The Effects of Family-Centered Intervention on Gross Motor Function and Performance, Balance in Children with Cerebral Palsy (뇌성마비 아동의 가족중심중재 프로그램이 대동작 기능과 수행력 및 균형에 미치는 영향)

  • Purpose : The purpose of this study was conducted to examine the effects of application family-centered intervention, by acknowledging the families of disabled children as experts and maintaining mutually cooperative relation throughout the entire processes of treatment and rehabilitation, on gross motor and balance in children with cerebral palsy. Methods : This study was executed with two group, pre-post test quasi-experimental study design. Among the 24 children with cerebral palsy as the subjects were allocated to the experimental and control group. Both groups were subjected to neuro-developmental treatment, with the experimental group performing family-centered intervention program for 40 minutes, 3 times a week for 12 weeks. Assessments were conducted before intervention, after 6 weeks and 12 weeks of intervention on gross motor function and performance, static and dynamic balance. Results : Although there was significant difference in the gross motor function and performance after 12 weeks of intervention, the mean scores of the experimental group increased more after 12 weeks of intervention than those of the control group with significant difference between the groups. Although there was significant difference in the static and dynamic balance after 12 weeks of intervention, the mean values of the experimental group decreased more after 12 weeks of intervention than those of the control group with significant difference between the groups. Conclusion : Therefore, these results suggest that on family-centered intervention on children with cerebral palsy can be provided as an beneficial and reliable clinical intervention program in development on gross motor and balance.

The Outcomes of Open Ankle Fractures in Patients Managed by Early or Delayed Definitive Fixation: A Comparative Analysis of 73 Patients

  • Purpose: Ankle fracture fixation is the gold standard of treatment but it does have its own complications. There is inadequate data regarding the comparative effectiveness of early vs. delayed fixation for open ankle fracture outcomes. This study compares the clinical and functional outcomes of open ankle fractures treated by early or delayed definitive fixation and identifies the limitations of both methods. Materials and Methods: All 73 patients enrolled in the study underwent surgical intervention within 24 hours of injury. The early fixation group (group A) consisting of 39 patients underwent definitive fixation as an index procedure, while the delayed fixation group (group B) consisted of 34 patients who underwent debridement and external fixator application as an index procedure and definitive fixation when soft tissue condition was conducive. All patients were evaluated at 2, 6, and 12 weeks postoperatively and then three monthly for a year. Results: Enneking and American Orthopaedic Foot and Ankle Society scores were markedly higher in the early fixation group at 6 months postoperatively (p-values <0.001 and 0.011, respectively). However, no discernible intergroup difference was evident at 12 months postoperatively. Between 6 and 12 months, group functional outcome scores were significantly different. At 6 months, there was a substantial difference in dorsiflexion between the two groups (p-values 0.001 and <0.001, respectively), but no difference was observed at 12 months postoperatively. At 6 and 12 months, group average plantar flexions were non-significantly different. Conclusion: Early definitive fixation of complex ankle fractures using a targeted approach produced promising results for lower grade open fractures (grades 1 and 2), and delayed definitive fixation, after initial external fixation to allow for soft tissue stabilization, produced promising results for higher grade open fractures (grades 3A and 3B). At 12-month follow-ups, clinical and functional outcomes achieved using these strategies were equivalent.

A Study on the Effects of Bee Venom Aqua-Acupuncture on Writhing Reflex (봉독약침(蜂毒藥鍼)이 Writhing reflex 및 척수내(脊髓內) 통증관련(痛症關聯) 신경세포(神經細胞)의 활성(活性)에 미치는 영향(影響))

