공공민간협력사업을 통한 한 민간병원의 결핵치료 성공률 향상 (Increasing the Treatment Success Rate of Tuberculosis in a Private Hospital through Public-Private Mix (PPM) Project)

  • Background: Stopping medical treatment by tuberculosis (TB) patients prior to completing treatment period is a major concern in private hospitals. We evaluated the impact of public-private mix (PPM) project on increasing treatment success rate of TB in a private tertiary hospital in Korea. Methods: Starting in February 2009, TB patients treated at Dankook University Hospital received health education and case monitoring activities by specially trained public health nurses (PPM project). On a retrospective basis, we reviewed medical records and compared the treatment outcome of TB patients treated under the PPM project (PPM group) to patients treated without the PPM project (control group) between January 2008 and June 2010. In addition, we also evaluated the risk factors treatment non-completion. Results: The number of patients in the PPM group and control group were 123 (85 pulmonary TB and 38 extrapulmonary TB) and 146 (101 pulmonary TB and 45 extra-pulmonary TB), respectively. The PPM group had demographic and clinical findings comparable to those of control group. The PPM group showed a significantly higher treatment success rate (93.5%) compared to the control group (77.9%). However, development of complications, proximity to the hospital, and presence of co-morbid disease did not influence the treatment success rate. Conclusion: The PPM project was effective at increasing the TB treatment success rate. An effort to improve and to expand the PPM project is needed in private Korean hospitals.

마루운동 제자리 뒤공중돌기 동작의 운동학적 분석 (Kinematical Analysis of the Back Somersault in Floor Exercise)

  • This study was to compare the major kinematic factors between the success and failure group on performing the back somersault motion in floor exercise. Three gymnasts(height : $167.3{\pm}2.88cm$, age : $22.0{\pm}1.0years$, body weight : $64.4{\pm}2.3kg$) were participated in this study. The kinematic data was recorded at 60Hz with four digital video camera. Two successful motions and failure motions for each subject were selected for three dimensional analysis. 1. Success Trail It was appear that success trail was larger than failure group in projection velocity, but success trail was smaller than failure trail in projection angle. Also it was appear that success trail was longer than failure group in the time required. Hand segment velocity and maximum velocity in success trail were larger than those in failure trail, and this result was increasing the projection velocity and finally increasing the vertical height of center of mass. At the take-off(event 2), flection amount of hip and knee joint angle was contributed to the optimal condition for the take-off and at the peak point, hip and knee joint angle was maximum flexed for reducing the moment of inertia. Also in this point, upper extremities of success trail extended more than those of failure trail. in this base, success trail in upward phase(p3) 2. Failure Trail It was appear that failure trail was smaller than success trail in projection velocity, but failure trail was larger than success trail in projection angle. Also it was appear that failure trail was more short than success trail in the time required. Hand segment velocity and maximum velocity in failure trail were smaller than those in success trail, and this result was reducing the projection velocity and finally reducing the vertical high of center of mass. 패널자료를 활용한 종단적 금연 성공요인 분석 (Longitudinal Analysis on Success Factors of Smoking Cessation Using Panel Data)

  • Objectives: This study has longitudinally analyzed male smokers in order to find out the trend rate of re-smoking after smoking cessation. Methods: The study was conducted with 800 male smokers who were provided with smoking cessation services for 3 years at public health centers from July 16, 2005 to July 15, 2008. Results: The results can be summarized as follows: First, the success rate of smoking cessation for at least 6 months during 3-year period was 54.4%. The failure rate of smoking cessation at the second year was 47.5% and the third 27.2%, which indicates that the failure rate diminishes as the period of smoking cessation extends. Second, the success rate of smoking cessation at the first trial was 35.0%, the second 24.3%, and the third 16.4%. The success rate diminished as the number of trial increased. Third, the continuation rate of smoking cessation for a year was 18.3%, for 2 years 13.4%. For the success group, the continuation rate of smoking cessation for a year was 52.5%, for 2 years 38.2%. For the failure group, the rate for a year was 21.5%. Fourth, in this longitudinal analysis, the most crucial variables that affect the success rate of smoking cessation are total number of consultation and the past experience of successful smoking cessation. Conclusion: This study shows that success of smoking cessation and the continuance of smoking cessation are both difficult, and that the past experience of smoking cessation plays an important role determining the present success of smoking cessation and continuance of smoking cessation. Thus, it is necessary to divide people into success group and failure group based on the results of the past experience of smoking cessation when they consult at smoking cessation clinics at public health centers. Further, in order to increase the continuation rate of smoking cessation, this study suggests that we need to consider ways to take care of those who successfully abstained from smoking for 6 months.

