The Effect of Botulinum Toxin-A on the Survival of Random-Pattern Cutaneous Flap in Rat (보튤리눔 톡신-A가 백서의 임의 피판 생존율에 미치는 영향)

  • Purpose: Botulinum toxin type A(BoTA) can block the release of vasoconstriction cotransmitters as well as acetylcholine in nerve terminal. The authors observed that BoTA increases flap survival by preventing sympathetic collapse of peripheral vessels. Methods: 10 Sprague Dawley rats were divided into control(n=5), and BoTA group(n=5). $3{\times}10cm$ sized random pattern cutaneous flaps were elevated on the dorsal side in both groups. In BoTA group, BoTA was injected into the flap via intradermal to subdermal route, 7 days before the flap elevation. Flap survival rates (survival area/total area) were measured 7 days after the elevation. Cutaneous blood flow was measured in proximal, middle and distal compartments of the flap using laser Doppler flowmetry initially, preoperatively, at immediate postoperation, and 7 days after flap elevation, respectively. Histological examination was performed 7 days after the flap elevation. The number and shape of the vessels were evaluated under microscope. Results: Mean flap survival was $53.18{\pm}6.58%$ in control group and $93.79{\pm}6.06%$ in BoTA group, displaying statistically significant difference(p=0.0008, p<0.05). In the control group, blood flow to the middle and distal compartments of the flap decreased significantly immediately after flap elevation. In the BoTA group, blood flow to the middle compartment did not decrease(p=0.002) and slightly decreased in the distal compartment(p=0.001). Cutaneous blood flow was significantly higher in all compartments of the flap in BoTA group than in control group, 7 days after the flap elevation. In histopathologic examination, greater number of vessels were noted in the BoTA group than in the control group. Conclusion: Botulinum toxin A can increase the survival of the random pattern cutaneous flap in rats by preventing the sympathetic collapse of peripheral vessels.

Alteration in Magnesium Level in Acute Myocardial Infarction

  • Sodium, potassium, calcium, zinc and magnesium levels in the serum of 31 patients diagnosed as acute myocardial infarction were analyzed on admission (within 24 Hours) and after 48 hours. The results were compared with those of 26 age matched controls. No significant difference was observed in the mean sodium, potassium, calcium and zinc levels between the cases and controls. Compared to the controls, however, the variation in the level of magnesium is highly significant at the time of admission as well as after 48 hours. When the risk factors like diabetes mellitus, hypertension, smoking and alcohol were considered, it is found that there is no significant difference between the risk groups as well as between the patients. The alteration in magnesium level in acute myocardial infarction is independent of these risk factors. Within the first 24 hours, the significant decrease in serum magnesium (35-51% fall when compared with the control group), correlates with its entry into the cell following ischemia. From this hypomagnesemic state, it rises to 9-22 times after 48 hours. This hyper-magnesemia after 45 hours is probably due to the shift of magnesuim from the intracellular fluid compartment to the extracellular fluid compartment that follows cellular recovery. Therefore, including magnesium in the immediate management of acute myocardial infarction will be beneficial in the early recovery.

  • Magnetic resonance imaging (MRI) has become a crucial tool for evaluating mediastinal masses considering that several lesions that appear indeterminate on computed tomography and radiography can be differentiated on MRI. Using a three-compartment model to localize the mass and employing a basic knowledge of MRI, radiologists can easily diagnose mediastinal masses. Here, we review the use of MRI in evaluating mediastinal masses and present the images of various mediastinal masses categorized using the International Thymic Malignancy Interest Group's three-compartment classification system. These masses include thymic hyperplasia, thymic cyst, pericardial cyst, thymoma, mediastinal hemangioma, lymphoma, mature teratoma, bronchogenic cyst, esophageal duplication cyst, mediastinal thyroid carcinoma originating from ectopic thyroid tissue, mediastinal liposarcoma, mediastinal pancreatic pseudocyst, neurogenic tumor, meningocele, and plasmacytoma.