  • Introduction : In spite of the use of Bee Venom aqua-acupuncture in the clinics, the scientific evaluation on effects is not enough. Bee Venom aqua-acupuncture is used according to the stimulation of acupuncture point and the chemical effects of Bee Venom. The aims of this study is to investigate the analgegic effects of the Bee Venom aqua-acupuncture, through the change of writhing reflex Materials and Methods : Pain animal model was used acetic acid method. The changes of writhing reflex of the mice which were derived pain by injecting acetic acid into the abdomen, after stimulating Bee Venom aqua-acupuncture on Chungwan($CV_{12}$) and non acupuncture point on the backside were measured. Results : 1. It showed that the writhing reflex were appeared on the groups which injected aceticacid only, and saline-acetic acid group(sample I), but not on the group bee venom-saline group(sample II). 2. The change of writhing reflex by Chungwan($CV_{12}$) Bee Venom aqua-acupuncture showed significant decrease in the order of Chungwan($CV_{12}$) Bee Venom aqua-acupuncture group III($2.5{\times}10^{-3}$g/kg), II($2.5{\times}10^{-4}$g/kg), and I($2.5{\times}10^{-5}$g/kg), compared with control group. There were significant decrease of number of writhing reflex in $5{\sim}10,\;10{\sim}15\;and\;15{\sim}20$ minutes intervals of Chung wan($CV_{12}$) Bee Venom aqua-acupuncture group I, and in $0{\sim}5,\;5{\sim}10,\;10{\sim}15\;and\;15{\sim}20$ minutes intervals of II and III, compared with control group. 3. The change of writhing reflex by non acupuncture point Bee Venom aqua-acupuncture showed significant decrease in the $0{\sim}5\;and\;5{\sim}10$ minutes intervals and the total number of writhing reflex in $2.5{\times}10^{-4}$g/kg group, compared with control group 4. The effects of writhing reflex of Chungwan($CV_{12}$) Bee Venom aqua-acupuncture group showed significant decrease, compared with non acupuncture point Bee Venom aqua-acupuncture group. Conclusion : This study shows that the Bee Venom aqua-acupuncture on Chungwan($CV_{12}$) decreases the numbers of writhing reflex. As the analgegic effects of Bee Venom aqua-acupuncture is recognized. Bee Venom aqua-acupuncture treatment is expected for pain modulation. In order to use it in many ways, more researches are needed for the dose and stability of Bee Venom aqua-acupuncture.

An Analysis of Competencies of Dietitians in Self-operated vs. Contracted Employee Foodservice by Worker-oriented Job Analysis Methodology (직영 및 위탁 사업체 급식소 영양사에 대한 작업자 지향적 직무분석(제 1 보) : 직무 수행 능력 요건)

  • The job characteristics and competencies of 285 dietitians in employee foodservices were investigated by worker-oriented job analysis methodology. The purposes of this study were to : a) determine the knowledge, abilities, skills and other characteristics(KASO, hereafter) needed for dietitians in two types of employee foodservice groups(self-operated vs. contracted) and b) the analysis of the degree of frequency and importance and entry requirements of KASO items listed below : their degree of frequency, the level of importance and their function as entry requirements. The 134 KASO items would be divided into 12 task categories. The questionnaire was mailed to 250 dietitians who are members of The Korean Dietetic Association Practice Group in self-operated foodservices(hereafter group A) and 250 dietitians who are employed in contracted foodservice companies(hereafter group B). Completed questionnaires were received from 285 dietitians(121 in self-operated, 164 in contracted) ; with a response rate of 57%. Statistical data analysis was completed using the SPSS/win and the SAS/win packages. The results of this study can be summarized as follows. 1) Analysis of the degree of frequency, importance and entry requirements of 134 KASO items in group A showed a high priority(score〉4.0) placed on 14 items in relation to dietetic tasks and 9 items in relation to human attributes, whereas in group B 19 items in relation to dietetic tasks and 11 items in relation to human attributes. 2) Comparing responses about the KASO items in the 12 task categories, there was a significant difference between the two sample groups with regard to degree of frequency : group A scored 3.29, group B scored 3.50(p〈0.01). With regard to importance of KASO items in 12 task categories, group A scored 3.78 and group B scored 3.88 showing no significant differences. With to entry requirements of KASO items in 12 task categories, group A scored 3.29 and group B scored 3.46 showing a significant difference(p〈0.05). The Effects of Oral Health Care Education for Nursing Staff on Halitosis and Nutrient Intake of the Elderly in a Long-term Care Facility (간호자 구강간호 교육프로그램이 요양시설 노인의 구취와 영양소 섭취량에 미치는 효과)

