Background : The purpose of this study was to determine whether cleansing the perineum and urethral meatus and using midstream urine affect the rate of bacterial contamination of urine specimens, and to determine the optimum urine collection method. We studied 41 asymptomatic healthy nursing school students. Women who were menstruating were not excluded from this study. Method : The first and midstream urine samples were collected during consecutive urinationsby each woman. The first sample was not a clean-catch specimen, and the second one was a clean-catch specimen. Both specimens were studied by urinalysis and bacterial culture with standard methods. Results : 41 women met the study criteria and 39 successfully completed the study. None of the urine cultures were positive. 68.3% of the non clean-catch first urine cultures, 53.7% of the non clean-catch midstream cultures, 33.3% of the first clean-catch urine culteres and 30.8% of the midstream clean-catch urine were found to be contaminated. There was a significant difference in the bacterial contamination rates between the first and midstream urine, and the clean-catch and non clean-catch urine(p=0.035, p =0.001 respectively). On urinalysis, 7.3% of the non clean-catch first urine, 7.3% of the non clean-catch midstream urine, 2.6% of the clean-catch first urine and 2.6% of clean-catch midstream urine were found to be above grade 2. Conclusions : According to our results, the bacterial contamination rate was the lowest in midstream and clean catch urine specimens. Threrfore it is recommended that the midstream clean-catch technique is the standard practice for collecting urine specimens for bacterial culture in women.
This study was designed to identify the relationship between perceived self-efficacy and self care behavioral compliance in type II diabetic patient. The subjects for this study were the 113 adults with type II diabetes that received hospital treatment or attended as outpatients in 2 general hospitals at Inchon City. The data were collected during the period from August 15 to semtember 15, 2000 by means of an interview. Collected data were analyzed by means of descriptive statistics, t-test, ANOVA, Pearson's correlation using the SPSS/PC+ program. The results of this study were as follows : 1. The mean score of self care behavioral compliance was 3.06. 2. There were statistically significant difference in self care behavioral compliance according to sex(p<0.05), age (p<0.001), BMI(p<0.05), education(p<0.001), occupation(p<0.001), experience of diabetic education(p<0.05) and complication(p<0.05). 3. The mean score of perceived self-efficacy was 3.45. Self care behavioral compliance and perceived self-efficacy had a positive correlation which was statistically significant (r=0.3879 p<0.001). 4. Stepwise multiple regression analysis was performed to identify the influencing factors for self care behavioral compliance. Education, experience of diabetic education and self-efficacy accounted for 41% of the variance in self care behavioral compliance. The results suggest that education, experience of diabetic education and self-efficacy are important variables in the compliance of self-care behavior with typeII diabetes.
The purpose of this study was to survey the ability of nurses to perform endotracheal suction to provide basic data on correct endotracheal suction. The participants in this study were 181 intensive care unit nurses in two university hospitals and one general hospital located in Daegu, one university hospital and two general hospitals located in Pohang, one university hospital located in Gyeongju. These data were collected from February 1, 2001 to March 10, 2001 using self reported questionnaires. The study instrument, which was developed by these investigators, included questions on general characteristics and performance of endotracheal suction. The results of this study are as follows : 1. Of the Participants 71.3% recognized correct endotracheal suctioning and 23.2% did not. 2. The greatest number, 57.5%, acquired knowledge about endotracheal suctioning at conferences on practice or education for staff. 3 There were significant differences in scores on performance of endotracheal suction according to department (F=2.60, P=.05) and hospitals (F=3.24, p=.01) among the general characteristics. 4. Slightly less than half of the participants, 47.8%, averaged correct scores for endotracheal suctioning procedures. Ventilator $O_2$ supply had the highest score (84.0%), normal saline exchange time, the lowest (14.4%) From the results, we suggest that education is needed to increase the knowledge and correct performance of endotracheal suction. Further study including direct observation of Performance of endotracheal suction is also needed.
