신발 밑창 높이에 따른 보행의 변화 (The change of gait on shoes sole height)

  • ■ Objectives This study is to examine change of gait parameters on shoes sole height(high heels, MBT shoes, house shoes) through gait analyzer. ■ Methods The subjects of this study were 12 women in their twenties. Gait analysis system is 5m in total length and gait is led to be comfortable. They put three kinds of shoes each and were led to walk 5m on gait analysis system. ■ Results There were significant differences in step length, single support and load response of gait parameters and in stride length and total double support at double support phase. ■ Conclusion Muscle activity differs in that different that shoes sole height and form because tibialis anterior muscle has strengthen and gastrocnemius has stretched. Therefore we think that patients with knee joint problem consider gait parameters when shoes select.

Changes of spatio-temporal gait parameters according to experience falls in post-stroke patients

  • Objective: Falls are defined as contact of the body with the floor after losing balance during activities of daily living. Falls commonly occur among the elderly, and stroke patients in particular are at a high risk of falling. The purpose of this study was to investigate the changes of temporal and spatial gait parameters and gait symmetry according to experience falls in post-stroke patients. Design: Cross-sectional study. Methods: Fifty three patients with stroke were recruited on a voluntary basis from the rehabilitation unit, who currently undergoing physical therapy. All participants were asked to answer questions regarding the frequency of falls in the past 1 year. Fifty-three patients with stroke were allocated 2 groups according to experienced falls: stroke with falls (n=26) during past 1 year and stroke without falls (n=27). The spatial and temporal gait parameters and gait symmetry ratio were measured using GAITRite system. Results: The spatial gait parameters and the temporal gait parameters were significantly different between the stroke with falls group and the stroke without falls group (p<0.05). Furthermore, step length was the only significantly different among symmetry ratio (p<0.05). Conclusions: Experience of falls can lead to impairment of gait ability in stroke patients. This result is expected to be used as a basic data for rehabilitation program development to prevent a fall of post-stroke patients.

Spatiotemporal Gait Parameters That Predict the Tinetti Performance-Oriented Mobility Assessment in People With Stroke

  • The purpose of this study was to find which spatiotemporal gait parameters gained from stroke patients could be predictive factors for the gait part of Tinetti Performance-Oriented Mobility Assessment (POMA-G). Two hundred forty-six stroke patients were recruited for this study. They participated in two assessments, the POMA-G and computerized spatiotemporal gait analysis. To analyze the relationship between the POMA-G and spatiotemporal parameters, we used Pearson's correlation coefficients. In addition, multiple linear regression analyses (stepwise method) were used to predict the spatiotemporal gait parameters that correlated most with the POMA-G. The results show that the gait velocity (r=.67, p<.01), cadence (r=.66, p<.01), step length of the affected side (r=.49, p<.01), step length of the non-affected side (r=.53, p<.01), swing percentage of the non-affected side (r=.47, p<.01), and single support percentage of the affected side (r=.53, p<.01) as well as the double support percentage of the non-affected side (r=-.42, p<.01) and the step-length asymmetry (r=-.64, p<.01) correlated with POMA-G. The gait velocity, step-length asymmetry, cadence, and single support percentage of the affected side explained 67%, 2%, 2%, and 1% of the variance in the POMA-G, respectively. In conclusion, gait velocity would be the most predictive factor for the POMA-G.

만성 편마비 환자의 비마비측 발바닥굽힘근 근피로가 시·공간적 보행변수에 미치는 영향 (The Effects of Fatigue in the Non-Paretic Plantarflexor Muscle on Spatial and Temporal Gait Parameters during Walking in Patients with Chronic Stroke)

  • Purpose: The purpose of this study was to obtain detailed and quantified data concerning the effects of plantarflexor muscle fatigue induced in the non-paretic side on the spatial and temporal gait parameters of the bilateral lower extremities during walking in stroke patients. Methods: This study was conducted on 20 patients with chronic stroke. The load contraction fatigue test was applied to induce muscle fatigue in the non-paretic plantarflexor muscle. Step length, stride length, double support, gait velocity and cadence, and functional ambulatory profile (FAP) score in the bilateral lower extremities were measured using a gait analysis system in order to investigate changes in temporal and spatial gait parameters caused by muscle fatigue on the non-paretic side. The statistical significance of the results was evaluated using a paired t-test. Results: A review of the results for gait parameters revealed a significant increase in double support (p<0.05) and a significant decrease in step length, stride length, gait velocity and cadence, and FAP score (p<0.05). Conclusion: These results indicate that the muscle fatigue in the non-paretic side of the stroke patients also affected the paretic side, which led to a decrease in gait functions. This implies a necessity to perform exercise or training programs in a range of clinical aspects not causing muscle fatigue.