당뇨병성 족부 궤양에 의한 절단술후 상처 치유와 발목-상완 지수, 족지-상완 지수, 족지압의 관계 (Prognostic Factors of Wound Healing after Diabetic Foot Amputation; ABI, TBI, and Toe Pressure)

  • Purpose: The purpose of this study is to establish guidelines for ankle-brachial index (ABI), toe-brachial index (TBI) and toe pressure with regard to healing of diabetic foot amputation wound. Material and Methods: We designed a retrospective study that included patients with diabetic foot ulcer. From 2008 to 2011, 46 patients who had suffered from amputation of a foot due to diabetic foot ulcer were included in this study. We divided them into amputation-success group and amputation-revision group, and compared their ankle-brachial index (ABI), toe-brachial index (TBI) and toe pressure between two groups. Amputation-revision group is that first forefoot amputation is failed to heal successfully and need to have another proximal amputation. Results: Toe pressure was 78 mmHg (54~107) in the amputation success group, 0 mmHg (0~43) in the amputation revision group (p=0.000). Ankle-brachial index was 1.1650(1.0475~1.1975) in the amputation success group, 0.92(0.5275~1.0750) in the amputation revision group (p=0.05), and toe-brachial index was 0.6100(0.4050~0.7575) in the amputation success group, 0.00(0.00~0.4150) in the amputation revision group (p=0.04), respectively. Conclusion: ABI, TBI, toe pressure of amputation success group were significantly higher than those of amputation revision group.

Influencing factor on the prognosis of arthrocentesis

  • Objectives: The purpose of this article is to evaluate factors influencing prognosis of arthrocentesis in patients with temporomandibular joint (TMJ) disorder. Materials and Methods: The subjects included 145 patients treated with arthrocentesis at the Dental Center of Ajou University Hospital from 2011 to 2013 for the purpose of recovering mouth opening limitation (MOL) and pain relief. Prognosis of arthrocentesis was evaluated 1 month after the operation. Improvement on MOL was defined as an increase from below 30 mm (MOL ${\leq}30mm$) to above 40 mm (MOL ${\geq}40mm$), and pain relief was defined as when a group with TMJ pain with a visual analog scale (VAS) score of 4 or more (VAS ${\geq}4$) decreased to a score of 3 or more. The success of arthrocentesis was determined when either mouth opening improved or pain relief was fulfilled. To determine the factors influencing the success of arthrocentesis, the patients were classified by age, gender, diagnosis group (the anterior disc displacement without reduction group, the anterior disc displacement with reduction group, or other TMJ disorders group), time of onset and oral habits (clenching, bruxism) to investigate the correlations between these factors and prognosis. Results: One hundred twenty out of 145 patients who underwent arthrocentesis (83.4%) were found to be successful. Among the influencing factors mentioned above, age, diagnosis and time of onset had no statistically significant correlation with the success of arthrocentesis. However, a group of patients in their fifties showed a lower success rate (ANOVA P=0.053) and the success rate of the group with oral habits was 71% (Pearson's chi-square test P=0.035). Conclusion: From this study, we find that factors influencing the success of arthrocentesis include age and oral habits. We also conclude that arthrocentesis is effective in treating mouth opening symptoms and for pain relief.

의류 브랜드의 성공 요인에 관한 연구 (A Study on Success Factors of Apparel Brand)

  • This study is to examine the factors affecting success of business withhigh accomplishments in customer recognition and financial aspect in the fashion industry. In addition, through survey on consumers and interviews on industry officials who concerned fashion brand, perceived success factors were compared between consumer and industry groups. This study selected a total of 20 brands, which were 5 brands per 4 categories(e.g., women, men, sports and casual wear), with high customer satisfaction and customer loyalty in consumer's perspective and high revenues, revenue growth, and profit rate in 2004 in a financial perspective. The survey on consumer group and interviews on officials in industry were conducted simultaneously. One thousand respondents were obtained from survey on consumers and 40 respondents were obtained from interviews on industry officials. Multiple regression analysis and t-test were used for data analysis via the SPSS 12.0 program. The result of this study was as following. From a consumer perspective, respondents recognized that both consumer satisfaction and brand revenues were positively related to brand factors of consumer, product and marketing. From an industry's perspective, consumer, product and marketing factors affected consumer satisfaction as a brand success factor. In comparison of perception difference in brand success factors between consumer and industry groups, industry group was more likely to concern about the importance of brand success than consumers. In addition, the consumer group perceived the most highly consumer factor as a brand success factor, followed by product and marketing and external environment factors, while industry group did in order of consumer, marketing, product, and external environment factors, which indicated significant difference in perceptions of the two groups. Through this study on consumer satisfaction to improve positive and amicable buying behavior and comparative analysis on difference of perception of consumers and industry on factorsfor financial ability and revenue increase, the foundation for strategy establishment of brand distinction in fashion industry can be provided.

