Convolution Neural Network Based Auto Classification Model Using Endoscopic Images of Gastric Cancer and Gastric Ulcer (내시경의 위암과 위궤양 영상을 이용한 합성곱 신경망 기반의 자동 분류 모델)

  • Although benign gastric ulcers do not develop into gastric cancer, they are similar to early gastric cancer and difficult to distinguish. This may lead to misconsider early gastric cancer as gastric ulcer while diagnosing. Since gastric cancer does not have any special symptoms until discovered, it is important to detect gastric ulcers by early gastroscopy to prevent the gastric cancer. Therefore, we developed a Convolution Neural Network (CNN) model that can be helpful for endoscopy. 3,015 images of gastroscopy of patients undergoing endoscopy at Gachon University Gil Hospital were used in this study. Using ResNet-50, three models were developed to classify normal and gastric ulcers, normal and gastric cancer, and gastric ulcer and gastric cancer. We applied the data augmentation technique to increase the number of training data and examined the effect on accuracy by varying the multiples. The accuracy of each model with the highest performance are as follows. The accuracy of normal and gastric ulcer classification model was 95.11% when the data were increased 15 times, the accuracy of normal and gastric cancer classification model was 98.28% when 15 times increased likewise, and 5 times increased data in gastric ulcer and gastric cancer classification model yielded 87.89%. We will collect additional specific shape of gastric ulcer and cancer data and will apply various image processing techniques for visual enhancement. Models that classify normal and lesion, which showed relatively high accuracy, will be re-learned through optimal parameter search.

Hookworm Infection Caused Acute Intestinal Bleeding Diagnosed by Capsule: A Case Report and Literature Review

  • Hookworm infections are rare causes of acute gastrointestinal bleeding. We report a middle aged man with primary nephrotic syndrome and pulmonary embolism. During the treatment with steroids and anticoagulants, the patient presented acute massive hemorrhage of the gastrointestinal tract. The results of gastroscopy showed red worms in the duodenum. Colonoscopy and CT angiogram of abdomen were unremarkable. Capsule endoscopy revealed fresh blood and multiple hookworms in the jejunum and ileum. Hookworms caused the acute intestinal bleeding. The patient responded well to albendazole. Hematochezia was markedly ameliorated after eliminating the parasites. Hence, hookworm infection should be considered in the differential diagnosis of a patient with obscure gastrointestinal bleeding. Capsule endoscopy may offer a better means of diagnosis for intestinal hookworm infections.

Clinical Study on Admission Patients of North Korean Refugees at a Department of Korean Internal Medicine (1개 한방병원 한방내과에 입원한 북한이탈주민 환자에 대한 임상적 분석)

  • Objectives: The purpose of this study was to investigate the health status of hospitalized North Korean refugees in a department of Korean internal medicine. Methods: From March 1st, 2011 to June 30th, 2014, 57 North Korean refugee patients were hospitalized 96 times in a department of Korean internal medicine. Their demographic and medical information was approached retrospectively. Results: Among the 57 North Korean refugees, 46 (80.7%) were women, 11 (19.3%) were men and their average age was 49.7. Except for 3 people, the other 54 (94.7%) have been to an outpatient hospital of Department of Western Medicine, National Medical Center. Orthopedics, Neurology and Gastroenterology were most frequently visited and hospitalized. Among the total 96 times of hospitalization in a Department of Korean internal medicine, low back pain was the majority chief complaint. Among the 96, 78 (80.4%) took herbal medicine and tonifying and replenishing formula (補益劑) was most frequently prescribed. As western medication, 32 (33.0%) took ones related to the musculo-skeletal system and 30 (31.0%) took ones related to the nervous system. Qi stasis was highest among 8 types of pattern identification and Deficiency pattern (虛症) was more frequent than Excess pattern (實證). Conclusions: There are several factors that have aggravated the health status of North Korean refugees, and Korean medicine can perform a proper role to improve their health.

Bowel Sound Recording for Structural Deformity in the Pylorus-Duodenum: Report of 3 Cases (각기 다른 유문-십이지장의 기질적 병변 3증례에 대한 장음 측정 보고 : 장음의 주주파수를 중심으로)

