지체장애인의 운동실천을 위한 동기면담 프로그램 개발 및 평가 (Development and Evaluation of a Motivational Interviewing Program for Exercise Improvement in Persons with Physical Disabilities)

  • Purpose: The aims of this study were to develop a motivational interviewing program for exercise improvement in persons with physical disabilities and to examine the effect of this motivational interviewing intervention. Methods: The study employed a nonequivalent control group pretest and posttest design. A total of 62 persons with physical disabilities (30 in the experimental group, 32 in the control group) were recruited from 2 community rehabilitation centers. The experimental group received 8 sessions of a group motivational interviewing program, scheduled once a week, with each session lasting 60 minutes. Test measures were completed before the intervention, immediately after the end of the intervention, 2 weeks later, and 6 weeks after the end of the intervention. Measures included self-efficacy for exercise, decisional balance for exercise, stage of change for exercise, regularity of exercise, exercise maintenance, and independent living ability. Data were analyzed using the ${\chi}^2-test$, Fisher's exact test, Independent samples t-test, and repeated measures ANOVA, conducted using IBM SPSS Statistics version 18. Results: The experimental group showed a significant increase in self-efficacy for exercise (F=50.98, p<.001), benefit (pros) of exercise (F=24.16, p<.001), and independent living ability (F=50.94, p<.001), and a significant decrease in loss (cons) of exercise (F=26.50, p<.001). There were significant differences between the two groups in stages of change for exercise (p<.001), regularity of exercise (p<.001), and exercise maintenance (${\chi}^2=26.61$, p<.001). Conclusion: The motivational interviewing program has the potential to improve exercise levels in persons with physical disabilities.

회전근개봉합술 후 닫힌사슬운동 적용 시점에 따른 효과 비교 (Comparison of the Rehabilitation Program after Rotator Cuff Repair by Time Closed Chain Exercise)

  • PURPOSE: This study aimed to examine the pain, range of motion (ROM), upper extremity task performance, and functional levels of patients after rotator cuff repair according to the timing of a closed chain exercise thereby presenting basic data for an effective rehabilitation program. METHODS: The intervention was applied three times per week, one hour per day, for four weeks to 40 participants, 78 of whom had undergone rotator cuff repair. The participants were divided into four groups and assigned to usual general physical therapy and an open chain exercise. Group I consisted of the open chain exercise only. The closed chain exercise was applied to group II after the 4 times, group III after the 7 times, group IV after the 10 times. Measurement were used ROM, visual analogue scale (VAS), box and block test (BBT), and shoulder pain and disability index (SPADI). A one-way analysis of variance was conducted to test differences. RESULTS: There were significant differences in the internal/external rotation between group I and group II. The VAS significantly differed between group II and group I, group III, and group IV. The BBT results of group II and group I were significantly different compared to those of group IV. The SPADI significantly differed between group II and group I and between group II and group IV. CONCLUSION: The closed chain exercise was effective for patients following rotator cuff repair from the second week after active exercise was prescribed, verifying its applicability in rehabilitation programs.

정체 운동이 거북목 증후군의 경부통증과 경추 기능장애지수 및 목 길이에 미치는 효과 (Effects of Straight Body Exercise Therapy on Neck Pain, Necks Disability Index and Neck Length Among Turtle Neck Syndrome Patient)

  • 본 연구는 정체 운동이 거북목 증후군의 경부통증과 경추 기능장애지수의 감소 효과와 목 길이 변화에 미치는 효과를 파악함으로써 거북목 증후군의 중재방안을 마련할 뿐만 아니라 추후 이들 분야의 기초자료를 제공하고자 시행되었다. 연구대상은 사전 검사를 통해 선별된 총 60명 중 정체 운동군 21명, 운동군 19명, 대조군 20명 이었으며, 정체 운동군은 5가지 정체 운동요법을 적용하였고, 운동군은 8가지 경추 강화 운동을 적용하였으며, 대조군은 아무런 운동을 시행하지 않았다. 운동 전.후 및 10주 후에 경부통증, 경추 기능장애지수 및 목 길이를 측정하였다. 수집된 자료는 SPSS(v18.0) 통계프로그램을 이용하여 분석하였다. 정체 운동이 거북목 증후군의 경부통증과 경추 기능장애지수의 감소 효과와 목 길이 변화에 미치는 효과를 파악 정체 운동군과 운동군은 실험 전. 후 목 길이, 경부통증, 경추 기능장애지수에서 통계적으로 유의하게 감소하였으며, 대조군에서는 유의한 차이가 없었다. 정체 운동군이 운동군, 대조군에 비해 거북목 증후군의 경부통증 완화와 경추 기능장애지수 및 목 길이 감소에 보다 효과적이었고, 또한 8주 운동 후 실험을 종료하고 2주 뒤 사후검사 결과에서 8주 상태를 유지되었다.

