Improvements in Balance of Patients with Hemiplegia A Literature Review (편마비환자의 균형기능 향상에 관한 문헌적 고찰)

Background: Hemiplegic patients usually have difficulty maintaining balance. Balance training is a major component of there habilitation program for patients with neurological impairment. The purpose of this study was to investigate the effects of PLS(Posterior Leaf Spring), FES(Functional Electrical stimulation), treadmill training, and neurodevelopmental treatment on the improvement of balance in patients with hemiplegia. Methods: We looked into published studies from Dankook University’s electronic library databases of RISS4U, KMbase, NCBI, and MEDLIS concerning the effectiveness of any form of intervention leading to improvement of balance. All types of studies relevant to the topic that were published in English during the time period of 1986 to 2005 were included. Results: 1. There were significant differences in standing balance, dynamic activity balance, and gait speed between barefoot subjects and subjects who wore SPAFO and HPAFO(p<.05). 2. The changes in ROM and FRT related to sex, age, height, and weight part of the diagnosis, as well as experience relapse, was of meaningless value. Changes in ROM related to the duration of pain and experiences of falling down were also meaningless. However, FRT showed significant static differences(p<.05). 3. The body-weight-support treadmill training scoring of standing balance, step length, and a timed 10m walking test showed definite improvement. 4. The proprioceptive control approach improved dynamic balance in patients with hemiplegia. Conclusion: Consequently, further study is needed to verify methods when physical therapists are researching balance ability in hemiplegic patient.
Purpose: The purpose of this study was to examine the influence of foot angles on plantar pressure and the center of pressure (COP) trajectory length during level walking. Methods: The study subjects were 30 female university students without orthopedic diseases in the foot. The foot angle was divided into three forms (out-toeing, normal, in-toeing). The subjects practiced each type of gait, and then performed each of level walking, three times, and their averages were calculated. A plantar pressure measurement instrument was used, and the maximum force was obtained by dividing the foot into nine regions covering the anterior medial-lateral, middle medial-lateral, and posterior medial-lateral. The COP trajectory length was statistically processed by obtaining medial-lateral, anterior-posterior, and entire travel distance. Results: During normal walking, the maximum force was significantly higher in the anterior lateral than in the other areas, and the COP trajectory length was significantly shorter in the front-back and entire travel distances (p<0.05). During stair climbing. Conclusion: Walking at abnormal foot angles does not cause appreciable problems in the short term as pressure is concentrated on a specific plantar part. However, it becomes the cause of deformed foot structures and can result in musculoskeletal disabilities in the long term. Therefore, a kinesiatrics-based intervention is required to maintain normal foot angles.
This study was conducted as a part of long-term project on the development of a set of design guidelines for a crutch as mobility aids for the handicapped in the lower extremity and the suggestion a practical solution for a crutch design. The purpose of this study is to develop a design of a crutch and a set of prototypes that reflects the characteristics of crutch-gait and has a realistic possibility for mass production-based industry. TOGO, a axillary crutch as the result of this study, shows a number of characteristics distinguished from ordinary crutches. These are (1)Minimize the shock associated with planting of the crutch tips by improving the form and structure of crutch tip and axillary pad, (2)Ergonomically designed crutch in accordance with users' body movement while walking on crutches, (3)Easy length control to maximize mobility and maneuverability by changing the cross section of the crutch revolutionary, (4)Minimize possibilities of safety hazards, and (5)Attractive shape of the crutch to keep user self-esteem. The revolutionary crutch derived from this study results has been patented, and the company is seeking to mass-produce and find ways to commercialize it after reviewing the potential problems that may arise in the mass production environment.
In this study, three axillary crutches(Mobilegs, KM crutch, Pro In-motion) were tested with 12 healthy male subjects by measuring the EMG activity of the latissimus dorsi and gastrocnemius and the user-perceived levels focusing on the comfort and ease of use. The result showed that the EMG activity of the gastrocnemius was significantly reduced during ambulation using Mobilegs and Pro In-motion compared with the ambulation using KM crutch, a conventional crutch. However, no statistically significant difference was found for EMG activity of the latissimus dorsi. Subjects perceived the Mobilegs to be more comfortable and easy to use than the other crutches. Theses result may delivered mostly from the characteristics of spring-loaded axillary pad and ergonomic crutch handle. The results indicate that ambulation using the spring-loaded crutch is much easy with asking less exertion at gastrocnemius. In addition, the characteristics of crutch design, such as embedded spring, makes muscles to consume less energy. When the reasonable structure and form of the axillary pad and crutch handle are applied herein, the ambulation using the crutches are perceived to comfort.
