슬관절 부위에서 자화전이 위상감도법에 의한 자화전이율 영상 평가 (Evaluation of Magnetization Transfer Ratio Imaging by Phase Sensitive Method in Knee Joint)

  • 슬관절 퇴행성 질환을 진단하기 위하여 자기공명영상(magnetic resonance imaging: MRI)이 많이 사용되지만 간혹 슬관절 병후 및 예후를 잘못 진단하는 경우가 종종 발생한다. 본 연구에서는 슬관절 질환의 진단에 도움을 주기 위하여 자화전이(magnetization transfer: MT) 영상법을 소개하고자 한다. 슬관절 환자 7명으로부터 스핀 에코(SE) T2 강조 영상(3,400-3,500/90-100 ms)과 슬관절 환자 7명으로부터 FSE T2 강조 영상(4,500-5,000/100-108 ms)과 또한 슬관절 환자 3명으로부터 gradient echo (GRE) T2 강조 영상들(9/4.56 ms, 50 flip angle, NEX 1)을 획득하였다. 6명의 슬관절 환자에서 지방 억제가능 T2 강조 STIR 펄스시퀀스(TR/TE=2894-3215 ms/70 ms, NEX 3, ETL 9)를 사용하였다. 지방 포화도에 있어서 위상감도 방법은 Larmor frequency 차이에 따른 위상 차이를 이용하므로, 각각의 픽셀에 대한 자화전이율(magnetization transfer ratio: MTR)의 측정은 포화된 영상과 포화되지 않은 영상의 비율에 따라 산출하였다. 따라서 각 입력된 영상들은 동차원성을 가지고, 시각적으로 신호강도 정도가 회색의 명암도만으로 평가될 수 있기 때문에 생리학적, 정량적 진단을 위하여 3차원 등방성 체적영상과 자기공명 삼원색을 매핑하였고 정량적 특정은 자화전이율 지도로서 표현하였다. 자화전이율 영상은 병변 부위에서 높은 대조도를 나타내어 환자의 병태를 추적하는데 도움을 주었고 정량화하였다. 자화전이율 영상들과 기존의 MRI 사이에 명암도 차이는 회색상으로 표현되며, 자화전이율 영상화의 효과에 대해 프로파일 그래프는 자화전이 펄스로 인하여 신호강도에 있어서 정량적 측정값이 상대적으로 감소하였다. 슬관절의 정확한 병리상태를 진단하기 위해 프로파일 그래프의 측정값을 영상과 함께 표현하였다. 본 연구에서 수행한 예비적 데이터들을 통하여 슬관절의 자화전이율 영상들이 매우 임상학적으로 유용함을 확인하였다. 자화전이율 영상에 대한 물리적 변화를 관찰함으로써 자화전이율 영상에 대한 물리적, 기술적 기반에 대한 더 많은 통찰력을 제공할 수 있다. 무릎 질환 환자의 자화전이율 영상들을 이용하여 매우 높은 대조도를 확보할 수 있으므로 슬관절 질환의 정밀 진단에 매우 도움을 줄 수 있다.

심층신경망을 이용한 레이더 영상 학습 기반 초단시간 강우예측 (Very short-term rainfall prediction based on radar image learning using deep neural network)

  • 본 연구에서는 강우예측을 위해 U-Net과 SegNet에 기반한 합성곱 신경망 네트워크 구조에 장기간의 국내 기상레이더 자료를 활용하여 심층학습기반의 강우예측을 수행하였다. 또한, 기존 외삽기반의 강우예측 기법인 이류모델의 결과와 비교 평가하였다. 심층신경망의 학습 및 검정을 위해 2010부터 2016년 동안의 기상청 관악산과 광덕산 레이더의 원자료를 수집, 1 km 공간해상도를 갖는 480 × 480의 픽셀의 회색조 영상으로 변환하여 HDF5 형태의 데이터를 구축하였다. 구축된 데이터로 30분 전부터 현재까지 10분 간격의 연속된 레이더 영상 4개를 이용하여 10분 후의 강수량을 예측하도록 심층신경망 모델을 학습하였으며, 학습된 심층신경망 모델로 60분의 선행예측을 수행하기 위해 예측값을 반복 사용하는 재귀적 방식을 적용하였다. 심층신경망 예측모델의 성능 평가를 위해 2017년에 발생한 24개의 호우사례에 대해 선행 60분까지 강우예측을 수행하였다. 임계강우강도 0.1, 1, 5 mm/hr에서 평균절대오차와 임계성공지수를 산정하여 예측성능을 평가한 결과, 강우강도 임계 값 0.1, 1 mm/hr의 경우 MAE는 60분 선행예측까지, CSI는 선행예측 50분까지 참조 예측모델인 이류모델이 보다 우수한 성능을 보였다. 특히, 5 mm/hr 이하의 약한 강우에 대해서는 심층신경망 예측모델이 이류모델보다 대체적으로 좋은 성능을 보였지만, 5 mm/hr의 임계 값에 대한 평가결과 심층신경망 예측모델은 고강도의 뚜렷한 강수 특징을 예측하는 데 한계가 있었다. 심층신경망 예측모델은 예측시간이 길어질수록 공간 평활화되는 경향이 뚜렷해지며, 이로 인해 강우 예측의 정확도가 저하되었다. 이류모델은 뚜렷한 강수 특성을 보존하기 때문에 강한 강도 (>5 mm/hr)에 대해 심층신경망 예측모델을 능가하지만, 강우 위치가 잘못 이동하는 경향이 있다. 본 연구결과는 이후 심층신경망을 이용한 레이더 강우 예측기술의 개발과 개선에 도움이 될 수 있을 것으로 판단된다. 또한, 본 연구에서 구축한 대용량 기상레이더 자료는 향후 후속연구에 활용될 수 있도록 개방형 저장소를 통해 제공될 예정이다.

