Concomitant Avulsion Injury of the Subclavian Vessels and the Main Bronchus Caused by Blunt Trauma

  • Concomitant rupture of the subclavian vessels and the left main bronchus caused by blunt trauma is a serious condition. Moreover, the diagnosis of a tracheobronchial injury with rupture of the subclavian vessels can be difficult. This report describes the case of a 33-year-old man who suffered from blunt trauma that resulted in the rupture of the left subclavian artery and vein. The patient underwent an operation for vascular control. O n postoperative day 3, the left main bronchus was found to be transected on a computed tomography scan and bronchoscopy. The transected bronchus was anastomosed in an end-to-end fashion. He recovered without any notable problems. Although the bronchial injury was not detected early, this case of concomitant rupture of the great vessels and the airway was successfully treated after applying extracorporeal membrane oxygenation.

어선에 종사하는 초급 해기사의 직업 지향에 관한 연구 (A Study on the Occupational Orientation of the Junior Ship Officer in Fishing Vessel)

  • In contemporary society, many fishery industries face a serious problem of having an insufficient number of junior ship officers and engineers. Even though many fishery high schools in Korea made significant effort to produce many talented junior officers and engineers, but after graduation, the students are reluctant to get a job on the fishing vessels. Furthermore, Korean government also invested a large amount of money to train students for ocean-going and costal & near-ocean fishing vessels. However, despite the government's great effort on the training program and education, only a few students wish to work on the fishing vessels. In this article, the author tried to find answers for various questions such as what are the factors that the student prefer, where the student to want to get a job in fishery industries, and how student feel about working at fishing vessels. Furthermore, the author also analyzed the differences between high school student group and Korea Institute of Maritime and Fisheries Technology(KIMFT) student group about occupational orientation by utilizing SPSS statistical program. Additionally, the author surveyed the current onboard status of job position of the high school and KIMFT alumni. This study's results and data will be used for aiding further stimulation and advertisement of job position in fishing vessels and continued daily lives on board.

  • In general, the lock structure is built at place of having great deal range for dealing with effective ships operation such as in the west coast of Korea. In Inchon harbour, the two locks of 10 KT and 50 KT tonnage class in total has been constructed for several years age, however, it has been recorded many waiting vessels at outer harbour due to the increment of oceangoing vessels & shortage of berthing facilities in accordance with beyond expectation of cargo amount increment. This paper attempts to solves the waiting vessels problem at outer harbour by simulation in applying queing theory. It is found that the simulation results such as average queue time, service time, and queue length during lock operation can be applied to find the minimum of the cost function for determination of optimum Lock Size

돈분과 톱밥혼합물의 연속 및 간헐 통기 퇴비화에서 암모니아 휘산 (Ammonia Emissions from Composting Hog Manure Amended with Sawdust under Continuous and Intermittent Aeration)

  • Ammonia emissions during composting of hog manure mixed with sawdust were studied in four runs comprising a total of 22 pilot-scale reactor vessels. These four runs extended previous work and both verified and extended the previous conclusions. The pilot-scale vessels were 205 L insulated stainless steel drums that were aerated either continuously (high/low thermostatically controlled fans) or intermittently (5 min high fan 55 min off). Temperature ammonia emissions air flow rates carbon dioxide production and oxygen utilization moisture and dry matter reduction initial and final chemical compositions were measured. Ammonia emissions from the intermittently aerated vessels were only about 50% as great as those from the continuously aerated ones but this was found to be a result more related to total air flow than to aeration technique. All of the data for total result more related to total air flow were fitted with a linear regression line y=0.139x+29.835 where y is ammonia expressed as g of N and x is air flow in kg with $R^2$=0.6808. this general trend indicates that about 50% reduction in ammonia emissions can be achieved with 75% reduction in air flow. For the aeration techniques used the minimum oxygen level in te exhaust gas from the vessels was 5% and this is probably a resonable lower limit constraining air flow reduction. However within this constraint lower air flow now appears to be a technique that can reduce odorous ammonia emissions.

