The Effects of the Short Message Service Informing Patients' Operation Status on the Family Members' Nursing Satisfaction (SMS를 활용한 수술진행 정보제공이 간호요구 만족도에 미치는 효과)

  • Purpose: The purpose of this study was to investigate the effect of the SMS(Short Message Service) informing patients' surgical operation status to his or her family members on their nursing satisfaction. Methods: A non-equivalent control group non-synchronized design was used. The subjects were the family members of patients underwent spine surgery operation at a general hospital in B city. Twenty five subjects in the experimental group received SMS services whereas the other twenty five subjects in the control group received no intervention. Results: There was a significant difference in the level of nursing satisfaction in the experimental group compared to that of the control group(t=7.14, p=.001). Conclusion: The result above indicates that the level of nursing satisfaction of family members of the patients under surgical operation can be raised by providing SMS informing surgical operational status during the operation. This finding leads to the conclusion that such information service can be used as one of the efficient interventions for improving patient and family satisfactions with nursing care in a surgical unit.

The effect of flap operation and metronidazole gel combined therapy on the treatment of the juvenile periodontitis (치은박리소파술시 metronidazole gel 병용이 유년성치주염 치료에 미치는 영향)

  • The aim of the study was to investigate the effect of surgical therapy combined with the usage of metronidazole gel in the treatment of juvenile periodontitis by comparing clinical indices of flap operation along with application of metronidazole gel and flap operation only. Comparing clinical indices of the baseline, 3 months after surgery, 6 months after surgery statistically, the results are as follows; 1. Bleeding on probing (BOP) was decreased significantly both in the experimental group and the control group at 3 months after surgery (p<0.05). BOP increased 0.9% in the control group 6 months after surgery, while in the experimental group, decreased 4.7% (p>0.05). However, there was no statistically significant difference in these groups (p>0.05). 2. Pocket probing depth (PPD) was decreased significantly both in the experimental group and the control group at 3 months after surgery (p<0.05). PPD increased 0.lmm in the control group 6 months after surgery, while in the experimental group, no increase of the depth could be observed (p>0.05). However, there was no statistically significant and difference in these groups (p>0.05). 3. Loss of attachment level(LOA) was decreased significantly both in the experimental group and the control group at 3 months after surgery (p<0.05). LOA increased 0.2mm in the control group 6 months after surgery, while in the experimental group, increased 0.3mm However, there was no statistically significant difference in these groups (p>0.05). In conclusion, flap operation was effective on the treatment of juvenile periodontitis. However, combined therapy of metronidazole gel could not give rise to any significant adjunctive effect on the treatment outcome.

The Effectiveness of Meridian Acupressure Intervention Using Sticker Needles to Bowel Movement on Post Spinal Operative Patients (스티커 침을 이용한 경혈지압이 척추수술 후 배변에 미치는 영향)

  • This study was conducted to identify the effect of meridian acupressure on defecation of patients with post spinal operation. The nonequivalent control group posttest only design was used. The data were obtained from 77 post spinal operative patients, 34 in the experimental 43 in control group in Y Hospital, Seoul, Korea. The neurosurgical unit A and B ward, assigned by matched sample by the name of operation such as laminectomy and posterior lumbar interbody fusion are performed. Meridian acupressure meant the method that an examiner presses response points distributed in the pass of energy vessel. In this study, meridian acupressure program was performed on as points in order of Hegue (LI-4), Zhigou (TE-6), Zusanli (S-36), Shangjuxu (S-37), Xiajuxu (S-39), Tianshu (S-25), Taichong (L-3) which was known to be related to large intestine. Data were collected from 1, July 2003 to 1, September 2003. Meridian acupressure program was carried out for 20 minutes 4 hours after operation twice daily. In order to evaluate the effect of meridian acupressure intervention, they were asked time of bowel recovery, gas passing, and defecation though questionnaire method. Data were analyzed by the SPSS/ WIN 11.0 program. The results of this study were summarized as follows; 1. Homogeneity tests of general characteristics and operation related characteristics of the experimental group and the control group were performed. General characteristics included age, sex, defecation habit, eating pattern, fluid intake, life style, activity, usage of laxative and etc. 2. Hypotheses were verification as follows; 1) Recovery of bowel sound of the experimental group who received meridian acupressure intervention was faster than that of the control group after spinal operation (t=-6.770,P=.000). 2) Time of gas passing of the experimental group who received meridian acupressure program was faster than that of the control group after spinal operation (t=-8.003, P=.000). 3) Time to defecation of the experimental group who received meridian acupressure program was faster than that of the control group after spinal operation (t=-9.026, P=.000). 4) Abdominal discomfort due to defecation of the experimental group who received meridian acupressure program was lesser than that of the control group after spinal operation (t=-3.431, P=.001). From these results, meridian acupressure intervention was effective for recovery of bowel sound, reduce time to gas passing, time to defecation and lessen abdominal discomfort due to defecation on post spinal operative patients. And therefore this intervention can probably considered on clinical practice.

