Purpose: Elderly people are thought to be more susceptible to periodontal disease due to reduced immune function associated with aging. However, little information is available on the nature of immune responses against putative periodontal pathogens in geriatric patients. The purpose of this study was to evaluate the serum IgG antibody responses to six periodontal pathogens in geriatric subjects. Methods: The study population consisted of 85 geriatric patients and was divided into three groups: 29 mild (MCP), 27 moderate (MoCP), and 29 severe (SCP) chronic periodontitis patients. Serum levels of IgG antibody to Porphyromonas gingivalis, Tannerella forsythia, Treponema denticola, Aggregatibacter actinomycetemcomitans, Fusobacterium nucleatum, and Prevotella intermedia were measured by enzyme-linked immunosorbent assay (ELISA) and compared among the groups. Results: All three groups showed levels of serum IgG in response to P. gingivalis, A. actinomycetemcomitans, and P. intermedia that were three to four times higher than levels of IgG to T. forsythia, T. denticola, and F. nucleatum. There were no significant differences among all three groups in IgG response to P. gingivalis (P=0.065), T. forsythia (P=0.057), T. denticola (P=0.1), and P. intermedia (P=0.167), although the IgG levels tended to be higher in patients with SCP than in those with MCP or MoCP (with the exception of those for P. intermedia). In contrast, there were significant differences among the groups in IgG levels in response to F. nucleatum (P=0.001) and A. actinomycetemcomitans (P=0.003). IgG levels to A. actinomycetemcomitans were higher in patients with MCP than in those with MoCP or SCP. Conclusions: When IgG levels were compared among three periodontal disease groups, only IgG levels to F. nucleatum significantly increased with the severity of disease. On the contrary, IgG levels to A. actinomycetemcomitans decreased significantly in patients with SCP compared to those with MCP. There were no significant differences in the IgG levels for P. gingivalis, T. forsythia, T. denticola, and P. intermedia among geriatric patients with chronic periodontitis.
Background: This study involved a geriatric oral care expert who developed the competencies of students in the Department of Dental Hygiene and conducted preliminary research to develop an effective curriculum. Methods: A questionnaire was conducted in the last week of class targeting students who took courses in geriatric dentistry, geriatric dental hygiene, and practice. In order to confirm the educational effect according to the differences in the geriatric dental hygiene curriculum, the differences in students' achievement of major competencies, awareness of the geriatric dental hygiene process, class satisfaction, and ageism were analyzed. Results: Regarding major competency attainment, 'communication competency' was significantly higher in PBL education that combined theory and practice than that theory-oriented PBL education (p=0.038). For ageism, the tendency to avoid older adults was low in PBL education, which combined theory and practice, and was statistically significant (p=0.040). For class satisfaction, the rate of responding 'very high' for the 'class atmosphere' was significantly high (p=0.025) for PBL education that combined theory and practice. Conclusion: The PBL teaching method can be useful as a geriatric dental hygiene curriculum. However, it would be more effective to create a curriculum so that education in geriatric dental hygiene care practice can be combined with theory rather than a theoretical education alone.

The aim of this systematic review was to determine the success rate of nitrous oxide-oxygen procedural sedation (NOIS) in dentistry. A systematic digital search was conducted for publications or reports of randomized controlled trials evaluating the clinical performance of NOIS. Abstracts of research papers were screened for suitability, and full-text articles were obtained for those who met the inclusion and exclusion criteria accordingly. The quality of the studies was assessed using the revised Cochrane risk-of-bias tool (RoB 2). A total of 19 articles (eight randomized clinical trials with parallel intervention groups and 11 crossover trials), published between May 1988 and August 2019, were finally selected for this review. The studies followed 1293 patients reporting NOIS success rates, with a cumulative mean value of 94.9% (95% CI: 88.8-98.9%). Thirteen trials were conducted on pediatric populations (1098 patients), and the remaining six were conducted on adults (195 patients), with cumulative efficacy rates of 91.9% (95% CI: 82.5-98.1%) and 99.9% (95% CI: 97.7-100.0%), respectively. The difference was statistically significant (P = 0.002). Completion of treatment and Section IV of the Houpt scale were the most used efficacy criteria. Within the limitations of this systematic review, the present study provides important information on the efficacy rate of NOIS. However, further well-designed and well-documented clinical trials are required and there is a need to develop guidelines for standardization of criteria and definition of success in procedural sedation. Currently, completion of treatment is the most used parameter in clinical practice, though many others also do exist at the same time. To maximize NOIS efficacy, clinicians should strictly consider appropriate indications for the procedure.
Eating-swallowing disorder (dysphagia) is a very important functional problem for the elderly, and it has a significant impact on the quality of their life. Because the eating and swallowing processes are affected by oral tissues including teeth, tongue, and oral muscles, it is natural that the dentist as a specialist in oro-facial region, intervenes in the diagnosis and treatment of the disorder. Nevertheless, Korean dentists still lack interest and understanding of dysphagia. In aged society, it is necessary that the dentists understand the functional disorders as well as oral diseases. The purpose of this study is to introduce the evaluation and treatment methods of eating and swallowing functional disorders in order to cope with eating-swallowing disordered patients who are frequently encountered in aged society.
