제2형 당뇨병 환자의 기능적 헬스 리터러시가 주관적 건강에 미치는 영향: 일개 대학병원 외래 환자를 대상으로 (Factors Related to Perceived Health Status in Patients with Type 2 Diabetes)

  • Objectives: This study was performed to identify factors related to perceived health status among patients with type 2 diabetes. Methods: This is cross-sectional observational study. Respondents were 106 visitors in an outpatient diabetes clinic of a university hospital. Self-report questionnaire which included general information inquiry, diabetes-related, sociopsychological factors, functional health literacy and perceived health status was used for this study. The data was analyzed by using descriptive statistics, independent simple t-test, one-way ANOVA, and hierarchical multiple linear regression. All analysis were conducted using SAS 9.3. Results: Among the respondents, 43.4% engaged in poorly perceived health status. After adjusting for control variables, functional health literacy is significantly related to perceived health status(${\beta}$=0.095, p=0.016). Conclusion: Independent of diabetes-related, sociopsychological factors, higher functional health literacy is associated with better perceived health status of patients with type 2 diabetes. In order to improve perceived health status in the type 2 diabetes patients, it is necessary to develop strategy to enhance the functional health literacy.

서울 및 경기지역 노인의 건강자가평가에 따른 기능적 건강 및 영양위험 평가 (Evaluation of Functional Ability and Nutritional Risk according to Self-Rated Health (SRH) of the Elderly in Seoul and Kyunggi-do)

  • A functional ability and adequate nutritional status are the major determinants of health status, Self-rated health (SRH) is a worldwide method to assess health status and it is recognized as a predictor of morbidity and mortality in the elderly, This study was designed to evaluate the functional ability and nutritional risk according to SRH in the elderly. Four hundred nine free-living elderly people (118 male, 291 female), aged $\geq$ 65 years were interviewed by trained interviewers using structured questionnaires including demographic information, SRH, anthropometric measurements, functional ability, general health status, and nutritional risk. SRH was divided into three status such as “Good”, “Moderate” and “Poor” status. And all the data were analyzed by oneway ANOVA, spearman correlation, and x$^2$ analysis using SPSS 9.0 version at p 〈 0.05. Of all the subjects, 48.9% perceived their health status as “poor”, and their functional abilities (activities of daily living, instrumental activities of daily living) were more impaired than their counterparts (“good” and “moderate”). Poor self-rated health was also related to: a higher prevalence of illnesses (p 〈 0,001) especially in hypertension, arthritis. Self-rated health was significantly related to food security (p 〈 0.001), food enjoyment (p 〈 0.001) ,and nutritional knowledge (p = 0.0 13). Also NSI checklist total score was the highest in “poor” health status (p 〈 0.001). Better self-rated health was related to better food security, and better food enjoyment. However, smoking, alcoholic intake, exercise, eating behaviors, and demographic characteristics were not significantly different among the three SRH status. SRH was closely related to chronic diseases, functional ability, and nutritional risk in the elderly. Therefore, public health strategies for the elderly should be focused on the elderly who are “poor” in SRH, to improve nutritional status and functional ability, and to reduce risk factors of chronic diseases.

일개지역 저소득층 노인의 신체적 기능상태에 관한 연구 (A Survey on Functional Status among Low-Income Older Adults Living at Home)

  • The purpose of the study was to analyze the functional status of low income elderly living at home according to their socio-economic factors, sensory function, health status, medical service utilization, commodity and types of chronic disease. Method: Functional status was defined by the level of mobility, ADL and IADL categorized as independently functional, mildly impaired, moderately disabled, and severely disabled. The data was collected by home-visit interviews with 567 community dwelling adults who were 65 years of age or more with low a income status subsidized by government in ChonAn. Results: 9.9% of community dwelling older adults were severely disabled, and 44.4% were moderately disabled in their functional status. There were significant differences in the functional status by age, education, religion, and types of family structure. The older adults with hearing impairment or dental problems had a significantly higher rate of severe disability. Self-rated health status and medical service utilization were also significant factors to the differences in functional status. The functional status of older adults was also significantly related to the presence of chronic health problems such as chronic back pain, stroke, and Alzheimer-dementia. Conclusion: The results confirmed that community dwelling older adults with low income status were more functionally disabled in comparison to general older adults at national level, while the relating factors to their functional status seemed similar to other studies on older adults. Further studies were suggested to look into functional status longitudinally and focus on the changes of functional status by managing modifiable influencing factors.

