Clinical Application of Functional MRI : Motor Cortex Activities by Acupuncture

  • We report a preliminary fMRI evidence of modulation of somatomotor areas by acupuncture in GB34 acupoint. GB34, located in the back of the knee, is known to be effective in recovering motor function after stroke. Using 3T MR scanner, functional MR imaging was performed in five normal volunteers in two stimulation paradigms; acupuncture manipulation on GB34 and sham points. Group analysis from five individuals showed that bilateral sensorimotor areas (BA 3,4,6 and 7) showed stimulation related BOLD signal contrast of approximately 6% whereas very few areas were activated when sham stimulation is given. Our results suggest that acupuncture fMRI study can be safely conducted in 3T environment, and stimulation in GB34 modulate the cortical activities of the somatomotor area in human.

  • The purpose of this study is to analyze the facility standards of the elderly welfare facilities applying the seven principles of the universal design. The facility standards of the elderly welfare facilities were limited to the provisions of the "Enforcement Regulation of the Elderly Welfare Act". There are few legal regulations applying the universal design in the construction of the elderly welfare facilities. The facility standards are set only with the minimum requirements such as securing the safety of the user, securing the space area, functional convenience, and risk prevention. However, the contents of the regulations are limited to the minimum standards for functional convenience and safety required for living. In conclusion, in the elderly welfare facilities, the notion of universal design, which considers not only the elderly, but also service providers, employees, and general visitors, should be considered.

3.0T 자기공명영상장치를 이용한 사람의 간지럼자극과 감각중추 자극의 활성화 차이 (Difference of fMRI between the Tickling and Sensory Stimulation Using 3.0 Tesla MRI)

  • 기능적 자기공명영상법(fMRI)을 이용하여 일반 감각 자극과 다른 간지럼 자극 과제를 수행 할 때 대뇌 감각중추 신경 연결망을 규명하고, 간지럼이 웃음의 기전과 어떤 차이가 있는지를 알아보고자 하였다. 건강한 성인 남녀 16명(평균 : 28.9세)을 대상으로 두 종류의 감각 자극 과제 수행동안 3.0T 자기공명영상장치를 사용하여 기능적 자기공명영상을 얻었다. 감각 자극은 피험자마다 역균형화하여 제시되었으며, 블록 설계로 자극 제시와 영상 획득이 이루어졌다. 획득된 영상 데이터는 SPM 99 분석하였으며, 개별 분석과 그룹 분석을 실시하였다. 개별 분석 결과 두 과제 모두 체감각 영역의 활성화가 관찰되었고, 간지럼 자극 조건은 감각자극 조건에 비해 베리니케 영역(BA40)에서 더 많은 활성화를 보였다. 또한, 그룹 분석결과 일반 감각 조건에서는 양쪽 체감각 피질 영역(BA 1,2,3)이 활성화되었으며, 간지럼 조건에서는 양쪽 체감각 피질 뿐만 아니라 시상, 대상회, 대뇌섬엽 영역에서 커다란 활성화를 보였다. 간지럼 자극에서 감각자극을 뺀 결과에서는 우측 대상회와 좌측 MFG 영역 및 좌측섬엽 에서 유의미한 활성화를 보였다. 촉각을 통한 간지럼자극을 인지하는 대뇌영역에 대해 검증하였고, 간지럼과 같은 가장 원초적인 자극이 다양한 사회적 활동에서 중요한 기능을 담당하는 웃음과 밀접한 관련이 있음을 알 수 있었다.