Effects of Resistance and Resistance & Stabilization Exercises on the Strength of Lumbar and Lower Limbs of the Elderly

  • The purpose of this study is to identify the effective lumbar area exercise program to prevent falls of the elderly by dividing 14 female elders over 65 living in Daegu into two groups such as resistance exercise group and resistance & stabilization exercise group, and applying a 60 minute exercise program three times a week for 12 weeks. The followings are the results. First, both of resistance exercise and resistance & stabilization exercise brought improvement of lumbar strength and there was no significant difference between two groups. Second, there was no significant difference in lower limb strength in resistance exercise group but resistance & stabilization exercise group showed significant increase in lower limb flexor strength. To make a summary of the above results, it can be said that 12 weeks' resistance & stabilization exercise is effective to the lumbar strength and lower limb strength of the elderly. Accordingly, if combining resistance exercise and stabilization exercise appropriately, we can build a successful preventive program which even the elderly who cannot perform resistance exercise easily can follow.

당뇨 환자의 운동실천에 따른 건강행태, 의료이용 및 의료비용 비교 (The Comparison of Health Behaviors, Use of Health Services, and Health Expenditures among Diabetic Patients according to the Practice of Exercise)

  • Purpose: The purpose of this study was to compare the use of health services and health expenditures between non-exercise and exercise groups of diabetic patients and among three groups divided according to exercise intensity. Methods: Data were obtained from the Korean Health Panel Survey of 2011. The participants of this study were 864 diabetic patients who did exercise (walk, moderate exercise, or vigorous exercise) or not. Data were subsequently analyzed using the SPSS 21 Program. Results: The exercise group showed higher percentages of medication compliance, non-smokers, and regular diet than the non-exercise group. The hospitalization percentage, the number of outpatient hospital visits, and health expenditures were higher in the non-exercise group than in the exercise group. There was no difference among the three groups divided according to exercise intensity in the use of health services and health expenditures. Conclusion: These results show that exercise is a way to reduce diabetic patients' use of health services and their health expenditures.

노인의 운동의도, 운동습관, 운동자기효능감이 규칙적 운동행위에 미치는 영향 (The Influence of Exercise Intention, Exercise Habits, and Exercise Self-efficacy on Regular Exercise Behaviors in the Elderly)

  • Purpose: The purpose of this study was to investigate the effects of exercise intention, exercise habit, exercise self-efficacy on exercise behavior with elders at welfare center. Methods: The subjects were 152 elders at the welfare center at J city. Data were collected using the questionnaires which consisted of the Exercise intention scale and the Exercise habit scale developed by Lee and Gu (2018), and the Exercise self-efficacy scale developed by Lee and Chang (2001). The collected data were analyzed with the SPSS/WIN 22.0 program, which was used for frequency, percentage, mean, standard deviation, x2 test, and multiple logistic regression. Results: The regular exercise group consisted of 92 elders (62.5%) and the irregular exercise group of 60 elders (39.5%). The scores for exercise intention, exercise habits, exercise self-efficacy were 3.28±0.70, 3.92±0.61, 2.80±0.88 in regular exercise group, and 2.38±0.99, 3.00±1.07, 2.04±0.73 in irregular exercise group, respectively. Two groups were significantly different between the two groups. Multiple logistic regression analysis showed that exercise intention (OR: 2.26, 95% CI:1.19~4.28), exercise habits (OR: 1.79, 95% CI:1.01~3.15), exercise self-efficacy (OR: 1.99, 95% CI:1.11~3.57) were affecting factors for regular exercise in elders. Conclusion: To increase the regular exercise behaviors of the elderly at the welfare center, it is needed the strategies to increase exercise intention, exercise self-efficacy, and exercise habits.

목 안정화와 호흡 재교육 운동이 만성 뇌졸중 환자의 목 깊은 굽힘근육의 두께, 노력성 폐활량과 최대 기침 유량에 미치는 효과 (Effects of the Neck Stabilizing Exercise Combined With the Respiratory Reeducation Exercise on Deep Neck Flexor Thickness, Forced Vital Capacity and Peak Cough Flow in Patients With Stroke)

