Epidemiologic Study of Human Epidermal Growth Factor Receptor 2 Expression in Advanced/Metastatic Gastric Cancer: an Assessment of Human Epidermal Growth Factor Receptor 2 Status in Tumor Tissue Samples of Gastric and Gastro-Esophageal Junction Cancer

  • Purpose: The Trastuzumab for gastric cancer (GC) trial identified human epidermal growth factor receptor 2 (HER2) as a predictor of successful treatment with trastuzumab (HER2 receptor targeting agent) among patients with advanced/metastatic GC. To date, the prevalence of HER2 overexpression in the Korean population is unknown. The present study aimed to assess the incidence of HER2 positivity among GC and gastroesophageal (GE) junction cancer samples and the relationship between HER2 overexpression and clinicopathological characteristics in Korean patients. Materials and Methods: Tumor samples collected from 1,695 patients with histologically proven GC or GE junction enrolled at 14 different hospitals in Korea were examined. After gathering clinicopathological data of all patients, HER2 status was assessed by immunohistochemistry (IHC) at each hospital, and IHC 2+ cases were subjected to silver-enhanced in situ hybridization at 3 central laboratories. Results: A total of 182 specimens tested positive for HER2, whereas 1,505 tested negative. Therefore, the overall HER2-positive rate in this study was 10.8% (95% confidence interval=9.3%-12.3%). The HER2-positive rate was higher among intestinal-type cases (17.6%) than among other types, and was higher among patients older than 70 years and 50 years of age, compared to other age groups. Conclusions: Our evaluation of the HER2 positivity rate (10.8%) among Korean patients with GC and GE junction indicated the necessity of epidemiological data when conducting studies related to HER2 expression in GC and GE junction.

Evaluation of HER-2/neu Overexpression in Gastric Carcinoma using a Tissue Microarray

  • Background: Amplification and overexpression of human epidermal growth factor receptor 2 (HER2/neu) oncogene has considerable prognostic value in breast and gastric cancers. This study aimed to evaluate the frequency, overexpression pattern, clinical significance, and concordance between the results for protein expression and gene amplification of HER-2/neu in gastric and gastro-esophageal junction carcinomas. Materials and Methods: In this study, 101 gastric tissue samples which were included in tissue microarray were immunohistochemically examined for overexpression of HER2/neu. Chromogenic in situ hybridization (CISH) was used for HER-2/neu amplification. The correlation of HER2/neu amplification with clinicopathological parameters was also assessed. In addition, concordance between CISH and IHC was detected. Results: This study demonstrated a significant difference in the overexpression of HER2/neu in gastric tumors. The overexpression of HER2/neu was significantly higher in intestinal type, poorly differentiated grade, large size ($5cm{\leq}$) and positive nodal involvement tumors (p-value=0.041, 0.015, 0.038 and 0.071, respectively). Also, amplification of HER2/neu according to CISH test, had a significant positive correlation with tumor size and tumor type (p-value=0.018 and 0.058, respectively).Concordance between CISH and IHC was 76.9% in 101 evaluable samples. Conclusions: IHC/CISH differences were attributed to basolateral membranous immunoreactivity of glandular cells resulting in incomplete membranous reactivity and/or a higher rate of tumor heterogeneity in gastric cancers compared to breast cancers. Therefore, this can be a potential marker for targeted therapy of malignant gastric tumors.

위식도접합부선암의 임상병리학적 특성 (The Clinicopathological Characteristics of Adenocarcinoma of the Gastro-esophageal Junction)

  • 목적: 위식도접합부선암의 임상병리학적 특성은 서구와 아시아 간의 많은 차이를 보인다고 알려져 있다. 그러나, 한국의 경우 위식도접합부선암의 유병율이 매우 낮고 이에 대한 연구 또한 부족하다. 따라서 저자들은 우식도접합부선암의 Siewert 분류에 따른 임상병리학적 특성을 조사하여 서구와 다른 국내 위식도접합부선암의 특성을 연구하고자 하였다. 대상 및 방법: 2004년 5월부터 2008년 2월까지 본원에서 위암으로 수술을 시행 받은 환자들을 대상으로 수술소견 및 병리 사진을 토대로 Siewert 분류법에 따라 위식도접합부선암을 분류하고, 의무기록을 바탕으로 환자들의 임상병리학적 특성들을 조사하였다. 결과: 위식도접합부선암은 전체 1,778예 중 70예(3.9%)였다. Siewert 1형은 3예(4.3%)로 매우 드물었으며, 2형과 3형이 각각 30예(42.8%) 37예(52.8%)였다. 2형이 3형에 비하여 평균연령이 높았으나(64세 vs 59세, P=0.049), 남녀비에는 차이가 없었다. 2형과 3형간에 Barrett 식도의 발현 정도나 Lauren 분류의 차이는 없었으나, 저분화암의 빈도는 3형에서 유의하게 높았다(P=0.045). 68예(97.1%)에서 근치적 절제가 시행되었으며, 각 아형간에 TNM 병기의 차이는 없었다. 결론: 국내의 위식도접합부선암은 매우 드물며 특히 1형의 유병율이 매우 낮다. 본 연구에서 서구의 보고와 부분적으로 일치하지 않는 병리학적 특성을 보였다. 향후 보다 많은 환자들을 대상으로 한국인의 위식도접합부선암의 특성을 연구할 필요가 있다.