하악 우각부 골절의 소형금속판 내고정 후 기능시 골접합선의 안정도에 관한 방사선학적 평가 (RADIOLOGICAL EVALUATION OF FRACTURE LINE STABILITY DURING FUNCTIONAL LOADING AFTER MINIPLATE FIXATION OF MANDIBULAR ANGLE FRACTURES)

  • After miniplate fixation of mandibular angle fractures, fracture line stability during functional loading was evaluated. Using panoramic radiographs, 15 mandibular angle fracture patients who were treated by open reduction and one miniplate fixation along the external oblique ridge, were evaluated at postoperative 1, 4 and 8 weeks. At each time, 2 radiographs were taken: one taken during maximum biting of hardened silicone sheet on the affected side molar area and the other on the non-affected side. The distraction gap of inferior border of mandible at each time and each side was measured and these data were analysed statistically with clinical findings. The differences of inferior border distraction gap during hardened silicone sheet biting on the affected side molar area and on the non-affected side molar area at 4 week radiographs were smaller than those of 1 week's except one case. At 8 week's radiographs, the fracture lines were so stabilized that it was almost impossible to find the gap differences except one case and there were increased radiopacity along the entire fracture lines. Clinically, bony union was confirmed in all cases during plate removal performed at postoperative 6 month. By statistical analysis(paired t-test), the inferior border distraction gap during biting of hardened silicone sheet on the affected side was significantly reduced during 1 and 4 week interval(p<0.01). The differences of inferior border distraction gap during biting on the affected side molar area and on the non-affected side molar area were also significantly reduced at 1 and 4 week interval(p<0.01). But the inferior border distraction(compression) gap during non-affected side biting was not significantly changed. From these findings, it could be concluded that fracture line stability during functional loading after one miniplate fixation of mandibular angle fractures stems mainly from reduction of inferior border distraction gap during affected side biting on time interval. According to these radiographic and clinical findings, the clinical superiority of one miniplate fixation technique in mandibular angle fracture treatment could be confirmed.

Acupuncture stimulation for motor cortex activities: Evidence from 3T functional MRI study

  • Purpose: To investigate whether or not acupuncture of GB34 produces a significant response of the modulation of somatomotor areas by functional magnetic resonance imaging (fMRI) study. Methods: The acupoint, GB34, located in the back of the knee, is known to be effective in recovering motor function after stroke. Using 3T MRI scanner, functional MR imaging of the whole brain was performed in 12 normal healthy subjects during two stimulation paradigms; acupuncture manipulation on GB 34 and sham points. This study investigates the activation of the motor cortex elicited by a soft and an intensified stimulation of GB 34. Three different paradigms were carried out to detect any possible modulation of the Blood Oxygenation Level Dependent (BOLD) response in the somatomortor area to motor stimulation through acupuncture. Results: Group analysis from seven individuals showed that bilateral sensorimotor areas (BA 3,4,6 and 7) showed stimulation related BOLD signal contrast of approximately 6% whereas very few areas were activated when sham stimulation is given. Conclusions: The present study shows that acupuncture fMRI study can be safely conducted in 3T MRI environment, and acupuncture stimulation in GB34 modulates the cortical activities of the soma- to motor area in human. The present findings may shed light on the CNS mechanism of motor function by acupuncture and form a basis for future investigations of motor modulation circuits in the stroke patients. Influence of oxyfluorination on activated carbon nanofibers for CO2 storage

  • The oxyfluorination effects of activated carbon nanofibers (OFACFs) were investigated for $CO_2$ storage. Electrospun CFs were prepared from a polyacrylonitrile/N,N-dimethylformamide solution via electrospinning and heat treatment. The electrospun CFs were chemically activated in order to generate the pore structure, and then oxyfluorination was used to modify the surface. The samples were labeled CF (electrospun CF), ACF (activated CF), OFACF-1 ($O_2:F_2$ = 7:3), OFACF-2 ($O_2:F_2$ = 5:5) and OFACF-3 ($O_2:F_2$ = 3:7). The functional group of OFACFs was investigated using X-ray photoelectron spectroscopy analysis. The C-F bonds formed on surface of ACFs. The intensities of the C-O peaks increased after oxyfluorination and increased the oxygen content in the reaction gas. The specific surface area, pore volume and pore size of OFACFs were calculated by the Brunauer-Emmett-Teller and density functional theory equation. Through the $N_2$ adsorption isotherm, the specific surface area and pore volume slightly decreased as a result of oxyfluorination treatment. Nevertheless, the $CO_2$ adsorption efficiency of oxyfluorinated ACF improved around 16 wt% due to the semi-ionic interaction effect of surface modificated oxygen functional groups and $CO_2$ molecules.

