Purpose: This study aimed to explore the level of importance perceived by caregivers of elderly on the education of long term care nursing assistants (LTCNAs) taking care of elders with dementia or stroke. Method: Data was collected from 296 participants (112 families, 98 NAs, and 86 RNs) from October 2006 to February 2007. A structured questionnaire was used for data collection. Result: The item of 'attitude while caring for the aged' was identified as most important. The family group reported that 'position change' and 'understanding of geriatric diseases' were the most important for education, while LTCNAs, highly identified, 'bathing', and 'bed sheet change'. The RNs group regarded 'position change', and 'bathing' as most important. Institutions demanding LTCNAs' activities were special facilities for elderly care and special hospitals for the aged, and the need was also high in families with an elder incapacitated by illness. Conclusion: Nurses need to take an interest in education for enhancing the quality of LTCNAs and thus advance the quality of nursing care as well as the quality of life for the aged.
Recently the Elderly with dementia is increasing in Korea with rapid aging of the population. In addition to increasing a number of nuclear family and women's advancement in society, people are less capable of supporting family with dementia. In response, the Seoul city increased welfare facilities for the elderly with dementia about two times for one year. I think that the quantitative portion of facility get to meet the requirements but it is lack of qualitative one in fact. Therefore, the space for the elderly can keep up with the falloff of mental and physical functions, dementia and geriatric diseases according to aging and careful consideration about the space considering stability and convenience is required. At this point, this study has the purpose to plan a daycare center which reflects the characteristics of the elderly to protect them within the community safely and comfortably. As for the research method, case study was conducted to understand the overall situation on the space plan of the daycare center for 3 daycare centers located in Seoul and found out problems. From the generalization of the above study, a space plan for a daycare center considering the characteristics of the elderly was provided. The composition of Dementia welfare facility should be carefully planned to fit characteristics of the patient's behaviors.
This study aims to explore the difference in life satisfaction and daily activities of elderly solitaries through a survey conducted from May to June 2004 to 152 who hare moi the criteria set forth for this study. Used for the survey were such tools as 11 items of questions concerning characters of the subjects, Life Satisfaction and Daily activities(K-ADL). The outcomes could be summarized as follows: 1. The majority of 152 subjects are females, aged $70{\sim}79$, living in the urban areas, non-educated with miscellaneous religions(including non-believers), jobless, self-dependent (or living expenses, without leisure, with disease, suffering from sickness despite of treatment and, thus not self-confident in health. 2. The Life Satisfaction and K-ADL of all subjects appear in general to be good with $20.03{\pm}9.74$ and $8.25{\pm}2.74$, respectively. 3. The difference in Life Satisfaction by characters of the subjects is found significantly high in those who are females, living in rural area, educated, with religions and jobs, have diseases treated and currently confident in health. 4. The difference in K-ADL by characters of the subjects is significantly high for those who are older than 80, Buddhists, jobless, affordable with living expenses by themselves, sick currently and not confident in health. Thus, it is thought vastly important to help the elderly solitaries to realize quality of life by social (including family) assistance, economic self-reliance and health promotion and prevention. Measures drawn from the analysis into issues of the elderly solitaries from various aspects may definitely contribute to avoiding and complimenting the causes of various issues relating to the elderly people and to assisting them to cope with the problems in the highly geriatric society to come.
Background: Tuberculosis (TB) is an infectious disease caused by Mycobacterium tuberculosis that can affect many organs of the body but usually affects the lungs. The prevalence of TB in Korea is considerably higher than that in other countries with similar economic levels, and is much higher in elderly people. Pharmacotherapy is important in the treatment of TB and requires relatively high compliance for a prolonged duration. Methods: We analyzed sample data of elderly patients obtained from the Health Insurance Review and Assessment Service. We used logistic regression analysis and frequency analysis to identify factors that could affect prevalence of TB in elderly patients, compliance with prescribed medication regimes in these patients, and use of medical institutions. Korean Standard Classification of Diseases, version 7 (KCD-7) was used to diagnose pulmonary TB, and medications were analyzed using Korean standardized drug classification codes. Results: 1,276,331 patients were analyzed in the sample of the elderly population, and 16,658 TB patients were included in the study. The mean age of the TB patients was 76.19 years (SD 6.899). A total of 699 patients were prescribed isoniazid, rifampicin, ethambutol, or pyrazinamide at least once. Of these, 352 (50.4%) were prescribed all four medications and 101 (14.4%) were prescribed only isoniazid, rifampicin, and ethambutol. The mean duration of prescription was 28.75 days (SD 36.13). Conclusion: In the elderly population, old age and poor socioeconomic conditions correlated with TB prevalence. Most patients did not meet the criteria for effective pharmacotherapy of TB.