Currently, health promotion is an important issue in health policy for world citizens. Aerobic workout may be one way to attain health promotion. In recent years, there ave been many studies to evaluate the effects of exercise on obese people and various patients. However, there are few studies in which both physiological and psycho-perceptual aspects of health romotion for healthy people have been evaluated. Purpose: This study focused on evaluating the holistic effect of an 8 week aerobic workout for health promotion in young women. Method: This quasi-experimental study was designed as a nonequivalent control group pre-post test study. Forty healthy college women, aged between 20 and 25 volunteered for the study. Twenty women took 30 minutes of exercise a day for five days a week over 8 weeks with Jane Fonda's aerobic workout program. Twenty women participated as controls. The aerobic workout program consisted of warm-up for arms, waist, abdominal, leg, hips, buttocks and cool down exercises. Result: After the eight weeks of aerobic exercise, changes in health related fitness components through the In Body Test, skin fold, circumference of body, Body image, Self concept, Mood and Self-efficacy were compared. The results areas follows ; 1. Following the 8 week aerobic workout, health related body components, and fitness scores, increased significantly (P < 0.05) and skin fold for four sites (right upper arm, scapular, abdomen, supra ilium), and circumference of mid thigh decreased significantly (P<0.05, p<0.005, P<0.001) compared to the control group. 2. Following the 8 week aerobic workout, there was a significant increase in Self-concept and Mood (P<0.005 and 0.001) compared to the control group. Conclusion: According to these results, it is concluded that 8 weeks of aerobic workout have a positive effect on young women's holistic health promotion.
Purpose: The purpose of this study was done to investigate the effects of oral care using 0.12% chlorhexidine and saline solution on oral status, incidence of oral pathogens and pneumonia among children who had cardiac surgery. Methods: The study participants were 84 children who underwent cardiac surgery and were admitted to the SICU of a university hospital. Oral care was provided with 0.12% chlorhexidine to the experimental group (n=43) and with saline solution to the control group (n=41), 3 times a day for 3 days. Before and after the intervention, the oral status, oral swab culture, and CPIS were checked. Results: There was no significant difference in oral status score between the two groups. The incidence of oral pathogens in the experimental group was significantly lower than that of the control group (t=-5.780, p<.001). The CPIS in experimental group was significantly lower than that of control group (t=-3.665, p<.001). Conclusion: The oral care with 0.12% Chlorhexidine is more effective than with only saline solution for reducing incidence of oral pathogens and pneumonia.
The purpose of this study was to investigate the level of alienation, depression, suicidal ideation and delinquency in high school students and to explore the relationships between the variables. The respondents, 226 students attending a high school located in Chonnam province made up the convenience sample. Data were collected using a structured questionnaire which included the alienation scale, suicidal ideation scale, BDI, and delinquency scale. Data collection was done between June 20 and July 6, 2001. Data were analyzed with the SAS program and included descriptive statistics, t-test, ANOVA and Pearson's correlation coefficients. There were no significant association between alienation and general characteristics of the students. The scores for depression were significantly higher in female students, in students who had recently dropped in their school ranking compared to students who had improved. The scores of suicidal ideation were significantly higher in male students. Also, the scores of delinquency were significantly higher in male students, and students in the high economic class and those with low school rankings. There were significant positive correlations between alienation and other ; alienation and depression (r= .432, p= .000), alienation and suicidal ideation (r= 267. p= 000), and alienation and delinquency (r= 150, p= .024). Findings from this study suggest that subsequent practical study in consideration of Korean sociocultural background is needed to identify the cause of alienation and develop supportive strategies to maintain mental health in high school students. Also, study on alienation of students should be based on family, community and popular culture in combination with improvements in school environment.
Purpose: This study was done to determine whether providing auditory and vestibular stimulation to preform infants would have an effect on stress hormones. Methods: The design was a nonequivalent control group protest-posttest design in a quasi-experimental study Seventy-nine preform infants were assigned either one of two experimental groups or to a control group: 27 in the auditory stimulation group, 25 in the vestibular stimulation group and 27 in the control group. The criteria for inclusion in this study were 1) gestational age of less than 37 weeks, 2) birth weight of less than 2,500g, 3) the absence of congenital anomalies or specific diseases, 4) recovering physiological weight loss, and 5) weaned from ventilatory assistance or oxygen. The data were collected from March 2002 to May 2003. The auditory stimulation, a music audiotape, was provided 20 minutes twice a day for 10 days and the vestibular stimulation, an infant waterbed, was provided for 10 days. On day 1 and day 10 of the study, 24 hour urine sample was collected for norepinephrine, epinephrine, and cortisol assays. In the data analysis SPSSWIN 10.0 program was utilized for descriptive statistics, ANOVA and t-test. Results: General characteristics of the three groups showed no significant differences, thus three groups were found to be homogenous. The 24 hour urine cortisol for the auditory (t=3.489, p=.001) and for the vestibular (t=2.638, p=.013) stimulation group were significantly reduced compared to the control group after 10 days. Conclusions: The results suggest that auditory and vestibular stimulation can be used to reduce 24 hour urine cortisol in preform infants. Therefore, music audiotapes and waterbeds provided in incubator are be recommended for reduction of the stress in preform infants who are hospitalized in neonatal intensive care units.
