Purpose: The purpose of this study was to identify the patterns of activities of daily living (ADL) functional status and to examine the relationships between sense of coherence(SOC), depression, and uncertainty in gynecologic cancer patients who were receiving chemotherapy. Method: A prospective, longitudinal design with repeated measures was utilized. Women reported depression, SOC, and uncertainty at the beginning of the first cycle of chemotherapy, and they recorded ADL functional status everyday for two consecutive treatment cycles. the The instruments used were the Karnofsky Performance Status Index, Orientation to Life Questionnaire, Beck Depression Inventory, and Mishel Uncertainty in Illness Scale-Adult Form. Result: Data from 42 women showed that the ADL functional status during the second cycle was better than that of the first cycle with significant improvement each week. However, it did not completely recover to the baseline level even three weeks after the treatment ended in both cycles. SOC was correlated with depression(r=-.64, p<.001) and uncertainty(r=-.62, p<.001). Uncertainty was related to depression (r=.66, p<.001) and to functional status during the second cycle(r=-.45, p<.05), while the scores of the functional status during the two cycles were not related. Conclusion: Changing patterns and level of functional status during the treatment phase would be useful information for cancer patients to prepare coping strategies for positive health outcomes.
Purpose: This study aimed to explore changes in symptom distress and functional status in gynecologic cancer patients during the entire treatment cycles of chemotherapy. Methods: A prospective and longitudinal study with repeated measures was designed. Symptom Distress Scale and Karnofsky Performance Status Index were included in a daily log developed for self-administration. A total of 39 patients with a mean age of 48.4 years participated. Results: The levels of symptom distress and functional status changed significantly over the six cycles. Symptom distress kept increasing until its peak at the fourth cycle, while the functional status scored lowest at the first cycle, then it improved as the cycle repeated. In each cycle, symptom distress was marked higher during the first 6 days accompanying poor functional status. However, both changes did not recover completely until the end of each cycle. Conclusion: Nursing assessment and intervention need to be provided based on these changing patterns to help cancer patients cope and adjust successfully during the long treatment period. Further studies are needed to examine the impact of the major symptoms on psychological responses, quality of life, and outcomes of the cancer treatments.
Purpose: This study was performed to identify factors affecting the functional status in disabled persons in the Jeiu Province. Method: Data were collected from 318 disabled persons in the Jeiu province during the period from the 6th of July to the 11th of October in 2006. The functional status was assessed by the disability assessment schedule II (Whodas II) of the World Health Organization and collected data were analyzed using the SAS 8.0 program. Result: The mean score of WHODAS II was 29.9. According to the results of multiple regression, factors affecting the functional status were perceived health condition(t=3.44, p<.001), brain disorder disability(t=2.55, p<.001), treatment status(t=-1.95, p=.05), drinking(t=2.09, p=.04), stress(t=-2.72, p=.01), depression(t=-2.70, p=.01). heart disease(t=2.62. p=.01) and anemia(t=2.20, p=.03). Conclusion: The functional status was affected by health behaviors, diseases, and the type of disability. Thus, future efforts to promote the functional status of disabled persons may need to take into account all these factors.

Purpose: The purpose of this study was to identify the factors that influence the functional status of patients with heart failure. Method: A descriptive, correlational study design was used. The participants in this study were 260 patients with heart failure who were admitted at Y University and U University in Seoul, Korea. Between September 2005 and December 2005 data was collected by an interview using a questionnaire and from medical records. The Functional status was measured with KASI. Physical factors (dyspnea, ankle edema, chest pain, fatigue, and sleep dysfunction), psychological factors (anxiety and depression), and situational factors (self-management compliance and family support) were examined. Result: In general, the functional status, anxiety, depression, self-management compliance, and family support was relatively not good. The level of fatigue was highest and the level of ankle edema was lowest for physical symptom experiences. In regression analysis, functional status was significantly influenced by dyspnea(23%), age(13%), monthly income(7%), fatigue(3%), ankle edema(2%), depression(1%), and length of stay in the hospital(1%). These factors explained 50% of the variables in the functional status. Conclusion: These results suggest that psycho-physiological symptoms management should be a focus to improve the functional status in patients with heart failure.
Objectives: This study was performed to identify factors related to perceived health status among patients with type 2 diabetes. Methods: This is cross-sectional observational study. Respondents were 106 visitors in an outpatient diabetes clinic of a university hospital. Self-report questionnaire which included general information inquiry, diabetes-related, sociopsychological factors, functional health literacy and perceived health status was used for this study. The data was analyzed by using descriptive statistics, independent simple t-test, one-way ANOVA, and hierarchical multiple linear regression. All analysis were conducted using SAS 9.3. Results: Among the respondents, 43.4% engaged in poorly perceived health status. After adjusting for control variables, functional health literacy is significantly related to perceived health status(${\beta}$=0.095, p=0.016). Conclusion: Independent of diabetes-related, sociopsychological factors, higher functional health literacy is associated with better perceived health status of patients with type 2 diabetes. In order to improve perceived health status in the type 2 diabetes patients, it is necessary to develop strategy to enhance the functional health literacy.