  • A total of 185 vasovasostomies were carried out for 11 years. Various factors which are of importance in fluencing the successful vasovasostomies are analysed and presented as follows: In recent years, demand of reanastomosis of the vas deferens increased considerably. An average age of the subjects is 39, and that of their wives, 33 in this series. An average interval between vasectomy and vasovasostomy is 4 years. An average number of living chidren is 2.6 at the time of vasectomy. and 2.1 at the time of time of vasovasostomy. Merchant and public official are the most commonly encounterd occupation in the present series. Coital frequencies are 2.2/week after vasovasostomy. The most common reasons for requesting the vasovasostomy are remarriage and deaths of children, especially son. Success rate is considerably higher among younger group than that of the older. Success rate is somewhat higher among groups of shorter interval between the operations (vasectomy and vasovasostomy) than that of longer interval group. Success rate is higher among bilateral vasovasostomy group than that of any others. Success rate is higher in solid splint group and no splint group than hollow splint group. The closed dressing technique of the end of splint on the scrotum is found to be reasonably effective in preventing infection. Ordinary end-to-end anastomosis and folded side-to-side anastomosis techniques are proved to be the most ideal form of modified operation for the successful vasovasostomies. The most common causes of failure are infection, injuries of blood supply, avascular necrosis due to extensive mobilization, inadequate approximation of both vasal end, hematoma, changes of epididymal environment, and early ambulation in this series. Overall success rate of the author's series is found the 81 per cent, and impregnation rate is 35 per cent. Overall results reported by other workers are that success rate regarding appearance of sperm in the semen after operation is 64 per cent, and that of impregnation after vasovasostomy is 33 per cent. The results of semen assessment performed on same cases prior to vasectomy and following vasovasostomy showed that post-vasovasostomy sperm count scarcely reached three-quaters of pre-vasectomy count.

악안면 외상 후 식립된 임플란트의 예후에 관한 연구 (THE STUDY ON THE PROGNOSIS OF DENTAL IMPLANTS WHICH HAS BEEN INSTALLED AFTER MAXILLOFACIAL TRAUMA)

  • Purpose The purpose of this study was to evaluate the success rate of dental implant on post-traumatic region to rehabilitate missing teeth. Patients and Methods 5 patients who had been treated due to maxillofacial trauma during 2000 to 2004 at samsung medical center were selected for this study. 22 dental implants were installed to them for restoring missing teeth. The success rate of these dental implants was evaluated with criteria by T. Albrektsson. We divided installed 22 implants into 2 categories. One category consisted rough surface implants group and smooth surface implants group. And the other category consisted more healing group and less healing group. The healing time was calculated from point of trauma. Results The success rate of dental implants is 68.2% that is lower than other studies because of poor vascularity of bone bed and mechanical stress according to trauma. Rough surface implants group and More healing group showed superior success rate to others. (P < 0.05). Conclusion We found that to increase success rate, it may need rough surface implants and longer healing period.

Success and Failure Factors of Technology Commercialization: A Korean Case

  • We wanted to show the different group dynamics of factors for success and failure cases for technology commercialization in small technology-based firms. Existing studies are based on product level, project level, division level or firm level. We deal with technology level, and at small-technology-based firms. This is a longitudinal case study based on 8 cases from Korea. Our study on technology level is a first trial in success and failure studies unlike all existing studies. As a first step, we introduced new categories and factors such as technology attributes and CEO reflecting data, and especially a new concept of launch readiness level. Finally, we adopted correspondence analysis to show the group dynamics. The results are as follows; Technology factors are the most important factors. Second, resource-based factors are more critical in failure cases than success cases and technology factors are more critical to success.

Clinical and radiographical evaluation of mineral trioxide aggregate, biodentine and propolis as pulpotomy medicaments in primary teeth

  • Objectives: The purpose of this study was to evaluate the efficacy of mineral trioxide aggregate (MTA), Biodentine and Propolis as pulpotomy medicaments in primary dentition, both clinically and radiographically. Materials and Methods: A total of 75 healthy 3 to 10 yr old children each having at least one carious primary molar tooth were selected. Random assignment of the pulpotomy medicaments was done as follows: Group I, MTA; Group II, Biodentine; Group III, Propolis. All the pulpotomized teeth were evaluated at 3, 6, and 9 mon clinically and radiographically, based on the scoring criteria system. Results: The clinical success rates were found to be similar among the three groups at 3 and 6 mon where as a significant decrease in success rate was observed in Group III (84%) compared to both Group I (100%) and Group II (100%) at 9 mon. Radiographic success rates over a period of 9 mon in Groups I, II, and III were 92, 80, and 72%, respectively. Conclusions: Teeth treated with MTA and Biodentine showed more favorable clinical and radiographic success as compared to Propolis at 9 mon follow-up.
