요양병원 간호 인력의 간호행위 수행 실태 (A Survey of the Nursing Activities Performed by Nursing Staffs in Long-term care Hospitals)

  • 본 연구는 요양병원의 간호 인력의 간호행위 수행 실태를 파악한 조사연구이다. 연구 자료는 설문지를 이용하여 15곳의 요양병원에 근무하는 131명의 간호사를 대상으로 2013년 3월 7일부터 6월 30일까지 수집하였다. 자료 분석은 SPSS 17.0프로그램을 이용하여 기술통계 및 t-test, ANOVA를 시행하였다. '간호진단하기', '간호목표 설정하기', '환자평가표 작성'을 제외한 88항목의 간호행위를 다양한 범위 내에서 간호사와 간호조무사가 함께 수행하고 있는 것으로 나타났으며 기관의 총 환자 수와 간호사 수에 따라 간호조무사의 간호업무 수행정도는 유의한 차이가 나타났다(p<.001). 따라서 요양병원의 간호업무 중 위임할 수 있는 업무와 위임할 수 없는 업무를 분류하고, 위임에 대한 알고리즘 개발이 필요하며, 위임에 대한 법적, 제도적 규정을 마련할 필요가 있다.

노인유사체험이 노인병동 간호사의 노인에 대한 태도와 업무만족에 미치는 영향 (Effect of Senior Simulation on Geriatric Unit Nurses' Attitude and Job Satisfaction toward the Elderly)

  • 본 연구는 노인병동에서 근무하는 간호사를 대상으로 노인유사체험을 실시한 후 노인에 대한 태도와 노인간호 업무만족에 미치는 효과를 검증하고자 실시된 단일군 전후설계이다. 연구대상자는 대구광역시 소재 1개 종합병원 내 노인병동에서 근무하는 간호사를 대상으로 하였고 연구의 목적을 설명하고 참여를 허락한 30명을 대상으로 하였다. 자료수집은 2006년 8월 23일부터 9월 6일까지 이루어졌으며, 연구는 노인체험복을 착용하고 45분정도 일상생활을 체험하는 것으로서, 노인유사체험 전, 유사체험 직후, 유사체험 1주일 후에 노인에 대한 태도, 노인간호 업무만족과 유사체험 직후에 노인체험 경험을 묻는 질문으로 이루어졌다. 자료분석방법은 SPSS/WIN 12.0 통계 프로그램을 이용하여 분석하였고, 대상자의 일반적 특성과 노인 경험관련 특성은 서술통계를 이용하여 분석하였다. 대상자의 노인유사체험 전, 후, 일주일 후 노인에 대한 태도와 업무 만족도의 차이를 검증하기 위해 반복측정 분산분석(Repeated measures ANOVA)의 방법을 이용하였으며, 연구도구의 신뢰도를 보기 위해 Cronbach's α 계수로 분석하였다. 이상의 연구결과에서 볼 때, 간호사를 대상으로 노인유사체험을 실시한 결과 체험 전과 비교 시 체험 직후와 체험 1주일 후의 조사에서 노인에 대한 태도와 업무만족 모두 유의한 차이를 보여 노인유사체험이노인에 대한 태도와 노인간호 업무만족에 효과가 있음을 입증하였다. 따라서 노인유사체험은 간호사들이 노인 대상자를 간호할 때 공감적인 이해와 태도를 바탕으로 업무만족도를 높여 능동적인 간호를 가능하게 해주는 중재방법으로 사료되며 노인간호를 배우는 간호대학생이나 노인병동에서 노인을 간호하는 건강관리자들에게도 유용한 방법이 될 것으로 사료된다.

퇴원계획중재가 응급실 내원 호흡기 질환 노인의 퇴원준비도, 간호서비스 만족도, 재입원율에 미치는 효과 (The Effects of Discharge Planning for the Elderly with Pulmonary Disease in the Emergency Room)

  • Purpose: The purpose of this study was to examine the effects of discharge planning on patient satisfaction, the readmission rate and preparedness for discharge in the elderly admitted to the emergency room (ER) for pulmonary disease. Methods: A quasi-experimental intervention study design was used. Older adults with pulmonary health problems in the ER in one general hospital were randomly allocated to either an experimental (n=21, 74.2 years) or control group (n=19, 70.7 years). The experimental group participated in a discharge planning program by a geriatric nurse practitioner. Data were collected from medical records, physical measurements and structured questionnaires including information on demographics, patient satisfaction, readmission, and preparedness for discharge. Results: Participants in the experimental group had significantly better outcomes with regard to patient satisfaction with nursing services (p=.003) and preparedness for discharge (p=.034). However, there was no significant effect on the readmission rate (p=.392) Conclusion: The results suggested that a discharge planning program could bolster nursing service satisfaction and preparedness for discharge in older patients admitted to the ER for pulmonary health problems. To clarify the effects of discharge planning on older patients admitted to the ER, a larger sample population, better instruments for various measures, a new manual on discharge planning and frequent follow-up will be necessary.

