  • A tray inserting machine is developed for automation of inserting plastic mold tray into a cardboard box. Plastic mold tray is used for protecting breakdown of glass bottles. In this paper, two types of processes to divide the plastic mold tray are proposed. As a result, adhesion method by vacuum pad is accepted. And also, static and modal analysis for the machine are carried out to check the machine design using the commercial software, CATIA V5.

Effects of Proprioceptive Neuromuscular Facilitation Exercise on Forced Expiratory Volume at One Second, Pain, and Functional Disability Index of Chronic Low Back Pain Patients (고유수용성신경근촉진법 운동이 만성허리통증환자의 1초간 노력성 날숨량과 통증 및 기능장애지수에 미치는 영향)

  • Purpose: The purpose of this study was to investigate the effects of neck patterns in proprioceptive neuromuscular facilitation (PNF) for neck movement and the neck disability index (NDI) among adults with forward head posture. Methods: Thirty-nine subjects were randomly assigned into two groups. Subjects in the proprioceptive neuromuscular facilitation exercise group (PNFG, n = 20) received 20 minutes of PNF neck pattern (flexion-Rt. lateral flexion-Rt. rotation followed by extension-Lt. lateral flexion-Lt. rotation) 3 times weekly for 4 weeks. Outcomes were measured using absolute rotation angle (ARA), anterior weight bearing (AWB), range of flexion and extension motions (RFEM), and neck disability index (NDI) methods before and after the 4-week intervention period. Results: There were significant effects for the PNFG, pre- and post-intervention, in ARA, AWB, RFEM, and NDI. There were significant differences in ARA, AWB, RFEM, and NDI compared with CG. Conclusion: The results of this study suggest the PNF neck pattern could be beneficial for adults with forward head posture.Purpose: This study investigates how abdominal muscular exercise based on proprioceptive neuromuscular facilitation (PNF) can affect chronic low back pain patients in terms of their pulmonary function, pain, and functional disability indexes. Methods: Fourteen target subjects with chronic low back were randomly assigned to the control group (n = 7) that performed abdominal muscle exercises and the experimental group (n = 7) that performed PNF abdominal muscular exercises. The exercises were performed five times a week for six weeks. To check the change in pulmonary function, the forced expiratory volume at one second (FEV1) and visible analogue scale (VAS) were measured to check the pain level. The disability level caused by back pain was measured by the Oswestry Disability Index (ODI). A paired t-test was applied to compare the differences between the groups before and after the intervention, and an independent t-test was used to compare the differences between the groups. The level of statistical significance was set as ${\alpha}=0.05$. Results: Before and after the intervention, the experimental group showed a significant change in FEV1 (p < 0.01), and both the experimental and the control groups showed significant changes in VAS and ODI (p < 0.01). A comparison of the differences between the groups indicated that the experimental group showed more significant changes in FEV1 (p < 0.05). Conclusion: According to the study results, PNF abdominal muscular exercise effectively improved pulmonary function, pain, and functional disability indexes in subjects with chronic back pain. The proposed program can be applied to chronic back pain patients as a useful therapy.

Arsenic Speciation Aanalysis in Bamboo Salts by Hydride Generation-ICP-AES (수소화물 발생-유도결합 플라즈마 원자 방출 분광법을 이용한 죽염중의 비소 종분리 분석)

  • As(III) and As(V) were analyzed in bamboo salts by hydride generation ICP-AES. In order to quantify the total amount of As in bamboo salts, $AsH_3$ was producted by reacting with 6 mol/L hydrochloric acid and $NaBH_4$, which was then analyzed by hydride generation ICP-AES. As(V) was quantified simultaneously after selectively quantifying As(III). As(III) was quantified by determining the total amount of As and then correcting for the amount of As(III). To improve the reliability of the analysis we repeated the experiment several times to check the detection limit, quantification limit, and measurements of our testing methods. According to the result of our quantification analysis of As existing in bamboo salt, the range of total As content was 0.05 mg/kg~0.2 mg/kg and As(V) was over 90% of the total As.

  • The aim of this study is to check the application criteria of the conventional techniques and clarify the effects of breaker depth, seabed conditions on the stability in relation to the effects of uncertainty of storm duration and directional irregular waves. The typical damage modes were divided by the direct wave force on the armor unit and by the local scouring around the toe of a breakwater head by the model experiments. The destruction modes are defined, and some criteria on the damage modes and scouring/deposition at the toe of a breakwater head in relating the wave-bottom-structural conditions can be checked using the multi-directonal irregular wave generator system. According to the results, it is emphasized that the 3-D effects on the stability should be analyzed in the design of multi-purpose/function coastal structures in consideration of the evaluation of spatial variation of damage modes and hydraulic characteristics as well as the wave distribution along the structures.

