  • A new method for HRV(heart rate variability} detection from the R-wave signal, based on the IPFM (integral pulse frequency modulation) model and its similarity to pulse position modulation, is presented. The proposed method exert lowpass filtering with a Kaiser window. In this paper, The proposed method presents a powerful, but simple, tool for investigation of HRV. It also guarantees real-time behavior, simplicity in design, and phase linearity. Even without the basic assumption of IPFM model, the new algorithm can still be used on-line and with higher performance. It is thoroughly proved that lowpass filtering is an ideal method for PSD (Power Spectrum Density) analysis of HRV.

무월경 환자의 DITI와 HRV, 체성분 분석을 통한 특성 연구 (A Study on Characters of DITI, HRV and Body Composition Analysis in Amenorrhea)

  • Purpose : The purpose of this study is investigate the characters of DITI, HRV and Body composition analysis in Amenorrhea. Methods : We studied 47 patients visiting OO hospital from 1st October 2007 to 19st September 2009. The subjects were categorized in two groups, amenorrhea group(26) and normal group(21). We studied the difference of DITI, HRV and Body composition analysis between two groups by Mann-whitney test using SPSS for windows(version 12.0). Results : there is not statistically significant difference of DITI, HRV and BMI between amenorrhea group and normal group. But there is statistically significant difference of percent body fat and waist-hip ratio between amenorrhea group and normal group. Conclusion : Percent body fat and Waist-Hip ratio of amenorrhea groups is lower than normal groups. Percent body fat and Waist-Hip ratio can be diagnosis index.

  • 신경심장학의 발전으로 중추신경 활성이 심장의 전기적 활동도에 미치는 영향이 밝혀지고 있고, 심박동수변이(Heart rate variability, HRV)가 정신적 스트레스와 심혈관질환의 기계적 연관성을 나타내는 중요한 지표로 제기되고 있다. HRV란 자율신경계의 정상적 상호작용에 의해 나타나는 생리적인 심박수 변동을 타나내며, 자율신경 기능을 정량화하여 측정할 수 있는 방법론이다. 분석 방법은 시간 영역 분석과 주파수 영역 분석으로 나눌 수 있으며, 측정 시간대에 따라 단기간 측정과 24시간 측정 등의 방법이 확립되어 있다. 임상적으로는 변이 정도, 교감신경과 부교감신경의 활성과 균형도를 파악하여 다양한 질환과 스트레스 등의 진단과 치료 및 예후 판정에 활용되고 있다. HRV를 이용한 바이오피드백도 최근 자율신경 관련 질환의 치료에 활용되고 있다. 향후 다양한 프로토콜의 개발과 임상적 적용이 필요할 것으로 보인다.

  • Objectives : We performed this study to analyze correlation among hair tissue minaral ratio, autonomic function and obesity. Methods : Subjects were gathered from January 2005 to March 2007. This study was carried out on 263 subjects who had visited Garosero oriental clinic and had no previous cardiovascular disease and thyroid disease. Heart rate variability (HRV) parameters, tissue mineral ratio and obesity degree were statistically compared with correlation and T-test analysis. Results : The results of this study are summarized as follows : 1. Normal group were predominant over obesity group in HRV parameters(SDNN, RMSSD, VLF, LF, HF). 2. Ca/P, Ca/K, Na/K and Fe/Cu, Na/Mg ratio in hair tissue mineral ratio have correlation with BMR, BMI and waist circumference. 3. Ca/P ratio has correlation with LF norm in HRV, and Na/K with HF, Na/Mg with LF, equally. Conclusions : Taken together these results may suggest that there are significant relationships between hair tissue mineral analysis and HRV.

