개의 위운동성에 대한 전침술의 영향 (Effects of Electroacupuncture on Gastric Motility in Dogs)

  • The effects of electroacupuncture on gastric motility in dogs were investigated in this study. The acupoints used in this experiment were Zu San Li (ST-36), Feng Long (ST-40), Jie Xi (ST-41), and Li Dui (ST-45) which belong to stomach meridian, Wei Shu (BL-21) which belongs to urinary bladder meridian, and Zhong Wan (CV-12) which belongs to conception vessel meridian. The animals were stimulated with 2-4 volt and 5 Hz for 20 min. Electroacupuncture at Zu San Li and Wei Shu acupoint increased the gastric motility, but Zhong Wan decreased the motility. There were no change in gastric movement after electrostimulation at Feng Long, Jie Xi and Li Dui acupoints. It was considered that electroacupuncture at Zu San Li and Wei Shu acupoints to promote the gastric movement, and at Zhong Wan acupoint to depress the movement might be effective in veterinary practice.

  • 본 연구는 침술이 개의 위 운동성에 미치는 효과에 관하여 알아보기 위하여 실시하였다. 건강한 잡종견 6두의 유문부 근층에 전극을 장착한 후 위근전도를 (electrogastromyograms, EGMG) 기록하여 위 운동성을 평가하였다. 침술은 ST-36, ST-40, ST-41, ST-42, ST-45, BL-21 및 CV-12 혈 위에 실시하였다. ST-36 및 BL-21 혈 위에 침술을 실시한 결과 위 운동성이 증가되었으며, CV-12 혈 위에서는 위 운동성이 감소되었다. 그러나 ST-40, ST-41, ST-42 및 ST-45 혈위의 침술은 위 운동성에 영향을 미치지 않았다

개에서 위운동에 관한 초음파 관찰 (Ultrasonegraphic Observation of Gastric Motility in Dogs)

  • The Ultrasonographic method alas been widely applied to evaluating gastric motility with safety and reproducibility ill human medicine but few reference to its use in veterinary medicine is appeared. Therefore, in this study, the gastric motility was evaluated with ultrasonography by the cri-terion of mean cycle lime, short and lony axis and the area of pyloric antrum in dogs, fed with liquid of semisolid meals. Furthermore, the animals were evaluated for the effect of metoclopramide on the motility of pyloric antrum. Healthy 5 mongrel male dogs were fed with either 400 ml of milk a: a liquid meal or a mixed meal of 200 ml of milk with two pieces of bread as a semisolid meal. Mean cycle time of pyloric antrum of dogs was significantly delayed after feeding either of liquid and , semi- solid meals(P<0.05), alls it was returned to the fasting state at 60 min. after feeding of liquid meal and 160 min. after feeding of semisolid meal. Mean area of pyloric antrum of dogs was gradually decreased and was returned to the lasting state at 80 min. in doss fed liquid meal. but 1600 min. in dog\ulcorner fed semisolid meal. The administration of metoclopramide (1.0 mg/kg of of B.W.) accelerated the mean cycle time of pyloric antrum from 20 mill. to 60 min. after feeding of liquid meal and from 40 min. to 120 min. after feeding of semisolid meal. From this study, the ultrasonography was confirmed as a valuable diagnostic method leer evaluating the gastric motility and gastric area in dogs. It is non-invasive, safe and reproducible, and provides a method for the study of the effect of drugs and diseases states on gastric motility.

지출환(枳朮丸)의 흰쥐 위(胃) 운동성(運動性)에서 정상 상태 및 위벽 국소손상 후에 대한 효능 (Effect of Jichul-hwan on Gastric Motility in Conscious and Gastric Wall Injured Rats)

  • Background & Objective : Jichul-hwan(JCH) has been used for the treatment of functional dyspepsia, regarded as a gastric dysmotility disease. We investigated the effects of JCH on gastric motility and its mechanisms of action in rats. Methods : The gastric wall was injured by tracting a part of stomach body in rats. Gastric emptying was measured after administration of normal saline(NS) or JCH in normal rats and gastric wall injured rats. To evaluate the mechanism of JCH under delayed gastric emptying conditions, normal rats were treated with atropine sulfate(1mg/kg, s.c.), quinpirole HCl(0.3mg/kgg, i.p.), $NAME(N^{G}-nitro-L-arginine$ methyl ester, 75mg/kg s.c.) and cisplatin(10mg/kg, i.p.). The gastric slow waves were measured for 30 minutes before and after administration of each solution(NS, JCH). Results : JCH 110.1mg/kg improved gastric emptying for 2 hrs(p=0.014). JCH 110.1mg/kg improved gastric emptying in the gastric wall injured rats(p=0.001). Under the delayed gastric emptying, JCH 110.1mg/kg improved gastric emptying in the group treated with atropine $sulfate(1.83{\pm}0.96$ vs $8.43{\pm}8.46$, p=0.003), but aggravated it with quinpirole $HCl(4.7{\pm}2.9$ vs $1.61{\pm}2.09$, p=0.021). Administration JCH 110.1mg/kg increased EGG power in rats. Conclusions : JCH stimulates gastric motility through the cholinergic pathway, so we expect that it would be effective in the treatment of dysmotility-like functional dyspepsia with low activity of vagus nerve.

