

  • Hypopharyngeal perforation secondary to tracheal intubation is rare, but may result in severe airway complications that include retropharyngeal abscess, pneumothorax, pneumonia, mediastinitis and death. The most common site of hypopharyngeal perforation is the pyriform sinus and the region of the cricopharyngeus muscle. We report a 62-year old man with intubation-induced hypopharyngeal injury presenting as deep neck infection. The patient presented with dyspnea and pain on the neck. Neck CT scan identified fluid and air collection on the neck from the hyoid bone to the thoracic inlet level. Despite of delayed diagnosis, we successfully operated him by using strap muscle myofascial transposition flap. The patient was followed up for 3 months without any complications.
