Nurses' Needs for the Spatial Composition of Geriatric Hospital (노인요양병원의 공간구성에 대한 간호사 요구)

  • Purpose : The purpose of this study was to examine the nurses' needs for the spatial composition of geriatric hospital. Methods : The survey method was used for collecting the data. The subjects were 110 nurses who worked at 6 geriatric hospitals in Busan. They answered the questionnaire by the self-administered method. The questionnaire consisted of items which asked the spatial composition of patient room, bathroom, dining space, resting space, care-givers' room, and garden. Results : 1) The patient room using a bed on ondol floor would be useful. Also, it would be desirable that most patient rooms consisted of 4 or 6 persons per patient room and the others were 1 or 2 persons per room. 2) The bathroom for only patients and the shower room for their families or care-givers are needed in the geriatric hospital. 3) The dining room for patients in each floor would be useful to old patients. In addition to that, the cafeteria for their families or care-givers are needed. 4) The resting space for patients and their families, for the staff, and for care-givers are needed. 5) The personal locker and refrigerator are necessary fixtures in care-givers' room. Also, table, sofa, sink, and shower booth are also needed. 6) On the base of activities of care-givers, the space for preserving, washing, and heating foods are needed. Also, the storage space for personal items and the fitting space are needed. Implications : The results of this study would be the fundamental data for space planning of the geriatric hospital.

A Study on the Evaluation of the Cognition of the Circulation Efficiency in Geriatric Hospital (노인요양병원 진료부의 경로인지 효율성 평가에 관한 연구)

  • The aim of this study is to quantitatively deduce the circulation efficiency and space arrangements of the outpatient department in geriatric hospitals in terms of spatial structure and to find a methodology that could promote the improvement of accessibility and spatial cognition while using the facilities. All space units of the selected twelve geriatric hospitals were located on the first floor. The study adopted the psychological distance concept of space syntax to the unitized spaces, and presented a set of indices, such as TD, PN, RF, PNT and TRF. These indices were calculated according to the standard clinical use procedures. The study evaluated the effectiveness of circulation. The analysis of circulation was based on the four major geriatric illnesses that were selected in this study. The study established the standard clinical use procedure with consideration of the medical treatment, examinations and space units of outpatient departments. The results are as follows: 1) Based on the statistical data, four major geriatric illnesses are examined, and space requirement and standard procedures are established. Development indicators are applied in order to quantitatively deduce the efficiency of spatial cognition. 2) The percentage of the risk of cognitive decline(TRF) has an inversely proportional correlation to the circulation efficiency(PNT) at a certain point. TRF is more influenced by the clinical use procedure than the scale. 3) The arrangement of the waiting space and types of corridor greatly affect the percentage of the risk of cognitive decline. 4) Vulnerable facilities are selected on the basis of the evaluation, and two alternatives that could promote the circulation efficiency are presented. The result of the reanalysis showed that cognitive efficiency had much improved compared to the existing facilities.

Correlation analysis of factors and the geriatric oral health-related quality of life in Gumi (구미 일부지역 노인의 구강건강관련 삶의 질 수준과 요인간 상관관계 분석)

  • Objectives : The objective of this paper is to clarify the factors of the geriatric oral health influencing oral health-related quality of life by using the contracted OHIP-14 tool. Methods : This research conducted individual interview for 177 seniors using senior citizen center by using structured questionnaires. SAS(Ver.9.2) Program was used for the collected data to perform frequency analysis, reliability and scale analysis, t-test, ANOVA, correlation analysis and multiple regression analysis. Results : The oral health-related quality of life level related to oral health according to the demographical characteristics showed that it was better in case that they are younger, married, more educated and have more living expenses. Except for age, oral health-related quality of life was connected to scholastic achievement, living expenses, subjective health condition and subjective oral health condition. The factors influencing the oral health condition were subjective health condition, marriage, scholastic achievement, living expenses, age and sex. As the subjective health condition is better, in case of cohabitation of only a couple and as the age or scholastic achievement is higher and the living expenses are more, the oral health condition was better. The factors influencing oral health-related quality of life were subjective oral health condition, marriage, sex, subjective health condition, scholastic achievement and living expenses. As the subjective oral health condition and health condition were better and in case of sole living and cohabitation of only a couple, male's oral health-related quality of life was higher. Conclusions : It is considered that because the geriatric oral health condition becomes an important factor to oral health-related quality of life, the development of the geriatric oral health business and the geriatric heal education program to maintain and improve oral health is required and the activation of the oral health insurance policy for preventive dental service is necessary.

