신경근 훈련이 기능적 발목 불안정성 축구선수의 자세조절과 점프 수행력에 미치는 영향 (Effect of Neuromuscular Training on Postural Control and Jump Performance in Functional Ankle Instability Soccer Player)

  • The purpose of this study was to investigate the effects on jump performance and balance in soccer player with functional ankle instability of difference of neuromuscular training. In 33 male college soccer player with functional ankle instability subjects of this study randomization, combined training group (group I, n=11), balance training group (group II, n=11) and control group (group III, n=11) that included in the plyometric training and balance training was classified group. The intervention was conducted three times a week for 8 weeks. Before and after intervention, measured in surface area ellipse and countermovement jump and countermovement jump with arm swing. Showed a significant improvement in postural control and jump performance from the combined training group and balance training group compared to the control group. Showed a significant improvement in countermovement jump from the combined training group compared to the balance training group. Combined training and balance training showed the increased jump performance and postural control in soccer player with functional ankle instability.

족관절 외측 불안정성 재건 시 시행하는 탐험적 관절경의 유용성 (Chronic Lateral Ankle Instability: Efficacy of Combination of Ankle Arthroscopic Exploration and Modified Brostrom Operation)

  • Purpose: To evaluate the efficacy of the arthroscopic exploration combined with modified Brostrom operation (MBO) for the treatment of chronic lateral ankle instability (CAI). Materials and Methods: Sixty patients who diagnosed a CAI were performed MBO. We divided to 2 groups, whether simultaneously inspected by arthroscopy (group B) or not (group A). The both group's results were compared according to American Orthopedic Foot and Ankle Society Ankle-hindfoot score (AOFAS), functional ankle score and visual analog scale (VAS) at preoperative and final follow-up period. Results: There were no significant differences of AOFAS, functional ankle score and VAS between both groups at final follow-up. However, in group A, 2 cases associated with medial ankle instability and syndesmotic injuries were did not diagnosed preoperatively, showed poor prognosis. In group B, one case had a permanent peroneal nerve symptom. The match rate of intra-articular lesions between preoperative diagnosis and postoperative arthroscopic diagnosis was 30% in group B. Conclusion: Combination of arthroscopic exploration and MBO is effective strategy for intra-operatively discrimination of intra-articular associated lesions for CAI.

발목 불안정환자에 대한 전신음파진동기와 토구를 이용한 불안정 지지면 운동의 효과 비교 (Comparison of the Effects of Unstable Support Exercise Using Whole Body Sonic Vibrator and TOGU for Patients with Ankle Instability)

  • Purpose : Chronic ankle instability can lead to problems in balance and gait due to weakness of the ankle muscles and decreased proprioceptive sensation. Balance training that stimulates proprioceptors is necessary to improve ankle stability. We aimed to compare the effects of unstable support balance exercises using whole body sonic vibration and an aero-step (TOGU) on proprioception and balance in individuals with unilateral functional ankle instability. Methods : Thirty-six participants with unilateral functional ankle instability were randomly recruited and divided into two groups (group 1 = sonic whole body vibration, group 2= TOGU). Individuals in each group participated in training for 5 weeks, 40 minutes per session, 5 times per week, and performed weight-bearing exercises in five postures on different unstable support surfaces. Proprioception was measured by digital inclinometer (Dualer IQ), and balance was measured by force platform (Biodex balance system). Results : Significant differences were observed in proprioception before and after intervention within both group (p<.05). Significant differences were also observed in the balance index before and after intervention within both groups (p<.05). Conclusion : As a result of this study, it is suggested that for individuals with ankle instability, unstable support surface training using a whole body sonic vibrator and TOGU can have a positive effects on proprioception and balance ability.

  • This paper describes a technique, which analyzes the functional instability of the ankle using three-dimensional scanner. The technique is based on the structured light pattern projection method, which is performed by using one digital still camera and one LCD projector. This system can be easily realized with the low cost. The measuring result has high accuracy. The measuring error is about 0.2 mm or less. Using this technique the three-dimensional posture of the leg and foot of the target person are measured and analyzed.