$GafChromic^{(R)}$ EBT2 Film Dosimetry를 위한 품질 관리용 초기 프로그램 개발 (Development of Preliminary Quality Assurance Software for $GafChromic^{(R)}$ EBT2 Film Dosimetry)

  • GafChromic EBT2 필름 dosimetry에 필요한 품질 관리용 소프트웨어를 개발하였다. 개발한 프로그램에서는 EBT2 필름특성에 맞게 붉은색, 초록색, 파란색 및 회색 채널에 따른 필름 교정이 가능하도록 하였다. 또한 평판형 스캐너의 빛의 산란 효과나 필름 내 방사선에 반응하는 물질(active layer)의 두께 차이가 선량 검증에 미치는 영향을 평가할 수 있도록 하였다. EBT2 필름을 이용한 측정 결과는 방사선 치료계획 시스템, ECLIPSE 또는 2차원 이온 전리함 배열의 선량 값과 비교할 수 있다. 개발한 소프트웨어를 이용한 GafChromic EBT2 필름의 선량 검증은 파일 입력, 잡음 제거, 배경 보정(background) 및 반응 물질 보정(active layer correction), 선량 계산 및 평가 단계를 통해서 이루어진다. 절대적 또는 상대적 배경 보정 방법을 선택적으로 적용할 수 있으며 필름 교정 결과 및 교정 곡선에 대한 적합식(fitting equation)은 결과 파일로 출력할 수 있다. 선량 행렬의 화소 크기 조정을 위한 보간법, 대화식 영상 이동 및 회전 기능을 이용하여 선량 행렬 간 구조적 위치를 일치시킨 후, 빔 측면도(beam profile) 및 등선량곡선(isodose curve)을 비교할 수 있다. 또한 거리 및 선량 차이에 대한 허용값을 적용하여 gamma index 및 gamma histogram을 이용한 선량 분석이 가능하다. 60도 동적 쐐기 조사면과 전립선 세기조절방사선치료의 조사면을 이용하여 개발한 소프트웨어의 기초 성능 평가를 수행하였을 때, 동적 쐐기 조사면에서 ECLIPSE와 EBT2 필름 간 절대적 빔 측면도는 3% 오차 범위 내에서 일치하였다. EBT2 필름을 이용한 두 종류의 선량 검증 모두, 99% 이상의 영역이 3 mm, 3%의 gamma index의 평가 기준을 만족하였다. 개발한 선량 검증용 소프트웨어를 이용하여 주기적으로 수행되는 일반적인 품질관리뿐만 아니라 빔의 세기가 조절된 복잡한 조사면의 품질관리에도 활용할 수 있으며, Radiochromic 필름을 이용한 선량 평가에 필요한 유용한 분석 툴을 제공할 수 있다.

뇌피질 이형성증의 자기공명영상소견: 병리적 등급 및 유형과의 연관성에 대하여 (MR Imaging Findings of Cortical Dysplasia of the Brain: Correlation with Pathologic Grades and Subtypes)