Double-Gauss Optical System Design with Fixed Magnification and Image Surface Independent of Object Distance (물체거리가 변하여도 배율과 상면이 고정되는 이중 가우스 광학계의 설계)

  • A change in object distance would generally change the magnification of an optical system. In this paper, we have proposed and designed a double-Gauss optical system with a fixed magnification and image surface regardless of any change in object distance, according to moving the lens groups a little bit to the front and rear of the stop, independently parallel to the direction of the optical axis. By maintaining a constant size of image formation in spite of various object-distance changes in a projection system such as a head-up display (HUD) or head-mounted display (HMD), we can prevent the field of view from changing while focusing in an HUD or HMD. Also, to check precisely the state of the wiring that connects semiconductor chips and IC circuit boards, we can keep the magnification of the optical system constant, even when the object distance changes due to vertical movement along the optical axis of a testing device. Additionally, if we use this double-Gauss optical system as a vision system in the testing process of lots of electronic boards in a manufacturing system, since we can systematically eliminate additional image processing for visual enhancement of image quality, we can dramatically reduce the testing time for a fast test process. Also, the Gaussian bracket method was used to find the moving distance of each group, to achieve the desired specifications and fix magnification and image surface simultaneously. After the initial design, the optimization of the optical system was performed using the Synopsys optical design software.

A absorbed and effective dose from the full-mouth periapical radiography using portable dental x-ray machine and panoramic radiography (ORIGINAL ARTICLE - 이동형 구내방사선촬영기로 촬영한 치근단 방사선촬영과 파노라마방사선촬영의 흡수선량과 유효선량 평가)

  • Purpose: The purpose of this study was to measure the absorbed dose and to calculate the effective dose for full-mouth periapical radiography using the portable dental x-ray machine and panoramic radiography Material and Method: Thermoluminescent chips were placed at 25sites throughout the layers of the head and neck of a tissue-equivalent human skull phantom. The man phantom was exposed with the portable dental x-ray machine and panoramic unit. During full-mouth periapical radiography the exposure setting was 60 kVp, 2 mA and 0.15 ~ 0.25 seconds, while during panoramic radiography the selected exposure setting was 72 kVp, 8 mA and 18 seconds. Absorbed dose measurements were obtained and equivalent doses to individual organs were summed using ICRP 103 to calculate of effective dose. Result: In the full-mouth periapical radiography, the highest absorbed dose was recorded at the mandible body follow with submandibular glands and cheek. Using panoramic unit, the highest absorbed dose was parotid glands and the following was back of neck and submandibular glands. The effective dose in full-mouth periapical radiography using portable dental x-ray machine was 46 ${\mu}Sv$. In panoramic radiography, the effective dose was 38 ${\mu}pSv$. Conclusion: It was recommended to panoramic radiography for general check in the head and neck area because that the effect dose in the panoramic radiography was lower than the dose in the full-mouth periapical radiography using portable dental x-ray machine.

Survival of a Neartotally Amputated Auricle by Simple Closure and Medical Leech (불완전 절단된 귀손상에서 단순봉합수술 및 거머리를 이용한 접합 치험례)

  • Purpose: In an amputated auricle, a microvascular anastomosis is the best treatment of choice. But, the neartotally amputated auricle which is connected by very narrow tissue pedicle to the head, can survive by simple attachment without a microvascular anastomosis owing to the rich vascular network through the superficial temporal artery and posterior auricular artery. In cases of venous congestion resulting from a lack of vein anastomosis, medical leeches (Hirudo medicinalis) can solve the problem. We are reporting the case of a 6-year-old boy who had a neartotally amputated auricle with successful results by simple closure and medical leech treatment without a microvascular anastomosis. Methods: A 6-year-old male patient had an left auricular injury by an escalator accident. The left auricle was neartotally amputated from the temporal head with connection only by very narrow skin and subcutaneous pedicle (about 1 cm in width) at the helical root of upper and anterior part of auricle. Marginal bleeding from the avulsed auricle was noted and the arterial blood was supplied from a branch of upper auricular branch of the superficial temporal artery. The auricle was repaired by simple closure including cartilage and skin without any vascular anastomosis. After simple closure, the auricle showed good circulation with pink color. But on the 2nd day after the operation, there was a venous congestion with severe swelling, which resulted in a purplish colored auricle. The venous congestion disappeared after using medical leeches by the 5th day after the operation. Results: The repaired auricle showed aesthetically and functionally satisfactory result with normal development at the 9 months follow-up check after the operation. Conclusion: In cases of neartotally amputated auricles of children or crushing injury in which microsurgery is difficult, we can try simple closure with the use of medical leeches in treating a of venous congestion for a successful result.

