Human Papillomavirus Vaccine

  • Human papillomavirus (HPV) infection is the most common sexually transmitted disease in the United States. An estimated 6.2 million people are infected with HPV every year. Randomized controlled studies consistently show that HPV vaccine is effective in preventing infection and HPV related cervical lesions. In June 2006, Gardasil (qadrivalent HPV recombinant vaccine) was approved by the FDA for use in females 9-26 years of age. This article reviews published data to evaluate the effectiveness of HPV vaccine for the prevention of cervical cancer.

  • In India, cervical cancer is the most common woman-related cancer, followed by breast cancer. The rate of cervical cancer in India is fourth worldwide. Two vaccines, Gardasil and Cervarix, both targeting HPV-16 and 18 which account for 70% of invasive cervical carcinomas, are licensed in the United States and numerous countries worldwide. Both vaccine formulations have shown excellent efficacy with minimal toxicity in active female population but numerous questions arise in vaccinating like cost effectiveness, lack of proven efficacy against other HPV strains, social acceptance of HPV vaccination and other ethical issues. The main objective of this study is to emphasis the advantages and disadvantages of the vaccination in India.

부산지역 여성의 자궁경부질환과 HPV의 상관관계 (Correlation between Uterine Cervical Lesion and HPV in Busan Region)

  • 2013년 1월부터 2016년 3월 사이 인제대학교 해운대백병원에 내원한 환자를 대상으로 HPV genotype 분석 후 세포학적 검사 결과, 조직학적 검사 결과와 비교하였다. 총 검사대상 2,130건 중 58.9%인 1,254건은 HPV 양성으로, 41.1%인 876건은 HPV 음성으로 분석되었다. HPV 양성검체 중 단순감염은 58.4%인 732건, 복합감염은 41.6%인 522건이었다. 감염비율은 HPV 16, 68, 56의 순으로 각각 7.1%인 152건, 4.6%인 97건, 3.8%인 80건으로 나타났다. HR HPV 감염은 40대, 30대, 50대 순으로 높은 감염률을 보였고, LR HPV 감염은 40대, 50대, 30대 순으로 높은 감염률을 보였다. 조직병리학적 분석 결과 CIN 2 이상으로 나온 HPV 16, 68, 56 건수는 329건 중 155건으로 47.1%(155/329)로 분석되었다. 부산지역 여성의 HPV subtype 감염은 주로 16, 68, 56, 58, 51과 관련이 있었으나, 이중 68, 56, 51형은 현재 시판 중인 Gardasil 9가 백신으로도 예방할 수 없는 유전자형이었다. 이 연구를 통해 부산 지역의 HPV 예방 접종을 위한 프로그램에 대한 중요한 기준 데이터를 제공할 수 있을 것으로 사료된다.

Human papillomavirus Infection and its Vaccines: Knowledge and Attitudes of Primary Health Clinic Nurses in Kelantan, Malaysia

  • Background: Cervical cancer though preventable is still the leading cause of cancer death among women secondary to breast cancer. Persistent infection with HPV has been causally linked to the disease. A school based HPV vaccination program was introduced in late 2010 in Malaysia and nurse support is essential for its success. Objectives: To determine nurses knowledge and attitudes about HPV infection and its vaccines, and factors associated with their knowledge. Materials and Methods: This cross-sectional study was conducted among nurses working at primary health clinics in Kelantan from mid-June till the end of July 2014. Its involved 330 nurses selected through multistage random sampling. A validated self-administered questionnaire consisting of 11 items for the knowledge domain and eight items for the attitude domain was used. Results: The response rate of the study was 93.7%. The mean knowledge and mean attitude (SD) scores were 5.37 (1.76) and 29.8 (3.51) respectively. Only 24% knew that HPV is the most common sexually transmitted infection and 67% correctly answered that Gardasil vaccine can protect against four types of HPV. Nearly 60% of participants wrongly answered that HPV vaccines cannot be offered to sexually active women. Likewise, 70.9% participants were not aware that HPV vaccine may be appropriate for females aged 9 through 26 years. Though 90% of participants believed that the vaccine is safe, nearly half of them were unsure about efficacy. From multiple linear regression analysis, among the factors tested only participant's level of education showed a statistically significant association with the HPV knowledge score (p <0.001). Conclusions: This study indicates nurses have favorable attitudes towards HPV vaccination; however they have significant knowledge deficit and major misunderstanding in critical knowledge items. Among the factors tested, nursing qualification is the only factor that is significantly associated with the nurses knowledge score.

Neutralization of Human Papillomavirus by Specific Nanobodies Against Major Capsid Protein L1

  • The human papillomavirus (HPV) is the main cause of cervical cancer in developing countries. Rapid diagnosis and initiation of treatment of the HPV infection are critical. Various methods have been employed to reduce the immunogenicity of antibodies targeting HPV serotypes. Nanobodies are the smallest fragments of naturally occurring single-domain antibodies with their antigen-binding site compromised into a single domain. Nanobodies have remarkable properties such as high stability, solubility, and high homology to the human VH3 domain. In this study, a phagemid library was employed to enrich for nanobodies against the L1 protein of the human papilloma virus. Binding reactivity of the selected clones was evaluated using phage enzyme-linked immunosorbent assay (phage-ELISA). Finally, two nanobodies (sm5 and sm8) with the best reactivity against the Gardasil vaccine and the purified HPV-16 L1 protein were expressed and purified using a $Ni^+$-NTA column. The accuracy of expression and purification of the nanobodies was confirmed by sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS-PAGE) and immunoblotting assays. In vitro studies demonstrated that neutralization was achieved by the selected nanobodies. The ease of generation and unique features of these molecules make nanobodies promising molecules for the new generation of HPV diagnosis and therapy.