Purpose: To survey the functional status, self-esteem and depression of a community of indwelling clients with total laryngectomy, and to evaluate the correlation among these three variances. Methods: From May 2002 to October 2002, 76 clients who had undergone total laryngectomy more than three months ago were surveyed with using Baker's functional status in head & neck cancer-self report, Rosenberg's self rating self-esteem scale and Zung's self rating depression scale. Results: 1. The functionaI status score was $48.67{\pm}10.12$. The subjects aged 70 years or older had significantly higher scores than the subjects aged $50{\sim}59$ years. The group of clients who were operated on more than five years ago showed higher scores than the patients operated on within a year. 2. The self-esteem score was $30.47{\pm}6.44$. 3. The depression score was $47.26{\pm}8.01$. The subjects who bad three family members and the subjects who had five family members had significantly higher scores than the subjects who had two family members. 4. A significant correlation found between the functional status and self-esteem. Significant negative correlations were found between the functional status and depression and also between self-esteem and depression. Conclusion: Remarkable correlations were observed among functional status, self-esteem and depression. Nurses should consider the status of layngectomy patients and pay more attention to their pre- and post- operative nursing care and rehabilitation.
Purpose: This purpose of this study was to investigate the effects of initial cognitive status on the recovery of functional status in patients with subacute stroke. Methods: The participants were 111 patients with subacute stroke, divided into two groups: mini-mental state examination (MMSE) <20 (n=49) group and MMSE ≥20 (n=62) group. Clinical evaluation scores were collected before and after rehabilitation. The repeated measurements ANOVA was used to confirm the changes in functional status before and after intervention in the two groups. Changes in functional status within the group were examined through a paired test. A Pearson correlation coefficient analysis was performed to identify the correlation between MMSE change amount and functional status score. Results: In each of the two groups, according to the initial cognitive status, the clinical evaluation score increased statistically significantly, but there was no difference between the two groups in the degree of significant increase. When examining the correlation between the MMSE change amount, according to the initial cognitive state and the functional state score change amount, it was found, only in the group with MMSE <20, that the larger the change in the MMSE score, the greater the functional state change of Berg balance scale, Rivermead Mobility Index, and motor assessment scale. This did not apply to the group with MMSE ≥20. Conclusion: Initial cognitive status should be considered when setting the patient's goal, and considering cognitive improvement when constructing a rehabilitation program is thought to have a positive effect on rehabilitation services.
Purpose: This study was conducted to extract international classification of functioning, disability, and health (ICF) core sets for measuring functional status in acute stroke patients, and to evaluate clinical applicability of the core sets. Methods: A set of 22 ICF items on functional status in acute stroke patients were extracted from the Korean general ICF core sets and ICF core sets for stroke patients. The extracted ICF functional items were assessed at the time of admission and discharge among 100 stroke patients who were admitted in a university hospital. Results: Comparing to functional status at admission of acute stroke patients, the overall functional status at discharge was improved. However, functions on defecation, skin protection, and relationship with immediate family at the time of discharge were not significantly changed. Conclusion: The set of ICF functional items identified in this study may be reliable and valid to assess acute stoke patients' body functions, activities and participation and environmental factors in the holistic and comprehensive nursing context. Nursing interventions on bowel elimination and skin protection for acute stroke patients need to be developed.
Objective: Setting therapeutic goals assist to identify patient priorities and predict problem that may arise in performing daily and functional activities. Goal setting is associated with improved rehabilitation outcomes in individuals with hemiparetic stroke. This study was conducted to assess the effects of having awareness and satisfaction of goals on the functional status of individuals with hemiparetic stroke. Design: Cross-sectional study. Methods: Eighty-one people who had been diagnosed with stroke participated in this study. The study was conducted through a face-to-face surveys to identify participants' awareness and satisfaction of goals and standard assessment tools were used to evaluate patients' functional status. This study measured functional status through three clinical measures including the manual function test (MFT), Berg Balance scale (BBS), and modified Barthel index (MBI). Results: In participants with hemiparetic stroke, the MBI, BBS, and MFT scores were significantly higher in the participants who were aware of the therapeutic goal than in those who were unaware (p<0.05). The MBI, BBS, and MFT scores were not significant between the participants who were satisfied with the goal and those who were unsatisfied. Conclusions: This study suggests that having awareness in therapeutic goal-setting could lead to improvement of functional status in those with hemiparetic stroke. This study highlights how goal-setting can have a positive effect on persons with hemiparetic stroke.
Recovery of functional status and affecting factors it were examined at 1 and 2 months after delivery in 111 women who defined as the resumption of household, self-care, social & community, and occupational activities, and assumption of infant care responsibilities. The data were collected by Tulman & Fawcett (1988)'s IFSAC questionnaire at 2 local hospitals & health center in Kwangju city & Chonnam province. The results were as follows : 1. Total mean scores of functional status were 2.3 points and increased in total functional status between 1 & 2 months after childbirth. The analysis revealed significant changes in 3 dimensions-household, social & community, and occupational activities-of IFSAC between 1 & 2 months after childbirth. 2. Mean scores of 5 dimensions of IFSAC were : self-care activities, 1.9 points : household activities, 2.7 points ; infant care responsibilities, 3.6 points ; occupational activities, 1.5 points ; and social & community activities, 1.6 points. 3. The job affected significantly the household, occupational activities and infant care responsibilities. And the complication of infant or mother affected significantly the infant care responsibilities in association with recovery functional status. Based on the findings and a review of the literature in regard to our understanding of recovery of functional status, the following recommendations were derived. 1. Future research needs to be policied concerning length of maternity leave after delivery on a firmer basis throught longitudinal study. 2. The data also nay be used to develop individual interventions to facilitate recovery from childbirth. 3. The IFSAC may be used to clinical assessments of functional status in the case of women cancer or obstetric and gynecologic operation.
