Effect of Deep Neck Flexor Performance on the Stability of the Cervical Spine in Subject With and Without Neck Pain

  • This study compared the stability of the cervical spine according to the presence of neck pain and deep neck flexor performance. Thirty subjects with neck pain, and thirty subjects without neck pain were recruited for this study. The Cranio-cervical flexion (CCF) test was applied using a pressure biofeedback unit to classify the subjects into four subgroups; no cervical pain and good deep neck flexor performance (NG group), no cervical pain and poor deep neck flexor performance (NP group), cervical pain and good deep neck flexor performance (PG group), and cervical pain and poor deep neck flexor performance (PP group). The head sway angle was measured using a three-dimensional motion analysis system. A 3-kg weight was used for external perturbation with the subject sitting in a chair in the resting and erect head positions with voluntary contraction of the deep neck flexors. A one-way analysis of variance (ANOVA) was performed with a Bonferroni post hoc test. The deep neck flexor performance differed significantly among the four groups (p<.05). The NG group had significantly greater deep neck flexor performance than NP and PP groups. The stability of the cervical spine also differed significantly among the four groups in the resting head position (p<.05). The head sway angle was significantly smaller in NG group as compared with the other groups. The PP group had the greatest head sway angle in the resting head position. However, there was no significant difference in the stability of the cervical spine among the groups in the erect head position with voluntary contraction of deep neck flexors (p=.57). The results of this study suggest that the deep neck flexor performance is important for maintaining the stability of cervical spine from external perturbation.

Can Head and Neck Cancers Be Detected with Mean Platelet Volume?

  • Background: Mean platelet volume (MPV) is a marker which has been investigated in many cancers but data for head and neck lesions are limited. We aimed to study the MPV levels in head and neck cancers as a diagnostic marker. Materials and Methods: A total of 96 head and neck cancer patients and 31 control patients who did not meet exclusion criteria were enrolled in the study. The cancer locations, the platelet and MPV levels at the first diagnosis time were collected. Results: The head and neck cancer location distribution between these patients was 2 (2.1%) buccal, 9 (9.4%) tongue, 6 (6.3) lip, 1 (1%) gingiva, 1 (1%) hypopharynx, 1 (1%) ear, 58 (60.4%) larynx, 2 (2.1%) maxilla, 2 (2.1%) nasal, 1 (1%) nasopharynx, 2 (2.1%) palatal, 3 (3.1%) primary unknown, 1 (1%) retromolar, 1 (1%) thyroid, 2 (2.1%) tonsil, and 4 (4.2%) salivary gland. MPV levels were significantly different between cancer and control group (p=0.002). The cut-off point for MPV predicting head and neck cancer is >10 fL (sensitivity=55.21, specificity=87.10). Conclusions: MPV level increase, a readily assessable parameter which does not bring extra costs can warn us regarding head and neck cancer risk.

상악동내 발생한 악성 섬유성 조직구종 1예 (A Case of Malignant Fibrous Histiocytoma in Maxillary Sinus)

  • Malignant fibrous histiocytoma (MFH) is a sarcoma that occurs principally in soft tissue and typically involved the skeletal muscle and deep fascia. Although it is one of the most common types of soft tissue sarcoma in late adult life, the involvement of the head and neck area is relatively rare. MFH shows variable histologic appearance, and may be classified into several subtypes(storiform-pleomorphic, myxoid, giant cell, inflammatory, angiomatoid) and the storiform-pleomorphic type is the most common type. The authors have recently experienced a case of MFH in the right maxillary sinus, and report it with a brief review of the related literature.

두경부에 발생한 육종의 임상적 고찰 (Clinical Analysis of Head and Neck Sarcoma)

  • Introduction : Sarcoma of the head and neck region is a very rare disease entity. This retrospective study investigated the clinical characteristics of head and neck sarcomas and analyzed its treatment methods and outcomes. Subjects and Methods : Eighty-five patients who were diagnosed as sarcomas of the head and neck region in Shinchon Severance Hospital between 1985 and 2005 were included in the study. Data concerning age, sex, symptoms, location and size of tumor, histopathologic characteristics, treatment methods, recurrence, and distant metastasis were reviewed. Result : Overall 5 year survival rate was 38% and the 5 year survival rate in the pediatric population was 60%. The 5 year survival rates for each osteosarcoma and soft tissue sarcoma cases were 42% and 37% respectively. The 5 year survival rate was significantly higher in the cases where complete surgical resection was achieved. Conclusion : In managing head and neck sarcomas, it is important to perform wide resection and to achieve complete resection.

이하선에 발생한 비호지킨스 림프종 1예 (A Case of Non Hodgkin's Lymphoma of Parotid Gland)

  • About 20% of non-Hodgkin' lymphoma occur in the extra-nodal region in the head and neck area and more than half of extra-nodal lymphoma in the head and neck area involves the Waldeyer' ring. Malignant lymphoma arising in the parotid glands are relatively rare, although 90% of all salivary gland lymphomas, appear in parotid gland and are classified as extranodal non Hodgkin' lymphoma. We experienced one rare case of non-Hodgkin' lymphoma in the right parotid gland, with a painless swelling of the cheek region as the chief complaint. We treated with CHOP(cyclophosphamide, $750mg/m^2$ i.v. day1 ; doxorubicin, $50mg/m^2$ i.v. day 1 ; vincristine, $1.4mg/m^2$ i.v. day 1;prednisone, $50mg/m^2$ orally days 1 to 5. repeat every 21 days) and Rituximab combination therapy. We aimed to report here one case of non-Hodgkin' lymphoma in the right parotid gland with review of literature.

