양하지 소력을 주소로 하는 길랑-바레 증후군 환자에 대한 한방치료 증례보고 1례 (A Clinical Case Study on Guillain-Barre Syndrome Complaining Both Lower Extremity Weakness with Oriental Medical Treatment)

  • This study was performed to report the effectiveness of oriental medical treatment on a patient with Guillain-Barre syndrome complaining both lower extremity weakness. The patient was diagnosed as Guillain-Barre syndrome and treated with acupuncture, electroacupuncture, physical therapy and herbal medicine. We have evaluated the efficacy of oriental medical treatment by measuring changes of motor grade and ambulatory condition according to period of hospitalization. After treatment, motor grade and ambulatory condition were improved. These results suggest that oriental medical treatment was effective on the patient with Guillain-Barre syndrome.

  • Intravenous immunoglobulin (IVIG) is a safe treatment to treat various neurological disorders, but fatal thrombotic events as rare complications have been reported. A 54-year-old woman with Guillain-Barre syndrome complained of dyspnea during IVIG treatment. She was finally diagnosed with pulmonary thromboembolism. To the best of our knowledge, this is the first case of pulmonary thromboembolism associated with IVIG treatment in a Korean patient with Guillain-Barre syndrome.

  • Background: In the present study to investigate the immediate effects of standing position of the trunk stabilization exercise on Guillain-Barre syndrome patient's balance and gait abilities were examined. Design: Case report and conducted as a single-blind. Methods: Standing position of trunk stabilization exercise was conducted for five Guillain-Barre syndrome patient's who met the selection criteria were recruited. The subjects were conducted conservative exercise and trunk stabilization exercise. Conservative exercise was implemented for thirty min, five times for 8 weeks, and trunk stabilization exercise was implemented for 15 min. The participants' balance was measured via force plate (COP), functional reach test (FRT), timed up and go test (TUG), gait was measured using the 10 m walk test, and 6 minute walk test. Results: After training, the change values of the balance and gait ability in the subjects were significantly greater than pre-test. participants showed significant improvements in COP, FRT, TUG, 10 m walk test and 6 minute walk test pre and post the training (p<0.05). Conclusions: standing position of the trunk stabilization exercise was effective on Guillain-Barre syndrome patient's balance (COP, FRT, TUG) and gait (10 m walk test and 6 minute walk test) abilities were examined.

  • Guillain-Barre syndrome(GBS) is one of the common motor unit diseases and defined as acute postinfectious polyneuropathy, It is not known most effective medical intervention for GBS, but generally benefits from an intensive physical therapy program. In this report, hydrotherapy was applied for a patient with GBS to improve muscle power and functional abilities. Two weeks later, the patient's functional abilities, muscle power, and physical endurance were improved without complications. Since the hydrotherapy does not load maximally to the patient with GBS, the patient can exercise himself actively for the maximal tolerance. It is the reason of fast recovery. In summary, the hydrotherapy is important to the patient with GBS and it must apply to these patients more frequently.

  • The writers have treated in hospital a two-year and 4 month boy child with Guillain-Barre syndrome whose chief complaints are flaccid tetraparalysis and hypesthesia for about three months from December 2, 1996. In the early stage, the principles and methods of treatment such as wind expelling and removal of dampness(?風濕) and obstruction in meridians and collaterals(通經活絡) were used and in the late stage the principles and methods of treatment to invigorate the spleen, replenish Ki(健脾益氣), and nourish the liver and kidneys(滋補肝腎) were applied, together with acupuncture, indirect moxibustion and massotheraphy. Since then, the symptom has gradually improved. Six months after onset, the boy child was recovered to a nearly normal condition. Therefore, this case of treatment is reported, together with consideration of literature.

A Case of Visceral Autonomic Neuropathy Complicated by Guillain-Barre Syndrome Accompanied with Cyclic Vomiting Syndrome-like Disorder in a Child

  • We present a case of an 8-year-old boy with visceral autonomic neuropathy complicated by Guillain-Barre syndrome. In this pediatric patient, gastroparesis was the major symptom among the autonomic symptoms. Due to the gastroparesis, there was no progress with the oral diet, and nutrition was therefore supplied through a nasojejunal tube and gastrojejunal tube via Percutaneous endoscopic gastrostomy (PEG). After tube feeding for 9 months, the patient's gastrointestinal symptoms improved and his oral ingestion increased. The pediatric patient was maintained well without gastrointestinal symptoms for 3 months after removal of the PEG, had repeated vomiting episodes which lead to the suspicion of cyclic vomiting syndrome. Then he started treatment with low-dose amitriptyline, which resulted in improvement. Currently, the patient has been maintained well for 6 months without recurrence, and his present growth status is normal.

  • We report a case with squamous cell lung cancer with concomitant Guillain-Barre syndrome (GBS) as a paraneoplastic syndrome. A 67-year-old patient who was previously diagnosed as metastatic squamous cell lung cancer developed mild symmetrical weakness, paresthesia and sensory ataxia. Nerve conduction study showed sensorimotor polyneuropathy. Analysis of cerebrospinal fluid showed high tilter for monospecific anti-GD1b IgG antibody without onconeuronal antibodies. After treatment with intravenous immunoglobulin, the patient's symptoms improved.

  • Guillain-Barre syndrome (GBS) is an auto-immune disease of peripheral nerve system. It occurs mainly after preceding infection such as upper respiratory or gastrointestinal infection and other antecedent events as tetanus vaccinations. However, any case of GBS after tetanus infection has not been reported. Recently, when analyzed the clinical aspects of 13 tetanus patients including ours, 2 GBS occurred after tetanus infection. We report the neurological and electrophysiologic findings of two cases of Guillain-Barre Syndrome after generalized tetanus.

  • Guillain-Barre syndrome (GBS) after trauma and general orthopedic surgery is rare. A 74-year-old woman showed ascending paralysis symmetrically, dysarthria, dysphagia and areflexia on 14 days after minimally invasive endoscopic thermoannuloplasty on L4-5 level. Brain and lumbar magnetic resonance imaging demonstrate no abnormal findings. The electrodiagnostic study showed prolonged distal motor, sensory latencies and F-wave latencies and reduced amplitude of compound muscle action potential in nerves of upper and lower extremities. In the cerebrospinal fluid (CSF) examination, total protein and IgG were increased. We diagnosed Guillain-Barre Syndrome based on clinical features, electrodiagnostic study and CSF examination and the patient improved symptoms after immunoglobulin injection and rehabilitation. Because the occurrence of GBS after minimally invasive procedure has not been reported, we report a case of GBS after minimally invasive procedure with literature review.