심박변동 신호의 시-주파수 분석을 이용한 스트레스 분석에 관한 연구 (The Analysis of Mental Stress using Time-Frequency Analysis of Heart Rate Variability Signal)

  • FFT 나 AR 방법에 기초해서 파워스펙트럼을 구하는 방법은 생체신호와 같이 시간에 따라 그 특성이 빠르게 변화하는 신호에는 적합하지 않다. 반면, 시-주파수 방법은 시간에 따라 특성이 변하는 신호를 분석하기에 유용한 장점을 지니고 있다. 본 논문에서는 시-주파수 분석방법을 이용하여 시-주파수 영역에서 심박변동신호를 분석하는 방법을 연구하였다. 여러 종류의 시-주파수 커널 함수들 중에서 심박변동 신호에 적합한 방법을 찾기 위해서 모의 HRV 신호를 이용하였다. HRV 모의신호를 이용하여 커널 함수들의 시간-주파수 분해 능력과 교차성분제거 능력에 대한 평가를 하였고 시-주파수 영역에서 심박변동신호의 주파수 성분들을 추출해 내는 방법들을 제시하였다. 17명의 피험자들을 대상으로 동전쌓기 실험을 3분씩 시행하여 심전도 실험을 측정한 다음 이를 심박변동 신호로 재구성하여 분석하였다. 실험결과 피험자들에게 가해진 정신적 스트레스는 교감신경을 활성화 시켜 LF/HF 증가의 원인이 되는 것으로 나타났다. 실험수행능력에 따라 두 그룹으로 나누어 관찰하였는데 스트레스를 많이 받는 그룹이 그렇지 않은 경우에 비해 작업능력이 떨어짐을 알 수 있었다.

리턴맵을 이용한 자율신경계 영향 평가 (Estimation of the effect on the autonomic nervous system using the return-map)

  • 본 연구에서는 마취 중 리턴맵 분석 방법을 적용하여 ECG 신호에서 R-R 간격의 변화로 표시될 수 있는 HRV 신호를 분석하였다. HRV신호는 자율신경계(autonomic nervous system : ANS)의 상태변이에 따른 심혈관계(cardio vascular system : CVS)의 변화 양상에 대한 객관적인 정보를 구할 수 있으므로 수술중 자율신경계의 변화를 관찰하여 마취심도를 평가할 수 있다. 리턴맵 분석 방법은 일련의 시계열 HRV 신호를 위상공간으로 사상하기 위해 지연시간과 매립차원을 구한 후 2차원의 위상공간에 신호를 재구성하였다. 위상공간에 재구성된 신호 분포를 타원형으로 근사화 한 후 장축과 단축의 길이를 구하여 마취심도를 구별하는데 이용하였다. 마취 단계별 마취심도를 평가하기 위하여 마취 단계를 7단계로 구분하여 분석하였다. 외부자극이 아주 강한 마취유도단계에서 장축과 단축 모두 통계적으로 유의하게 큰값을 나타내었으며, 외부 자극이 가해지지 않은 수술중 단계에서는 장축과 단축의 길이 모두 작은 값을 나타내었다. 따라서 2차원의 위상공간에 매립된 수술중의 HRV 신호를 이용하여 자율신경계의 영향을 판단하여 마취심도를 구분 할 수 있었다.

구강작열감증후군 환자의 임상 특징에 따른 심박변이도 분석 (Heart Rate Variability Analysis According to Clinical Characteristics in Patients with Burning Mouth Syndrome)

  • Objectives: This study aimed to identify the characteristics of the heart rate variability (HRV) index in patients with burning mouth syndrome (BMS) and to evaluate the predictive value of HRV for Korean medical treatment responsiveness to tongue pain in BMS. Methods: Among BMS patients who visited the Oral Diseases Clinic of Kyung Hee University Korean Medicine Hospital from January 1, 2018, to May 31, 2021, we reviewed the clinical records of 217 women aged 50 years and over. HRV and unstimulated saliva flow rate (USFR) measurements were carried out, and all patients were asked to indicate their degree of tongue pain using the visual analog scale (VAS). The subjects were classified into subgroups according to the presence or absence of hyposalivation and VAS improvement, and each subgroup was compared using the HRV index. Logistic regression analysis was conducted to confirm whether cause-effect relationships were present between statistically significant values. Results: BMS patients had lower LF and HF than healthy people. Subgroup analysis demonstrated that there were some statistically significant differences in the HRV index according to salivation rate and degree of pain. In addition, as the LF/HF ratio increased, the pain improvement rate decreased after treatment. Conclusion: The autonomic nerve activity of BMS patients was lower than that of healthy people; however, autonomic balance was not impaired. In addition, tongue pain in BMS patients responded more favorably to Korean medical treatment when patients fell within a normal range of the LF/HF ratio, suggesting that autonomic imbalance could be used as one of the predictable factors in clinical practice.