Purpose: The purposes of the study were to examine the knowledge and practice about Middle East Respiratory Syndrome (MERS) isolation precaution, and to explore influencing factors of the isolation practice among hospital nurses. Methods: A total of 182 nurses were recruited from four general hospitals where MERS patients had been treated. The knowledge and practice of MERS isolation precaution were measured by the scales developed based on the CDC guidelines. The collected data were analyzed by SPSS/WIN 22 with descriptive statistics, T-test, One-way ANOVA, Pearson correlation coefficients, and hierachical multiple regression analysis. Results: The nurses were 30 years old in average, and half of them had less than 5 years of clinical experience. and knowledge on droplet precautions (${\beta}=.171$, p=.019) were the significant predictors, explaining 19.6% of variance in the MERS isolation guideline practice. Clinical experience (${\beta}=.225$, p=.002), working at infection unit (${\beta}=-.203$, p=.011). Conclusion: The knowledge on droplet precaution and general knowledge on MERS were the important modifiable factor to improve the MERS isolation guideline practice among hospital nurses, even after adjusting clinical experience and demographic variables. It is necessary to develop an efficient education program on specific guidelines for prevention and management of infection by improving the knowledge on infectious disease such as MERS as well as droplet precaution which are modifiable factors.
Nurse's caring behaviors are essential to help patients cope with their health problems. The purpose of this study was to investigate the importance of nurse's caring behaviors as perceived by staff nurses and patients. The subjects were 295 staff nurses and 340 patients from five medical centers in Taegu. Data collection was done from Dec. 10, 1997 to Jan. 31, 1998. The tool for this study was a 7 point Likert type scale with 27 earing behaviors developed by Lee(1996). Cronbach alpha of the tool was .9701 in staff nurses and .9618 in patients. Data were analyzed with SAS using t-test, ANOVA and Spearman rank correlation program. The results are as follows : 1. The a verage score of perceived importance of nurse's taring behaviors for the 27 items was 5.65 in staff nurses and 4.97 in patients. 2. The most important caring behaviors perceived by the nurses was 'Nurse gives painless and cautious injection and treatment for patients' and for the patients, 'Nurse gives hygienic treatment and nursing care'. 3. The caring behavior which showed a significant difference between the ranks of perceived importance by staff nurses and patients was 'to treat the patient with tenderness and friendship'. 4. The Spearman rank correlation coefficient between the ranks of importance perceived by staff nurses and patients was 0.8302. With the results of this study, the staff nurses could be recognized to have much enthusiasm and passion about caring. It is suggested that nurses let the public know about the nurses' caring roles. In the future, research to investigate the actual practice of nurses' caring behavior is necessary.
Purpose: The purpose of this study was to investigate the occurrence of first onset of constipation during the rehabilitation stage and risk factors for constipation in patients with cerebral vascular disease (CVD). Methods: Participants for this retrospective study were 214 CVD patients admitted to rehabilitation units. First onset of constipation and factors influencing constipation such as dependency level were recorded for 14 days during the rehabilitation stage. Survival analysis with Cox proportional hazard model and descriptive statistics were conducted. Results: Age, patients' severity, types of diet, and dependency level were different between constipation and non-constipation groups. In survival analysis, 99% of participants developed constipation within 14 days after admission to rehabilitation units. Median constipation occurrence time was 7.4 days. Patients who were immobile and had tube feedings were more likely to develop constipation 4.07 times (95% CI: 1.018~16.301, p=.047) and 2.09 times (95% CI: 1.001~4.377, p=.050) respectively compared to patients who were independent and had a regular diet, respectively. Conclusion: Most CVD patients experienced constipation within 2 weeks after entering the rehabilitation stage. Constipation was linked to types of diet and dependency level. These factors associated with constipation should be considered when caring for patients in rehabilitation.