선천성 삼첨판 폐쇄증 (수술 2례 보고) (Tricuspid Atresia: Two Cases of Successful Palliative Surgery)

  • Two patients operated upon for tricuspid atresia by Glenn operation are presented. They were five years old and four years old females who were cyanotic shortly after birth, and remained cyanotic. On physical examination, cyanosis on digits and lips, clubbing of fingers, thrill and grade III systolic murmur on 4th I.C.S. along left sternal border were noted. In the first case, chest roentgenograms showed normal pulmonary markings, and the electrocardiogram was interpreted as showing left ventricular hypertrophy with left axis deviation and peaked P-wave in lead IL Right heart catheterization showed high pressure in right atrium and the catheter tip was easily inserted into the left atrium through septal defect. In the second case, chest roentgenograms showed cardiomegaly with sparse pulmonary vascular markings and narrowed vascular pedicle, and the electrocardiogram showed left ventricular hypertrophy with some element of right atrial hypertrophy. Angiocardiogram showed changes characteristic of tricuspid atresia, including "right ventricular window" The findings of right heart catheterization were similar to those of first case. On the basis of these observations, they were diagnosed as tricuspid atresia, and Glenn operation was performed. Normal position of great vessels combined with ventricular septal defect and pulmonary stenosis were noted on the first case, and on second case, transposition of great vessels was additional finding. Postoperative course was uneventful and favorable outcome was obtained.

Pulmonary Root Translocation with the Lecompte Maneuver: For Transposition of the Great Arteries with Ventricular Septal Defect and Pulmonary Stenosis

  • A five-month-old boy who had undergone previously transcatheter balloon atrioseptostomy at 3 days of age for complete transposition of the great arteries with ventricular septal defect and pulmonary stenosis underwent pulmonary root translocation with the Lecompte maneuver. This operation has the advantages of maintaining pulmonary valve function, preserving the capacity for growth, and avoiding problems inherent to the right ventricular to pulmonary artery conduit. This patient progressed well for 9 months postoperatively and we report this case of pulmonary root translocation with the Lecompte maneuver.

  • Interruption of the aortic arch may be defined as discontinuity of the aortic arch in which either an aortic branch vessel or a patent ductus arteriosus supplies the descending aorta. This uncommon lesion was described first by Raphe Steidele in 1778 and was later classified into 3 types by Celoria and Patton. This anomaly rarely occurs as an isolated anomaly. Most commonly, a ventricular septal defect, patent ductus arteriosus, and abnormal arrangement of the brachiocephalic arteries occurs together with arch anomaly. Rarely, more complex anomaly, such as transposition of the great vessel, or single ventricle, is coexistent. We present the case of an 6 year-old boy with D-transposition of great vessel single ventricle, patent ductus arteriosus and patent foramen ovale with interruption of the aortic arch (Type A).

  • There have been few reports documenting the outcome of arterial swich operations(ASO) in selected patients with transposition of great arteries(TGA) and with left ventricular outflow tract obstruction(LVOTO). In the case of TGA with LVOTO, if the atrial septal defect(ASD) is large and the ventricular septal defect(VSD) is restricive, this deprives the left ventricle(LV) of approporiate preload and could lead to underdevelopment of the ventircular mass and lead poor LV performance after the arterial switch operation, dspite a high pressure in the LV preoperatively. Because an increase in the systolic ventricular pressure is not necessarily paralleled by an increase in ventricular mass, which is also essential for optimal ventricular performance after the operation. We report here a case of rapid LV training after ASO in TGA with unprepared LV (because of large ASD and restrictive VSD) despite a high pressure in the LV(due to LVOTO) preoperatively.

  • The patient in whom corrected transposition of the great vessels is an isolated congenital anomaly has a physiologically normal circulation. A 19 year old male was perforemd with surgical correction for congenitally corrected transposition of great arteries (SLL) associated with ventricular septal defect (type I) and severe pulmonary stenosis. After right sided ventriculotomy, ventricular septal defect was repaired with Dacron patch and to provide adequate relief of severe valvar stenosis of pulmonary artery, valvulotomy was done. On immediate postoperative period, complete heart block and lower cardiac output state were ocurred, but adequate blood pressure could be maintained under using of inotropic agent and temporary pace-maker.