Evaluation of End-of-Life Nursing Education Consortium-Geriatric Train-the-Trainer Program in Korea

  • Background: Few nurses are trained in palliative care for long-term care in Korea. The End-of-Life Nursing Education Consortium (ELNEC)-Geriatric training program improves nurses' ability to promote palliative care for the elderly. Purpose: The aim of this study was to evaluate nurses' satisfaction and knowledge following the attendance at the ELNEC-Geriatric curriculum on nurses' knowledge of palliative care. Methods: Nine ELNEC-Geriatric modules were presented to 203 interdisciplinary professionals on July 1 and 3, 2010, in Seoul, South Korea. The Palliative care quiz for nursing (PCQN) was used to evaluate nurses' knowledge. Of all the participants, 128 nurses were completed the questionnaire. Of these nurses, 45.2% were staff nurses and 73.4% were hospital nurses. Results: Approximately eight nine percent of the nurses reported previous experience in caring for dying patients and attending various hospice palliative care training programs. Overall program satisfaction of the participants was 4.03 on a 5-point scale, and their mean of the total PCQN score was 12.75 out of 20 after participating in ELNEC-Geriatric course, which was a significant improvement (p=.022) from the pretest. Conclusion: The results of this study demonstrate that ELNEC-Geriatric curriculum was successfully implemented and significantly contributed to increasing the nurses' knowledge for palliative care in long-term care in Korea.

The Influence of Job Satisfation on Turnover Intention of Nurses in Geriatric Hospital (요양병원 간호사의 직무만족이 이직의도에 미치는 영향)

  • This study aims to analyze the effects of job satisfaction on turnover intention among the nurses in geriatric hospitals. In terms of study methods, this study developed structured questionnaires, and distributed them to nurses working in 11 geriatric hospitals in metropolitan area. A total of 418 questionnaires (response rate: 87.0%) were used for final analysis. We conducted multiple regression analyses to analyze the influence of job satisfaction on turnover intention, adjusting for the characteristics of the research subjects. The results of this study are as follows. First, both job satisfaction and turnover intention were statistically significantly related to individual and hospital characteristics. Second, turnover intention was lower in older age group, and higher in smaller hospitals. As job satisfaction increases, turnover intention decreases. Third, the results of the regression analysis adjusting for subject's characteristics and job satisfaction's sub-categories show that the turnover intention is lower in nurses who are older, day worker, and work in bigger hospitals. Fourth, the result of subgroup analysis by hospital volume show that both of two groups are influenced by hospital ownership and satisfaction about compensation. In addition, professionalism had a significant effect on decreasing turnover intention in smaller hospitals. However, there was no statistically significant relationship between job satisfaction's sub-categories and turnover intention in larger hospitals. The results of this study confirms that job satisfaction lowers turnover intention among nurses in geriatric hospitals. It is suggested that the administrators of those hospitals continue to monitor critical factors associated with job satisfaction, and thus, reduce turnover intention, which helps improve quality of services.

Calculation of the Costs and Optimal profits per Inpatient-day of the Geriatric Hospitals (노인병원의 재원환자 1인당 일평균 원가 및 적정이윤 계산)

  • It has been asserted that per diem payment system should be introduced, in place of the current fee-for-service system, for payment of the inpatient services of the geriatric hospitals, Based on the assentation, this study aims at calculating costs and profits per inpatient-day of the geriatric hospitals, and thereby at contributing to the managerial improvement from the both sides of the Government and the hospitals. Relevant data of the three months, May to August, 2002 were collected from the five geriatric hospitals, and per inpatient-day costs and profits were calculated for the three disease groups. Major results and conclusions are as follow : Firstly, total costs per insured inpatient-day of the geriatric hospitals are 65, 389 won for dementia (including optimal profit of 3,858 won), 69,730 won for stroke (including optimal profit of 4,117 won), and 70,085 won for other diseases (including optimal profit of 4,134 won). Secondly, the amount of the non-insured costs per inpatient-day occupies 34.5% of the total costs for dementia, 30.3% for stroke, and 30.1% for other diseases. Thirdly, the total amount of the per inpatient-day costs calculated including the optimal profits is, on the average, higher by 12% than the present price level calculated for the current fee-far-service system. This implies that the present price level should rise by 12% when the current fee-far-service payment system be maintained, and Finally, introduction of a sliding-scale payment system should be considered for the inpatient medical management fees for the length of stay over six months or more that are being cut in the claim examination process by the insurance corporation.