Effect of Cimetidine and Antacid on Pharmacokinetics of Minocycline (시메티딘 및 제산제가 미노싸이클린의 약물동태에 미치는 영향)

  • Effects of aluminum magnesium hydroxide (A) and cimetidine (C) on the pharmacokinetics of minocycline (M) were investigated in female rats. Blood samples were collected at various time intervals until 36 hrs following oral dosing of drugs. Plasma minocycline concentrations were determined by HPLC. Control group (M), $T_1$ group (M+A), $T_2$ group (A+M after 2 hrs), $T_3$ group (M+A after 2 hrs), $T_4$ group (M+C) and $T_5$ group (C+M after 2 hrs) were divided to examine interaction of the drugs with minocycline. Plasma minocyline level-time curves were well described by two-compartment open model with first-order absorption in rats. Antacid treatment was associated with reduced of 71.0, 45.9, 35.7% in minocycline absorption rate $constant(K_{\alpha})$, maximum plasma $concentration(C_{max})$, and relative $bioavailability(F_{rel})$, respectively. Cimetidine treatment group exhibited no significant changes in plasma level-time curve when compared with control group and did not affect minocycline absorption as by any of these three parameters.

Pharmacokinetics of ciprofloxacin in chickens (닭에서 ciprofloxacin의 체내 동태에 관한 연구)

  • The purpose of this experiment was to develop a simple and reliable HPLC method for the detection of ciprofloxacin in chicken serum and to provide a basic data on pharmacokinetic parameters after oral and intramuscular administration. The results obtained were as follows: 1. 0.2% meta-phosphoric acid: acetonitrile(7:3, v/v) solution had a high and regular recovery rates and was selected as an extraction solution. 2. The recovery rates of ciprofloxacin were 83-97% with the selected solution in chicken serum and the detection limit was 50ng/ml in serum. 3. Ka(abosorption rate constant) were 3.652 1/h in fasted group and 0.880 1/h in non-fasted group, and Ke (elimination rate constant) were 0.061 1/h and 0.133 1/h, respectively. 4. The highest concentration in serum after intramuscular injection was 840ng/ml within 15-30min and 160-324ng/ml in 1.1-3.2 hours after oral administration. 5. The time course of blood concentration fits well into a 2 compartment model. 6. On oral administration of ciprofloxacin with feed, ciprofloxacin was absorbed more slowly and the amount of absorbed was smaller than that of in fasted chickens. 7. Blood concentration of ciprofloxacin increased in a dose-dependent manner after intramusclular and oral administraiton.

The Latest Trend of Dynamic MR Defecography for the Chronic Constipation Patient (만성 기능성 변비 환자에서 동적 MR Defecography의 최신동향)

  • With advancement of the medical imaging technology, the dynamic pelvic MRI (magnetic resonance imaging) has been introduced and used for dynamic MR defecography to improved diagnosis of the patients. At the early stage of its use, it was mostly used to diagnose enterocele or cystocele, then its use was extended to diagnose the organ prolapse and other dysfunctional pelvis organs. There now have been many reports of other diseases such as the functional constipation and others. This paper introduces the pelvis MRI and the dynamic MR defecography and reports the future trend in their clinical applications. Until recently, the studies with pelvic MRI were mostly focused on observing the movement of the pelvis in the supine position. Yang and 26 others reported good result in observing the patients with the pelvic organ prolapse by using the pubococcygeal line as the anatomical index. Using the fast gradient recalled acquisition (fast GRASS), they compared cystocoele, genitourinary prolapse, enterocoele and rectocoele with the control group. Kruyt et al. observed the posterior compartment and reported that MRI was more helpful than the fluoroscopy. Healy et al. applied the dynamic MRI test on the patients with constipation or incontinence as well as the control group without those symptoms. Since then, MRI technology has further advance by Lienemann, who was able to attain the more detailed images using the fast T2 weighted turbo spin echo technology, and others. If its limitation in diagnosing intussusception and the like, since the observation can be made only from the supine position, can be overcome with open MR or others, it is envisages that the method can eventually replace the radiological defecography.

The Role of Lymphocyte Compartment and Cytokine in Coal Workers Pneumoconiosis (진폐증환자에서의 임파구 분획 및 싸이토카인의 역할)