Effect of Ankle Stabilization Training Using Biofeedback on Balance Ability and Lower Limb Muscle Activity in Football Players with Functional Ankle Instability

  • Purpose: This study focuses on influence of ankle stabilization training on balance ability and lower limb muscle activation of soccer player with functional ankle instability. Methods: Subjects were grouped into ankle stabilization training group using biofeedback comprised of 15 subjects and general exercise group of 15. The training was conducted for 30 minutes, 3 times a week for 8 weeks in total. All 30 football players conducted plyometric training for 30 minutes before main training. To evaluate balance ability, biorescure was used to measure whole path length and surface area and surface electromyography (EMG) system was used to measure tibialis anterior, tibialis posterior, and soleus to evaluate lower limb muscle activation. Results: The experiment group showed significant difference to the comparison group in regard of whole path length and surface area which represents balancing capability and muscle activation of tibialis anterior, tibialis posterior, and soleus. Conclusion: Therefore, ankle stabilization training using biofeedback is more effective in enhancing balance ability and lower limb muscle activation than general exercise.

Effect of Jumping Exercise on Supporting Surface on Ankle Muscle Thickness, Proprioception and Balance in Adults with Functional Ankle Instability

  • Background: Functional ankle instability (FAI) indicating a decrease in muscle strength, proprioception, neuromuscular control, balance and postural control function. Objective: To investigate the effect of jumping exercise on the supporting surface on the ankle muscle thickness, proprioceptive sensation, and balance in adults with FAI. Design: Randomized Controlled Trial. Methods: Twenty young people with FAI were randomly assigned to the unstable supporting surface jump group (N=10) and the stable supporting surface jump group (N=10). The intervention was conducted three times a week for eight weeks, and for 30 minutes per session. Trampoline was used as an unstable support surface and the stable support surface was carried out on a regular floor. The thickness of the tibialis anterior muscle and medial gastrocnemius muscle was measured by ultrasonography, and the proprioception of dorsiflexion and plantarflexion was measured using an electrogoniometer. The dynamic balance was also measured with a balance meter. Results: The the muscle thickness of the medial gastrocnemius muscle was significantly higher in the stable supporting surface jump group than in the unstable supporting surface jump group (p<.05). Furthermore, the plantar flexion proprioception and dynamic balance were significantly improved in the unstable supporting surface jump group than in the stable supporting surface jump group in the intergroup comparison (p<.05). Conclusions: The conclusion has been reached in this study that the jumping exercise on the unstable supporting surface could be a more effective in improving FAI than the regular surface.

기능적 발목 불안정성시 하지 근피로에 의한 동적균형이 족저압에 미치는 영향 (The Effect of the Plantar Pressure on Dynamic Balance by Fatigue of Leg in the Subjects with Functional Ankle Instability)

  • 본 연구는 기능적 발목 불안정성시 하지 근피로도에 따른 동적균형이 족저압에 미치는 영향에 대해 알아보고자 하였다. 본 연구는 설문조사를 통해 기능적 발목 불안정성으로 진단된 14명의 발목불안정집단과 16명의 발목안정집단 총 30명을 대상으로 실시하였다. 하지의 근피로는 Biodex system III, 족저압은 Zebris FDM-S system으로 측정하였고, 한발로 점프 후 착지 검사로 동적균형을 실시하였다. 동적균형시 족저압 분포비율은 발목불안정집단의 전족부(p2, p3, p4)에서 높게 나타났다(p<0.05). 동적균형시 근피로 유발 전후 족저압 분포 비율은 발목 불안정집단에서 발목 안정집단보다 전족부(p2, p3, p4)와 외측 중족부(p6)에서 유의하게 높았으나 종골부(p7)에서는 낮았다(p<0.05). 동적균형시 족저압 중심(CoP)의 전후이동거리(AP)와 좌우이동 폭(ML)은 발목 불안정집단이 발목 안정집단에 비해 근피로 유발 전보다 유발 후 유의하게 길었다(p<0.05). 기능적 발목불안정시 하지 근피로도에 따른 동적균형은 족저압 분포와 족저압 중심이동에 영향을 준 것으로 사료된다. 향후, 기능적 발목 불안정이 있는 다양한 연령대를 대상으로 족저압 차이를 객관화, 척도화 시키는 연구가 이루어져야 할 것으로 사료된다.

