  • In this study, we developed a robotic gait trainer which induces the active gait training based on predefined continuous proper lower extremity joint movements. AC servo motors and linear actuators were used to control hip and knee joints of patients and the weight support system was used to support the patient's weight during the gait training. We also implemented a Gill program to set the gait training pattern with several training parameters and to confirm states of patients and the system through the visual feedback. The effectiveness of the gait training system will be determined by the long-term clinical experiments in the future. We expect that the developed robotic gait training system could be applied very practically to recover gait abilities for persons with gait disorder.

  • Modem concepts of gait rehabilitation after stroke favor a task-specific repetitive approach. In practice, the required physical effort of the therapists limits the realization of this approach. Therefore, a mechanized gait trainer enabling nonambulatory patients to have the repetitive practice of a gait-like movement without overstraining therapists was constructed. In this study, we developed an active gait training system for patients with gait disorder. This system provides joint movements to patients who cannot carry out an independent gait. It provides a normal stance-swing ratio of 60:40 using an eccentric configuration of two gears. Joint motions of the knee and the ankle were evaluated with using the 3D motion analysis system and compared with the results from the multi-body dynamics simulation. In addition, clinical investigations were also performed for low stroke patients during the 6-week gait training. Results from the dynamics simulation showed that joint movements of the knee and the ankle were affected by the gear size, the step length and the length of the foot plate, except the radius of curvature of the foot guide plate. Also, the 6-week gait training revealed relevant improvements of the gait ability in all low subjects. Functional ambulation category levels of subjects after training were 2 in three patients and 1 in a patient. The developed active gait trainer seems feasible as an adjunctive tool in gait rehabilitation after stroke.

  • People with somatoform disorder have a number of different symptoms that typically last for several years. Their symptoms can't be traced to a specific physical cause. In people with somatoform disorder, medical test results are either normal or don't explain the person's symptoms. The symptoms of somatoform disorder are similar to the symptoms of other illnesses. People with this disorder may have several medical evaluations and tests to be sure that they don't have another illness. In this case, we described a 56-year old man who was diagnosed as R/O) ALS, because he has many similar symptoms of ALS (amyotrophic lateral sclerosis) such as gait disturbance, dysarthria, dyspnea. But we diagnosed his case as somatoform disorder and treated with herbal medication, acupuncure treatment ,negative cupping and relaxation training. Through these treatment, the patient showed improvement of chief complains and accessory symptoms.

  • Background: To investigate the differences of locomotor dynamics between Parkinson's disease (PD) patients with tremor dominant symptom and patients with postural instability dominant symptom. Methods: 66 subjects with PD were classified into two subgroups, tremor-dominant group and postural instability and gait disorder group by Unified Parkinson's disease rating scale (UPDRS). The spatial, temporal and electrodynamic gait parameters were recorded automatically using computerized 3-D motion analysis system with electrogoniometer. Results: There was no significant difference in cadence, pelvic tilt range, hip flexion range, knee flexion range and ankle dorsiflexion range. Postural instability and gait disorder group showed decreased gait velocity, short stride length, decreased range of motion in pelvic obliquity, pelvic rotation and ankle plantar flexion. Conclusions: There was meaningful difference in locomotor dynamics between Parkinson's disease(PD) patients with tremor dominant symptom and patients with postural instability dominant symptom.

  • We diagnosis two patients who have severe vertigo and gait inbalance as ‘dizziness due to retention of phlegm’ (痰暈) and gave Zexie-tang(澤瀉湯). They improved enable to walk balance and no more vetigo. Therefore we could assume that Ku-ren-xuan-mao(苦人眩冒) which is the main symptom of Zexie-tang means the vertigo and gait imbalance due to vestibular disorder.

  • Propionic acidemia is an autosomal recessive metabolic disorder caused by a defect of propionyl CoA carboxylase with resultant accumulation of toxic organic acid metabolites. This disorder is biochemically characterized by metabolic acidosis, ketoacidosis, hyperglycinemia and hyperammonemia. Clinical symptoms are very heterogeneous and present as a severe neonatal-onset or a late-onet form. We describe one case of propionic acidemia in a 4-year-old boy who has developed gait disturbance after acute metabolic decompensation.

  • The purpose of this study was to investigate the effects of the increasing ankle range motion program on ambulation and balance for the elderly with balance disorder. Eighteen elderly subjects were administered with a timed test twice; approximately 4 weeks apart. The exercise group participated in a fall prevention exercise class at the Y.S. Senior Welfare Center of the Seoul Metropolitan Government. The session consisted of a stationary cycle, static stretching ankle joints, balance boards, and progressive resistive exercises using the Thera-band. The results were as follows: Firstly, the increasing ankle range of motion program was effective on the exercise group. Gait-speed was improved (p<.005). Secondly, balance was significantly improved (p<.000). Thirdly, increasing ankle range of motion program was effective in ankle dorsiflexion which was the major risk factor for falls among the elderly. Differences in gait and balance between the groups were examined using a paired t-test (p<.05). The exercise group demonstrated significantly higher values of gait-speed, ankle range of motion, and balance when compared with the non-exercise group. The results of this study suggest that the increasing ankle range of motion program is an effective intervention for the elderly with balance disorder.

  • Objectives : This is a clinical report about the Conversion Disorder, Wei symptom patient. Methods : The patient was treated by acupuncture, herb medication, moxibustion, physical therapy and bio feedback. The improvement of the patient was judged by lower limb elevation test and gait condition test. Results : During the 1st duration, lower limb elevation angle of the patient had improved. And the patient's gait condition had improved. But he received stress so that same symptom appeared again. Then, after the second treatment, He could walk alone again. Conclusions : Oriental medicine treatment including Acupuncture, Herb medication was very effctive to improve the Conversion disorder patient's symptoms. It is necessary to have more observation and many cases of patients with Conversion disorder.

  • The purpose of this study is to report the effects of Korean medicine treatment on the gait disorder. Patient with myelin oligodendrocyte glycoprotein-associated disorder. We treated the patient using Korean medicine treatment including Motion-Style-Acupuncture-Treatment, pharmacopuncture, acupuncture, and herbal medicine during 30 days. Numerical rating scale (NRS), EuroQol five dimension (EQ-5D) index, Oswestry Disability Index (ODI), Modified Barthel Index (MBI), and the changes of symptoms were measured for assessment. After 30 days inpatient treatment, NRS decreased from 4 to 2, EQ-5D index, ODI, MBI, and the symptoms of the patient also were improved. In conclusion, this case shows Korean medicine treatment might be an effective treatment for gait disorder Patient with MOG-associated disorder.
