협응적 이동 훈련 프로그램 적용이 편마비 환자의 균형과 보행능력에 미치는 효과 (Effects of Coordinative Locomotor Training Program on Balance and Gait of Stroke Patients)

  • Purpose: This study sought to examine the effect of coordinative locomotor training (CLT) program on the balance and gait of stroke patients and to develop effective programs and training methods to improve the functions of such patients. Methods: Subjects included 29 patients with hemiplegia caused by stroke. The subjects were randomly divided into an experimental group (n=14) that participated in CLT program and a control group (n=15) that participated in general exercise therapy. The experimental group underwent CLT program, while the control group underwent general exercise therapy, for 30 minutes, 3 days per week for a 6-week period. timed up and go test (TUG), four square step test (FSST), figure-of-8-walk test (F8WT), and 10m walking test (10MWT) were conducted to evaluate changes in balance and gait. Results: After the intervention, significant differences (p<0.05) were seen in the TUG, FSST, F8WT, and 10MWT in both groups. The experimental group showed more significant improvement than the control group(p<0.05). Conclusion: The results from this study indicate that a CLT program is extremely effective for improving the balance and gait in stroke patients.

다발성감각운동자극 치료가 뇌졸중 환자의 보행과 낙상위험도에 미치는 효과: 무작위배정예비임상시험 (Effect of Multi-Sensorimotor Training on Gait Ability and Fall Risk in Subacute Stroke Patients: A Randomized Controlled Pilot Trial)

  • Purpose : To determine whether an advanced rehabilitation therapy combined with conventional rehabilitation therapy consisting of sensorimotor exercises that would be superior to a usual treadmill training in gait ability and fall risk in subacute stroke patients. Methods : Thirty subjects randomly assigned to either multi-sensorimotor training group (n=19) or treadmill training group (n=18). Both groups first performed conventional physical therapy for 30 min, after which the multi-sensorimotor training group performed multi-sensorimotor training for 30 min, and the treadmill training group performed treadmill gait training for 30 min. Both groups performed the therapeutic interventions 5 days per week for 8 weeks. Gait ability was evaluated using the GAITRite system and Fall risk was measured using the Biodex Balance system before intervention and after 8 weeks. Results : There were no intergroup differences between demographic and clinical characteristics at baseline (p>.05). Both groups showed a significant improvement in gait ability (p<.05) and Fall risk (p<.05). In particular, the multi-sensorimotor training group showed more significant differences in gait velocity (p=.05), step length (p=.01) and stride length (p=.014) than the treadmill training group. Conclusion : The multi-sensorimotor training program performed on multiple types of sensory input had beneficial effect on gait ability. A large-scale randomized controlled study is needed to prove the effect of this training.

  • Purpose: This study aimed to assess the effectiveness of gait training with the use of an electromechanical gait trainer with functional electrical stimulation (FES) for patients that had undergone subacute stroke. Methods: The study subjects included nine subacute stroke patients of the Korea National Rehabilitation Center in Seoul, Korea. Outcome was measured using the timed Up and Go test, Fugl-Meyer-L/E assesment, with determination of the comfortable maximal gait speed, composite spasticity score, functional ambulatory category and Berg balance scale. All measured scores were recorded before, during, and after rehabilitation and at an eight-week follow-up. Results: Patients who received electromechanical-assisted gait training in combination with FES after subacute stroke were more likely to achieve independent walking, functional activities, balance and gait speed. Conclusion: The outcome of our gait-training program demonstrates that it may be practical to integrate FES into electromechanical gait training without any adverse effects. However, further randomized controlled studies are needed to evaluate if patient outcome after combined training is superior to outcome after the use of electromechanical gait trainer treatment alone or conventional gait training alone.