An Effect of Group Art Therapy by Way of Reminiscence on the Depression of the Elderly Inpatients with Geriatric Illness (회상을 활용한 집단미술치료가 노인성질환 입원환자의 우울에 미치는 효과)

  • This thesis is to inquire into the effect of group art therapy by way of reminiscence on the depression of the elderly inpatients with geriatric illness. The subjects are 20 inpatients whose age is over sixty in the Elderly Nursing Hospital located in K county. Among them, 10 inpatients are arranged as the experimental group, and the rest 10 inpatients, as control group. The research period is from February 28, 2009 to August 21, 2009. For the experimental group, the group art therapy by way of reminiscence is carried out during 21 sessions once a week for 60 minutes per session. For both the experimental group and the control group, pre and post test is carried out, and then after four weeks, the further test is carried out. The measurement tools are KGDS(Korean Form of Geriatric Depression Scale) and KHTP. The data are analyzed by SPSS(for Windows) and SAS(ver. 9.1.2), by which one-way ANOVA, two-way ANOVA, and Scheffe test are carried out. The results are as follows. First, the average of the whole elderly depression levels is shown significantly in the post test and the further test between the two groups. The art therapy group is more effective on the reduction of depression of the elderly inpatients with geriatric illness than control group. Second, it is shown that there is significant difference between the two groups in the the elderly depression by KHTP. The art therapy group is more effective in the improvement of the reduction of the elderly depression in view of KHTP. In conclusion, the group art therapy is an efficient intervention which enables to reduce the depression of the elderly inpatients with geriatric illness.

  • The purpose of this study is to analyze the effect of health status on sleep quality and quality of life among elderly patients in geriatric hospitals. A survey was held to the elderly patient of 5 geriatric hospitals in Kyunggi-do and Busan from May 8 to May 28, 2013. As result of factor analysis, the health status divided to the 7 domain: Emotional function, Strength, Daily activity, social role, Change of body, Social function and pain. Pain and Emotional function revealed the negative effect on sleep quality and Change of body and Daily activity were positive effect on quality of life. In conclusion, it is need to improve sleep quality through finding pain and emotional problem in elderly patient, and in order to upgrade nursing in geriatric hospitals, it have to develop nursing strategies considering the geriatric hospital for health screening and health promotion to take care of daily activity and change of older's body.

The Effects of the Stress on Depression of Elderly Inpatients in Geriatric Hospital : Mediating Effects of Self-Esteem (요양병원 노인 입원환자의 스트레스가 우울에 미치는 영향: 자아존중감의 매개효과 분석)

  • The purpose of this study was to examine the effects of the stress on depression of elderly inpatients in geriatric hospitals and verify the mediating effects of self-esteem on the relationships between stress and depression. Data collected from 218 elderly inpatients of geriatric hospitals in Seoul and Gyeonggi region from Sept. 4th to 22nd, 2017 were analyzed with SPSS 23.0. As a results, there was a statistically significant difference in the level of stress, depression, self-esteem of elderly inpatients in geriatric hospitals depending on their Socio-demographic and Hospital-utilization characteristics. Stress of elderly inpatients had a positive influence on their depression(${\beta}=.297$, p=.000), the self-esteem had a partial mediating effect on the relationships between stress and depression(Sobel Z=2.034). These results suggest that the development and provision of self-esteem improvement programs as well as stress management of elderly inpatients are necessary to prevent depression of elderly inpatients in geriatric hospital.

  • The purpose of this study is to investigate the effect of gratitude disposition of geriatric hospital employees on job satisfaction and life satisfaction, and the moderating effect of the gratitude diary experience. A total of 485 copies of the research questionnaire were selected for workers in four geriatric hospitals who are promoting campaigns related to the gratitude diary. For data processing, a simple or hierarchical regression analysis was performed using SPSS 26.0. As a result of the study, The gratitude disposition had a statistically significant effect on job satisfaction and life satisfaction. However, the effect of gratitude disposition on job satisfaction and life satisfaction and the moderating effect of the audit diary experience were not statistically significant. The purpose of this study is to develop programs for job satisfaction and life satisfaction of geriatric hospital employees and provide basic data on manpower management of geriatric hospital.
