A Novel Nonmechanical Finger Rehabilitation System Based on Magnetic Force Control

  • This paper presents a new nonmechanical rehabilitation system driven by magnetic force. Typically, finger rehabilitation mechanisms are complex mechanical systems. The proposed method allows wireless operation, a simple configuration, and easy installation on the hand for active actuation by magnetic force. The system consists of a driving coil, driving magnets (M1), and auxiliary magnets (M2 and M3), respectively, at the finger, palm, and the center of coil. The magnets and the driving coil produce three magnetic forces for an active motions of the finger. During active actuations, magnetic attractive forces between M1 and M2 or between M1 and M3 enhance the flexion/extension motions. The proposed system simply improves the extension motion of the finger using a magnetic system. In this system, the maximum force and angular variation of the extension motion were 0.438 N and $49^{\circ}$, respectively. We analyzed the magnetic interaction in the system and verified finger's active actuation.

A Tele-rehabilitation System with an Automated Pegboard Utilizing Radio Frequency Identification

  • Due to the expense of health care and the need to contain costs, many stroke patients are discharged from hospitals while still in an impaired condition. Using Tele-rehabilitation, these patients can receive rehabilitation services remotely. A pegboard is a conventional rehabilitation therapeutic device that integrates cognition, sensation and hand motor function. This study proposes a Tele-rehabilitation content with automated pegboard and shows its functional feasibility. The evaluation of the pegboard session was automated with RFID (radio frequency identification), and a 16-hole pegboard was rapid-prototyped. After a pegboard session is completed, the session result is uploaded to a server automatically for viewing on a web browser by a remote therapist. The therapist can also send messages to remote patients to encourage them or to manage the rehabilitation process.

  • This paper propose the test method for performance of myoelectric hand prosthesis(MHP) according to myoelectric signal generated in the human muscle. The MHP consists of a mechanical hand, a surface myoelectric sensor(SMES) for measuring myoelectric signal, a control system and a charging battery. The propose test method is proved the reliability of MHP.

The Impact of Shoulder Flexion Angle on Hand Grip Strength in Male and Female Undergraduate Students (견관절 굴곡 각도가 남·녀 대학생의 악력 변화에 미치는 영향)

  • Purpose : This study's purpose is consideration about change of the hand grip strength according to different posture and shoulder flexion angle. The shoulder joint permits the greatest mobility and carries out the important function of stabilization for hand use. Hand grip activity is important to evaluate while assessing loads of shoulder in hand mobilities. Methods : Thirty(15 male, 15 female) college students with unknown shoulder dysfunction participated subject in five different positions of elbow extension with sitting and standing posture, different positions is followed : (1) shoulder $0^{\circ}$ flexion (2) shoulder $45^{\circ}$ flexion (3) shoulder $90^{\circ}$ flexion (4) shoulder $135^{\circ}$ flexion (5) shoulder $180^{\circ}$ flexion. Results : On the average, in the hand grip strength, the standing posture is higher than sitting posture. Sitting posture showed a most high level at the man's $0^{\circ}$ and woman's $135^{\circ}$. And standing posture showed a most high level at the man's $135^{\circ}$ and woman's $90^{\circ}$. Conclusion : The paired t-test was used to determine the different in grip strength between sitting and standing posture by shoulder angle change. There was no significant difference between the five position by sitting and standing posture. In man, correlation analysis revealed significant connection for all five position by sitting and standing posture. And in woman, correlation analysis revealed connection for all five position by sitting and standing posture.

The Status of Medical Rehabilitation of the CVA Patients (뇌졸중환자의 재활의료실태)

  • This research tries to find out the methods to enhance the rehabilitation ability of Cerebro Vascular Accident(CVA) patients and their family members. For this purpose, the authors identify the factors that may affect the medical rehabilitation process as well as the rehabilitation itself. Furthermore, the current status of the rehabilitation ability and the desires of CVA patients and their family members are surveyed in this research. The results of this research are as follows: Firstly, the surveyed CVA patients, most of whom are over fifties in the age, have cerebral infection and hemorrhage in a numerical order. The survey showed that the larger cities they live in, the more CVA occur to them. Secondly, the survey for the treatment cost demonstrated that the lower are their living level and their education level, the more pressure is put on the CVA patients and their family. The statistical data proved this conclusion is reasonable. Thirdly, as for the safisfaction for the rehabilitation treatment, the statistical survey showed that it is directly proportional to their financial income. Moreover, those who have lower income generally want the rehabilitation services from a public health center. On the other hand, those who have higher income want the home therapy services.

Is the Legal Basis for Physical Therapist's Home-Based Rehabilitation Appropriately Prepared?

  • Purpose: This study examined the legal basis related to a physical therapist's home-based rehabilitation. Methods: The policy data were referenced to the comprehensive plan for national health promotion and the guidebook Community Health Promotion Project issued by the Korea Health Promotion Institute and other institutes. The legal data were referenced to the Bill information system on the National Assembly website. Results: The physical therapist's home-based rehabilitation did not have a legal basis in the community-based rehabilitation project (CBR) of the HP 2020 project. On the other hand, according to the Home health care part of the community health promotion project that began in 2013, physical therapists were allowed to play a role from 2020 under Article 16-2 (Public Officials in Exclusive Charge of Visiting Health Management) of the REGIONAL PUBLIC HEALTH ACT. Conclusion: Policies and laws are being developed in the field of healthcare, but the necessary policies and laws in the field of rehabilitation are still insufficient. A bill to rehabilitate in a variety of fields and spaces by modifying the scope of work of physical therapists will be needed.