COVID-19와 두경부암 (Head and Neck Oncologic Care During the COVID-19 Pandemic)

  • It is well established that cancer patients are more susceptible to infection because of the immunosuppressive status caused by both disease itself and anticancer treatment, including surgery and chemoradiation. Head and neck cancer (HNC) patients are generally at high-risk for coronavirus disease 2019 (COVID-19) infection and serious adverse outcomes. Although there is an urgent need for guidance in the delivery of safe, quality oncologic care, no international consented recommendation addressed the management of HNC patients in COVID-19 due to limited data. In this review, we summarized the consideration for head and neck oncologic care in the context of the COVID-19 pandemic, based on the data and the very recent recommendations from the Korean Cancer Association and National Cancer Center. COVID-19 should be taken into consideration in the comprehensive management of HNC patients, and multidisciplinary evaluation of multilevel surgical-risks, discussion of optimized strategy, and shared-decision-making with the patient are needed to maximize both the safety from infectious pandemic and outcome of surgical and oncologic care.

호산구 증다증을 동반하지 않은 기무라병 1예 (A Case of Kimura's Disease Without Eosinophilia)

  • Kimura's disease is a rare disease of unknown etiology, commonly presenting with slow-growing head and neck subcutaneous nodules. It primarily involves the head and neck region, presenting as deep subcutaneous masses and is often accompanied by regional lymphadenopathy and salivary gland involvement. Clinically it is often confused with a parotid tumor or lymph node metastasis. It is difficult to diagnose before surgery, and fine needle aspiration cytology has only limited value. Even though this disease has not shown any malignant transformation, it is often difficult to cope with because of its high recurrence rate. Surgery, steroids, and radiotherapy have been used widely as the first-line recommendation, but none of them is standard procedure until now because of high recurrence rates. The recurrence of the disease reported up to 62%. We recently experienced a case of Kimura's disease, not accompanying peripheral eosinophilia, on the parotid gland treated by surgical resection in an 82-year-old woman with polycythemia vera. Here, we report this case with a review of the literature.

Knowledge of Human Papillomavirus and its Association with Head and Neck Benign and Malignant Lesions in a Group of Dental Patients in Pakistan

  • Human papillomaviruses (HPVs) remain a serious world health problem due to their association with cervical and head and neck cancers. While over 100 HPV types have been identified, only a few subtypes are associated with malignancies. HPV 16 and 18 are the most prevalent oncogenic types in head and neck cancers. Although it has been proven that some subsets of benign and malignant head and neck lesions are associated with HPV, the general population have very little awareness and knowledge of their association with HPV. Therefore, the purpose of this study was to determine the knowledge of HPV and its links with head and neck benign and malignant lesions in a group of Pakistani dental patients who attended the Dental Department of the Sandeman provincial hospital in Quetta, Pakistan. One hundred and ninety-two patients were recruited and requested to answer a questionnaire. It was revealed that there was a low level of knowledge about HPV and its association with head and neck benign and malignant lesions among the participants. This result suggested that more education regarding the relationship of HPV in inducing head and neck benign and malignant lesions is required in this group of patients.

The Adverse Effect of Proprioceptive Sense in Head-Neck according to Smartphone Usage

  • Purpose: Most studies have reported pain in the head-neck and upper-limbs according to smartphone usage, which is related to the proprioception sense in the head and neck, but there have been few studies. Therefore, the aim of this study was identify the adverse effects of the proprioceptive sense in the head-neck according to smartphone usage. Methods: Twenty-seven young adults (male: 9, female: 18) were enrolled in this study. The proprioceptive sense was measured through the joint reposition sense error and neural positon error in the head-neck during smartphone usage for 0, 5, and 20 minutes. The Noraxon MyoMotion system was used to record the joint position angle and neutral positon in the head-neck. One-way repeated ANOVA was used to identify the differences between the three smartphone use durations and the least-squares difference was used as a post hoc test. The data were analyzed using SPSS 18.0 software. Results: The joint reposition sense error and neural positon error in the head-neck were significantly different among the 0, 5, and 20 minutes of smartphone usage (p<0.05). In the post hoc test, the joint reposition sense error and neural positon error showed a significant difference between smartphone use for 0 minute and 5 minute, and between smartphone use for 0 minute and 20 minutes. Conclusion: This study suggests that smartphone use within 5 minutes can have adverse effects on the proprioceptive sense. Therefore, it is necessary to consider the appropriate use time and break time when using smart phones.

경구강 적출이 가능했던 부인강 이형성종 1예 (A Case of Parapharyngeal Pleomorphic Adenoma Removal Intraorally)

  • Primary tumors of the parapharyngeal space are rare and account for only 0.5% of head and neck neoplasm. About 80% of parapharyngeal tumors are benign and 20% are malignant. Parapharyngeal space is classified into the Prestyloid space and the Poststyloid space. The Poststyloid tumors are usually benign lesions such as neurogenic tumors, paragangliomas, vascular tumors, or aneurisms. The origins of prestyloid tumors are much more diverse pathology, the pleomorphic adenoma in parotid deep lobe is most common type. Several surgical approaches have been introduced for management of parapharyngeal tumor, such as transcervical, transparotid-transcervical and the transcervical-transmandibular approaches. This paper is aimed to present a large parapharyngeal space tumor removed via transoral approach. It is possible to remove easier by using microdebrider. The pathologic diagnosis was pleomorphic adenoma.
