일부 노인의 치과보철물 상태가 구강건강관련 삶의 질에 미치는 영향 (The effects of dental prostheses to the quality of life among the elderly)

  • 연구목적: 사회복지관 이용 노인들을 대상으로 치과보철물 상태를 역학적으로 조사하고 이러한 치과보철물 상태가 구강건강관련 삶의 질에 미치는 영향을 살펴봄으로써, 향후 노인들의 치과보철물과 관련된 연구에 필요한 기초자료를 제공하고자 하였다. 연구 재료 및 방법: 대전광역시 소재 7개 사회 복지관을 이용하는 60세 이상의 노인 표본 275명을 대상으로, 주관적으로 느끼는 구강건강인식 및 삶의 질에 대한 설문조사와 치아 및 치과보철물 상태에 대한 구강검사를 실시하여 통계 분석하였으며, 삶의 질은 노인구강건강평가지수 (Geriatric Oral Health Assessment Index; GOHAI)를 이용하여 측정하였다. 결과:1. 연령이 낮을수록, 교육수준이 높을수록, 주관적인 경제상태가 좋을수록, 부부가 함께 사는 가족형태에서GOHAI는 높게 나타났다. 2. 가철성 의치를 처음 사용한 평균연령은 62.11세였고 의치의 평균사용수명은 10.76년이었다. 전체 조사 대상자의 57.5%가 가철성 의치를 사용하고 있었으며 총의치 사용자는 전체의 13.8%이었다. 3. 가철성 의치를 사용하지 않고 고정성 보철물로 수복한 경우나 총의치에서 편악 보다는 상 하악을 함께 총의치로 사용하는 경우에 저작 불편감과 발음문제를 적게 느끼며 저작능력지수와GOHAI는 높게 나타났다. 4. 의치 만족도가 높고, 항상 의치를 사용할 수 있으며 새로운 의치의 필요성이 없고 의치적합도가 양호한 경우에 GOHAI는 높게 나타났다. 결론: 본 연구의 결과로 볼 때 구강건강관련 삶의 질에 가장 관련이 있는 것은 저작이 가능한 기능 치아를 구강 내에 많이 가지고 있는 것이었다. 자연치아의 상실이 많은 노인의 구강건강관련 삶의 질을 향상시키기 위해서는 적절한 치과보철물을 통하여 치아를 수복하여 줌으로 정상적인 저작기능을 회복하여 주는 것이 필요함을 보여준다.

  • Objectives: The purpose of the study was to investigate factors affecting the assessment of objective and subjective masticatory ability in the elderly, and to evaluate masticatory ability assessment more accurately. Methods: A total of 112 participants were recruited after oral examination in senior citizen welfare facilities. The participants' masticatory ability was evaluated objectively (Mixing ability index; MAI), and subjectively (Key food intake ability; KFIA). Participants' general characteristics and oral health-related variables were also recorded. Based on masticatory ability assessment, participants were classified as either high or low. IBM SPSS Statistics Ver.23.0 was used for all analyses, including descriptive statistics, Chi-square test, Mann-Whitney U test, Spearman rank correlation, and Logistic regression analysis. Results: Higher masticatory ability was positively correlated with higher scores on MAI and KFIA. Additionally, there was a significant positive correlation between MAI and KFIA. When analyzing factors affecting objective and subjective masticatory ability assessments, Functional tooth units (FTUs) were revealed as a related factor. In subjective masticatory ability assessment, oral moisture, difficulty in chewing, and the Geriatric Oral Health Assessment Index (GOHAI) were also influential factors. Conclusions: In order to accurately assess masticatory ability, it is necessary to use both objective and subjective measures. Additionally, to improve the masticatory ability in the elderly, treatment should be provided to improve overall oral health and satisfaction.

대도시 노인의 구강건강관련 삶의 질에 관한 연구 (A study on oral health-related quality of life of among elderly in metropolis)

  • Objectives : The present study purposed to survey the oral health-related quality of life among elderly in metropolis Methods : We had individual interviews and oral survey using a questionnaire with 336 elderly who were using seniors' centers in Metropolitan City during the period from the $9^{th}$ to $31^{st}$ of July, 2007. Results : Factors of GOHAI showed the significant relation with types of health insurance, a large number of medication, perceived oral health status, perception of dental care needs those with pains in the temporomandibular joint, the number of natural teeth and the explanatory power or the final model was 25.5%. Factors of OHIP-14 showed the significant relation with types of health insurance, a large number of medication, perception of dental care needs those with pains in the temporomandibular joint, gingival bleeding, the number of natural teeth and the explanatory power or the final model was 26.6%. Conclusions : Oral health-related quality of life among elderly as factors by general characteristics of study subjects, by perceived heath status and oral health status, perceived oral symptoms and oral health status that were relevant. Accordingly, for the healthy maintenance of elders' natural teeth, it is considered necessary to develop and execute continuous oral health management systems and oral health education programs that promote preventive activities and enhance the perception of oral health.
