Minimally Invasive Radio-guided Parathyroid Surgery (최소침습 방사능 유도 부갑상선 수술)

  • Introduction: Since 1996, Dr. James Norman has successfully performed mimimally invasive radio-guided parathyroidectomy(MIRP) using intraoperative nuclear mapping with $^{99m}Tc$ sestamibi scanning and radioactivity detection probe. Objectives: We aimed to introduce this new surgical technique and evaluate it's efficacy by our own experiences. Method: From May to October 1999, five consecutive patients with primary hyperparathyroidism underwent parathyroidectomy by using modified MIRP technique. $^{99m}Tc$ sestamibi scanning was performed 1.0 or 1.5 hour before operation. After intraoperative localization of the tumor under the guidance of quantitative gamma counting with a NEVIGATOR probe, an unilateral small skin incision(3.0-4.0cm) was placed. Without a skin flap, the strap muscle was directly divided with the use of a Harmonic scalpel. After careful dissection, the parathyroid tumor was removed. Result: In all patients, a single adenoma could be easily detected and removed by this new technique. Mean incision length was 3.2cm(3.0-4.0cm) and operative time ranged from 40 to 110minute. All the patients were discharged within 2 days of surgery without any complication. Conclusion: This new operative technique could become the most minimally invasive alternative to the standard operative procedure for parathyroid adenoma.

Mapping and estimating forest carbon absorption using time-series MODIS imagery in South Korea (시계열 MODIS 영상자료를 이용한 산림의 연간 탄소 흡수량 지도 작성)

  • Time-series data of Normal Difference Vegetation Index (NDVI) obtained by the Moderate-resolution Imaging Spectroradiometer(MODIS) satellite imagery gives a waveform that reveals the characteristics of the phenology. The waveform can be decomposed into harmonics of various periods by the Fourier transformation. The resulting $n^{th}$ harmonics represent the amount of NDVI change in a period of a year divided by n. The values of each harmonics or their relative relation have been used to classify the vegetation species and to build a vegetation map. Here, we propose a method to estimate the annual amount of carbon absorbed on the forest from the $1^{st}$ harmonic NDVI value. The $1^{st}$ harmonic value represents the amount of growth of the leaves. By the allometric equation of trees, the growth of leaves can be considered to be proportional to the total amount of carbon absorption. We compared the $1^{st}$ harmonic NDVI values of the 6220 sample points with the reference data of the carbon absorption obtained by the field survey in the forest of South Korea. The $1^{st}$ harmonic values were roughly proportional to the amount of carbon absorption irrespective of the species and ages of the vegetation. The resulting proportionality constant between the carbon absorption and the $1^{st}$ harmonic value was 236 tCO2/5.29ha/year. The total amount of carbon dioxide absorption in the forest of South Korea over the last ten years has been estimated to be about 56 million ton, and this coincides with the previous reports obtained by other methods. Considering that the amount of the carbon absorption becomes a kind of currency like carbon credit, our method is very useful due to its generality.

Higher Order Shimming for Ultra-fast Spiral-Scan Imaging at 3 Tesla MRI System (3 Tesla MRI 시스템에서 초고속 나선주사영상을 위한 고차 shimming)

  • Purpose: To acquire high-resolution spiral-scan images at higher magnetic field, high homogeneous magnetic field is needed. Field inhomogeneity mapping and in-vivo shimming are important for rapid imaging such as spiral-scan imaging. The rapid scanning sequences are very susceptible to inhomogeneity. In this paper, we proposed a higher-order shimming method to obtain homogeneous magnetic field. Materials and Methods: To reduce measurement time for field inhomogeneity mapping, simultaneous axial/ sagittal, and coronal acquisitions are done using multi-slice based Fast Spin echo sequence. Acquired field inhomogeneity map is analyzed using the spherical harmonic functions, and shim currents are obtained by the multiplication of the pseudo-inverse of the field pattern with the inhomogeneity map. Results: Since the field inhomogeneity is increasing in proportion to the magnetic field, higher order shimming to reduce the inhomogeneity becomes more important in high field imaging. The shimming technique in which axial, sagittal, and coronal section inhomogeneity maps are obtained in one scan is developed, and the shimming method based on the analysis of spherical harmonics of the imhomogenity map is applied. The proposed technique is applicable to a localized shimming as well. High resolution spiral-scan imaging was successfully obtained with the proposed higher order shimming. Conclusion: Proposed pulse sequence for rapid measurement of inhomogeneity map and higher order shimming based on the inhomogeneity map work very well at 3 Tesla MRI system. With the proposed higher order shimming and localized higher order shimming techniques, high resolution spiral-scan images are successfully obtained at 3 T MRI system.

  • Matrix converters have been studied for eliminating dc link of conventional converter-inverter system, and various undulation strategy have been proposed. Therefore, matrix converter have no energy storage component except for small ac later for the elimination of switching ripple, and can be made compact and highly reliable compare with the do link inverter system. Matrix converter, however, directly connected the input and the output terminals by bidirectional static switch. As a result if the input voltage are asymmetrical, and contain harmonics, the influence of the distortions directly appear on the output terminal. This problem is a major obstacle to the matrix converter. A new control method using average comparison strategy have been proposed in this paper. This control method realizes sinusoidal input and output current unity input displacement factor regardless of load power factor. Moreover, compensation of the asymmetrical and/or harmonic containing input voltage is automatically realized, and calculation time of control function is reduced.