중량조끼를 착용한 뇌졸중으로 인한 보행장애 환자의 즉각적인 보행 개선 효과 1례 (Case Study of the Immediate Gait Improvement in a Post-Stroke Gait Disturbance Patient Equipped with a Weighted Vest)

  • Objective: To confirm the immediate gait improvement in a post-stroke gait disturbance patient equipped with a weighted vest. Methods: We selected a patient who was able to walk without another’s help or with tools. The selected patient had an unstable gait because she had only started an independent gait within the past week, so we thought that a weighted vest could be very helpful for her. We first collected gait parameters using a treadmill gait analysis system while the patient walked on the treadmill without the weighted vest. After a 10-minute break, gait parameters were collected again while the patient walked on the treadmill while wearing the weighted vest. The gait parameters we collected included step length (cm), stance phase (%), swing phase (%), SW/ST, and gait line length (mm). For objective evaluation of gait improvement, we calculated the ratio of gait parameters of the right and left limbs. Results: The gait of the post-stroke patient was more symmetrical when wearing the weighted vest than without the weighted vest. Without the weighted vest, her step length ratio was 0.78, stance phase ratio was 0.88, swing phase ratio was 1.50, SW/ST ratio was 1.70, and gait line length ratio was 0.91. With the weighted vest, her step length ratio was 0.88, stance phase ratio was 0.90, swing phase ratio was 1.38, SW/ST ratio was 1.54, and gait line length ratio was 0.98. No side effects were observed due to the weighted vest.

대동작 기능 평가도구, 대동작 수행능력 측정도구, 그리고 시공간적 보행변수와의 상관관계 (Correlations Among GMFM, GMPM, and the Spatiotemporal Gait Parameters)

  • These were two main purposes of this study. The first was to research the relevance between gross motor function measurement (GMFM) and the spatiotemporal parameters of gait in children with cerebral palsy. The second was to research the relevance between gross motor performance measure (GMPM) and the spatiotemporal gait parameters. Twelve children ($6.0{\pm}1.8$ years) with cerebral palsy participated in this study. GMFM and GMPM were performed and the spatiotemporal parameters of gait were measured by using WalkWay MG-1000. There were no significant correlations between the GMFM score and the stride length, step length, step width, cadence, and velocity (p>.05). The GMPM score also had no significant correlation with the spatiotemporal gait parameter (p>.05).

편마비 환자에서 트레드밀 보행훈련이 보행에 미치는 효과 - 지면 보행훈련과의 비교 - (Effects of Treadmill Gait Training on Gait Patterns in Hemiplegic Patients comparison with conventional gait training)

  • The aim of this study was to investigate the effects of treadmill gait training on the functional characteristics and the temporal-distance parameters of gait in hemiplegic patients, as compared with conventional gait training. The subjects of this study were 32 hemiplegic patients who had been admitted or were visited out-patients at Kangdong Sacred Heart Hospital, Hallym University, from March 3 through April 25, 2003. These subjects were randomly divided into treadmill gait training group or conventional gait training group. We evaluated the gait ability, motor functions, muscle strength, spasticity, physiological cost index, and temporal-distance parameters. We analyzed the changes between pre and post training in each groups, and the difference between two groups. Temporal-distance parameters were obtained using the ink footprint method and then energy consumption using physiological cost index. The results were as follows: 1. After a six-week training, treadmill gait training group significantly improved, as. compared to pre-training, in gait ability, motor functions for the leg and trunk and gross function, muscle strength of the lower limb, gait speed, cadence, step length both on the affected and on the unaffected side, step length symmetry, and energy consumption(p<0.05). 2. After a six-week training, conventional gait training group significantly improved, as compared to pretraining, in gait ability, motor functions for the leg and trunk, muscle strength of the lower limb, spasticity the upper limb, gait speed, cadence, step length both on the affected and on the unaffected side, and energy consumption(p<0.05). 3. After a six-week training, the treadmill gait training group significantly improved, as compared to the conventional gait, training, in gait speed and step length on the unaffected side. These results show that treadmill gait training was improved gait speed and step length on the unaffected side of hemiplegic patients, as compared with conventional gait training. Further research is needed to confirm the generalization of these findings and to identify which hemiplegic patients might benefit from treadmill gait training.