족삼리(足三里) 침자(鍼刺)가 위운동(胃運動)에 미치는 영향(影響)에 관한 신경학적(神經學的) 관찰(觀察) (Neurologic Study of Acupuncture ST36(足三里) on Gastric Motility in Rats)

  • To investigate the acupuncturing function of $ST_{36}$(足三里) on the gastric activity, four different treatment methods such as intact, local anesthesia, ligation and nerve dissection were implemented. Followings are the results from the present study. 1. When a simple acupuncture with intact, the stomach motility and frequency were increased without significance compared with control. 2. Under local anesthesia on femoral region, the stomach motility and frequency showed no such changes compared with control. 3. With ligation of femoral region, the stomach motility and frequency were decreased significantly compared with control. 4. When the siatic nerve was dissected, the stomach motility and frequency showed no such changes compared with control. Overall, the effect of acupuncture on the $ST_{36}$(足三里) was very closely related with the condition of surrounding area. It is assumed that further studies should be done for the investigation of mechanism behind the acupuncturing function in conjunction with the nervous system.

침술자극이 개의 위 배출시간에 미치는 영향에 관한 X선 및 초음파학적인 평가 (Radiographic and Ultrasonographic Evaluation of Gastric Time of Dogs after Acupunctural Stimulation)

  • This study was performed to evaluate the effect of acupuncture on gastric motility in dogs with radiography and ultrasonography. All experimental dogs were fed with the soup with barium (3% W/V), and thereafter gastric emptying time(GET) was measured with radiography and ultrasonography with Bolondi's method. The acupoints were CV-12, PC-6, ST-36 and BL-21 in this experiment. On these acupoints, electroacupunctral stimulation(15 MHz, continous wave) was applied for 20 minutes. The results were as follows: In the control group, normal GET was 70-85 minutes with ultrasonography, and 110-130 minutes with radiography. In acupuncture stimulation groups, there was no effect of gastric motility in CV-12, PC-6, and 57-36, whereas there was significant stimulating effect of gastric movement in BL-21 with ultrasonography (P < 0.05) and radiography (P < 0.01). Above these results, it is concluded that acupunctural stimulation of BL-21 was more effective in gastric motility compared to those of CV-l2, PC-6 and ST-36.

소적건비환과립(消積健脾丸顆粒)이 흰쥐의 위운동성(胃運動性)에 미치는 영향 (Effect of Sojukgunbihwan granule on Gastric Motility in Rats)

  • Background & Objective : The herbal medicine Sojukgunbihwan granule has been used for the treatment of functional dyspepsia, generally categorized as a gastric dysmotility disease. However, its mechanisms are not yet well known. Therefore, the effects of Sojukgunbihwan granules on gastric motility in rats was investigated. Methods : The myoelectrical activity of the gastric smooth muscle was recorded by a bipolar electrode placed at the gastric antrum of rats. The contractile waves were measured for 30 minutes before and after intragastric administration of each solution(normal saline, Pyungwisan 40mg/kg, Sojukgunbihwan granules 60mg/kg) and expressed as the motility index. Gastric emptying was measured by the number of glass beads expelled from the stomach(containing one hundred 1mm glass beads) within an hour after glass beads and test drugs(normal saline, Pyungwisan 40mg/kg, Pyungwisan 120mg/kg, Sojukgunbihwan granules 60mg/kg, Sojukgunbihwan granules 180mg/kg) were administered. In another series of experiments to evaluate the mechanisms of Sojukgunbihwan granules under delayed conditions, the rats were treated with atropine sulfate(1 mg/kg, s.c.), cisplatin(10mg/kg, i.p.), and NAME($N^G$-nitro-L-arginine methyl ester, 75mg/kg, s.c.) respectively. Results : Intragastric administration of Sojukgunbihwan granules increased the myoelectrical activity significantly, gastric motility index rose 25%, and gastric emptying of glass beads was significantly enhanced over a period of 60minutes. Under the delayed gastric emptying induced by atropine sulfate, cisplatin, and NAME, Sojukgunbihwan granules aggravated gastric emptying in the atropine sulfate($5.71{\pm}3.45\;vs\;3.71{\pm}4.42$) and cisplatin($13.86{\pm}3.53\;vs\;5.14{\pm}5.05$, p<0.01) treated groups, but enhanced gastric emptying in the NAME treated group($5.00{\pm}3.21\;vs\;11.71{\pm}5.65$, p<0.05). Conclusions : Results suggest that Sojukgunbihwan granules stimulate gastric motility through cholinergic and 5-hydroxytryptamine 3 receptors. Results are indicative of Sojukgunbihwan as an especially effective remedy in dysmotility-like functional dyspepsia with impaired reservoir functions such as gastric adaptive relaxation.