Pompe disease is a genetic disorder caused by a deficiency of acid
The purpose of the study is to examine the effects of Gifted Education Isnstitute of Korea (GEIK) programs for gifted children especially in the areas of reasoning skills and creativity, thereby proving the effectiveness of the program. The subjects are 136 (103 boys and 33 girls) fourth, fifth, and sixth grade gifted children, who have participated in GEIK programs for more than six moths. They were stratified by the length of participation in GEIK programs. Ninety four children have participated for more than one year. Forty-two children have participated for less than one year. Both groups are rather homogeneous in IQ scores and school achievement levels at the time of enterance into GEIK programs. Both a Group Assessment of Logical Thinking (GAIT) and a Creativity test were used for the study on reasoning skills and creativity. GALT, developed by V. Roadranka, R. H. Yeany and M. J. Padilla in 1983, consists of 12 questions. It is classified into six subscales: conservation, proportional reasoning, controlling variables, provability reasoning, correlational reasoning, and combinatorial reasoning. The reliability of this test is .85. This test recommends to classify the stages of child development as follows according to the total test score. 0-4 point: Concrete Stage, 5-7 points: Transitional Stage, and 8 and above points: Formal stage. The Creativity Test was developed by Y. Lee and W. Chung (1971). It consists of four components: fluency, flexibility, originality, and openness. Only both fluency and openness were used in this study. In order to analyze data, T-Test, Intercorrelational Analyses, ANOVA, and Nultiple Regression were used. Followings are the results deduced from the above analoyses of the data. First, 43.48% of the subjects were on Concrete Stage, 36.78% were on the Transitional Stage, and 19.86% were on the Formal Stage in the developmental level classified by Piaget. Second, the students who have participated in GEIK programs more than one year acquired significantly higher score in GALT than the students who have participated in GEIK programs less than one year. Third, boys showed higher score in GALT than girls did. Fourth, there were statistically significant intercorrelations between six subscales of GALT. Fifth, the students who have participated in GEIK programs more than one year acquired significantly higher score in openness of creativity test than the students who have participated in GEIK programs less than one year. There were no significant differences in openness of creativity test between boys and girls. Sixth, the students who have participated in GEIK programs more than one year acquired significantly higher score in fluency of creativity test than the students who have participated in GEIK programs less than one year. Girls showed higher score in fluency of creativity test than boys did. Seventh, the students who acquired higher score in GALT showed higher score in both openness and fluency of creativity test. Followings are the conclusions deduced form the above results. First, the developmental level of reasoning skills of the fourth grade students participationg in GEIK programs is the same as that of 7th grade of normal Korean students and the same as those of 10th grade of U.S.A. and Philipoine students. Second, the GEIK programs are effective in improving reasoning skills. Third, the GEIK programs are effective in improving creativity. Fouth, reasoning skills and creativity can be improved by well planned programs. In conclusion, this study suggests that beyond reasoning skills and creativity, other areas such as areas in science skills, mathmatical skills, or verbal skills, etc., should be studied in the future.
Multiple epiphyseal dysplasia (MED) is one of the more common skeletal dysplasias. MED is characterized by joint pain and stiffness, a waddling gait, and/or mild short stature in childhood. Radiographic findings include delayed and irregular ossification of the epiphyses in multiple joints. Mutations in at least six different genes (COMP, MATN3, COL9A1, COL9A2, COL9A3, and DTDST) can cause MED, and it can be either dominant or recessive inheritance. Molecular diagnosis is important for accurate prognosis and genetic counselling. COMP mutation is the most common form of MED in Western. But, MATN3 mutation was reported as the most common type of MED in Korea. Here, we describe a boy who was diagnosed as MED by clinical and radiological features. Hip radiograph of the patient was suggested MATN3 mutation. But knee radiograph was suggested COMP mutation. MATN3 and COMP mutations direct sequencing, but were no mutation. So we tested whole exome sequencing, but significant variant was not detected as known MED six genes mutations. The patient was diagnosed as having MED clinically and radiologically. Further study to identify the other responsible genes for MED is needed.
The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times.