Y-균형 운동이 만성적 발목 불안정성을 가진 사람들의 시거리 보행 변수에 미치는 영향 (Effects of Y-Balance Exercise on Spatio-temporal Gait Parameters in Subjects with Chronic Ankle Instability)

  • Background: This study aimed to investigate the effect of Y-balance exercise on spatio-temporal gait parameters in subjects with chronic ankle instability. Design: Randomized Controlled Trial. Method: A study was conducted on 43 people with chronic ankle instability. Subjects performed modified Y-balance exercise 3 times a week for 50 minutes, 4 weeks. Gait parameters were measured using a gait analysis treadmill before exercise, 2 weeks after exercise, and 4 weeks after exercise. A gait analysis treadmill (FDM-T AP1171, Zebris, Germany) was used to measure gait parameters. Mean values were compared using Repeated measured two-way ANOVA. Result:: When comparing the results of three measurements taken before exercise, 2 weeks after exercise, and 4 weeks after exercise, there were significant differences in the qualitative and quantitative aspects of gait in gait variables such as step distance, step time, step ratio, and sway ratio. Conclusions: These results suggest that the Y-balance exercise and various exercises combining balance and proprioception are effective for subjects with chronic ankle instability.

삼차원 가상현실 운동 프로그램이 노인의 시공간적 보행변수에 미치는 효과 (Effect of virtual reality training using 3-dimensional video gaming technology on spatiotemporal gait parameters in older adults)

  • PURPOSE: This study was conducted to investigate the effect of the virtual reality training (VRT) using 3-dimensional video gaming technology on spatiotemporal gait parameters in older adults. METHODS: The study participants were divided into two groups: the VRT group and the control group. Those in the VRT group were enrolled in a VRT, which was conducted for 60 min per day, two times a week, during the 6-week research. The Wii-Fit balance board game was used for the VRT intervention. The VRT consisted of 6 different types of games, namely, jogging, swordplay, ski jump, hula hoop, tennis, and step dance. A 3-dimensional TV was used for 3-dimensional display. Participants in both the groups received 3 sessions of fall prevention education, at the first, third, and fifth weeks. Their gait parameters were measured by using OptoGait. RESULTS: After 6 weeks of the VRT, the spatial gait parameters of the participants, that is stride length and step length, were significantly improved compared with those of the control group participants (p<0.05). The temporal gait parameters, such as velocity, cadence, stride time, and step time, also showed improvement after the completion of the VRT training (p<0.05). Both the temporal and spatial gait parameters of the VRT group participants showed improvement after 6 weeks of the program compared with those of the control group participants (p<0.05). CONCLUSION: The VRT using 3-dimensional video gaming technology might be beneficial for improving gait parameters to prevent falls among older adults.

Immediate Effects of Posteroanterior Cervical Mobilization on Pressure Pain Threshold and Gait Parameters in Patients with Chronic Neck Pain: A Pilot Study

  • Background: Mobilization and cranio-cervical flexion exercise has been reported in reducing pain from cervical part and improving its motor function; also, has been represented that alleviate of neck pain and recover of neck muscles improve the normal gait performance. However, few studies have identified the effects of mobilization and exercise on pain and gait parameters with preceding issues. Objective: To examine the effects or changes of pressure pain threshold (PPT) and gait parameters in patients with chronic neck pain. Design: Cross-Sectional Clinical Trials Methods: Twenty patients with the history of neck pain (>3 months) performed the cervical mobilization and cranio-cervical flexion exercise. Gait parameters were assessed with wireless device and collected data were transmitted to the personal computer via Bluetooth. The PPT was measured posteroanterior direction at the prone position and the mean of subsequent three PPT measurements was used for the final analysis. Results: Both cervical central posteroanterior mobilization (CCPAM) (p<.000) and sling-based cranio-cervical flexion exercise (SBCCFE) (p<.000) group showed a significant increase in the PPT and the gait parameters, cadence (p<.023), was significantly increased in the CCPAM group, however slightly increased in the SBCCFE group. The comparison between the CCPAM and the SBCCFE groups after treatment did not show significant differences for the score on the PPT and gait parameters. Conclusions: This study suggests that CCPAM and SBCCFE increase PPT, cadence, and gait speed.
