We experienced a case of the papillary thyroid carcinoma seen as a huge solid lateral neck mass. The mass grew very slowly over the period of 30years up to 10cm in diameter while relatively well sparing the surrounding tissues. Physical examinations, CT scan, and fine needle aspiration cytology did not reveal any strong suggestions for evidence of malignancy. But it was pathologically diagnosed as metastatic thyroid carcinoma by excisional biopsy. We emphasize that for a large solitary neck mass which persists for several decades, head and neck surgeons should always keep in mind the possibility of metastasis from the malignancy of thyroid gland.
The reconstruction of huge surgical defect is one of the major problems in the surgical treatment of the cancer of the head and neck. The latissimus dorsi myocutaneous flap, which is one of the most versatile myocutaneous flap, is a reliable method of reconstruction for extensive wounds in the head and neck. Due to the difficult patient positioning, its uses are reserved for the extensive defects or for the cases in which other traditional flaps have failed. The authors successfully reconstructed large surgical defects in the head and neck region using LDMC flap in five patients.

Epidermoid cysts can be found anywhere in the body, particularly in areas where embryonic elements fuse together. Most cases have been reported in the ovaries, the testicles, as well as the hand and feet. Epidermoid cysts in mouth are uncommon and account for less than 0.01% of all oral cysts. The treatment of choice is complete surgical excision and prognosis is excellent. The purpose of this article is to describe two cases of an epidermoid cyst in the oral cavity mucosa and a review of the literature.

A group of 49 patients with neurogenic tumor in head and neck except intracranial tumor & Von-recklinghausen's disease was treated at the Department of Surgery. Presbyterian Medical Center during 12 years from January, 1980 to December, 1991. Of the 49 cases, 24 cases were neurilemmoma, 23 cases neurofibroma and 2 cases malignant schwannoma. The lateral cervical region was the commonest location of the neurogenic tumors, 24 cases (49%) arose from posterior triangle of neck and 12 cases from anterior triangle of neck. The origin of nerve was identified in 28 cases(57%). Cervical plexus(10 cases) and brachial plexus(7 cases) were most commonly affected. The most common site of neurogenic tumors arose from cranial nerve was anterior cervical triangle.
Papillary thyroid carcinoma frequently invades the lymph node, trachea, esophagus and perithyroid tissue. However, direct extension to posterior pharyngeal area is known to be rare. A 64-year-old male was referred to our clinic presenting as posterior pharyngeal mass during gastrofiberscopy. The neck CT scan showed soft tissue mass in retropharynx and lymph node in right level III with calcifications. We performed the total thyroidectomy with selective(level II, III, IV) and anterior compartment neck dissection. In operative findings, the right thyroid mass were connected to the retropharynx through the posterior portion of inferior constrictor muscle. Histopathologic findings revealed the papillary thyroid carcinoma extended to retropharynx. We report a unique case with a literature review.
Malignant peripheral nerve sheath tumor (MPNST) is an extremely rare sarcoma of soft tissue. It is locally invasive, frequently recurred and metastasizes distantly, therefore has a very poor prognosis. the most involved sites are human body and extremities, and MPNST occurs and involved very rarely in the parapharynx of neck. MPNST does not react to radiotherapy and chemotherapy, therefore complete surgical resection is the most important treatment. Since immunohistochemical staining is essential for the final diagnosis, it is common to be confirmed after surgery. We report a case of giant MPNST without symptom in the head and neck involving the right parapharynx with multiple distant metastases in a 74-year-old patient on a review of the literature.
Malignant lymphoma of the thyroid gland is a reletively rare and constitute up to 1-5% of all thyroid malignancies. It is known to be frequently associated with Hashimoto's thyroiditis. The clinical presentation includes an enlarging neck mass, accompanied by dysphagia, hoarseness or tenderness, with its most common histologic type being the diffuse large B cell non-Hodgkin's lymphoma. We have experienced a case of thyroid lymphoma arising from Hasimoto's thyroiditis in a 57-year-old woman, who presented with an anterior neck mass. We present this case with a review of the literature.

In recent years, a large number of study for rear impact has been conducted and as a result of study, researchers proposed the neck injury criteria and test procedures. But many questions, related to injury criteria and dummy biomechanical levels, remain unresolved. In recently reports, rear impact motions of BioRIDIIg is not humanlike but better than other dummy(HybridIII, RID3d). So, in this paper, 4 times sled test would be done to find the substitutable neck injury criteria in BioRIDIIg. To review corelation trend with neck injury critera and head-neck movement, we compared with recently announced neck injury criteria(NIC, Nkm, T1 ect.) and head-neck X-direction movement in BioRIDIIg. Finally, we find the head-neck X-direction movement on head C.G to T1 point may be considerable as the additional neck injury criteria.

Background and Objectives: Adenoid cystic carcinoma (ACC) is a unique tumor characterized by frequent and delayed distant metastasis (DM) with uncommon regional lymph node metastasis. We evaluated the factors affecting DM of ACC and survival after appearance of DM. Materials and Methods: Medical records, radiographs and pathologic slides were reviewed for 94 patients from 1979 through 2001. Results: DM of ACC occurred in 46 patients, and developed more frequently in patients with tumors of the solid histologic subtype than in patients with tubular or cribriform subtypes. DM occurred less frequently in the sinonasal tract, and development of DM was not affected by tumor stage. Disease-specific 5- and 10-year survival rates were 88% and 72% for patients without DM, respectively and 76% and 48% for those with DM(p=0.02). Regarding the site of DM and its impact on outcomes, 30 patients had lung metastasis alone, 5 patients bone metastasis alone and 6 patients developed both lung and bone metastasis. Median survivals after appearance of DM among patients with isolated lung metastases and those with bone metastases with or without lung involvement were 54 and 21 months, respectively (p=0.04). Conclusions: Development of DM in ACC is predicted by solid histologic subtype, and major salivary gland or oral/pharyngeal rather than sinonasal primary site. Those patients with bone involvement with our without lung metastases had worse outcomes than those with pulmonary metastasis only.

Growth factors and the receptors play an important role in the regulation of the growth and development of mammalian cells. In particular, epidermal growth factor is a polypeptide with potent mitogenic activity that stimulates proliferation of various normal and neoplastic cells through the interaction with its specific receptor(EGFR). EGFR has been described as a parameter of poor prognosis in many human neoplasms such as breast, bladder, and vulvar cancers. The objectives of this study are the evaluation of the expression of EGFR and cell cycle analysis in the head and neck squamous cell carcinomas(SCC), and the evaluation of the correlation between clinico-patholgic features and expression of EGFR and S-phase fraction. 37 head and neck squamous cell carcinoma specimens were evaluated for expression of EGFR by Western blot analysis and S-phase fraction by cell cycle analysis using the flow cytometry. The obtained results were as follows : 1. The expressions of EGFR were observed in 20 specimens(54%) among 37 head and neck SCC specimens. In case of oral SCC, 15 specimens(56%) out of 27 specimens were observed, and in case of nasopharyngeal SCC 5 specimens(50%) out of 10 specimens. 2. There was no correlation between clinical features(location, stage) of head and neck SCC and expression of EGFR (p>0.05). 3. There was a significant correlation between histo-pathological differentiation of head and neck SCC and expression of EGFR (p<0.02). 4. There was a significant correlation between expression of EGFR and S-phase fraction of cell cycle in the head and neck SCC (p<0.05). The above results suggest that expression of EGFR and S-phase fraction of cell cycle are adjunctive prognostic marker in the head and neck squamous cell carcinomas.