This study examined two differences in physical and psychological recovery patterns after surgery in the elderly. The sample consisted of 40 patients with abdominal surgery In five large hospitals in Seoul. The data for this study were collected from Apr. 20 to Nov. 26 by structured questionnaire, chart review and call. Physical recovery was assessed by ADL, a Cantril Ladder Scale and a Visual Analogue Scale. Psychological recovery was measured by the Geriatric depression Scale and a Cantril Ladder Scale. The data were analyzed using frequency, percentage, Pearson Correlation Coefficient, and MANOVA by SPSS/WIN. The result are as follows : 1. Physical recovery indicated significant improvement over time with the exception of ADL(F=.812 p=.449). Perceived physical health were significantly improved(F=6.189 p=.004). Pain & discomfort was significantly decreased(F=3.927 p=.025). 2. Perceived psychological health was significantly improved over time(F=20.648 p=.000), but depression showed no statistical significance improvement over time(F=1.393 p=.256). 3. There were no significant effects of sex, age, complication and combined chronic diseases on physical and psychological recovery patterns. 4. There were significant correlations between operation time and pain(r=-.331 p=.020), recovery time and perceived psychological health(r=-.320 p=.024), recovery time and pain(r=.404 p=.005). There were significant correlations between admision period and ADL(r=-.418 p=.004), perceived physical health(r=-.354 p=.014), depression(r=.280 p=.042), and perceived psychological health(r=-.447 p=.002). BRAS showed significant correlation with ADL(r=-.458 p=.002). 5. With an increase in the degree of perceived health(physical and psychological), ADL was significantly increased. With an increased in the degree of depression and pain, ADL and perceived health(physical and psychological) were significantly decreased. In conclusion, the elderly patient recovered significantly over time with the exception of ADL and depression. It these we suggested to considered when planning care for elderly patients.
Purpose: An exercise program, which was composed of motions simple and easy to follow, was developed and applied to aged women as a way to improve their health conditions. Method: To ascertain the effects of the exercise, which was conducted twice a week and 16 times in total for 8 weeks from September 10 to November 5, 2005 in two experimental groups (31 Persons) and a control group (31 persons), this study conducted quasi-experiments of nonequivalent control group pre-post test design. Data were analyzed using SPSS/PC Win 10.0, the reliability of the measuring tools using Cronbach's alpha, the verification of equivalence for general properties and dependent variables using Chi-square test and t-test, and the verification of hypotheses using t-test and ANCOVA. Results: After the exercise program. the aged women showed profitable evaluation of decisional balance, high feeling of self-efficacy as well as improved subjective awareness of health. Their diastolic blood pressure and total cholesterol were also reduced significantly after the exercise. Conclusion: After the conduct of the exercise program for 8 weeks, aged women showed increased profitable decisional balance, self-efficacy and subjective awareness of health. This suggests that continuous and proper exercise is effective as an intervention for health improvement for the aged as well as the prevention of and recovery from geriatric diseases.
Death is one of the population movement phenomena used as an important health index in most society. Especially it is regarded as group phenomenon in a specific group rather than individual one and considered important in public health field because the level and cause of death is related to health of public. The auther examined the changes of the Korean mortality pattern to evaluate the status of the Korean public health by studying mortality indicator using the population census and other materials from 1930 to 1980. The results are as follows: First, the Korean crude death rate was reduced to one third in 1980 compared to that in 1930, but the crude birth rate was constant from 1930 to 1960 causing the increase of population. So the population pattern is changing from the classic pyramic shape to bell shape and the dependency ratio was reduced from 78 in 1930 to 61 in 1980. Second, the infant mortality rate decrease rapidly. In 1980 it was one seventh of that in 1930 which was proved by the change of the age-specific death rate curve from U to J shape. Third, the male mortality reduction after the age of forty was much less than that of female, which explained the specific death pattern of high mortality in Korean middle and oldaged males. Fourth, the main cause of death was changed from infectious, parasitic, respiratory and digestive system disease to circulatory of tumorous diseases. Considering the above results, Korean health problem is now changing from the infant infection to geriatric chronic regressive disease. That naturally the direction of health service should be turned from the infant stage maternal and child health to the health problems of old people.