Thyroid Hemiagenesis Associated with Papillary Thyroid Carcinoma : Report of a Case and Review of the Literature (갑상선 편측형성부전에 동반된 유두 갑상선암 1예)

  • Thyroid hemiagenesis is a rare anomaly, which is the result of failure of embryologic development of a lobe of thyroid gland. It is more frequently found in the left lobe and in female patients. We, herein, report an extremely rare case of thyroid hemiagenesis associated with papillary thyroid carcinoma. A 69-year-old female presented with an incidentally discovered thyroid nodule in the right thyroid during a routine medical check-up. Ultrasonography(US) and computed tomography(CT) disclosed $0.7{\times}0.5cm\;and\;2.8{\times}2.2cm$ sized nodules in the right thyroid. The left thyroid, however, was not seen in the imaging studies of US and CT. Fine-needle aspiration of the small and large nodules showed papillary thyroid carcinoma and adenomatous hyperplasia, respectively. The patient underwent a right total thyroidectomy with central compartment node dissection. The operative findings and histologic examination confirmed the absence of the left thyroid associated with papillary thyroid carcinoma and ademonatous hyperplasia of the right thyroid.

Pile-soil-structure interaction effect on structural response of piled jacket-supported offshore platform through in-place analysis

  • In-place analysis for offshore platforms is essentially required to make proper design for new structures and true assessment for existing structures, in addition to the structural integrity of platforms components under the maximum and minimum operating loads when subjected to the environmental conditions. In-place analysis have been executed to check that the structural member with all appurtenance's robustness have the capability to support the applied loads in either storm or operating conditions. A nonlinear finite element analysis is adopted for the platform structure above the seabed and pile-soil interaction to estimate the in-place behavior of a typical fixed offshore platform. The SACS software is utilized to calculate the dynamic characteristics of the platform model and the response of platform joints then the stresses at selected members, as well as their nodal displacements. The directions of environmental loads and water depth variations have significant effects in the results of the in-place analysis behavior. The most of bending moment responses of the piles are in the first fourth of pile penetration depth from pile head level. The axial deformations of piles in all load combinations cases of all piles are inversely proportional with penetration depth. The largest values of axial soil reaction are shown at the pile tips levels (the maximum penetration level). The most of lateral soil reactions resultant are in the first third of pile penetration depth from pile head level and approximately vanished after that penetration. The influence of the soil-structure interaction on the response of the jacket foundation predicts that the flexible foundation model is necessary to estimate the force responses demands of the offshore platform with a piled jacket-support structure well.

Genotypes of Hepatitis C Virus in Relapsed and Non-respondent Patients and their Response to Anti-Viral Therapy in District Mardan, Khyber Pakhtunkhawa, Pakistan

  • Hepatitis C is a blood-borne infectious disease of liver, caused by a small enveloped, positive-single stranded RNA virus, called the hepatitis C virus (HCV). HCV belongs to the Flaviviridae family and has 6 genotypes and more than 100 subtypes. It is estimated that 185 million people are infected with HCV worldwide and 5% of these are in Pakistan. The study was designed to evaluate different genotypes of HCV circulating in District Mardan and to know about the behavior of these genotypes to different anti-viral regimes. In this study 3,800 patients were exposed to interferon alfa-2a plus Ribavirin treatment for 6-months and subjected to real-time PCR to check the viral response. Among these 3,677 (97%) patients showed no detectable HCV RNA while 123 (3%) patients (non-responders) remained positive for HCV RNA. Genotypes of their analyzed showed that most of them belonged to the 3a genotype. Non-responders (123) and relapsed (5) patients were subjected to PEG-interferon and Ribavirin therapy for next 6 months, which resulted into elimination of HCV RNA from 110 patients. The genotypes of the persisting resistant samples to anti-viral treatment were 3b, 2a, 1a and 1b. Furthermore, viral RNA from 6 patients remained un-typed while 4 patients showed mixed infections. HCV was found more resistant to antiviral therapy in females as compared to mals. The age group 36-45 in both females and males was found most affected by infection. In general 3a is the most prevalent genotype circulating in district Mardan and the best anti-viral therapy is PEG-interferon plus Ribavirin but it is common practice that due to the high cost patients receive interferon alfa-2a plus Ribavirin with consequent resistance in 3% patients given this treatment regime.