PNF 훈련 프로그램이 건강한 노인의 기능적 평가 측정과 보행에 미치는 효과 (Effect of a PNF Training Program on Functional Assessment Measures and Gait Parameters in Healthy Older Adults)

  • Purpose: The purpose of this study was to examine changes in functional assessment measures (FAMs) and spatiotemporal gait parameters (STGPs) in healthy older adults before and after a 6-week intervention of a proprioceptive neuromuscular facilitation (PNF) training program. Methods: Thirty healthy older adults (mean age: $73.37{\pm}1.21$ age range: 65-79) were randomly assigned either to an experimental group (participating in a 6-week intervention of PNF training) or a control group (only performing daily activities). Participants in the control group did not receive any training program. Performance was assessed by recording changes in the FAMs and STGPs using functional assessment tools and GAITRite. Results: Participants in the PNF group showed significantly improved functional assessment measures and increased stride length, cadence, velocity, and step width (p<0.05). No significant differences in FAMs and STGPs were found in the control group (p>0.05). Conclusion: Participation in a PNF training program improves FAMs and STGPs, thereby increasing the ability of healthy older adults to maintain dynamic balance during functional performance and gait. These findings support the use of PNF training programs as effective fall-prevention programs for the elderly.

  • Purpose : This study was conducted to evaluate the effects of FES with abdominal muscle contraction before virtual reality training on balance and gait ability in patients of stroke patients. Methods : The subjects were 30 stroke patients who satisfied the selection criteria. They were randomly assigned to a group receiving functional electrical stimulation with a virtual reality training program (the experiment group, n=15) and placebo functional electrical stimulation with a virtual reality training program (the control group, n=15). The program consisted of three 30-min sessions per week for six weeks. The timed up and go test (TUG), the BT4, the G-WALK were used to estimate subjects' balance, gait before and after the program. For the experiment group, the functional electrical stimulation was applied to the external oblique and the rectus abdominis, For the control group, the same program and the placebo functional electrical stimulation were applied. Results : There were significant improvements in the subscales of the balance and gait ability test of those who participated in the functional electrical stimulation, while the control group showed no significant changes. Conclusion : Therefore, functional electrical stimulation with virtual reality effectively improved the balance and gait ability in patients with chronic stroke.

트레드밀 보행훈련이 편마비 환자의 족저압과 하지 근활성도에 미치는 영향 (The Effects of Plantar Foot Pressure and Muscular Activity on Treadmill Gait Training in Stroke Patients)

  • Purpose:This study was performed for effects of gait training on treadmill and stable surface which influenced on the lower limbs muscular activity needed in gait, plantar foot pressure with hemiplegic patients caused by cerebrovascular accident. Methods:Two groups of adult hemiplegia(n=20) were allocated randomly in this study: treadmill gait training group and control group. The gait training program was provided to experimental groups for 8 weeks (5 times a week). Measurements of pre and post experiment were plantar foot pressure. For measuring muscular activation rectus femoris, biceps femoris, tibialis anterior, gastrocnemius were detected. Results:The results of this study showed that in comparison of pre and post changes of gait training, the treadmill gait training group has noticeable changes than other groups in activity of rectus femoris and tibialis anterior, the control group revealed statistically significant differences in plantar foot pressure Toe2-5, M1, M3, M5, MF area, activity of gastrocnemius. Conclusion:These results mean gait training resulted by treadmill, stable surface provides effective muscle activation and plantar foot pressure with stroke.

  • In this study, we developed a robotic gait trainer which induces the active gait training based on predefined continuous proper lower extremity joint movements. AC servo motors and linear actuators were used to control hip and knee joints of patients and the weight support system was used to support the patient's weight during the gait training. We also implemented a Gill program to set the gait training pattern with several training parameters and to confirm states of patients and the system through the visual feedback. The effectiveness of the gait training system will be determined by the long-term clinical experiments in the future. We expect that the developed robotic gait training system could be applied very practically to recover gait abilities for persons with gait disorder.