A Study on Correlation Coefficients between Lumbosacral Balance and Low Back Pain (요천추부의 정렬과 설문을 통한 요통 자각도의 상호 연관성에 관한 연구)

  • Objectives : This study was designed to investigate the correlation coefficients among Oswestry low-back pain disability index(ODI), Roland-Morris disability questionnaire (RMD), visual analogue scale(VAS), lumbar lordosis angle(LLA), Cobb's angle and Ferguson's angle(FA). Methods : We measured LLA, Cobb's angle, and FA of 42 students. Then we researched ODI, RMD and VAS of all students, and analyzed correlations coefficient among all of them. Results : 1. There was significant correlation among VAS, RMD, ODI. 2. There was significant correlation between ODI and Cobb's angle. 3. There was no significant correlation between LLA, FA, Cobb's angle and VAS. 4. There was no significant correlation among LLA, FA, Cobb's angle and RMD. Conclusions : According to above results, there was no significant correlation between lumbosacral balance and low back pain except between ODI and Cobb's angle. On the other hand, there was significant correlation among RMD, ODI and VAS.

Changes in Grip Strength and Associations with Grip Strength in Breast Cancer Survivors Treated with Adjuvant Chemotherapy

  • Purpose: The aim of this study was to identify the change in hand grip strength (HGS) for consecutive 6 months in breast cancer survivors (BCS) undergoing postoperative adjuvant chemotherapy (adjCTX) and to determine the factors relevant to HGS after chemotherapy completion Methods: This study was a retrospective observational cohort study. BCS (N=38) who underwent breast cancer surgery and subsequent adjCTX were enrolled. The HGS of both sides was measured before adjCTX and 3 weeks, 2 months and 6 months after adjCTX. And we also collected body composition, anthropometric measurements, social demographics and clinical data. Then we analyzed the changes in HGS over time, and correlation of collected data with HGS at 6 months. Results: A significant increase in HGS of the unaffected side was observed within the first 3 weeks of adjCTX and maintained up to 6 months. In correlation analysis, HGS of both sides was positively correlated with the ipsilateral HGS at 3 weeks, and contralateral HGS at 6 months. And skeletal muscle mass was also positively correlated with the HGS of the unaffected side. In addition, subjects treated with targeted therapy had lower HGS of the unaffected side. However, HGS of the affected side was significantly lower in subjects with breast reconstruction and treated without radiotherapy. Conclusions: To maintain the early recovery of HGS, active and continuous exercise intervention for strengthening might be necessary.

The Effect of EMG-stim on Upper Limb Function in Chronic Stroke Patients (근전도 유발 신경근 전기자극치료가 뇌졸중 환자의 상지기능에 미치는 효과)

  • Purpose: This study examined the effect of EMG-stim related to the functional recovery of the upper extremity in chronic stroke patients with an intensive massed practice protocol. Methods: The subjects were assigned randomly to either the EMG-stim group (n=10) or sham treatment group (n=10). Both groups received conventional physical therapy, occupational therapy and FES, five times per week over a four week period. In the EMG-stim group, EMG-stim was applied to the hemiplegic wrist and finger extensors for 2 sessions for 30 minutes per day, 5 times per week over a 4 week period. As the pre- and the post-test, the following four motor tests were assessed as the function of the upper extremity clinical functional test: extensor digitorum strength test, Box and Block test, Fugl-Mayer Assessment, and Jebson-Taylor Hand Function Test. Results: In the Box and Block test and Fugl-Mayer Assessment, there were statistically significant differences between both groups as well as between pre- and post-test. The extensor digitorum and wrist extensor strength were similar in both groups. In the Jebson-Taylor Hand Function Test, there was a significant difference in simulated page turning but not in the other subtests. Conclusion: Intensive massed practice with EMG-stim intervention applied to the hemiplegic upper extremity is an effective therapeutic method for chronic stroke patients. However, a variety of intervention methods designed for stroke patients in clinical settings are needed.

The Effects of Community based Self-help Management Program on the Activity of Daily Life, Muscle Strength, Depression and Life Satisfaction of Post-stroke Patients (재가 뇌졸중 환자를 위한 자조관리프로그램이 일상활동, 근육 강도, 우울 및 삶의 만족도에 미치는 효과)

  • Stroke is one of the leading causes of death in Korea. Because of their sequelae, strokes are categorized as a sudden-onset, constant course chronic illness which needs continuous efforts for rehabilitation. Unfortunately, there are few community based rehabilitation program for post-stroke patients who stay at home. The authors developed a community based selp-help management program for post-stroke patients to enhance their rehabilitation process. The program consists of five sessions and each session contains health education. ROM exercise, ADL training, and stress management like foot reflexology. A professor and two graduate students of nursing college coordinated the program. To test the effects of the program we conducted a 5 week program to the 10 conveniently selected post-stroke patients who were living in Kang-buk district of Seoul. The Questionnaires about ADLs, IADLs, depression and life satisfaction were asked to the all subjects before and after program. The hand grisp power and muscle strength of four limbs were measured at the end of each sessions. The analysis of data revealed that the program was effective to increase the ADLs, IADLs, and muscle strength and to decrease the depression levels of subjects. However, there was no significant difference between pre and post hand grisp power and life satisfaction. Because the program was effective to Improve the physical and psycholocial function of subjects, we suggest continual development and Implementation of community based self-help management programs.