  • 목적 : 뇌피질 이형성증의 다양한 자기공명영상소견을 유형별로 분류하고 병리적 등급 및 유형과의 연관성을 분석한다. 대상 및 방법 : 수술 후 병리적으로 뇌피질 이형성증이 확진된 97명을 대상으로 하였다. 수술 전 MR 영상을 후향적으로 분석하였으며, 이상 소견의 유무에 따라 MR 양성군과 MR 음성군으로 나누었다. MR 양성군에서는 뇌회와 인접지주막하 공간의 크기, 뇌피질 두께, 피질하백질의 신호 강도, 뇌피질과 백질 경계부의 명확성에 의하여 MR 이상 소견을 분류하였다. 병리적 소견 역시 영상 소견을 모르는 상태에서 후향적으로 분석하였으며, 경도, 중등도, 고도와 비풍선세포형, 풍선세포형으로 나누었다. MR 양성군과 음성군 사이에서 그리고 MR 양성군의 네 가지 유형 내에서 각각 병리적 등급 및 유형의 차이가 있는지 분석하였다. 결과: MR 양성군과 음성군은 각각 39 (40%) 명, 58 (60%) 명이었다. MR 양성군 중 위축형은 13 (33%)명, 피질띠형은 9 (23%)명, 내측 만곡형은 9 (23%)명, 그리고 비특이적 형태는 8 (21%)명이었다. MR 양성군과 음성군 사이에서 병리적 등급은 의미 있는 차이가 없었으나, MR음성군에 비하여 MR 양성군에서 고도의 병리적 등급의 빈도가 높은 경향을 보였으며 또한 풍선 세포형의 빈도 역시 유의하게 높았다. (5% Vs 2l%, p<0.05). 특히 MR 양성군 중에 내측 만곡형은 고도의 병리적 등급과 풍선세포형의 빈도가 각각 78% (7/9), 56% (5/9)인 반면에, 위축형은 경도의 등급과 비풍선세포형의 빈도가 각각 77% (10/13), 100%(13/13)이었다. 결론: 뇌피질이형성증 환자의 MR 영상에서 반 이하에서만 이상 소견을 발견할 수 있었고, 특히 내측 만곡형은 병리적으로 고도의 등급 및 풍선세포형, 그리고 위축형은 경도의 등급 및 비풍선세포형과 높은 연관성을 보여 주었다.

한국인 뇌 표준판과 해부학적 및 세포구축학적 확률뇌지도를 이용한 뇌영상 정량화 (Quantification of Brain Images Using Korean Standard Templates and Structural and Cytoarchitectonic Probabilistic Maps)

  • 목적: 다수의 뇌 자료를 기반으로 구성된 통계적 화률뇌지도는 복잡하고 개인적인 편차가 큰 뇌의 형태학적, 기능적 특성을 분석하는데 유용하다. 특히 최근에 한국인의 정상 MR 영상을 기반으로 한 구조적 기능적 뇌 표준판과 구조적 확률뇌지도가 완성되었으며. 부검뇌의 조직절편을 활용한 세포구축학적 확률뇌지도가 도입되었다. 이 연구에서는 이들 자료를 활용하여 뇌 영상의 국소계수를 객관적으로 측정하기 위한 정량화 기법을 개발하였으며, 이를 이용하여 정상 한국인의 뇌 영상자료를 분석하였다. 대상 및 방법 : T1 MRI와 FDG PET에 대한 뇌 표준판을 성별, 연령별로 개발하였으며, 한국인 정상 MR 영상으로 만들어진 89개 뇌 영역의 구조적 확률뇌지도와 독일 율리히 연구센터에서 도입한 13개 브로드만 영역에 대한 세포구축학적 확률뇌지도를 뇌 표준판 위로 각각 비선형 변환하였다. 확률뇌지도에 정의된 각 뇌 영상의 국소계수는 확률에 의한 가중평균 또는 가중합으로 구하였다. 확률뇌지도를 이용한 예비 연구로 정상 노화에 따른 포도당대사의 변화가 대상회 내에서 전 후 위치에 따라 다르게 나타나는 가를 조사하였다. 결과: SPM과의 연계된 사용을 고려하여 Matlab 상에서 작성된 정량화 프로그램은 20초 미만에 1개의 공간정규화된 영상을 처리할 수 있었다. 대상회 포도당 대사에 대한 분석에서 후대상회에 대한 문측-전대상회 및 미측-전대상회의 계수 비는 정상 노화에 따라 매우 유의하게 감소하였다. 즉. 문측-전대상회/후대상회는 매 10년마다 3.1%씩 감소하였으며($P<10^{-11}$, r=0.81) 미측-전대상회/후대상회는 1.7%씩 감소하였다($P<10^{-8}$, r=0.72). 문측-전대상회와 미측-전대상회의 계수비 감소 역시 유의하였다(P<0.0005. r=0.52, 1.5%/year). 결론: 서양인 뇌에 기반한 확률뇌지도는 그동안 제한적으로만 사용되어 왔으나 한국인 뇌 표준판과 확률뇌지도 및 이 연구에서 개발한 정량화 기법은 일반에 공개하기로 하여 더욱 널리 쓰이게 되면 국내 뇌 연구 활성화에 기여할 것으로 기대 된다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury.