  • Background : Coal-worker's pneumoconiosis(CWP) is characterized by a chronic inflammatory lung reaction associated with macrophage accumulation in the alveolar spaces. CWP is usually divided into two stage : simple pneumoconiosis(SP) where there are a limited number of fibrotic lesions remain limited, with radiological opacities smaller than 1cm and progressive massive fibrosis(PMF), which is characterized by the development of a perifocal extensive fibrotic response of the lung and severe alterations in pulmonary function. In this study, the lymphocyte compartment and cytokine were evaluated by measuring the serum levels in the control, SP and PMF groups. Materials and Methods : The coal workers selected for this study were employees(patients?) of the Tae-Baek and Dong-Hae hospital. All were men, 45-76 years old and the mean duration of their exposure to coal dust was 23.2 years in the lymphocyte compartment and 24.3 years in the cytokine checked group. According to X-ray examination results, the patients were classified into either one of the SP, PMF categories. The normal controls examined were 26-70 years old men. The serum cytokine levels were estimated by using an end point enzyme immunoassay technique. Results : T lymphocyte, helper and suppressor T cells were highly related to pneumoconiosis in this study. A statistically significant decrease in the number of suppressor T lymphocytes was observed in the simple pneumoconiosis patients and at the same time, there was an increase in the lymphocyte index. Howevere, there was no statistically difference in the serum cytokines levels among the SP, PMF and control groups. Conclusion : T lymphocyte, helper T, and suppressor T cells may be highly related to the development of CWP compared to the control group particularly in the early stage of pneumoconiosis. The changes observed in the immunological system in patients with pneumoconiosis may lie at the bottom of the pathogenesis of fibrosis. Further study is needed to evaluate the lymphocyte compartment as a marker for pneumoconiosis development in the early stage.

The Effect of a Natural Zeolite (Clinoptilolite) on the Performance of Broiler Chickens and the Quality of Their Litter

  • The objective of this study was to assess the effect of clinoptilolite (a natural zeolite) on growth and performance of broilers as well as on the quality of their litter. A total of 5,200 one-day old broiler chickens (Cobb 500) were used in two consecutive rounds lasting 42 days each. The broilers were given free and continuous access to a nutritionally non-limiting diet (in meal form) that was either a basal diet (B) or a 'zeolite diet' (Z; the basal diet supplemented with clinoptilolite at a level of 2%). A $2{\times}2$ factorial design consisted of two feeding treatments (B and Z) and two bedding types, sawdust (S) and sawdust with zeolite (Sz, which was the result of adding $2kg\;zeolite/m^2$), was used. In each round the broilers were randomly assigned to one of four (n = 650), treatment groups: two fed on the basal diet (B) and had bedding of either sawdust (group BS) or sawdust and zeolite (group BSz) and two fed on the Z diet and had as bedding either sawdust (ZS) or sawdust and zeolite (ZSz). Average growth rates were significantly (p<0.05) different between broilers of different groups; broilers that were fed on the 'zeolite diet' (Z) and were placed either in a compartment with sawdust bedding or sawdust bedding and zeolite (ZS and ZSz) as well those that were fed on the basal diet in a compartment with sawdust bedding and zeolite (BSz) grew at a faster rate (p<0.05) compared with those of the control group (BS). The incorporation of NZ in broilers diets and in their bedding material decreased the organic content in litter samples throughout the experimental period. The lowest organic content was recorded in group ZSz where NZ was added in both feed and litter. Mean ammonia concentration (ppm) was significantly higher in group ZS in comparison to groups BSz and ZSz (27.00 vs. 20.55 and 21.71 respectively). The results of this study showed that the incorporation of the clinoptilolite both in feed and into the litter had a positive effect on broiler growth and also on the quality of their litter.

Preparation and Evaluation of Propofol Microemulsion for Parenteral Use (마이크로에멀젼을 이용한 프로포폴 주사제의 개발 및 평가)

  • Propofol(2,6-diisopropyl phenol) is a phenol derivative that is chemically distinct from other intravenous sedative hypnotics. It has been extensively used as a short term anesthetic agent, because of the rapid onset and short duration of action. Propofol microemulsion system was prepared with different concentrations of ethyl oleate, $Solutol^{(R)}$ HS 15 and $Kollidon^{(R)}$ 17 PF. Propofol microemulsions were studied by transmittance, viscosity, particle size, in vitro release and pharmacokinetics. The range of transmittance of A group with 4% ethyl oleate and that of B group with 5% ethyl oleate were 92.6~95.1 and 91.3~94.2%, respectively. Transmittance 1~2% decreased as concentration of $Kollidon^{(R)}$ 17 PF was increased and increased 0.8~3.3% when 10 times diluted with normal saline. The viscosity of A and B group were in the range of 3.9~4.1 mPaㆍsec and 4.4~5.3 mPaㆍsec, respectively. The particle sizes of A and B group increased as amount of $Kollidon^{(R)}$ 17 PF. Also, release of propofol was slowly increased as the amount of $Kollidon^{(R)}$ 17 PF was increased. Propofol plasma concentration by i.v injection showed 2-compartment model. Pharmacokinetics of A-5 was similar to that of commercial emuision(POFOL).
