A Case of Mikulicz's Disease in Parotid Gland (이하선에 발생한 Mikulicz's Disease 1례)

  • Mikulicz's disease is an autoimmune disorder characterized by asymptomatic chronic swelling of the salivary gland or lacrimal gland. It is also called as a benign lymphoepithelial lesion. The clinical manifestations are usually bilateral and symmetric. The diagnosis is confirmed by only histopathologic finding. Microscopically, the lymphoid infiltration and epimyoepithelial island appear as the solid nests surrounded and infiltrated by lymphoid cells. The treatment is symptomatic, but the patients are at an increased risk for the development of malignant lymphoma. So it need a careful observation.

  • Leiomyosarcoma of the larynx is very rare, and rarely been described in reference to the head and neck region. This tumor occurs mainly in the uterus, the gastrointestinal track, and the retroperitoneum. There have been 24 previously reported cases of this tumor involving the larynx in the literature. Histologic diagnosis remains extremely difficult. A case of leiomyosarcoma of the larynx, treated by total laryngectomy, is reported. We discuss the clinical presentation, diagnosis, and treatment of leiomyosarcoma with a review of the literature.

  • Basaloid squamous cell carcinoma(BSCC) is a high-grade variant of squamous cell carcinoma, with a prediction for multifocal involvement of the base of tongue, pyriform sinus, supraglottic larynx, hypopharynx and palatine tonsil. It primary affects men in the seventh decade of life with frequent cervical lymph-node metastasis at presentation. Grossly, these tumors are usually firm to hard, with associated central necrosis, occuring as exophytic to nodular masses. Histologically, the this infiltrating tumor offers a variety of growth patterns, including solid, lobular, cribriform, cords, trabeculae, nests and glands or cyst. We present a 55-year-old female who was treated with surgical excision and radiotherapy. She was firstly presented as a recurrent inflammatory neck mass and finally diagnosed with basaloid squamous cell carcinoma in the palatine tonsil.

  • Laryngeal amyloidosis is a rare benign disease. It is characterized by extracellular deposition of homogeneous and eosinophilic protein material in the form of fibrils. Diagnosis of this disease is made by histologic examination of involved tissue. It is ususlly primary or localized but rarely associated with a systemic or generalized disease. We present a case of primary laryngeal amyloidosis that was treated with KTP 532 laser vaporization.

  • Intrathoracic goiters can be classified anterior and posterior mediastinal goiter with its locations. Most intrathoracic goiters are retrosternally situated in the anterior mediastinal compartment. Posterior mediastinal goiters are rare, but might present a difficult diagnostic and surgical problem. Although thyroid goiters are nearly always amenable to a cervical approach, posterior mediastinal goiters may require a combined cervicothoracic approach with sternotomy or thoracothomy. We herein describe a case of posterior mediastinal goiter which was excised only by cervical approach. The relevant literature is briefly reviewed.

  • Giant cell reparative granuloma(GCRG) is a non-neoplastic rapidly expanding and locally destructive tumor that occurs almost exclusively within the mandible and maxilla. A 58-year-old man, complained of a mass on the left infra-auricular area starting 2 months ago. The radiologic finding suggests a mass that originate from mandible, pathology diagnosed the lesion as a giant cell reparative granuloma. The tumor was surgically excised without complications. We report the case with a review of literature.

  • Lymphadenoma is a rare benign neoplasm of the salivary gland. Herein, we present a 67-year old woman with lymphadenoma in the parotid gland. Physical and radiologic examinations of parotid lymphadenoma are not specific and preoperative diagnosis is usually difficult. Therefore, clinicians should consider the possibility that lymphadenoma may occur in the parotid gland mimicking the other more frequently observed lesions.

  • Benign lymphoepithelial cysts of parotid gland were first described by Miculicz in 1888, have been presented for a long time, uncommonly. But, recently it has been reported with increasing frequency since the onset of human immunodeficiency virus (HIV) infection. Benign lymphoepithelial cysts can be diagnosed by the fine needle aspiration in conjunction with the history and physical examination. The therapeutic options of benign lymphoepithelial cysts have been limited to aspiration, surgical resection, medical therapy, radiotherapy and sclerotherapy. Recently, we experienced a case of benign lymphoepithelial cyst of right parotid gland, so we report this case with review of the literatures.

  • Mucoepidermoid carcinoma is most frequent malignant tumor of parotid gland. However, mucoepidermoid carcinoma arising from pleomorphic adenoma has been disputed and rarely reported. 54-year-old woman with pleomorphic adenoma underwent superficial parotidectomy. And 15 years later, she diagnosed mucoepidermoid carcinoma at the same side. We report the case with a review of literature.

  • Brachial plexus schwannomas are rare tumors. They are benign nerve sheath tumors and only about 5% of Schwannoma arise from the brachial plexus. Due to its rarity and complex anatomical location they can pose a formidable challenge to surgeons. We present a case of a young patient who presented with an supraclavicular swelling three months, that were proven to be schwannoma on histopathology.