WHODAS II를 이용한 장애인의 기능상태와 관련 요인 (Functional Status and Related Factors of Disabled Persons Using WHODAS II)

  • Purpose: This study was performed to identify factors affecting the functional status in disabled persons in the Jeiu Province. Method: Data were collected from 318 disabled persons in the Jeiu province during the period from the 6th of July to the 11th of October in 2006. The functional status was assessed by the disability assessment schedule II (Whodas II) of the World Health Organization and collected data were analyzed using the SAS 8.0 program. Result: The mean score of WHODAS II was 29.9. According to the results of multiple regression, factors affecting the functional status were perceived health condition(t=3.44, p<.001), brain disorder disability(t=2.55, p<.001), treatment status(t=-1.95, p=.05), drinking(t=2.09, p=.04), stress(t=-2.72, p=.01), depression(t=-2.70, p=.01). heart disease(t=2.62. p=.01) and anemia(t=2.20, p=.03). Conclusion: The functional status was affected by health behaviors, diseases, and the type of disability. Thus, future efforts to promote the functional status of disabled persons may need to take into account all these factors.

  • Purpose: This study was conducted to explore the mediating effect of nutritional status on the relationship between symptom experience and functional status of patients with Chronic Obstructive Pulmonary Disease (COPD). Methods: A total of 141 COPD patients visiting D hospital and I hospital in B city were enrolled in this study. Data were collected from January 2017 to July 2017. Outcome variables were measured by Mini Nutritional Assessment Short-Form (MNA-SF) for nutritional status, The Memorial Symptom Assessment Scale (MSAS) for symptom experience, and The Functional Performance Inventory Short Form (FPI-SF) for functional status. The data were analyzed with descriptive statistics, Pearson's correlation, and path analysis using SPSS/WIN 21.0 and AMOS 25. Results: There was a significant negative correlation between symptom experience and nutritional status (r=-.61, p<.001), a significant negative correlation between symptom experience and functional status (r=-.40, p<.001), and significant positive correlation between nutritional status and functional status (r=.47, p<.001). Symptom experience had indirect effects on functional status through nutritional status. Conclusion: For enhancing functional status in COPD patients, it is necessary to develop nursing intervention programs to enhance symptom management as well as nutritional status.

  • Purpose: This study was conducted to explore the mediating effect of nutritional status in the relationship between symptom experience and functional status among in patients with lung cancer undergoing chemotherapy. Methods: Subject (N=139) were lung cancer patients visiting the D hospital in B city. Data collection was conducted from May 2016 to February 2017. Seven nutritional status measurement tools, 22 symptom experience tools, and 15 EORTC QLQ-C30 functional scales were used to measure nutritional, symptom, and functional levels. The collected data were analyzed by SPSS / WIN 21.0 using descriptive analysis, Pearson correlation, 3-step regression analysis of Baron and Kenny, and Sobel test. Results: There was a significant positive correlation between symptom experience and nutritional status, a significant negative correlation between symptom experience and functional status, and significant negative correlation between nutritional status and functional status. Symptom experience had direct effect and indirect effects on functional status through nutritional status. Conclusion: For enhancing functional status in lung cancer patients, it is necessary to develop nursing intervention programs to palliate symptoms and improve nutritional status.