  • Objectives: The purpose of this study is to report bowel sound patterns recorded in 3 diseases associated with structural deformity of the pylorus-duodenum. Methods: : Bowel sound recording is a useful non-invasive tool for the evaluation of gastrointestinal motility. However, the difficulty in manual attachment of the electronic stethoscope on the abdominal wall and noise production against bowel sound signals have prohibited its widespread use. Therefore, I developed a new apparatus that eliminates the noise; it contains a sound absorbing device and a holder for the head of stethoscope. Using the new bowel sound recording system, bowel sounds of the patients were recorded repeatedly. The endoscopic and abdominal computed tomography findings were obtained from other hospitals' medical record, and all patients were confirmed to have structural deformity in the pylorus-duodenum. The records of each patient were compared and assessed. Results: : Patients had either duodenal ulcer scar, pyloric stricture, or far advanced malignant stomach cancer, as diagnosed by the gastroscopy. Their dominant frequency of bowel sound obtained from the new system was checked more than 2 times at regular intervals. All 8 recordings in the 2 patients with duodenal ulcer scar (5 times) or pyloric stricture (3 times) showed a decrease in postprandial than fasting dominant frequency. One patient with stomach cancer showed no significant change between postprandial and fasting states at 2 recordings. Conclusions: The analysis of dominant frequency in bowel sounds can indicate the existence of pyloric obstruction that could delay gastric emptying.

Familial Gastric Dysmotility in Monozygotic Twins with Functional Dyspepsia and Their Mothers and the Effect of Korean Traditional Medicine on Symptoms and Gastric Dysmotility in Twins (기능성 소화불량증의 일란성 쌍생아와 이들 모친의 가족적 위 운동성 장애와 한방치료가 쌍둥이의 임상증상과 위 운동성에 미친 영향)

  • The aims of this study were to use the Rydoraku test, electrogastrography, and enterotachography to examine the presence of familial gastric dysmotility among monozygotic twins and their mothers; to determine the relationship between the symptoms and the indexes of the Rydoraku test, electrogastrography, and enterotachography; and to observe the therapeutic reaction for each differential treatment between twins with familial gastric dysmotility. The same herbal medication (Banhasasim-tang extract three times/day and Sojuckkunbi-tang extract three times/day) was given to each twin, but the younger twin also underwent manual acupuncture on the CV 10, 12, and 13 points of the abdominal wall and electrical stimulation of both ST 36 points of the lower leg 2-3 times per week. Evaluation of the therapeutic effect was followed after six weeks. The presence of familial gastric dysmotility was shown in the autonomic nerve system and gastric muscle and was thought to be a common pathophysiology induced by genetic co-ownership. Only the younger twin showed any marked relief of the dyspeptic symptoms associated with improvement of pyloric sphincter function, which was induced by acupuncture treatment. The Rydoraku test, electrogastrography, and enterotachography results showed the presence of familial gastric dysmotility. Although Korean traditional medicine had no effect on the familiar gastric dysmotility associated with genetic influences, the acupuncture treatment had a beneficial effect on the secondary disorder of pyloric sphincter function, which is associated with the relief of dyspeptic symptoms.

  • Objectives : The general and specific characteristics inherited in Korean traditional medicine research is to be identified and on the basis of this, the role of researchers, research institutes and government organizations is to be presented. Method : Analysis data targeted 399 cases of traditional medicine researches out of 696 cases, excluding overlapped 297 cases, that are relevanttokey-words, 'Oriental Medicine' and 'Herbal Prescription' registered in the National Institutes of Health Clinical Research Information Service(CRIS) as of 31 December, 2019. Including the regulation of the Ministry of Food and Drug Safety, research fund supporting, type of mediation, targeting disease and so forth, frequency analysis was carried out in order to confirm the general and specific characteristics of traditional medicine. Results : As a result of analyzing the data, researches of medicines(herbal medicines) and mediation(acupuncture, moxibustion) and were identified as the most and in the case of the targeting disease, it appeared in the order of targeting the research on musculoskeletal system and the healthy person.

Beneficial Effect of DA-9601, an Extract of Artemisiae Herba, on Animals Models of Inflammatory Bowel Disease

  • This study was conducted to investigate the effect of DA-9601, an extract of Artemisiae Herba, which is known to possess mucoprotective action either by free radical scavenging effect or increase of mucus secretion, against animal models of inflammatory bowel disease (IBD) induced by trinirobenzene sulfonic acid (TNBS) or other noxious agents. Experimental colitis was induced by intracolonic administration of TNBS in 50% ethanol, or 1 ml of 7% acetic acid solution (AA), by subcutaneous injection of indomethacin (INDO) in rats, or by supplementing drinking water with 5% dextran sodium sulfate (DSS) in albino mice. DA-9601 was treated orally for 4 to 7 days. Animals were euthanized 1 day after the last treatment for morphological and biochemical analysises. All the noxious agents including TNBS, AA, INDO and DSS elicited severe colitis. The animals treated with DA-9601 showed a consistent, dose-related reduction in the severity of colitis, grossly and histologically. The reduction was significant (p<0.05) after administration of DA-9601 at dose range of 10 mg/kg or above. In TNBS-induced colitis, the rats receiving DA-9601 showed significantly decreased mucosal myeloperoxidase (MPO) and thiobarbituric acid-reactive substances (TBA-RS), when compared to control and mesalazine groups. Mucosal proinflammatory cytokine levels were also decreased after DA-9601 treatment. In conclusion, DA-9601 ameliorated macroscopic and histologic scores in experimental colitis either through decreasing oxidative stress or by attenuating cytokines involved in inflammation. DA-9601 could be a promising drug for the therapy of IBD.

