A Study on the Degree of Burden and Quality of Life in Family Caregivers of Patients with Stroke (뇌졸중 환자가족이 인지한 부담감 및 삶의 질에 관한 연구)

  • This study was designed and undertaken to identify the degree of burden and quality of life in family caregivers of patients with stroke and to determine whether burden was directly related to quality of life. The data were collected from October 3rd, 1997 to March 20th, 1998. The subjects in this study were 126 caregivers as a family member and 126 patients with stroke who were hospitalized in two oriental medicine hospitals, three general hospitals located in Taegue City and Pusan City. The questionnaires consisted of questions regarding burden(25 items) and quality of life (18 items) of caregivers. Data were analyzed using percentages, mean, 1-test, ANOVA and Pearson-correlation coefficients done with the SPSS program. The results of thi study are as follows : 1. The score for family caregiver's burden was higher than the middle score. 2. The score for family caregiver's quality of life was relatively low. 3. The relationship between burden and quality of life was showed a significant inverse correlation. 4. The family caregivers' age had statistically significant differences in the degree of burden. 5. The education and monthly income of caregivers had statistically significant differences in the quality of life. That is, the higher the level of education and the higher the monthly income, the higher the degree of quality of life. 6. The age and sex of patients had affected the qualiry of life of caregivers sigmificantly. That is, caregivers felt more burden when caring for the patient group in sexties than any other age group and female patients than for male patients. 7. In the relationships between quality of life and general characteristics of the stroke patients, only the patients' sex was showed a statistically significant difference. That is, caregivers felt more quality of life when taring for male patients than female patients.

Is Season a Prognostic Factor in Breast Cancer?

  • Background: Some studies have indicated an inverse relationship between cancer risk and sunlight exposure. Others have reported that the prognosis of some cancers such as prostate, colon, ovarian and non melanoma skin cancer, were affected by the season in which the cancer was diagnosed. In our study, we evaluated whether season is prognostic in Turkish patients with breast cancer. Materials and Methods: A total of 517 patients from Kayseri Training and Research Hospital were analysed retrospectively. Patients were divided into 4 groups according to season of cancer diagnosis: winter, spring, summer and autumn. The prognostic factors for disease free survival and overall survival were investigated. Results: No significant differences were found among groups regarding prognostic factors overall. Only estrogen receptor status and lymphovascular invasion were independent prognostic factors (p=0.001 and p=0.001 respectively). We found significantly differences for mean disease free survival among groups (p=0.019). Winter group had better mean DFS while summer group had worse DFS. Mean overall survival was similar in the four groups (p=0.637). Conclusions: The season is not an independent predictive factor. However, due to interaction with other factors, we think that the season of cancer diagnosis is important for cancer prognosis.

A Study on the Distribution of the Elementary Girls' Size Dimensions according to Ages and Body Shapes (학령기 여아 연령별, 체형별 치수분포특성)

  • The purpose of this study was to analyse the body sizes of $7{\sim}12$ years elementary school girls and also to categorize KS size dimensions by the detailed information of ages and body shapes. For the study, the data of SizeKorea(2004) was analysed. Height, bust, waist, hip, the ratio of waist to height and hip to height were significant between age groups, but the ratio of bust to height was not. Therefore, the increase of bust size was resulted in growth of bust circumference, instead of bust volume. In the same height group, over 11 year girls had smaller waist, while over 12 year girls had bigger hip. For Grouping girls by ages and body shapes, the ages were divided into 2 groups, under 10 years old and 11 to 12 years old. The body shapes classified into 3 groups 'Stout-shape', 'Middle-shape', and 'slim-shape', by the ratios of bust to height and hip to height. 'Stout-shape' was significantly big at the almost sizes, but 'Middle-shape' was significantly big at only circumferences, not lengths. In addition, drop(the difference between bust and hip) and lower-drop(the difference between waist and hip) were in inverse proportion to the ratios of bust to height and hip to height. It meant the increases of bust-ratio and hip-ratio of 'Stout-shape' were resulted in overweight, rather than female matureness. The distribution of sizes over 0.5% were grouped for grading system and the subtotal percentiles of each group were calculated for industrial data. The groups which covered more than 10% of consumers were 2 to 6 and the 1 or 2 groups for 'Stout-shape' were also observed, so that children's ready-to wear companies could use them efficiently for their own consumer target.