  • The purpose of this study was to investigate the effects of an oral health care education program for care providers on the nutritional status of the elderly in a long-term care facility. This study was conducted at a long-term care facility located in K metropolitan city using a nonequivalent control group non-synchronized design. Fifty-four nursing home residents aged 65 years or older were assigned either to the intervention group (n=27) or the control group (n=24). The intervention group received oral health care from the care providers' intervention group, who provided oral health care for 6 weeks after 6 weeks of oral health care education. Data were collected from the control group and intervention group at the baseline 6 and 12 weeks after oral care education and were analyzed using SPSS windows 16.0. The halitosis was lower in the intervention group than the control group at 12 weeks (P<0.01). Body mass index of the intervention group at 12 weeks was higher than that at 6 weeks. Iron intake of the intervention group at 12 weeks was lower than that at baseline. At baseline, the intakes of riboflavin, folate, and potassium were lower than 75% of dietary reference intakes. In conclusion, an oral care education program for care providers was effective in improving the oral hygiene of nursing home residents, and dietary plans are needed to improve the nutritional status of them.

Nine months versus 12 months of adjuvant trastuzumab for patients with HER2-positive breast cancer

  • Purpose: This study aimed to compare the results of treatment with adjuvant trastuzumab for 9 months versus 12 months in human epidermal growth factor 2 (HER2)-positive breast cancer patients. The primary endpoint was disease-free survival. Secondary endpoints included cardiac safety, tolerability, and overall survival. Methods: The study included 60 non-metastatic HER2-positive breast cancer patients. All study patients underwent surgery, received adjuvant chemotherapy, radiotherapy and hormonal therapy if indicated. Thirty patients were randomized in each group. Group I patients received adjuvant trastuzumab for 12 months, while group II patients received adjuvant trastuzumab for 9 months. Patients were assessed by clinical examination and Echocardiography during treatment. Results: After median follow-up of 12 months, 90% of the patients in group I were disease free and 83.3% of patients in group II were disease free (P=0.402). All studied population in both groups I and II were alive at the end of the 1-year follow-up period after the completion of adjuvant trastuzumab treatment thus overall survival is 100%. Conclusion: Trastuzumab is tolerable and its side effects are reversible. Nine months of adjuvant trastuzumab treatment is more cost effective than the standard 12 months.

The Effects of 12 Weeks of Walking Exercise on Body Composition and Physical Fitness in Middle Aged Women (12주간 걷기운동이 중년여성의 신체조성과 건강체력에 미치는 영향)

  • Purpose: This study was conducted to examine the effects of walking exercise training on body composition and physical fitness in middle aged women. Method: The subjects were randomly assigned into an obese walking exercise training group (n=21), a non-obese walking exercise training group (n=9), or a control group (n=10). The women in the treatment groups were then subjected to brisk walking at 50~70% of maximum heart rate twice a week for 12 weeks. The results were then analyzed by repeated measures ANOVA using SPSS 12.0 for windows. Results: Body weight, BMI, % body fat, and WC were not significantly improved in the obese walking exercise training group and the non-obese walking exercise training group. However, muscular flexibility and muscular endurance were significantly improved in each of these groups (p<.05). Conclusion: These results indicate that 12 weeks of walking exercise training enhanced physical fitness. However, the treatment in this study did not improve the body composition of middle aged women.

Effects of Progressive Scapular Stabilization Exercise on Neck, Muscle Strength, Upper Extremity Function in Patients with Acute Whiplash Injury

  • Objective: The purpose of this study was to investigate the effects of a combination of progressive scapular stabilization exercises, neck and thoracic spine mobilization, and thermoelectric physical therapy on pain, range of motion, muscle strength, and function in patients with acute whiplash injury. Design: A randomized controlled trial design. Methods: A total of 24 subjects were included in this study. They were randomly assigned to an experimental group (n=12) that performed scapular stabilization exercises, neck and thoracic spine mobilization, and physiotherapy, and a control group (n=12) that only performed neck and thoracic spine mobilization and physiotherapy. The pre-test was conducted before the intervention and consisted of a total of 12 treatment sessions of 60 minutes each, three times a week. A posttest was conducted 4 weeks later. Pain, range of motion, muscle strength, and function were assessed before and after intervention. Results: The results of the study showed that there was a significant difference in pain reduction and range of motion and muscle strength improvement in the experimental group, and a significant difference was also found between the experimental group and the control group in terms of functional evaluation. Conclusions: The combination of exercise therapy and joint mobilization technique and physical therapy resulted in greater improvements in pain, range of motion, strength, and function assessment, contributing to improved overall function.