요양병원 간호사의 의사소통능력, 직무스트레스가 이직의도에 미치는 영향 : 감성지능 조절효과 중심으로 (The Effect of Communication Competency and Job Stress of Long Term Care Hospital Nurses on Turnover Intention focused on control effect of emotional intelligence)

  • 본 연구의 목적은 요양병원 간호사의 의사소통능력, 직무스트레스가 이직의도에 미치는 영향을 알아보고, 의사소통능력, 직무스트레스 이직의도의 관계에 감성지능의 조절 효과를 검증하는 서술적 조사연구이다. 요양병원 간호사 240명을 대상으로 2018년 2월 1일부터 28일까지 이루어졌다. 자료분석방법은 IBM SPSS Win 20.0 프로그램을 이용하여 기술통계, t-test, ANOVA. 상관관계, 단계적 다중 회귀분석과 조절효과는 위계적 다중 회귀분석으로 분석하였다. 연구결과 이직의도는 의사소통능력과 직무스트레스에 따라 높게 나타났다. 이직의도는 직무스트레스와 양의 상관관계가 있는 것으로 나타났으며 이직의도에 가장 큰 영향을 미치는 요인은 급여만족도, 직무스트레스, 연령으로 나타났다. 본 연구 결과를 토대로 요양병원 간호사의 이직의도를 줄이기 위해서는 임상특성을 반영한 체계적인 프로그램 개발과 교육이 필요하다.

Knowledge, Experience, and Attitudes of Nurses at Long-Term Care Hospitals regarding Advance Directives

  • Purpose: The purpose of this study was to promote awareness of the need for advance directives (ADs) and to provide baseline data for the development of a nurse training program about ADs. Methods: Nurses at eight long-term care hospitals in Busan and South Gyeongsang Province (N=143) were recruited using the random sampling method from December 2018 to January 2019. Data were obtained using a structured self-reported questionnaire to assess their knowledge, experience, and attitudes regarding ADs. Data were analyzed in SPSS 22.0 using descriptive statistics, the t-test, analysis of variance, the Scheffé test, Pearson's correlation coefficient, and stepwise multiple regression analysis. Results: The mean scores were 7.79±1.39 points for knowledge, 1.92±2.00 points for experience, and 2.80±0.24 points for attitudes regarding ADs. Knowledge and experience (r=0.32, P<0.001) had a positive correlation with knowledge and attitudes (r=0.17, P=0.39). Conclusion: According to the results, nurses generally had a high level of knowledge regarding ADs, which resulted in a positive attitude toward ADs. However, they had little experience with ADs. Therefore, nurses' must develop both direct and indirect experience with ADs using a practical training program to strengthen their clinical competency regarding ADs.

우리나라 가정간호제도화 방안에 관한 고찰 (A Consideration on the Instituting Home Health Care in Korea)

  • While the socioeonomic status of Koreas has been dramatically increasing in recent years, chronic and geriatric diseases have also been on the rise, bringing about many changes in our health care system. The basic goals of the home health care are to reduce health care costs, to increase the attrition rate in general hospitals, and to care for patients effectively and conveniontly at home. The purpose of this paper is to review and examine the current status of the home health care in Korea throughout the reports, surveys, other informations and education system of home health nurse. We identified the various types of home health care services programs, such as hospital-based home health care operated in public sector(demonstration project) and community-based home health care in health centers or in private sector, that is, Korean Nurse Association. Hospital based home heatlh care model was established as an alternative to traditional in-patiet services. Quality assurance and client satisfaction is an important measure of care received and establishment of payment and reimbursement for home health care services is important in promotng the home health care. We found out a fee-per-visit system composed of three kinds of fees : a basic service fee(16,000 Won), a travel fee(5,000 Won), and per-service fees (variables). Like fees paid for in-patient care, insureds pay 20% and insurers pay 80% of the basic and per-service fee. The travel fee is borne totally by the insured. Home health care continues to be viewed as not only the most preferred way to provide care to clients, but also the most cost effective. Home health care is that component of a continuum of comprehensive health care whereby health services are provided to individuals and families in their places of residence for the purpose of promoting, maintaining, or restoring health, or of maximizing the level of independence, while minimizing illness. Services appropriate to the needs of the individual patient and family should be planned and provided, nursing is to be a force for positive change and enhanced the nursing professionalism. Whatever type of involvement of home health care, it is essential to remember that home health care is highly service-oriented and highly touch health car deilvery system.

간호사의 섬망 지식수준과 섬망 사정에 대한 연구 (A Study of Nurses' Knowledge Level and Assessment Experience of Delirium)

  • Purpose: This study was conducted to examine the knowledge level and assessment experience of nurses in regards to delirium, and to utilize the study results as baseline data for planning delirium education programs for nurses. Methods: Subjects were 465 nurses who were working in a general hospital. A 'delirium related knowledge and assessment experience' questionnaire was used to collect data. Results: Knowledge levels regarding delirium averaged 70 out of 100, and at each domain, they scored 87 for etiology of delirium, 62 for symptoms, and 69 for nursing management. The knowledge level of delirium was significantly different according to educational level (F=3.851, p=.022), past education related to geriatrics(t=2.471, p=.014), and awareness of need for in-service education on geriatric nursing(F=2.663, p=.032). About 85% of nurses had past experience of dealing with delirious patients and only 7.7% of nurses used delirious state assessment tools. Conclusion: According to the above results, it is necessary, not only to provide delirium related educational programs for nurses, but also to emphasize the usefulness of applying the assessment tool.