  • Impaired respiratory function is common in patients with stroke. The purpose of this study were to investigate the effectiveness of exercises and to assess forced vital capacity and peak cough flow after completion of neck stabilizing and respiratory reeducation exercises (combining diaphragmatic breathing and pursed-lip breathing exercises). The 45 participants were randomly assigned to an experimental group 1 ($n_1=15$), experimental group 2 ($n_2=15$), and a control group ($n_3=15$). All subjects performed conservative physical therapy for 30 minutes. Experimental group 1 undertook the neck stabilizing exercise and the respiratory reeducation exercise. Experimental group 2 undertook the respiratory reeducation exercise. Additional exercise did not exceed 30 minutes, five times a week for six weeks. The subjects were assessed for deep neck flexor thickness and breathing function (forced vital capacity, forced expiratory volume at one second, forced expiratory volume at one second/forced vital capacity, peak expiratory flow, and manual assisted peak cough flow) at pre-post value. The results of this study were as follows. Experimental group 1 showed a significant increase only in deep neck flexor thickness change rate (p<.05). All groups showed significant increases in forced vital capacity, forced expiratory volume at one second, and peak expiratory flow in pre-post measurement (p<.05). Experimental groups 1 and 2 showed an increase in manual assisted peak cough flow in pre-post measurement (p<.05). There was no significant difference between experimental group 1 and experimental group 2, but experimental group 1 improved more than experimental group 2 in respiratory function as a whole. In conclusion, these findings suggest that the neck stabilizing exercise in combination with the respiratory reeducation exercise can improve forced vital capacity and peak cough flow in patients with stroke.

Effect of Hamstring Eccentric Exercise on Pain and Functional Activity in Patients with Chronic Low Back Pain

  • Objective: The aim of this study is to investigate the effect of hamstring eccentric exercise on pain and functional activity in patients with chronic low back pain. Design: Randomized controlled trial Methods: Participants comprised 27 subjects with back pain persisting for 3 months. They were randomly assigned to the experimental group (Eccentric exercise: n=13) and the control group (Stretching: n=14). The experimental group performed gastrocnemius, iliopsoas, and quadratus lumborum stretching along with hamstring eccentric exercise, while the control group engaged in the same stretches along with hamstring stretching. The eccentric exercise for the hamstrings was Nordic curl exercise. Each group practiced for 1 hour a day, 3 times a week, for 4 weeks. Pain, disability index, balance, and hamstring length were measured. Results: Significant differences were observed in the comparison of changes in back pain, back disability index, and hamstring length before and after exercise within each group (p<0.05). The balance ability of the experimental group showed a difference before and after exercise (p<0.05), whereas no significant difference in the control group. There were no significant differences in back pain and hamstring length between the groups. Changes in back disability index and balance ability significantly increased in the experimental group compared to the control group (p<0.05). Conclusion: Hamstring eccentric exercise using Nordic curls has a positive effect on back pain, back disability index, balance ability, and hamstring length changes.

The Exercise Capacity and Cardiovascular Factors in Patients with Exaggerated Blood Pressure Response during Treadmill Exercise Testing

  • Exaggerated blood pressure response during exercise has been found to increase the risk of future hypertension, left ventricular hypertrophy, cerebrovascular stroke, and CVD (cardiovascular disease) death. The aim of this study was to evaluate exercise capacity, cardiovascular factors in exaggerated blood pressure response during treadmill exercise testing. For research subjects, 72 subjects (normal blood response: 49 subjects, exaggerated blood response: 23 subjects) who received treadmill exercise test at J General Hospital were selected in this study. Exaggerated SBP (systolic blood pressure) response was defined as an SBP of 210 mmHg or greater during a maximal treadmill exercise test. The group with an exaggerated SBP response showed significantly higher values for RPP (rate pressure product) compared with the group with a normal SBP response. Subjects with METs (metabolic equivalents) had lower exaggerated SBP response than normal SBP response group. Subjects with recovery SBP had delayed exaggerated SBP response than normal SBP response group. Exaggerated SBP response to exercise is negative correlation with METs.

Psychological and Physical Effects of Schroth and Pilates Exercise on Female High School Students with Idiopathic Scoliosis

  • Purpose: The purpose of this study is to investigate effects of psychological factors on Schroth exercise and Pilates exercise to female high school students with idiopathic scoliosis. Methods: Sixteen subjects with over 20 of Cobb's angle were randomly divided into Schroth exercise group (SEG, n=8) and Pilates exercise group (PEG, n=8). Both training programs were carried out 3 day a week for 12 weeks. Psychological factors were measured using depression scale, self esteem scale and body esteem scale. Results: Both groups showed significant differences in depression scale, self esteem scale and body esteem scale. Schroth exercise group showed greater improvement than Pilates group. Conclusion: Having correct body image affects psychological factors to growing students. Schroth exercise is more effective method to physical and psychological approach of idiopathic scoliosis.