항콜린에스테라제 약물의 소화관 운동성에 대한 영향 (The Effects of Anticholinesterase Drugs on Gastric Motility)

  • 항콜린에스테라제 약물은 AChE를 억제하여 ACh을 콜린성 수용체에 축적시켜 지속적인 ACh 자극을 유도하는 약물이다. 이러한 특성이 적출한 위장의 운동성에 미치는 영향과 콜린에스테라제 활성화 약물의 영향을 조사하고, 에탄올이 AChE의 활성도를 감소시키는 보고를 바탕으로 항콜린에스테라제와 에탄올의 동시 투여시 나타나는 위장관의 수축운동의 변화를 규명하고자 다음과 같은 실험을 수행하였다. 흰쥐의 위장을 적출하여 $2{\times}10mm$ 환상근 절편을 만들어 적출근편 실험조에 고정하고 등척성 장력측정기를 이용하여 약물 처치에 따른 위장 절편의 수축 변화를 측정하였다. 비가역성 항콜린에스테라제인 fenthion은 ACh 유발 위장 절편의 수축을 증가시켰으며 콜린에스테라제 활성화 약물인 PAM은 이 작용을 길항하였다. 가역성 항콜린에스테라제인 physostigmine은 그 자체로 인해 위장 절편의 수축이 증가하였는데, 이는 비극성을 나타내어 위장 절편 조직으로의 흡수가 빠르기 때문이라고 생각되고, ACh에 의한 반응성 또한 현저히 증가하였다. PAM에 의한 반응성 감소효과도 대조군과 통계적으로 유의한 차이를 보였다. 최근 평활근 이완을 유도한다고 알려진 에탄올은 fenthion 처치에 의한 수축운동 증가를 억제하였고 이러한 결과로 볼 때 항콜린에스테라제와 에탄올의 동시 투여시에는 콜린에스테라제 활성도 감소보다 에탄올에 의한 위장관 운동성 감소가 많이 작용함을 의미하였다. 이상의 결론은 가역성 및 비가역성 항콜린에스테라제에 의해서 위장관 운동성 항진이 나타나고 이는 콜린에스테라제의 활성화 약물로서 길항되었다. 그리고 에탄올과 항콜린에스테라제에 동시에 노출된 경우는 에탄올의 평활근 이완 효과에 의해 위장관 운동성 감소가 나타남을 알 수 있었다.

이진탕(二陳湯)이 STZ 유발(誘發) 당뇨병(糖尿病) 흰쥐의 위(胃) 운동성(運動性)에 미치는 영향(影響) (Effect of Yijin-tang on Gastric Motility in STZ-Induced Diabetic Rats)

  • Backgrounds & Objectives : The aims of this study were to observe how morphology was changed and whether gastric motility was impaired in streptozotocin(STZ)-induced diabetic rats(DR), and whether Yijin-tang(YJT) was able to restore their impaired gastric motility. Methods : We investigated the change of body weight, feed intake and blood glucose between normal rats (NR) and DR for 12 weeks after induction of diabetes. At the time of 12 weeks after induction of diabetes, gastric surface area, gastric slow wave and gastric emptying rate were measured. Results : Decreased body weight, increased feed intake and increased gastric surface area were observed in DR, compared with NR. The percentage of normogastria decreased but that of bradygastria increased in DR, compared with NR. YJT 90mg/kg had no effect on the correction of gastric slow wave. YJT 90mg/kg and 270mg/kg had a significant effect on improvement of gastric emptying, more than normal saline (NS) in both NR and DR but the gastric emptying rate of DR was significantly lower than that of NR when YJT 90mg/kg and YJT 270mg/kg were administered. Conclusions : We can expect that administration of YJT would be effective on the improvement of gastric emptying and upper gastrointestinal symptoms as a juvantia.

Laparoscopic Partial Fundoplication in Case of Gastroesophageal Reflux Disease Patient with Absent Esophageal Motility

  • The surgical indications for the treatment of gastroesophageal reflux disease (GERD) in patients with esophageal motility disorders have been debated. We report a case of antireflux surgery performed in a patient with absent esophageal motility as categorized by the Chicago classification (2011). A 54-year-old man underwent laparoscopic Toupet fundoplication due to apparent GERD and desire to discontinue all medications. After surgery, his subjective symptoms improved. Furthermore, objective findings including manometry and 24-hour pH-metry also improved. In our experience, antireflux surgery can improve GERD symptoms patients, even with absent esophageal motility.
