This thesis is a kind of case report on the managerial difficulties and strategies to overcome them for the rural hospitals in Korea. We examined environmental situations and SWOT(Strength, Weakness, Opportunity, Threat) of the hospitals, and suggested some strategies to improve the hospital business through analysing secondary data and brief survey to staffs and patients at 3 case hospitals. Among the strategies were included establishment of their identity as community hospitals, positioning towards high-touch services rather than high-tech services to improve their competence. For this positioning, patient satisfaction should be emphasized to make rapport with the residents in the community, and a few service areas should be selected and strengthened. Emergency care and geriatric health services are recommended to be covered strongly, because rural hospitals are usually the one and only hospital in the region and rural communities have aged population structure. In addition to these, networking is necessary among rural hospitals nationwide and with local clinics and tertiary hospitals near urban areas. And also, complementary role division with community health post should be developed to lessen the competitive relationship between public and private sectors. To support these strategies, fresh organizational culture should be built up to make hospital staff creative and enthusiastic on their job. Finally, inducement of governmental assistance will be necessary to make the environment advantageous to the rural hospitals.

Purpose : The purpose of this study was to examine the nurses' needs for the spatial composition of geriatric hospital. Methods : The survey method was used for collecting the data. The subjects were 110 nurses who worked at 6 geriatric hospitals in Busan. They answered the questionnaire by the self-administered method. The questionnaire consisted of items which asked the spatial composition of patient room, bathroom, dining space, resting space, care-givers' room, and garden. Results : 1) The patient room using a bed on ondol floor would be useful. Also, it would be desirable that most patient rooms consisted of 4 or 6 persons per patient room and the others were 1 or 2 persons per room. 2) The bathroom for only patients and the shower room for their families or care-givers are needed in the geriatric hospital. 3) The dining room for patients in each floor would be useful to old patients. In addition to that, the cafeteria for their families or care-givers are needed. 4) The resting space for patients and their families, for the staff, and for care-givers are needed. 5) The personal locker and refrigerator are necessary fixtures in care-givers' room. Also, table, sofa, sink, and shower booth are also needed. 6) On the base of activities of care-givers, the space for preserving, washing, and heating foods are needed. Also, the storage space for personal items and the fitting space are needed. Implications : The results of this study would be the fundamental data for space planning of the geriatric hospital.

Purpose: We investigated the association between empathy for the elderly, the nursing work environment, person-centered care, and geriatric nursing practice among university hospital nurses and factors that affect geriatric nursing practice. Method: We administered questionnaires to 178 nurses between May 23 and June 21, 2022 at a university hospital in city D. We used descriptive statistics, the t-test, one-way analysis of variance, the scheffé test, Pearson correlation coefficients, and stepwise multiple regression analysis for data analysis. Results: We observed a statistically significant positive correlation between nurses' geriatric nursing practice and empathy for the elderly, the nursing work environment, and person-centered care. Empathy for the elderly was positively correlated with the nursing work environment and person-centered care, and the nursing work environment showed a statistically significant positive correlation with person-centered care. Empathy for the elderly and the nursing work environment significantly affected geriatric nursing practice. The overall explanatory power of the regression model was 33.4%. Conclusion: Greater awareness and implementation of various educational programs that promote empathy for the elderly are necessary to improve nursing care for the elderly population. Furthermore, adequate manpower and material support are important to improve the nursing work environment among nurses employed at university hospitals.

Purpose: The purpose of this study was to compare functional status and the level of health care needs in elderly Koreans in health care institutions. Methods: Data were collected from 2,521 elderly patients admitted in 50 health care institutions (hospitals, oriental hospitals, geriatric hospitals, and health care facilities) selected through proportional stratified sampling in 2008. We used a long-term care (LTC) assessment tool developed by the government, which consists of 52 items with 5 subscales. Results: Statistically significant differences were detected in functional status and the level of health care needs among the subjects in different health care institutions. Of the sample population in geriatric hospitals, 48.9% were eligible for LTC Category 1, 20.9% for Category 2, and 17.2% for Category 3, and 13.1% of the subjects were not eligible for any of the categories. Of the sample population in health care facilities, 29.9% were eligible for LTC Category 1, 20.5% for Category 2, and 21.8% for Category 3, and 27.0% of the subjects were not eligible for any of the categories. Conclusion: The findings of this study indicate the necessity of the development of an evaluation system that helps determine whether a subject is eligible for LTC.

Besides, the relation between Invested capital and Investment efficiency was opposite each other in the non-profit hospitals, and Region was an important element influencing over Productivity per Value Added. In addition, in the investment activity of non-profit hospitals in the light of Investment Efficiency, only hospitals among Medical Institution types had a character of Capital Intensive, and General Hospital and Geriatric & Long-term Medical Care Hospital among Medical Institution types showed a character of Labor Intensive in the light of Performance.
Purpose: We conducted a descriptive correlational study to determine a relationship between nurses' awareness of good death and attitudes toward terminal care, which in turn could be used as basic data for improvement of the quality of terminal care at geriatric hospitals. Methods: From April 3, 2013 through April 22, 2013, data were collected from 230 nurses working at geriatric hospitals. Results: Nurses' attitudes toward terminal care showed no significant correlation with awareness of good death, but it was positively correlated with a sense of closeness, a subfactor of awareness of good death. There was negative correlation between emotions regarding a deathbed, a subfactor of attitudes of nurses in charge of terminal patients, and awareness of good death. We found positive correlation between terminal care performance and awareness of good death. Conclusion: This study warrants the need for nursing education catered to characteristics of geriatric hospitals and development of diverse intervention strategies to help them to attain a positive attitude toward death by familiarizing themselves with the concept of good death and enhancing job satisfaction.
Purpose: The purpose of this study was to examine the relationships among knowledge, attitudes, and the use of negative control maintenance techniques toward older adults among nurses working in geriatric hospitals (n=86). Methods: Data were collected from April 4th to May 30th in 2006 by using questionnaires of Fact on Aging Quiz Part I, Aging Semantic Differential Scaling, and Control Maintenance Techniques. Results: The findings showed that the nurses displayed lack of knowledge concerning older adults and their average rate of correct answer was 45.6%. They also hold some negative bias and attitudes, especially in psychological and social aspects. The level of using negative control maintenance techniques in managing older patients' problematic behaviors was reported as the mean of 5.65 with a possible range of 0-40. Nurse's knowledge of older adults was positively associated with their attitudes toward older adults, but their knowledge and attitudes were not associated with the use of negative control maintenance techniques. Conclusion: It is necessary to provide educational programs for nurses that focus on a comprehensive understanding of aging with lifetime developmental perspectives. Further studies are needed to understand the factors associated with using negative control maintenance techniques and to evaluate the intervention programs in reducing the use of negative control maintenance techniques.