A Biomechanical Analysis of Lower Extremity Segment dur ing the Fouette en dehors Performed by Ballet Dancers (발레 무용수의 Fouette en dehors동작 시 하지분절에 대한 생체역학적 분석)

  • The purpose of this study was to quantitatively examine the biomechanical variables of Fouette turns for expert and beginner ballet dancers and to determine the difference in the variables between the two groups. sixteen female ballet dancers participated in this study. They were divided into an expert group(age, $25.38{\pm}1.92$ years; height, $168.38{\pm}4.66$ cm; mass, $49.63{\pm}4.41$ kg) and a beginner group(age, $20.88{\pm}1.13$ years; height, $161.63{\pm}7.42$ cm; mass, $48.88{\pm}3.64$ kg) depending on their ballet experience. Descriptive data were expressed as mean ${\pm}$ standard deviation (SD) for all variables including the duration, displacement of the center of the body, velocity of the center of the body, angle of the body segments, angular velocity of the body segments, ground reaction force, lower extremity torque, muscle activity, body weight, age, and body mass. An independence t-test was conducted to determine how the following variables differed between the beginners and experts: duration, displacement of the center of the body, velocity of the center of the body, angle of the body segments, angular velocity of the body segments, ground reaction force, lower extremity torque, and muscle activity. All comparisons were made at the p<0.05 significance level. The results show that the experts scored high on the biomechanical variables, although all the variables were not significant. Significant differences were found in the angle of body segments, angular velocity of the body segments, lower extremity torque, and muscle activity(p<0.05). The findings of this study demonstrate that the experts have the required skill to make an improved Fouette turn. The findings may also help ballet dancers to learn and understand the Fouette turn.

Prevention of Ischemic Damage in Working Rat Hearts by Calcium Channel Blocker and Calmodulin Inhibitors (흰쥐심장의 허혈손상에 대한 Calcium 통로봉쇄제와 Calmodulin 억제제의 예방효과에 대한 연구)

  • This study was investigated under the postulation that activation of intracellular calcium- calmodulin complex during ischemia-reperfusion leads to myocardial injury. The protective effects of calcium channel blocker, diltiazem and calmodulin inhibitors, trifluoperazine, flunarizine and calmidazolium from ischemic injury in rat hearts were observed by using Langendorff apparatus when the antagonists were infused for 3 min in the beginning of ischemia. Thereby, an increase in resting tension developed during 30-min ischemia was analyzed with regard to [1] the degree of cardiac functional recovery following 60-min reperfusion, [2] changes in biochemical variables evoked during 30-min ischemia. The results obtained were as follows: l. In the ischemic group, the resting tension was increased by 4.1*0.2 g at 30-min ischemia. However, the increase in resting tension was markedly reduced not only by pretreatment with diltiazem [3.3 p M] but also with calmodulin inhibitors, trifluoperazine [3.3 p M], flunarizine [0.5 p M] and calmidazolium [0.5 p M], respectively. 2. Recovery of myocardial contractility, +dF /dt and coronary flow were much reduced when evoked by reperfusion in the ischemic group. These variables were significantly improved either by pretreatment with diltiazem or with calmodulin inhibitors. 3. The resting tension increment evoked during ischemia was significantly inversely correlated with the degree of cardiac function recovered during reperfusion. 4. Following 30-min ischemia, the production of malondialdehyde and release of lysosomal enzyme were much increased in association with a decrease in creatine kinase activity. 5. The increases in malondialdehyde production and release of free lysosomal enzyme were suppressed by pretreatment with calmodulin inhibitors as well as diltiazem. Likewise, the decrease of creatine kinase activities was prevented by these calcium antagonists. With these results, it is indicated that a increase in resting tension observed during ischemia has an inverse relationship to the cardiac function recovered following reperfusion, and further, the later may be significantly dependent on the degree of biochemical alterations occurred during ischemia such as decrease in creatine kinase activity, increased production of malondialdehyde and increased release of free lysosomal enzyme. Thus it is concluded that calmodulin plays a pivotal role in the process of ischemic injury.

Evaluation of Balance in Diabetes Patients With Peripheral Neuropathy (당뇨병성 신경병증 환자의 균형기능 평가)

  • The purposes of this study were to determine the effect of different degrees of severity of diabetic neuropathy on balance function, and to evaluate dynamic balance and functional performance in diabetes patients. Twenty-four subjects with diabetes mellitus were divided into three groups according to results of sensory nerve conduction study. All subjects were evaluated for dynamic balance which was measured using computerized dynamic posturography, and functional performance which was measured using the Berg balance scale. One-way analysis of variance was used to determine whether there were any statistically differences of dynamic balance function and functional performance among the three groups. The Spearrnan's rank correlation was used to determine statistical significance between dynamic balance and age. The results were as follows: 1. Dynamic balance measured using computerized dynamic posturography was significantly lower in the no response group than in the normal amplitude group (p<0.05). 2. Functional performance tested by the Berg balance scale was not statistically different among the three groups (p>0.05). 3. an inverse relationship was found between dynamic balance measured using computerized dynamic posturography and age (r=-0.68, p<0.05). These results suggest that patients with severe diabetic neuropathy have loss of dynamic balance function. Therefore, patients with severe diabetic neuropathy need to have their balance evaluated and receive appropriate education.

