The Comparison of Glottal Area Waveform between Normal Person and Patient with Vocal Lesions (정상인과 후두질환 환자에서 Glottal Area Waveform의 차이에 관한 연구)

  • Background and Objectives : Glottal area waveform(GAW) shows the plot of glottal area versus time through the 1 cycle. This study is designed to see how it can be applied to laryngeal patients. Material and Methods : A GAW analysis was peformed on 11 patients before and after surgery and 22 persons without laryngeal problems were recruited for control. Acoustic and aerodynamic analysis with VHI(voice handicap index) evaluation were performed. Results. Significant changes in baseline offset, gross closing rate and VHI were observed postoperatively. Other parameters were changed but insignificant. Conclusion : GAW maybe used to evaluate patient with voice problems.

Quantitative Measurement of the Glottal Area Waveform(GAW) in Unilateral Vocal Fold Paralysis (편측성대마비환자에서의 성문면적파형(Glottal Area Waveform)의 정량적 측정)

  • Type Ⅰ thyuroplasty in conjunction with arytenoid adduction is one of the excellent techniques in the treatment of unilateral vocal fold paralysis. But perioperative objective evaluation of the patients is difficult. With the development of the videostroboscopy and image analysis program, we could quantify the Glottal Area Waveform(GAW) in patients with unilateral vocal fold paralysis and investigated the relationship between the glottal area and aerodynamic and acoustic parameters. Eight female patients who were performed type Ⅰ thyroplasty in conjunction with arytenoid adduction and 5 females with normal vocal function were involved in this study. Preoperative and postoperative videostroboscopy and vocal function study wire performed. GAW was analysed quantitatively with image analysis program (Kay Stroboscope Image analysis, KSIP) Peak Glottal Area(PGA), Baseline Offset(BO), and Closing Phase(CP) were increased in patients with unilateral vocal fold paralysis and they were reduced after the operation. Mean flow Rate (MFR) was well correlated with the PGA in normal control group and unilateral vocal fold paralysis patients. Noise to harmonic ratio(NHR) was correlated with PGA only in preoperative unilateral vocal fold paralysis patients. In conclusion quantitative measurement of the GAW is useful method in evaluation of unilateral vocal f31d paralysis patients.

A Comparative Study of Glottal Data from Normal Adults Using Two Laryngographs

  • A laryngograph was developed to measure the open and closed movements of vocal folds in our laboratory. This study attempted to evaluate its performance by comparing its glottal data with that of the original laryngograph. Ten normal Korean adults Participated in the experiment. Each subject produced a sustained vowel /a/ for about five seconds. This study compared f0 values, contact quotients of the duration of closed vocal folds over one glottal pulse, and area quotients of the closed over open vocal folds derived from glottal waves using both the original and new laryngographs. Results showed that the mean and standard deviation of the two laryngographs were almost comparable with a correlation coefficient 0.662 but minor systematic shift below those of the original laryngograph was observed. The absolute mean difference converged into 1 Hz, which indicates a possibility of adopting some threshold of rejecting inappropriate pitch values beyond a threshold value. The contact quotient of the normal subjects came out slightly over the 50% in a citation speech. Finally, the area quotient converged into 1. We will pursue further studies on the abnormal patients in the future.

Quantitative Analysis of Glottal Cycles According to Frequency and Intensity Variations in Normal Speakers (발성의 강도와 주파수 변화에 따른 성대 움직임의 정량적 분석)

  • To set up an objective basis for the evaluation of the stroboscopic findings, video-strobolaryngoscopic images of vocal fold vibration in 5 female and 5 male normal speakers were analyzed using an image analysis computer program called KSIP(Kay Storoboscopy Image Processing, Kay Elemetrics Corp., NJ, USA). Four consecutive vibratory cycles were compared in comfortable, louder, high-pitched /ee/ phonation for every subject. findings mostly replicated earlier studies including glottal chinks which were observed in most female speakers throughout the cycles and clear distinction between female and male speakers in their vibratory patterns as well as intensity and frequency-re-lated differences. However, there were some findings incompatible with those from previous studies which may be attributable to technical problems. This study may provide an objective basis of the stroboscopic findings such as image shape, amplitude, area, and their changes according to frequency and intensity variations. We anticipate that funker study with larger samples ran provide an objective criteria for normal vibratory characteristics of the laryngostro-boscopic findings.