감각운동 훈련을 병행한 순환식 과제 지향 운동프로그램이 뇌졸중 환자의 보행 및 균형에 미치는 영향 : 예비연구 (The Effects of Task-Related Circuit Exercise Program Combined with Sensorimotor Training on Balance and Walking in Persons with Stroke : A pilot study)

  • Purpose: The purpose of this study was to identify whether task-related circuit exercise program combined with sensorimotor training for 4 weeks could improve the balance and gait in stroke patients. Method: Fifteen stroke patients who had agreed with the study were randomly divided into 3 groups categorized as task-related circuit exercise program combined with sensorimotor training group (experimental group 1, n=5), task-related circuit exercise program group (experimental group 2, n=5), and control subjects performed conventional physical therapy (control group, n=5). The balance and gait were assessed by BT-4 force platform system, Berg Balance Scale, 10meter Walk Test and Smart Step at before training and after training. Wilcoxon signed rank test was used to analyze change before and after intervention in intra-group. Kruskal Wallis H test, Mann-Whitney U test and Bonfferoni correction were used to analyze changes of all variables in inter-groups. Result: The experimental group 1 showed significant improvements in postural sway area, BBS scores, walking velocity and plantar pressures of affected foot, whereas the experimental group 2 showed significant improvements in BBS scores, and the control group were no significantly different in all variables following training. The changes of postural sway area and BBS scores in the experimental group 1 were significantly greater than them of the control group. The changes of postural sway area in the experimental group 1 was significantly greater than that of the experimental group 2. Conclusion: The result of this study suggest the task-related circuit exercise program combined with sensorimotor training is an effective intervention to improve balance and gait in stoke patients.

The Effect of Side-Step Tasks Based Circular Training Program on Balance and Gait in Stroke Patients

  • Objective: The purpose of this study was to investigate the effect of the side-step tasks based circular training program (STCT) on balance and gait characteristics in stroke patients. Design: A randomized controlled trial Methods: Twenty-four stroke patients were randomly divided into two groups of twelve patients each. One group was applied with the STCT whereas the other group was treated with conservative physiotherapy (CP). The ability of gait was measured in 10m walking test and stride length on both side using BTS G-WALK (BTS Bioengineering S.p.A, Italy) and the ability of balance was measured in Berg Balance Scale (BBS) and Timed Up and Go Test (TUG). Results: The STCT group was significant differences in the balance parameters of BBS and TUG (p<0.05) and showed significant differences in gait variables in 10m walking speed, stride length of affected and non-affected side after the experiment before and after the experiment (p<0.05). In addition, the STCT group showed a significant difference in BBS compared to the control group (p<0.05). Conclusions: The results of this study confirmed that the side-step tasks based circular training program (STCT) improves balance and walking ability in stroke patients. STCT is expected to be used as a useful intervention method for stroke rehabilitation.

과제 지향적 그룹 운동 프로그램이 뇌졸중 환자의 운동 기능과 우울증에 미치는 효과 (Effects of the Group Task-related Program Training on Motor Function and Depression for Patient with Stroke)

  • Purpose : The purpose of this study is to assess the effect for gait, balance, and depression for stroke patients by group task-related program training based motor learning theory. Methods : The subjects of this study were administrated to the 11 stroke patients (9 male, 2 female) by 5 weeks, 3 times per week, 15 times. The group task-related program training were performed gait, balance, treadmill, muscle strengthening, and game program. Each program took 7~10 minutes and total time took 60 minutes including moving time. The difference of program training were compared using the paired t-test. Results : The results of this study revealed that Fugl-Meyer motor assessment, Chedoke-McMaster Stroke assessment of lower extremity and Berg balance scale were significantly correlated. However, impairment item of Chedoke-McMaster Stroke assessment, spatio-temporal gait parameters, Timed up and go test, and depression item of Minnesota Multiphasic Personality Inventory were not significantly correlated. Conclusion : These results support that group task-related program could be a useful treatment to improve the balance skills and motor function of lower extremity for the chronic stroke patients.