심박변이도를 통한 폐경 전 한국인 비만 여성의 비만 관련 대사체 농도 예측을 위한 회귀분석 (Predicting the Concentration of Obesity-related Metabolites via Heart Rate Variability for Korean Premenopausal Obese Women: Multiple Regression Analysis)

  • Objectives Advanced researches on the relationship between obesity and heart rate variability (HRV), heretofore, focused on characteristics of HRV depending on the state of obesity. However, the previous researches have not quantified predictive power of HRV toward the obesity-related variables, which is rather more meaningful for clinicians who regularly treat obese patients. Hence, we designed a research to investigate whether HRV could predict serum levels of obesity-related metabolites. Methods Ninety obese premenopausal women meeting the inclusion criteria were recruited. The HRV test, blood sampling, and measurement of physical traits were conducted. Multiple regression analysis of the measurement data was carried out, putting obesity-related metabolites (insulin, glucose, triglyceride, hs-CRP, HDL, LDL, total cholesterol) as outcome variables and the others as predictors. To select appropriate predictive variables, the Akaike's Information Criterion (AIC) was applied. Normality and homoskedasticity of residuals for each model were tested to identify if there were any violations of the regression analysis's basic assumption. Logarithm transformation was used for the values of the concentration of metabolites and the HRV. Results The regression model including Total Power (TP) value and BMI had significant predictive power for serum insulin concentration (F(2, 88)=835.7, p<0.001, $R^2=0.95$). The regression coefficient of ln (TP) was -0.1002. However, it was not sure if the HRV could predict concentrations of other metabolites. Conclusions The results suggest that the Total Power (TP) value of the HRV can predict the level of serum insulin. If the BMI could be assumed as being constant, when the TP value is multiplied by n, the predicted change of insulin could be drawn by multiplying $n^{-0.1002}$. The uncertainty of this model can be assumed as approximately 5%.

RSA분석과 자율신경기능을 평가하는 호흡주기 설정에 관한 연구 (A Study on an Optimal Respiration Rate for the ANS Assessment based on RSA Analysis)

  • Heart rate variability(HRV) is the clinical consequence of various influences of the autonomic nervous system(ANS) on heart beat. HRV can estimate the potential physiologic rhythm from the interval between consecutive beats(RR interval or HRV data), but cardiovascular system governed by ANS is in relation to respiration and autonomic regulation. It is known as RSA representing respiration-related HR rhythmic oscillation. Because the mechanism linking the variability of HR to respiration is complex, it has so far been unknown well. In this paper, we tried to evaluate 5-min RR interval segments under control of respiration in order to find out a proper respiration rate that can estimate the ANS function. 10 healthy volunteers were included to evaluate 5-min HRV data under 4 different respiration-controlled environments; 0.03Hz, 0.1Hz, 0.2Hz, and 0.4Hz respiration. HRV data were analyzed both in the frequency and the time domain, with cross-correlation coefficient(cross-coeff.) for HRV and respiration signal. The results showed maximum cross-coeff. of 0.84 at 0.1 Hz and minimum that of 0.16 at 0.4Hz respiration. Cross-coeff was decreased at a faster rate from 0.1Hz respiration. All mean SDNN, RMSSD, and pNN50 of time domain measures were 108.7ms, 71.85ms, and 28.47%, respectively, and LF, HF, and TP of frequency domain measures were $12,722ms^2,\;658.8ms^2$, and $7,836.64ms^2$ at 0.1Hz respiration, respectively. In conclusion, 0.1Hz respiration was observed to be very meaningful from time domain and frequency domain analysis in relation to respiration and autonomic regulation of the heart.
