혈액투석을 위한 상완동맥-두정맥단락 수술 후 발생한 거대한 정맥의 동맥류 -1예 보고- (Giant Venous Aneurysm after Brachiocephalic Arteriovenous Shunt for Hemodyalisis Access -A case report-)

  • 우리는 만성 신부전 환자에서 혈액투석을 위한 상완동맥-두정맥단락술(brachiocephalic arteriovenous shunt)을 시행한 후 최근 빠르게 성장하는 아주 거대한 정맥의 동맥류 1예를 경험하였기에 보고하는 바이다. 환자는 48세 남자로 7년 전 본원에서 동정맥단락술을 시행한 후 혈액투석을 해왔으나 좌측 상완의 정맥을 따라 점점 성장하는 정맥의 동맥류와 박동성 통증으로 혈액투석을 지속할 수 없었다. 내원 1개월 전 혈액투석을 지속하기 위해 타 의료기관에서 우측 손목관절부에 새로운 동정맥루를 조성하여 혈액 투석을 하고 있었다. 동정맥류가 너무 거대하여 동정맥단락 재건술은 시도하지 않았으며 수술은 전신마취하에서 동정맥루를 결찰하고 확장된 정맥의 동맥류를 모두 제거하였다. 환자의 술 후 경과는 양호하였다.

  • Giant popliteal artery aneurysm is an uncommon entity. If untreated, it results in life-threatening complications. It is usually seen in older patients (over 60 years of age), and atherosclerotic disease is its main cause. Few cases have been reported in young adults, and its incidence in teenagers is exceptionally rare, with scarce case reports in the literature. We report a case of left popliteal artery aneurysm in a 16-year-old and its successful surgical treatment through resection and repair with a synthetic interposition graft.

Treatment for Giant Fusiform Aneurysm Located in the Cavernous Segment of the Internal Carotid Artery Using the Pipeline Embolization Device

  • The pipeline embolization device (PED) is a new endovascular device for treatment of complex, fusiform and wide-neck intracranial aneurysms. The main mechanism of this stent is to divert the flow in the parent artery with reduction of inflow in the aneurysm leading to thrombosis. We treated a 40-year-old woman who had left facial pain and orbit discomfort. Angiography showed a giant fusiform aneurysm located in the cavernous segment of the left internal carotid artery. A PED was successfully deployed across the aneurysm. The procedure and post-procedural course were uneventful. After 3 months, angiography showed complete obliteration of the aneurysm with good patency of the branching vessels originating from the deployed segment. The patient's symptoms improved completely without complications.

  • 거대 우관상 동맥류를 동반한 우관상 동맥에서 좌심실로 유출되는 관상 동맥루는 매우 드문 질환이며 동맥류 파열과 심부전, 심내막염으로의 진행 가능성이 있으므로 수술적 치료가 요구되는 질환이다. 본 증례는 47세 남자 환자가 3개월 전부터 시작된 운동 시 호흡 곤란을 주소로 내원한 환자로 흉부 전산화 단층 촬영과 관상동맥 조영술을 통해 거대 우관상동맥류 및 관상동맥-좌심실루로 진단되어 거대 관상동맥류 절제술 및 관상동맥 우회술을 시행하여 성공적으로 치험하였기에 보고하는 바이다.

  • Apatient, 51 years old woman, had suffered form headache and decrease of visual acuity. She had $3{\times}3cm$ sized giant aneurysm originated in cavernous and clinoid portion of left ICA(C4,C5) in the cerebral angiography. Before craniotomy, left CCA was exposed and 6F double lumen catheter was inserted in left ICA through the sheath. Pre-operative angiography was done. balloon catheter was positioned at the petrous portion of ICA. Eyebrow approach was done and giant aneurysm was exposed. The proximal blood flow was controlled with balloon dilatation and suction and decompression was tried, then multiple clips were applied. The loss of distal blood flow under intra-operative angiography was notified after clipping. The position of clips were repositioned to preserve blood flow & the rich flow was confirmed at distal part of clipping. In the post-operative cerebral angiography, the same finding was shown.

  • 관상동맥류는 드문 질환으로 대개 주관상동맥의 주행을 따라 심외막공간에서 발생한다. 본 논문에서 우리는 우연히 발견된 좌심실의 하기저벽에 생긴 거대동맥류 사례를 보고하고자 한다. 가와사키병이나 고혈압을 비롯한 과거력이 없는 43세 여자 환자가 두 달간 지속된 심계항진을 주소로 내원하였다. 심초음파와 심장 컴퓨터단층촬영에서 좌심실에 거대동맥류가 있었고 이것은 좌회선지와 직접 동맥루를 이루었으며, 좌회선지는 우관상동맥과 교통하였다.

  • The superficial femoral artery (SFA) is a relatively rare location for lower limb aneurysmatic disease. In the literature, this disease is described an association between a relatively high growth rate and/or the rupture of aneurysms and chemotherapeutic agents. We report a case of the rupture of a giant SFA aneurysm in a patient during chemotherapy for acute lymphatic leukemia.

Successful Obliteration of Unclippable Large and Giant Middle Cerebral Artery Aneurysms Following Extracranial-Intracranial Bypass and Distal Clip Application

  • Large to giant middle cerebral artery aneurysm is a challenging disease, especially when incorporating important perforating arteries. Surgical risk increases by perforator infarction and anatomical complexity. In this clinical setting, extensive consideration of surgical options is needed. The two cases described here were unruptured and had rather stable wall. Because of their large and giant size, hardness and incorporated arteries, it was not affordable to isolate them by means of clipping or trapping. The procedure as the alternative to conventional treatment modalities, extracranial-intracranial bypass followed by clipping of only the efferent artery successfully treated the aneurysms.

Development of 'De novo' Aneurysm after Therapeutic Carotid Occlusion

  • Carotid occlusion is an inevitable therapeutic modality for the treatment of complex aneurysms such as giant, traumatic, and intracavernous aneurysms. Late complications of carotid occlusion include 'de novo' aneurysm formation at a distant site because of hemodynamic changes in the circle of Willis. We report a case of de novo aneurysm in a vessel that appeared to be normal on initial angiography. The patient developed an anterior communicating artery aneurysm and marked growth of a basilar bifurcation aneurysm 9 years after trapping of the left internal carotid artery for the treatment of a ruptured large saccular aneurysm involving ophthalmic and cavernous segments. We propose that patients who undergo therapeutic carotid occlusion should be periodically followed by magnetic resonance angiography or computed tomographic angiography to evaluate the possibility of de novo aneurysm formation; this advice is in line with previous reports.

Endovascular Treatment of a Large Partially Thrombosed Basilar Tip Aneurysm

  • Despite the remarkable developments in neurosurgical and neuro-interventional procedures, the optimal treatment for large or giant partially thrombosed aneurysms with a mass effect remains controversial. The authors report a case of a partially thrombosed aneurysm with a mass effect, which was successfully treated by stent-assisted coil embolization. A 41-year-old man presented with headache. Brain computed tomography depicted an $18{\times}18$ mm sized thrombosed aneurysm in the interpeducular cistern. More than 80% of the aneurysm volume was filled with thrombus and the canalized portion beyond its neck measured $6.8{\times}5.6$ mm by diagnostic cerebral angiography. Stent-assisted endovascular coiling was performed on the canalized sac and the aneurysm was completely obliterated. Furthermore, most of the thrombosed aneurysm disappeared in the interpeduncular cistern was clearly visualized follow-up brain magnetic resonance imaging conducted at 21 months. The authors report a case of selective coiling of a large, partially thrombosed basilar tip aneurysm.