The age structure has been experiencing substantial change due to the decreased birth rate as well as the increased life expectancy. The prediction that proportion of elderly people in need of oral health care will increase substantially is a new challenge to dentists in the future. The old paradigm that the aged person is just the person who was born earlier and needs the same conventional oral health care should be shifted to the new one. Elderly people tend to express their political interest related with health care system by actively participating in the national elections. The need to sustain economic status for the extended life span makes them seek eagerly esthetic health care to maintain sound social function. Most of them are under multiple chronic diseases and take related medicines. In addition, many studies report about mental change as well as physical change among the aged people. Since the prevalence of dental diseases among the aged is higher than other chronic devastating diseases, the aged seeking oral health care will increase. The aged who has different physical and psychological status as well as chronic disease and related medicine will show unexpected response to the conventional oral health care. In addition, the impact of tooth loss is substantial physically, mentally and emotionally. Dentist should prepare different approaches for the elderly dental patient.
Objectives: The purpose of the study was to investigate the relationship between oral health perception and oral health-related quality of life-14(OHIP-14) in a geriatric hospital. Methods: A self-reported questionnaire was completed by 230 elderly people in a long-term care institution. The questionnaire was consisted of items on general characteristics, oral health perception and OHIP-14. Except incomplete answers, 226 data were analyzed statistically with one-way ANOVA, t-test, Pearson's correlation coefficient and post hoc Tukey test and ANOVA when significance was set at 0.05. Results: The OHIP-14 showed a negative linear relationship(p<0.01) of r=-0.272 with the oral health perception. Among the subgroups of the OHIP-14, quantitative linear relationship(p<0.01) was shown in functional limitation(r=0.822), physical pain(r=0.825), psychological discomfort(r=0.568), physical disability(r=0.895), psychological disability(r=0.652), social disability(r=0.804) and handicap(r=0.818). Conclusions: In order to improve the oral health perception and OHIP-14 in a geriatric hospital, it is necessary to develop continuously a variety of oral health education and systematic oral health promotion program.

The aim of this article was to review age-dependent clinical recommendations for appropriate root canal instrumentation techniques. A comprehensive narrative review of canal morphology, the structural characteristics of dentin, and endodontic outcomes at different ages was undertaken instead of a systematic review. An electronic literature search was carried out, including the Medline (Ovid), PubMed, and Web of Science databases. The searches used controlled vocabulary and free-text terms, as follows: 'age-related root canal treatment,' 'age-related instrumentation,' 'age-related chemo-mechanical preparation,' 'age-related endodontic clinical recommendations,' 'root canal instrumentation at different ages,' 'geriatric root canal treatment,' and 'pediatric root canal treatment.' Due to the lack of literature with practical age-based clinical recommendations for an appropriate root canal instrumentation technique, a narrative review was conducted to suggest a clinical algorithm for choosing the most appropriate instrumentation technique during root canal treatment. Based on the evidence found through the narrative review, an age-related clinical algorithm for choosing appropriate instrumentation during root canal treatment was proposed. Age affects the morphology of the root canal system and the structural characteristics of dentin. The clinician's awareness of root canal morphology and dentin characteristics can influence the choice of instruments for root canal treatment.
In this study 240 geriatric patients, aged 60 year and over were evaluated using a questionnaire and oral examination for their oral status, demand and satisfaction of prosthesis treatment. 1. The single main complaint was regarding from the existing removable prosthesis. When the complaints were catagorized into larger groups, complaints regarding existing prosthesis were 32% and followed by 30% in caries and endodontic problems. 2. According to the result from questionnaire, 48% of patients wearing removable prosthesis and 43% with fixed prosthesis were not satisfied with existing prosthesis. Thirty-seven percentages of patients were not satisfied with aesthetics and unable to chew food properly. 4. Thirty-nine percentage of patients were wearing removable prosthesis. More complete dentures were found on maxilla and partial dentures on mandible. 5. Seventy-seven percentage of patients requested for new prosthesis and the majority of these patients had complaints of difficulties in chewing and discomfort.
A majority of patients who sustain injuries to the peripheral sensory nerves of the face and jaws experience a slow but gradual return of sensation that is functional and tolerable, if not the same as before the injuries. However, long-term effects of such injuries are aggravating for many patients, and a few patients experience significant suffering. In some of these patients, posttraumatic symptoms become pathological and are painful. The predominant painful components are (1) numbing anesthesia dolorosa pain, (2) triggered neuralgiaform pain, (3) burning and aching causalgiaform pain, and (4) phantom pain. This is a case report of conservative management of traumatic neuralgia and neuritis as part of posttraumatic pain syndromes in geriatric patients who have undergone the teeth extraction and alveoloplasty.
The geriatric population is the most rapidly growing part of the general population, which leads to an increase of chronic conditions and illnesses that will influence both oral and systemic health. The most common chronic diseases in elderly population are arthritis, hypertension, heart diseases, diabetes, and cognitive impairment. Chronic impairments such as visual, hearing, orthopedic, and speech disorders are also prevalent among older adults. All these chronic conditions have potential oral complications and the treatments of systemic diseases also has implications for the maintenance of oral health. Therefore, oral health care professionals should catch up with increasing knowledge in this field and can modify the treatment strategy for older adults. In addition, oral health care professionals should understand the changing need of oral health problems in this age and prepare the future demand.