  • The purpose of this study was to examine the differentiation of proprioception, invertor and evertor muscle strength, and time to peak torque at a velocity of $300^{\circ}/sec$ of the ankle joint in people with or without functional ankle instability (FAI). Nineteen subjects with a history of ankle sprain participated. All subjects were divided into FAI group ($n_1=9$, Cumberland ankle instability tool (CAIT)${\leq}24$) and a control group ($n_2=10$) based on their CAIT scores. Isokinetic dynamometer was used to measure the sense of active joint position of the ankle at mid-range and end-range of an inversion motion and invertor as well as the evertor muscle strength and time to peak torque at $300^{\circ}/s$. The FAI group showed a statistically reduction in invertor and evertor muscle strength and time to peak torque when compared to the control group (p<.05). Muscle strength and time to peak torque of the invertor and evertor, as well as the sense of active joint position at end-range were also lower in the FAI group than in the control (p<.05). Correlations between CAIT score and position sense at end-range (r=-.577) and invertor muscle strength (r=.554) were statistically significant (p<.05). Individuals with FAI showed reduction in invertor and evertor muscle strength and recruitment time as well as in proprioception of the ankle joint. Thus, proprioception and invertor and evertor muscle strength of the ankle joint at fast angular velocity may be investigated when examining and planning care for individuals with FAI.

만성 족관절 외측 불안정성의 수술적 치료: 봉합술과 재건술의 비교 (Surgical Treatment of Chronic Lateral Ankle Instability: Repair versus Reconstruction)

  • Surgical treatment to restore stability in the ankle and hindfoot and prevent further degenerative changes may be necessary in cases in which conservative treatment has failed. Anatomical direct repair using native ligament remnants with or without reinforcement of the inferior retinaculum is the so-called gold standard operative strategy for the treatment of lateral ankle instability. Non-anatomical lateral ligament reconstruction typically involves the use of the adjacent peroneus brevis tendon and applies only those with poor-quality ligaments. On the other hand, anatomic reconstruction and anatomic repair provide better functional outcomes after the surgical treatment of chronic ankle instability patients compared to a non-anatomic reconstruction. Anatomical reconstruction using an autograft or allograft applies to patients with insufficient ligament remnants to fashion direct repair, failed previous lateral ankle repair, high body mass index, or generalized ligamentous laxity. These procedures can provide good-to-excellent short-term outcomes. Arthroscopic ligament repair is becoming increasingly popular because it is minimally invasive. Good-to-excellent clinical outcomes have been reported after short and long-term follow-up, despite the relatively large number of complications, including nerve damage, reported following the procedure. Therefore, further investigation will be needed before widespread adoption is advocated.

테이핑이 기능적 발목 불안정성이 있는 선수들의 점프 착지 시 하지에 미치는 영향 (The Effect of Taping on Lower Extremity during lump Landing in Subjects with functional Ankle Instability)

  • 발목 테이핑은 상해 예방을 위해 임상적으로 이용되어 질 수 있는 보조적 수단으로 여겨지고 있다. 그러나 그동안의 연구들을 살펴보면 발목 자체에 대한 부분만을 언급하였다. 본 연구의 목적은 발목 테이핑의 적용이 기능적 발목 불안정성을 가진 선수들에게서 점프 후 착지 시에 하지 관절에 미치는 영향을 알아보는데 있다. 점프의 거리는 각 대상자 키의 50%로 결정하였으며 높이는 각 대상자 키의 20%로 결정하였다. VICON을 이용하여 하지관절의 데이터를 수집하였고, 점프 착지 동안 시상면, 관상면, 수평면의 관절 움직임의 변화를 측정하였다. 테이핑은 통계적으로 시상면에서 착지순간 저측 굴곡과 최대 저측 굴곡을 제한하는 효과가 있었으며, 반면에 착지순간 무릎 굴곡과 최대 무릎 굴곡, 착지순간 엉덩 굴곡과 최대 엉덩 굴곡을 통계적으로 유의하게 증가시키는 효과가 있었으며, 관상면에서는 최대 발목 내번, 착지순간 무릎 외반과 최대 무릎 외반, 최대 엉덩 외전을 감소시키는 효과가 있었고, 수평면에서는 최대 발목 외전, 최대 엉덩 내회전의 감소를 보였다. 발목 테이핑은 발목 관절 운동범위의 제한과 함께 무릎 관절이나 엉덩 관절에 영향을 미치는 것으로 나타났다.