Effects of Periodontal Treatment on Glycated Hemoglobin A Levels in Patients with Type 2 Diabetes: A Meta-Analysis of Randomized Clinical Trials

  • This systematic review aimed to investigate the effects of periodontal treatment on glycated hemoglobin A (HbA1c) levels in patients with type 2 diabetes who develop periodontal disease. The search of the MEDLINE, Embase, CINAHL, and Cochrane Library databases was completed on April 8, 2018. The study design was based on randomized clinical trials. Scaling and root planing was performed for the test group, whereas no periodontal treatment or simple oral training was performed for the control group. The main outcome variable was the change in HbA1c levels. We used the Review Manager statistical analysis software for the quantitative analysis of selected documents. Meta-analysis was performed using the inverse variance estimation method of the fixed-effect model to estimate the effects of periodontal treatment on HbA1c levels in patients with type 2 diabetes. A total of 1,011 documents were searched using search strategies, and 10 documents were included in the meta-analysis. The meta-analysis of the selected literature showed that periodontal treatment significantly reduced the HbA1c levels in patients with type 2 diabetes who develop periodontal disease (mean difference, -0.34; 95% confidence interval, -0.43 to -0.26; p<0.001). This study aimed to investigate the effects of periodontal treatment on HbA1c levels, which can be used as a basis for the increasing management of diabetic complications. To improve the quality of life and reduce the burden of medical expenses for patients with diabetes, periodontal disease management through nonsurgical periodontal treatment, such as scaling and root planing, is necessary.

New Algorithm for Arbitrary-ratio Image Resizing in DCT Domain (DCT 영역에서 영상의 임의 비율 크기 변환을 위한 새로운 알고리즘)

  • In Ubiquitous communication environment, various conversions of images are essential, and most digital images are compressed by standard methods such as the Joint Photographic Expert Group (JPEG) and Motion Picture Expert Group (MPEG) which are based on the discrete cosine transform (DCT). In this paper, various image resizing algorithms in the DCT domain are analyzed, and a new image resizing algorithm, which shows superior performance compared with the conventional methods, is proposed. For arbitrary-ratio image resizing in the DCT domain, several blocks of $8{\times}8$ DCT coefficients are converted into one block using the conversion formula in the proposed algorithm, and the size of the inverse discrete cosine transform (IDCT) is decided optimally. The performance is analyzed by comparing the peak signal to noise ratio (PSNR) between original images and converted images. The performance of the proposed algorithm is better than that of the conventional algorithm, since the correlation of pixels in images is utilized more efficiently.

Factors affecting Smartphone Addiction among Elementary School Students (초등학생의 스마트폰 중독에 미치는 영향요인)

  • The purpose of this study is to clarify the factors that affects smartphone addiction of elementary school students. The subjects were 263 students, from 4 elementary schools located in G-city. Data were collected from July, 2014 and analyzed using SPSS 19.0 program. The prevalence of addiction risk group and non-addicted group were 16.0% and 84.0%, respectively. The variables which had statistically significant differences with smart phone addiction of general characteristics are grade, living together family, economic status, school record, motivation for usage, advantage of usage and using time (p<.05). There was pure correlation between the smartphone addiction and impulsivity (r=.496, p<.001), daily stress (r=.471, p<.001). However, perceived parental attitude (r=-.375, p<.001) and self-esteem (r=-.444, p<.001) were inverse correlation with smartphone addiction. Higher using time, higher impulsivity and higher daily stress were all associated with increased of smartphone addiction level. These results suggest that more attentions should be given to early adolescents and could be effectively used as fundamental data to develop intervention programs, which can prevent the smartphone addiction.

Effectiveness of low-level laser therapy on recovery from neurosensory disturbance after sagittal split ramus osteotomy: a systematic review and meta-analysis

  • Background: Orthognathic surgery such as bilateral sagittal split ramus osteotomy (BSSRO) for the treatment of mandibular deformities is one of the most common procedures in maxillofacial operations that may lead to neurosensory disturbance. In this study, we aimed to evaluate the effectiveness of low-level laser therapy (LLLT) on augmenting recovery of neurosensory disturbance of inferior alveolar nerve (IAN) in patients who underwent BSSRO surgery. Methods: A comprehensive literature search was conducted by two independent authors in PubMed, Cochrane Central Register of Controlled Trials (CENTRAL), Scopus, Embase, and Google Scholar electronic databases. Besides, a manual search of all textbooks and relevant articles were conducted. Searches took place in August 2020 and were limited to published and peer-reviewed articles from 2000 to 2020. All analysis was performed using the comprehensive meta-analysis (CMA) and the STATA MP (version:16) software. The weighted mean difference (WMD) using the inverse variance method and the standard mean difference (SMD) was considered for continuous variables. Results: Seventy-four papers were retrieved after removing duplicate studies and finally, eight studies were assessed for qualitative synthesis and five for meta-analysis. Totally, 94 patients were included in the meta-analysis. Based on the meta-analysis, it was shown that LLLT was not effective in a short interval (0 to 48 h) after surgery, but in a period of more than 1 month after surgery, the positive results of treatment can be observed strikingly. Also, LLLT side/group showed no significant difference in some aspects of neurosensory recovery such as thermal sensation compared to the placebo side/group. Conclusions: The meta-analysis of randomized controlled trials revealed that LLLT generally improves IAN sensory disturbance caused by BSSRO. Further high-quality clinical trials with longer follow-up periods and larger sample sizes are recommended.