A Study of ADL Performance on Elderly Total Knee Replacement Patients (슬관절전치환술 환자의 일상생활동작 수행력에 관한 연구)

  • The purpose of this study was to examine the influence of total knee replacement to arthritis patients in pain intensity and functional impairment. For this study, over 50-year-old 30 patients who had osteoarthritis and 69 patients who got total knee replacement at the one of the departments of orthopedics in Daegu were interviewed from June, 2002 to March, 2003. The results of this study were as follows : First, pain intensity was decreased to experimental group after operation than control group(P<.01) And the presence of crepitation was also decreased to them(P<.05). On the other hand there's no significant difference was noted in the presence of stiffness, degree of flexion contracture and extension contracture between two groups. Second, over 91 days group after operation and visitor's group of physiotherapy unit were better than others in Level of knee function(P<.01). Third, over 91 days group after operation and visitor's group of physiotherapy unit were higher than others in Barthel Index of knee function(P<.01). Forth, over 91 days group after operation and visitor's group of physiotherapy unit were higher than others in Level of IADL(P<.05). Fifth, over 91 days group after operation and visitor‘s group of physiotherapy unit were higher than others in Katz Index of knee function(P<.01). Sixth, although Old Ages' Activity Index seemed to get better as time goes by, there's no statistical difference. Seventh, over 91 days group after operation and visitor's group of physiotherapy unit were higher than admission group and under 30 days group after operation in Social Activity Index(P<.05). Eighth, the function of the knee was connected with the abilities in the activity of the old and the social skill. The Barthel index was connected with the function of the knee joint and the activities of the old and Katz index. IADL function was connected with the activities of the old and the social activity. The Katz index was connected with the Barthel index. The activity of the old was connected with the Barthel index, the functional score of the knee joint andthe IADL score. The ability of social skill was connected with the activity of the old, IADL score and the function of the knee.

  • This study was conducted to evaluate the healing potential of hydroxyapatite and demineralized freeze dried bone in 5 dogs. Chronic periodontitis was induced by ligating elastic wire randomized as follows. The group in which only flap operation was performed was used as control. The group in which flap operation using nonresorbable nonporous hydroxyapatite (Orthomatrix)was performed was used as experimental I. The group which flap operation using resorbable porous hydroxyapatite (Biocoral) was performed as experimental II. The group in which flap operation using demineralized freeze-dried bone was performed was used as experimental III. Thereafter dogs serially sacrificed at the 1,2,4 and 8 weeks and the specimens were prepared, and stained with Hematoxilin-Eosin stain for the light microscopic evaluation. The results of the this study were as follows : 1. Control group : progressive inflammatory cell infiltration till 4 weeks and epithelial undergrowth. 2. Group I. : epithelial undergrowth and new bone formed with fibrous margin around HA granule. 3. Group II. : no epithelial undergrowth and direct bone formation at the porous granule 4. Group III. : could not see epithelial undergrowth but obviously new cementum formation.

Clinical Evaluation of Tooth Mobility Following Root Planing and Flap Operation (치근활택술과 치은박리소파술 후 치아동요도 변화에 관한 연구)

  • Tooth mobility may be the decisive factor that determines whether dental treatment of any kind is undertaken. Although tooth mobility in isolation says little in itself, the finding of increased tooth mobility is of both diagnostic and prognostic importance. Only the detection of an increase or decrease in mobility makes an evaluation possible. Thus prior to treatment, we must understand the pathologic process causing the observed the tooth mobility and decide whether the pattern and degree of observed tooth mobility is reversible or irreversible. And then it must be decided whether retention and treatment or extraction and replacement. The purpose of this study was to compare tooth mobility at different time period during root planing and flap operation and to relate changes in mobility to each treatment method. Twenty-one patients (287 teeth) with chronic adult periodontitis were treated with root planing(control group) and flap operation(experimental group), and each group was divided 3 subgroups based upon initial probing pocket depth (1-3mm, 4-6mm, 7mm and more). Tooth mobility was measured with $Periotest^{(R)}$ at the day of operation, 4 days, 1 week, 2 weeks, 3 weeks, 4 weeks, 8 weeks, 12 weeks after each treatment. Tooth mobility, attachment loss, radiographic bone loss, and bleeding on probing were measured at the day of operation, 4 weeks, 8 weeks and 12 weeks after treatment. 1. In group initial probing depth was 1-3mm, tooth mobility had no significant difference after root planing and flap operation. 2 . In group initial probing depth was 4-6mm, 7mm and more, tooth mobility had decreased in 12 weeks after root planing(p<0.01). And the mobility had increased after flap operation(p<0.01) and was at peak in 1 week, and decreased at initial level in 4 weeks, below the initial level in 12 weeks(p<0.01). 3. In 1 week, significant difference in tooth mobility between control and experimental group was found(p<0.01) but, in 12 weeks no difference between two groups was found. 4. Change of immediate tooth mobility after treatment was more larger in deep pocket than in shallow one. In group with the same probing pocket depth, the change of tooth mobility in molar group was greater than that of premolar group. 5. Tooth mobility before treatment was more strongly correlated with radiographic bone loss (r=0.5325) than probing depth, attachment loss and bleeding on probing, in 12 weeks after treatment, was more strongly correlated with attachment loss($r^2$=0.4761) than probing depth and bleeding on probing. Evaluation of the treatment effect and the prognosis after root planing and flap operation were meaningful on tooth initial probing depth 4mm and more. After flap operation, evaluation of the prognosis should be performed at least in 4 weeks and in 12 weeks after treatment, no difference in tooth mobility between two groups was observed. Radiographic bone loss and attachment loss were good clinical indicators to evaluate tooth mobility.

