The study on Correlation between Gross Motor Performance Measure and Pediatric Balance Scale in Children with Cerebral palsy (뇌성마비 아동의 대동작 수행평가(GMPM)와 아동용 균형척도(PBS) 사이의 상관성 연구)

  • Background : This study verified a correlation between gross motor performance measure (GMPM) and pediatric balance scale (PBS) for children, which are most widely used for assessment of children with cerebral palsy. Methods : The study subjects are 20 children aged between five and 13 who were diagnosed with cerebral palsy. Their mean age was $8.15{\pm}2.96years$ and consists of 15 males and 5 females. The subjects were divided into three groups according to muscle tone and motility disorders and then GMPM and PBS were assessed. In the assessment items in the GMPM, alignment, coordination, dissociated movement, stability, and weight shift were measured. The PBS consists of sitting to standing, standing to sitting, moving between chairs, standing without using hands, standing with one foot, turning 360 degrees, turning to look behind, retrieving object from floor, and reaching forward with outstretched arm while standing. A correlation of the collected data was analyzed using SPSS 18.0 for windows. Results : From Significant correlations were shown between items of GMPM and PBS as well as total scores of GMPM and PBS. Among them, coordination and weight transfer in the GMPM items had a high correlation with the PBS. In addition, the higher the GMPM total score was, the higher the PBS score was. Conclusion : This study result showed a significant correlation between GMPM and PBS, and this relation can be used to pediatric physical examination.

Correlations Among GMFM, GMPM, and the Spatiotemporal Gait Parameters (대동작 기능 평가도구, 대동작 수행능력 측정도구, 그리고 시공간적 보행변수와의 상관관계)

  • These were two main purposes of this study. The first was to research the relevance between gross motor function measurement (GMFM) and the spatiotemporal parameters of gait in children with cerebral palsy. The second was to research the relevance between gross motor performance measure (GMPM) and the spatiotemporal gait parameters. Twelve children ($6.0{\pm}1.8$ years) with cerebral palsy participated in this study. GMFM and GMPM were performed and the spatiotemporal parameters of gait were measured by using WalkWay MG-1000. There were no significant correlations between the GMFM score and the stride length, step length, step width, cadence, and velocity (p>.05). The GMPM score also had no significant correlation with the spatiotemporal gait parameter (p>.05).

  • 대동작 운동 수행능력 측정도구(GMPM)는 뇌성마비 아동의 움직임을 질적인 면에서 평가하기 위해 개발된 도구이다. 이 연구의 목적은 대동작 운동 수행능력 측정도구의 측정자간 신뢰도를 알아보는 것이다. 뇌성마비 아동 10명(평균 5.6세, 범위 4~8세)에게 GMPM 평가를 실시하였다. 평가 과정을 비디오로 녹화하여 각 속성 항목별로 3명의 평가자간의 급간내 상관계수로 일치도를 보았다. 전반적으로 측정자간 신뢰도는 '불량~보통'범주에 속했다. 이 연구의 결과는 충분한 교육을 받지 않고 평가하면 그 결과를 신뢰하기 어렵다는 것을 말해준다. 향후 임상에서 GMPM을 이용하여 평가할 때 측정자간 신뢰도에 어떤 변화가 있는지 알아보는 연구가 필요하다.

Effects of Functional Resistance Training on Gross Motor and Balance Abilities in Children with Dyskinetic Cerebral Palsy : Single Case Design (기능적인 저항훈련이 운동이상형 뇌성마비 아동의 대동작과 균형능력에 미치는 영향 : 단일사례설계)

  • Purpose : The purpose of this research is to find clinical effects of functional resistance training using weighted vest on gross motor and balance abilities of children with dyskinetic cerebral palsy. Methods : This study selects 3 subjects for 8~12 years old who were diagnosed with children with dyskinetic cerebral palsy. The Design is ABA design of single-subject research design. Baseline(A) and TypeII Baseline(A : 12weeks) phases were received with NDT treatment, Intervention(B : 12weeks) phase provided with 40 minute functional resistance training using weighted vest in a session twice a week. In order to analyze the measure results of gross motor function and performance, balance abilities in children dyskinetic cerebral palsy during baseline, intervention and typeII baseline phase. Result : A statistically significant differences in the total GMFM including walking/running/jumping during baseline, intervention, typeII baseline, but no significant differences in the lying/rolling, sitting, crawling/kneeling and standing. A statistically significant differences in the total GMPM including dissociated movement, coordination, weight shift, stability during baseline, intervention, typeII baseline, but no significant differences in the body alignment domains. A statistically significant differences in the length and surface area ellipse of center of pressure during baseline, intervention, typeII baseline. Conclusion : The intervention method to facilitate multi-joint and closed kinematic chain movement equipped weighted vest applied functional resistance training on children with dyskinetic cerebral palsy effectively improve on gross motor function and performance, balance abilities.

The Effects of Weighted Vest During Task-Oriented Training on Gross Motor Performance and Balance Abilities of Children With Spastic Diplegia : A Randomized Clinical Trial Study (경직형 양마비 아동의 과제지향훈련 시 무게조끼 적용이 대동작 수행력과 균형 능력에 미치는 영향: 무작위배정 위약비교 연구)

  • Objective : The purpose of this research is to find clinical effects of application of weighted vest during task-oriented training focused on gross motor performance and balance abilities of children with spastic diplegia. Methods : 34 subjects were divided by simple random sampling into two groups; experimental group (male : 9, female : 8, average age : 8.12) and placebo group (male : 9, female : 9, average age : 7.53). Both two groups underwent to 40 minute intervention, twice a week for 12 weeks. The intervention was task-oriented training focused on facilitating closed kinematic chain and multi-joint functional movement pattern. During the training, the experimental group received loaded-resistance weighted vest and placebo group also received weighted vest but without loaded-resistance. Participants in both groups underwent 8 to 10 reps of the task-oriented training and there were 3 minutes break time between tasks. There were pre-test of gross motor performance and balance abilities, and two times of post-tests were performed upon 6 weeks and 12 weeks after the intervention completed. And in final, an additional follow-up test was performed 12 weeks after the evaluation was finished in order to find any difference between the two groups over time. Results : There was significant difference in Gross Motor Performance Measure (GMPM) between two groups. It is found that average score of the experimental group increased more than the placebo group after 6 weeks and 12 weeks intervention (p<.05). There was significant difference in Pediatric Berg's Balance Scale (PBS) between two groups. It is found that average score of the experimental group increased more than the placebo group after 6 weeks and 12 weeks intervention (p<.05). Conclusion : Based on the results in this study, it is proposed that application of weighted vest into task-oriented training to facilitating closed kinematic chain and multi-joint movement can improve gross motor performance and balance abilities of children with cerebral palsy.