THE EFFECTS OF OVARIECTOMY ON TITANIUM IMPLANT OSSEOINTEGRATION IN RAT'S TIBIA (백서에서 난소절제가 티타늄 임플랜트의 골 유착에 미치는 영향)

  • The purpose of this study was to observe the titanium implant osseointegration in the osteoporosis-induced animal model. Seventy rats, 11 weeks of age, were divided into two groups : an ovariectomized group and a control group. Titanium screw implants(diameter, 2.0mm; length, 3.5mm) were placed into left tibias of 70 rats, 35 in the control group and 35 in the experimental group. The rats were sacrificed at different time interval (1, 2, 3, 4, 6, 8, and 12 weeks after implantation) for histopathologic observation, histomorphometric analysis and immunohistochemistry with fibronectin and CD34 antibody. The results obtained from this study were as follows: 1. Histopathologically findings, newly formed bone was seen at 3 weeks and became lamellar bone at 8 weeks, and mature trabecullar bone was seen at 12 weeks control group. In experimental group, thickness of regenerated bone increased till 8 weeks gradually and mature trabecullar bone was seen at 12 weeks. 2. By histomorphometric analysis, marrow bone density and contact ratio of marrow bone to implant decreased significantly from 8 to 12 weeks in experimental group compared to control group and also total bone to implant contact ratio decreased significantly from 8 to 12 weeks in experimental group. 3. Fibronectin immunoreactivity was strong at 3 weeks control group and reduced after 8weeks gradually. But it was continuously strong from 3 to 8 weeks in experimental group. 4. CD34 immunoreactivity was very strong in the newly formed osteoblasts from 3 to 8 weeks control group. But it reacted minimally later. While in experimental group, it reacted continuously strong from 3 to 12 weeks. The results of this study suggest that osteoporosis is not an absolute contraindication to dental implantation if sufficient period suggested after fixture installation till second stage surgery.

PREVENTION OF DENTAL CARIES BY XYLITOL GUM IN PRE-SCHOOL CHILDREN DURING 12-MONTHS (자일리톨 껌 저작에 의한 유치 우식증 예방효과 비교분석)

  • The effect of xylitol and sorbitol chewing gums on dental caries levels among 5-year old kindergarteners was investigated. Three kindergartens the operations of which were partly subsidized by municipal government of Daegu city, Korea, were chosen as the study sites. The child base of the kindergartens were considered demographically and ethnically similar, representing middle-income families. The subjects were examined at their own kindergarten by the two dentists. Participation in the program was voluntary. Over 12 months, 123 participants chewed xylitol chewing gums(X group; 42 subjects), sorbitol chewing gum(S group; 42 subjects), or did not receive chewing gum as a control group(C group; 39 subjects). Consumption of xylitol and sorbitol was 4.5 to 5.0 g/day/subject, consumed in five daily chewing episodes of 5 min. Oral examination, Dentocult-SM test and interproximal dental plaque collection were completed at baseline and 12 months later. The dmfs of group C increased 59.2%, but group S increased 33.4% and group X increased only 31.3% during 12 months study period. The caries prevention ratio was 47.1% at group X and 43.6% at group S. There also appeared the reduction of caries activity at group X(1.39), group S(1.50) than control group(1.79). Compared with groups S and C, there was a statistically significant reduction of S. mutans in interproximal plaque in group X. The results suggest that xylitol chewing gum can prevent dental caries of deciduous dentition, and may be a little more effective than a sorbitol-containing product in controlling some caries-associated parameters in kindergarten-age subjects.