농촌 지역 노인의 기능적 의료정보 이해능력 실태와 영향요인 (Influencing Factors on Functional Health Literacy among the Rural Elderly)

  • Purpose: The purpose of this study was to describe the status of the functional health literacy of the elderly living in a rural community and to identify the influencing factors on the functional health literacy. Methods: Subjects(n=238) aged over 60 years were selected for data collection. Functional health literacy was measured by Korean Health Literacy, which consisted of 15 items including eight numeracy and seven comprehension items. Descriptive statistics, t-test, ANOVA with Scheffe test, and stepwise multiple regression were used to analyze the data. Results: The mean score of total functional health literacy was $6.66{\pm}3.31$, numeracy was $3.52{\pm}1.76$, and comprehension was $3.41{\pm}2.10$. Of the subjects, 39.1% showed a low score below the median split. In the results of stepwise multiple regression, the factors affecting functional health literacy were basic literacy, age, number of chronic diseases, self-reported health status, and gender. These factors accounted for 33.0% of functional health literacy. Conclusion: These results suggest that interventions for improving functional health literacy are necessary to enhance elders' ability of self-care management. In addition, it is needed to take into account the individual characteristics of the subjects depending on their gender, age, and health status.

급성기 뇌졸중 환자의 기능상태 측정을 위한 ICF 핵심지표 (ICF Core Sets for Measuring Functional Status of Acute Stroke Patients)

  • Purpose: This study was conducted to extract international classification of functioning, disability, and health (ICF) core sets for measuring functional status in acute stroke patients, and to evaluate clinical applicability of the core sets. Methods: A set of 22 ICF items on functional status in acute stroke patients were extracted from the Korean general ICF core sets and ICF core sets for stroke patients. The extracted ICF functional items were assessed at the time of admission and discharge among 100 stroke patients who were admitted in a university hospital. Results: Comparing to functional status at admission of acute stroke patients, the overall functional status at discharge was improved. However, functions on defecation, skin protection, and relationship with immediate family at the time of discharge were not significantly changed. Conclusion: The set of ICF functional items identified in this study may be reliable and valid to assess acute stoke patients' body functions, activities and participation and environmental factors in the holistic and comprehensive nursing context. Nursing interventions on bowel elimination and skin protection for acute stroke patients need to be developed.

노인의 성공노화 구조모형 -선택.최적화.보상 전략을 중심으로- (Structural Equation Modeling on Successful Aging in Elders - Focused on Selection.Optimization.Compensation Strategy -)

  • Purpose: This study was designed to construct and test a structural equation modeling on specific domain health status and the Selection Optimization Compensation (SOC) strategy affecting successful aging in elderly people. Methods: The model construction was based on the SOC model by Baltes and Baltes. Interviews were done with 201 elderly people aged 65 or older. Interview contents included demographics, functional health status, emotional health status, social health status, SOC strategies, and successful aging. Data were analyzed using SPSS 15.0 and AMOS 7.0. Results: Model fit indices for the modified model were GFI=.93, CFI=.94, and RMSEA=.07. Three out of 7 paths were found to have a significant effect on successful aging in this final model. Functional health status had a direct and positive effect on successful aging. Emotional health status influenced successful aging through SOC strategies. Conclusion: This study suggests that interventions for improving functional health status and for strengthening SOC strategies are critical for successful aging. Continuous development of a variety of successful aging programs using SOC strategy is suggested.

입원 환자의 건강정보문해력에 관한 연구 (Health Literacy of Inpatients at General Hospital)

  • Purpose: The purpose of this study was to investigate the health literacy of inpatients at general hospitals. Methods: The sample of this study was 157 inpatients from secondary general hospitals located in a J city during April 1st to April 15th, 2011. The linguistic health literacy was measured by Korean Health Literacy Assessment Tool (KHLAT). The functional health literacy was measured by Korean Functional Health Literacy Test (KFHLT). Results: The mean score of the linguistic health literacy was $36.03{\pm}16.33$ (percent of correct answer: 54.6%). The mean score of the functional health literacy was $9.62{\pm}4.75$ (percent of correct answer: 56.6%). The linguistic and functional health literacy were by age, education level, marital status, socioeconomic status and their health status. There was statistically a significant correlation between the linguistic health literacy and the functional health literacy (r=.75, p<.001). Conclusion: The health literacy of inpatients was strongly related to education level and age. There is a need to develop the educational materials for adjusting the health literacy level of inpatients.