Advanced neuroimaging techniques for evaluating pediatric epilepsy

  • Accurate localization of the seizure onset zone is important for better seizure outcomes and preventing deficits following epilepsy surgery. Recent advances in neuroimaging techniques have increased our understanding of the underlying etiology and improved our ability to noninvasively identify the seizure onset zone. Using epilepsy-specific magnetic resonance imaging (MRI) protocols, structural MRI allows better detection of the seizure onset zone, particularly when it is interpreted by experienced neuroradiologists. Ultra-high-field imaging and postprocessing analysis with automated machine learning algorithms can detect subtle structural abnormalities in MRI-negative patients. Tractography derived from diffusion tensor imaging can delineate white matter connections associated with epilepsy or eloquent function, thus, preventing deficits after epilepsy surgery. Arterial spin-labeling perfusion MRI, simultaneous electroencephalography (EEG)-functional MRI (fMRI), and magnetoencephalography (MEG) are noinvasive imaging modalities that can be used to localize the epileptogenic foci and assist in planning epilepsy surgery with positron emission tomography, ictal single-photon emission computed tomography, and intracranial EEG monitoring. MEG and fMRI can localize and lateralize the area of the cortex that is essential for language, motor, and memory function and identify its relationship with planned surgical resection sites to reduce the risk of neurological impairments. These advanced structural and functional imaging modalities can be combined with postprocessing methods to better understand the epileptic network and obtain valuable clinical information for predicting long-term outcomes in pediatric epilepsy.

종합병원 로비의 기능요소에 따른 공간구조 분석 (Analysis of the Spatial Configuration of Lobby in General Hospital)

  • General hospitals in Korea have increased more in size since 2000, and as not only medical service but the quality of the environment also got important, the considerable changes have appeared revolving around the lobbies. This study is a basic research to find a direction of a plan for general hospital lobbies in response to the demands that change. It was examined the current condition of lobbies so as to derive the idea of types of lobbies and of functional elements, and based on the research, it was compared and analyzed in terms of the spatial configuration to draw the characteristics by type. The result of the research is as follows. (1) The lobbies were explored after being classified into four types; the plan(central type/street type) and the elevation(eclectic type/double-story type). (2) The hall and corridor were ranked high in order of importance in the whole spatial configuration, and it has been utilized in various ways. (3) Also, each type differed with the others in its main functional area, regarding traffic line efficiency and spatial intelligibility.

The Molecular Modeling of Novel Inhibitors of Protein Tyrosine Phosphatase 1B Based on Catechol by MD and MM-GB (PB)/SA Calculations

  • Binding modes of a series of catechol derivatives such as protein tyrosine phosphatase 1B (PTP1B) inhibitors were identified by molecular modeling techniques. Docking, molecular dynamics simulations and free energy calculations were employed to determine the modes of these new inhibitors. Binding free energies were calculated by involving different energy components using the Molecular Mechanics-Poisson-Boltzmann Surface Area and Generalized Born Surface Area methods. Relatively larger binding energies were obtained for the catechol derivatives compared to one of the PTP1B inhibitors already in use. The Molecular Mechanics/Generalized Born Surface Area (MM/GBSA) free energy decomposition analysis indicated that the hydroxyl functional groups and biphenyl ring system had favorable interactions with Met258, Tyr46, Gln262 and Phe182 residues of PTP1B. The results of hydrogen bound analysis indicated that catechol derivatives, in addition to hydrogen bonding interactions, Val49, Ile219, Gln266, Asp181 and amino acid residues of PTP1B are responsible for governing the inhibitor potency of the compounds. The information generated from the present study should be useful for the design of more potent PTP1B inhibitors as anti-diabetic agents.