Background: The risk factors of nocturia in older adults remain unclear. We aimed to investigate factors associated with nocturia using the National Health and Nutrition Examination Survey (NHANES) data. Methods: Among 40,790 participants, 4,698 participants aged ${\geq}65$ years were included from the NHANES dataset between 2005 and 2012. A multivariate logistic regression analysis was performed to determine the odds ratio (OR) for nocturia. A subgroup analysis was conducted based on sex and underlying diseases. Results: In the multivariate logistic regression model, obesity (OR, 1.46; 95% confidence interval [CI], 1.28-1.68), hypertension (OR, 1.28; 95% CI, 1.07-1.52), and diabetes mellitus (DM) (OR, 1.27; 95% CI, 1.11-1.45) were significantly associated with nocturia. These factors were associated with nocturia regardless of sex. In a subgroup of participants with hypertension, obesity (OR, 1.44; 95% CI, 1.25-1.67) and DM (OR, 1.26; 95% CI, 1.09-1.45) were associated with nocturia. In the additional analysis on patients with DM, nocturia was associated with obesity (OR, 1.33; 95% CI, 1.06-1.67) and duration of DM (OR, 1.02; 95% CI, 1.01-1.03). Conclusion: This study demonstrated that hypertension, DM, and obesity were significantly associated with the prevalence of nocturia in older adult patients regardless of sex. In particular, obesity was associated with nocturia in every subgroup analysis.
Xerostomia is defined as the subjective complaint of dry mouth with or without hyposalivation, which is insufficient salivary secretion from salivary gland. Xerostomia can lead to multiple oral symptoms such as dental caries, halitosis, burning mouth syndrome, and oral candidiasis, which can significantly impact the well-being of patients, especially in geriatric patients who may already have compromised health. Clinical findings of xerostomia include decreased salivary flow and alterations in salivary composition. These changes can lead to various oral health problems such as dental caries, periodontitis, swallowing and speaking difficulties, taste disturbances, halitosis, mucosal diseases, and burning mouth syndrome. Recognizing these clinical manifestations is essential for early diagnosis and appropriate management. Although several reasons and risk factors have been suggested for xerostomia such as aging, chemo-radiation therapy, systemic disease, and Sjögren's syndrome, the polypharmacy is recently highlighted especially in elderly patients. Understanding the etiology and risk factors associated with xerostomia is crucial for effective management. To manage xerostomia patients, a multidisciplinary guideline should be established beyond dental care. Through this literature review, we summarized consideration for diagnostic, therapeutic, nursing essentials for the clinical guideline. By addressing the underlying causes and implementing appropriate treatment strategies, healthcare professionals can improve the quality of life for individuals suffering from xerostomia.
This study is based on the utilization of pine mushroom for processing products by development of pine mushroom Jung-Gwa. The results were summarized as follows: Pine mushroom Jung-Gwa were prepared with four different amounts (20, 30, 40, 50%) of honey. Pine mushroom Jung-Gwa color value was the highest by 20% soaking honey-water product and 40% soaking honey-water product was the lowest. Strength and hardness of 20% soaking honey-water product was the highest. Appearance to product of 50% soaking honey-water was the highest preference by all age of except 20's. The honey content of more and more external appearance preference was became higher. Product of 40% soaking honey-water was best by flavor of pine mushroom Jung-Gwa. A sugary tastes comparison 50% soaking honey water to 40% soaking honey water was not distinction. The texture was appeared a statistically significant difference by products of 20%, 30% soaking honey-water. Product of 20% soaking honey-water was favorite in 20's and product of 30% soaking honey-water was favorite in 30's. In overall preference test for pine mushroom Jung-Gwa of 40% soaking honey water was preferred by all age. Oligosaccharide helped geriatric diseases and cheaper. Thus this study used oligosaccharide but the result was not a statistically significant difference of pine mushroom Jung-Gwa products quality by comparison honey-water to oligosaccharide. Therefore oligosaccharide utilization products was thought worth a lot more by processing price and functional.