Fasciola hepatica: Infection Status of Freshwater Snails Collected from Gangwon-do (Province), Korea

  • Fasciola hepatica is a trematode that causes zoonosis, mainly in cattle and sheep, and occasionally in humans. Few recent studies have determined the infection status of this fluke in Korea. In August 2015, we collected 402 samples of freshwater snails at Hoenggye-ri (upper stream) and Suha-ri (lower stream) of Song-cheon (stream) in Daegwalnyeong-myeon, Pyeongchang-gun in Gangwon-do (Province) near many large cattle or sheep farms. F. hepatica infection was determined using PCR on the nuclear ribosomal internal transcribed spacer 2 (ITS-2). Among the 402 samples, F. hepatica 1TS-2 marker was detected in 6 freshwater snails; thus, the overall prevalence in freshwater snails was 1.5%. The prevalence varied between collection areas, ranging from 0.0% at Hoenggye-ri to 2.9% at Suha-ri. However, F. gigantica ITS-2 was not detected in the 6 F. hepatica-positive samples by PCR. The nucleotide sequences of the 6 F. hepatica ITS-2 PCR-positive samples were 99.4% identical to the F. hepatica ITS-2 sequences in GenBank, whereas they were 98.4% similar to F. gigantica ITS-2 sequences. These results indicated that the prevalence of F. hepatica in snail intermediate hosts was 1.5% in Gangwon-do, Korea; however the prevalence varied between collection areas. These results may help us to understand F. hepatica infection status in natural environments.

The Role of Endoscopic Retrograde Cholangiopancreatography (ERCP) in the Treatment of Traumatic Pancreas Injury (외상성 췌장 손상에서 내시경적 담췌관 조영술의 역할)

  • Purpose: Blunt pancreatic injury has a high mortality rate, especially if adequate management is delayed. Although many guidelines exist for diagnosis and treatment, there is no consensus to date. Therefore, we analyzed the role of endoscopic retrograde cholangiopancreatography (ERCP) as a diagnostic and therapeutic tool for the treatment of traumatic pancreatic injury. Methods: We retrospectively reviewed the electronic medical records (EMR) database at Asan Medical Center (Seoul, South Korea) to identify all patients diagnosed with trauma to the pancreas between June 2003 and December 2010. Clinical and operative findings, CT (computed tomography) images, and ERCP findings were assessed. Results: A total of 40 patients were evaluated in this study. Of these, 14 patients underwent diagnostic ERCP, and 26 did not. Of the 14 patients who underwent diagnostic ERCP, 5 were found to have normal pancreatic ducts, thereby preventing a needless laparotomy in these patients. Of the patients diagnosed with ductal injury, four were treated with endoscopic intervention, and four underwent an exploratory laparotomy. The remaining patient was treated with radiologic intervention (percutaneous drainage) to manage pancreatic pseudocyst formation. Conclusion: Our findings suggest that ERCP is a beneficial diagnostic and therapeutic modality for the treatment of traumatic pancreatic injury.

Selection of Potential Virulence Factors Contributing to Streptococcus suis Serotype 2 Penetration into the Blood-Brain Barrier in an In Vitro Co-Culture Model

  • Meningitis caused by Streptococcus suis serotype 2 (S. suis 2) is a great threat to the pig industry and human health. Virulence factors associated with the pathogenesis of meningitis have yet to be clearly defined, even though many potential S. suis 2 virulence factors have been identified. This greatly hinders the progress of S. suis 2 meningitis pathogenesis research. In this study, a co-culture blood-brain barrier (BBB) model was established using primary porcine brain microvascular endothelial cells and astrocytes, and the whole genome library of S. suis 2 was constructed using phage display technology. Finally, a total of 14 potential virulence factors contributing to S. suis 2 adherence to and invasion of the BBB were selected by analyzing the interactions between the phage library and the co-culture model. Twelve of these factors have not been previously reported in meningitis-related research. The data provide valuable insight into the pathogenesis of S. suis 2 meningitis and potential targets for the development of drug therapies.
