Infection Control in Parenteral Nutrition Preparation and Compounding (정맥영양의 조제와 감염관리)

  • Recently, in Korea, the importance of preparation and use of injectable drugs has been emphasized due to successive fatal accidents caused by injection infections. Parenteral nutrition (PN) has also been identified as a cause of infection. Cases of infection due to PN have been reported not only in Korea, but also abroad, and contamination occurs mainly during the preparation of PN. Because sterile preparation and compounding of injections are very important for infection control and patient safety, this article reviews the major guidelines outlined thus far. The Korea Ministry of Food and Drug Safety in 2006 published guidelines and the KSHP (Korean Society of Health-System Pharmacists) recently issued guidelines for the aseptic preparation of injections. In addition, as US guidelines, the ASHP (American Society of Health-System Pharmacists) guidelines and United States Pharmacopeia (USP) <797> are also reviewed. The recent guidelines published by the KSHP have significance in that they were adopted in accordance with the domestic reality, even though they conform to foreign guidelines, and are expected to be guidelines for hospital pharmacists performing aseptic preparation work. In addition, the Korea Ministry of Health and Welfare is considering appropriate guidelines for the safe management of medications, training staff for infection prevention and strengthening staff capacity. Furthermore, the gradual expansion of aseptic compounding facilities and human resources, as well as the provision of adequate medical costs are also considered. Based on the establishment and standardization of injectable drugs compounding guidelines for Korean hospitals, it is believed that if human resources and facilities are supported and medical charges are improved, it will be possible to expect the safer preparation and use of injections.

  • Currently with the continuous development of ceramic and cementation materials, CAD-CAM(Computer-aided design/Computer-aided manufacture) restorations are becoming increasingly popular in esthetic dentistry. Preparation design is influenced by the selected restorative material, the fabrication method, and the ability to bond the restoration. For long-lasting CAD/CAM inlay/onlay restoration, clinicians should understand the basic knowledge of CAD/CAM restoration's cavity design to obtain the fracture resistance and proper fitting margin. This article gives an overview of preparation guidelines for CAD/CAM inlay/onlay restorations.

  • With the average life expectancy increasing thanks to better standards of living and medical technology, the number of elderly construction workers in construction sites rises every year to surpass the 450 thousand people in the construction industry (24.7% of all employees in the construction industry). Similarly, the percentage of fatal accidents involving elderly workers has stood at 41~46 percent for the past five years; note that this is significantly higher than the other age groups, making safety measures for dealing with this issue a matter of urgency. This study sought to propose appropriate safety guidelines for elderly construction workers aged 50 years and over by examining the changes in their physical and psychological functions and through the subsequent analysis of the current status and causes of fatal accidents involving them. The guidelines targeted ferroconcrete construction where accidents occur quite often; construction was classified into mold construction, reinforcing rod construction, and concrete construction. Mold construction was further classified into preparation, carry-in, processing, assembly, and disassembly, and reinforcing rod construction, into preparation/transport and processing/assembly. Safety guidelines for each process were presented by dividing them into three aspects considering the changes in the physical and psychological functions according to their ages and type of accident causes: work environment improvement, machinery and equipment improvement, and work method improvement.

Tooth preparation and Intraoral scanning for successful CAD/CAM restorations (성공적인 캐드캠 수복을 위한 치아형성과 구강스캔)

  • In recent years, with the introduction of various restorative materials, restorations using CAD/CAM equipment have been increasing in the esthetic dentistry. The critical steps in the fabrication of indirect restorations with CAD/CAM equipment are proper cavity preparation and making accurate impressions. The process of tooth preparation for CAD/CAM restoration should include a mechanical understanding of milling. In addition, during tooth preparation, the clinician should be familiar with additional equipment and techniques for obtaining the convenience. In order to obtain an accurate oral scan, the clinician should understand the limitations of the oral scan and be skilled at techniques for obtaining a successful image when making oral scans. This article focused clinical guidelines for the preparation of CAD/CAM restorations and introduced clinical methods for making successful impression of oral scans in narrow and deep tooth cavity areas.

  • The use of resin-bonded fixed partial dentures described in the early 1980s caused an conservative way to preserve tooth structure in the restorative dental community. The treatment of patients with requires long term analysis of clinical application and basic research. Failure rates of these prosthesis ranged from 3% to 55%. These varieties were orginated by different techniques, materials, tooth preparation methods and diverse clinical situations. This article review was focused on the standard long term results and in vitro studies on bond strength between metal and teeth. From this, many useful clinical guidelines to RBFPD could be adopted to clinical dentistry. For successful results, careful case selection and good clinical skills are needed. And appropriate techniques for each situations should be adopted. Also, RBFPD using new materials like all-ceramics, FRC/Ceromer was introduced.