A Clinical Study on Changes in the Concentration of Urine Potassium During and after Open Heart Surgery (개심술환자의 뇨중 $K^+$ 배설에 관한 임상적 연구)

  • A Clinical study was carried out to observe changes in concentration of serum and urinary potassium during and after open-heart surgery under extracorporeal circulation with Heart-Lung-Machine. The patients studied in the present work included eight cases of congenital heart anomalies and seven cases of acquired valvular diseases of the heart. Blood and urine samples were collected at intervals of 30 minutes during open-heart surgery and at intervals of 8 hours until 48 hours after the operation to determine concentration of serum and urinary potassium. 1.While the concentration of urinary potassium was not changed during the operation, the concentration of urinary potassium was rapidly increased after the operation. Eight hours after the operation, the concentration of urinary potassium was increased to the highest value of 86.97±43.96 mEq/L and was gradually decreased thereafter. The concentration of urinary potassium, however, still remained elevated even 48 hours after the operation, the concentration being greater than preoperative state by 19.90 mEq/L. 2.The concentration of urinary potassium after the operation was higher in the group who went through 90 minutes of extracorporeal circulation [$108.71{\pm}94.71mEq/L$] than in the group who went through 60 minutes of extracorporeal circulation [$86.58{\pm}50.87mEq/L$]. The Increased level of urinary potas-sium observed In the group who went through 90 minutes of extracorporeal circulation continued for longer period of time as compared with that in the group who went through 60 minutes of extracorporeal circulation. 3.The increased level of urinary potassium was observed in the both groups of patients with or without diuretics plus digitalis therapy. However, the increased level of urinary potassium appeared to con-tinue for longer period of time in the group of patients receiving diuretics plus digitalis therapy prior to the operation than in the group of patients receiving no diuretics plus digitalis therapy.

  • Purpose: The purpose of this study was to identify the effects of a pre-operation video assisted education program on residual urine, gas passing and state anxiety in women undergoing hysterectomy. Methods: Nonequivalent control group non-synchronized design was used for the study. In the research, video assisted education program was applied to the experimental group while a similar conventional education was done to the control group. The pre-operation state anxiety and post-operation residual urine and gas discharge of both the groups were measured. The data were analyzed using SPSS. Results: The experimental group was significantly higher than control group on gas passing (t=3.04, p=.00). However the residual urine (t=0.34, p=.73) and state anxiety (t=0.81, p=.82) did not make significant differences. Conclusion: This study is very meaningful in that it developed and provided a nursing intervention can positively affect hysterectomy patients. The pre-operation video assisted education program may be an effective nursing intervention that is clinically practical and useful to reduce time of the gas passing of hysterectomy patients after the operation.

  • Generally, an electrical distribution configuration is a radial system with one-way current in a distribution management system (DMS). All feeders in a DMS have tie-switches to make radial system. Sometimes, DMS should change a tie-switch for operation. In that case, the tie-switch has to be closed first; then a switch is opened as another tie-switch in order to prevent blackout for customers. At the moment when the tie-switch is closed, distribution system is operated in a loop state, not radial. Before the loop operation, DMS operator has to check any expected events for stable distribution system operation; and the most important event is a mis-operation of a protection relay. In addition, DMS operator should check voltage profile violation but a calculation method of section voltages had not been used. Thus, this paper proposes a calculation method of section voltages at a loop operation in a DMS. The proposed calculation algorithm is verified by Matlap Simulink.