  • Recently, Korean Food and Drug Administration (KFDA) has been attempting to modernize the review processes for generic drug products. Obviously, the modernization effort will lead to harmonize the guidelines in the process to the internationally acceptable level. In general, the Korean version of abbreviated new drug application (ANDA) consisted of similarly to those in developed countries (viz, specifications for bulk drug/preparation, bioequivalence, and stability); However, there exists a significant gap in the sophistication of the guidelines between the Korean version and those in the other countries.   • Time and temperature, pH and Aw, and microbiological evaluation were made to identify critical control points during various phases in product flow of chicken soup preparation in a university foodservice establishment. The results are summarised as follows: 1) Time and temperature data indicated that the phases of cooling after cooking, post-preparation, and holding ingredients at room temperature before assembly were critical. 2) pH and Aw values were in favorable for microbial growth. 3) Microbiological data indicated that the phases of basic ingredients. post-preparation and holding ingredients before assembly were critical. 4) Critical control points identified were; basic ingredients, cooling after cooking, post-preparation, holding before assembly and service, and assembly and service. 5) Several guidelines were suggested for the effective qualitly control program.

  • The community of women into society in various fields is expanding, and married late, and accordingly, the first child birth is increasing age. Accordingly, dystocia has also increased. Listed in the literature that the traditional method of birth seemed to oriental medical interpretation of one of the ancient wisdom of the predecessors to explore. The result of this study is summarized as follows. 1. Child birth custom divide into childbirth preparation, childbirth and postpartum care. Postpartum care divide into preparation of the birthplace, food for pregnant woman and birth preparation of the necessary things. 2. Im-san-ye-zi-bub(臨产豫智法) cosist of birth place, abdominal pain Precautions, food, care after childbirth, remove the plecenta posion, method of cut navel, bath, protection. Through various court records of the situation, according to the guidelines are properly applied based on dong-ui-bo-gam(東醫寶鑑).

Disaster Preparation of Visiting Nurses in Public Health Centers (보건소 방문간호사의 재난대비 및 재난간호핵심능력 영향요인)

  • Purpose: The purpose of this study was to identify the factors influencing personal disaster preparation and disaster nursing core competency among visiting nurses in public health centers. Methods: A descriptive survey study was adopted. A convenience sample was taken from 277 subjects in three regions. Data were analyzed by descriptive statistics, t-test, ANOVA, correlation, Pearson correlation coefficient and multiple regression. Results: The mean personal disaster preparation and disaster nursing core competency scores were 11.13 and 76.87, respectively. Personal disaster preparation was statistically significant by experience of disaster victims and disaster management guideline. Disaster nursing core competency was statistically significant by participation in future disaster recovery. As a result of multiple regression analysis, personal disaster preparation accounted for 14.9 of the variance by experience of disaster victims and disaster management guidelines; disaster nursing core competency accounted for 8.9 of the variance by perception of disaster nursing. Conclusion: Although visiting nurses are ill-prepared for disaster, global natural and man-made disasters can occur regardless of time and place. Disaster education and training should therefore be included in nurses' fundamental education.

Evaluation of Dietitians' Perception of Importance about HACCP Guidelines in Foodservice Facilities (급식소 HACCP 관리항목에 대한 영양사의 중요성 인지도 평가)

  • The purpose of the study was to evaluate the perception of importance about HACCP guidelines of dietitians. A questionnaire was developed to achieve research objectives and sent to random samples of 500 dietitians by a mail ; 418(84%) responses were analyzed. The respondents used three-point-scale to rate their perception of importance about HACCP from 1-"will be necessary" to 3-"very important". All statistical analyses were conducted using SAS package(version 8.12) for Windows. Means and standard deviations were computed for all variables. One-way analysis of variance and Duncan's multiple range test were used to assess differences in the perception of importance about HACCP guidelines among business and industry, health care and school foodservice managers. The study results were summarized as follows. Dietitians especially had lower perception of importance on the contents of Critical Control Points(CCPs), such as; adequate thawing methods and keeping records(1.95) and correct cleaning and sterilizing for raw vegetables and fruits(1.93). The rates of perception of importance about HACCP guidelines were significantly different from 19 of the 37 contents among business and industry, health care and school foodservice(p<.05 or p<.01 or p<.001). Generally, the item related to purchasing and inspection management had the highest perception level score and the item related to pre-preparation management had the lowest perception level score in foodservice production process(p<.001). Results indicate that there is a need for increased education of dietitians about HACCP principles and appropriate practices.
