ICNP를 적용한 학교간호현상 및 특성과 초.중.고등학교의 학교간호현상 비교 (Comparison of School Nursing Phenomena at Elementary, Middle and High Schools by Applying ICNP)

  • Purpose : this study purposed to investigate school nursing phenomena in Korea by applying ICNP, the international standard nursing classification system developed by ICN, and to compare school nursing phenomena at elementary, middle and high schools. Method : The subjects of this study are 110 nursing teachers from 82 elementary schools, 16 middle schools, and 12 high schools. In the survey, subjects were asked to score the degree to which phenomena and characteristics of Korean school nursing, identified in previous research, on a 5 point scale. Questionnaires were distributed and recovered by mail and email. The period of data collection was 6 months from July to December 2003. The general characteristics of schools and nursing teachers were represented with frequencies and percentages, the phenomena and characteristics of school nursing with the mean score of the questions, and the phenomena school nursing by school grade with ANOVA and Duncan's posterior analysis. Results : 1) As for the characteristics of schools according to school nursing phenomena related to human behavior, the mean score of questions on inadequate stress management was highest at 3.24 points followed by the score on inadequate weight control (3.23), inadequate eating habits (3.22), the risk of spine disorders (2.68), inadequate emergency management (2.62), inadequate response to sex -related problems (2.19), and smoking and drug use (1.85). 2) As for the characteristics of schools according to school nursing phenomena related to human function, the mean score of questions on oral health management was highest at 3.11 points followed by the score on the risk of digestive system disorder (2.87), improper eyesight management (2.81), the risk of respiratory system disorders (2.75), lack of sexual identity (2.52), and inadequate contagious disease control (2.12). 3) As for the mean score according to school nursing phenomena related to environment, the score of the risk of accidents in classroom was highest as 2.68 points followed by the score of the risk of accidents around the school (2.65), maladjustment to school (2.62), the risk of accidents outside the classroom (2.43), inadequate learning environment (1.83), the risk of exposure to socially and physically harmful environment factors (1.82), and inadequate waste disposal (1.77). 4) This study tested the mean scores of questions corresponding to each school nursing phenomenon in order to see if there is a difference in the school nursing phenomenon among elementary, middle and high schools, and performed Duncan's posterior comparison for school nursing phenomena. A significant difference was found at p<.1. According to the results, school nursing phenomena found to be significantly different among elementary, middle and high schools was smoking and drug use (p<.05), which appeared more problematic in high school than in elementary school. phenomena such as inadequate eating habits, inadequate weight control, inadequate response to sex-related problems and inadequate waste disposal were also found to be statistically different at p<.1; however, according to the result of Duncan's posterior comparison, no difference was found among groups in improper eating habits and improper response to sex-related problems, and a significant difference was found between middle and high schools in inadequate weight control and inadequate waste disposal. Conclusion: Based on the results of this study, it is necessary to plan school health projects focused on the characteristic school nursing phenomena that had high scores and to develop and execute school healthe projects in accordance with the characteristics of elementary, middle and high schools. Considering that the scores of school nursing phenomena related to human behavior are high, it is necessary to introduce school health promotion projects in a systematic way.

  • The achievement of notable social reoforms attained during the period of 19th and 20th centuries needlessly speaking remodelded the social environmental into several different patterns such as :1) high industrialization 2) propensity to consume 3) up graded overall social stands. The thecnique quoted in producing buttons are hand craft work inlay work precision casting press mosic dye etching, processing, engraving and embossed carving etc. 3. The major designs used in the buttons in -dustries are : Round shape however elliptical column angular and edge shape often used. 4. The colors used are : The multi-colors were highly used than mono-colored materials such as : Adjoining Color and Contrast Color. The highest consideration to be considered in choosing the colors for the buttons are harmonization and matching factor with the garment or dresses to be wore. 5. The major design(incused design) on the buttons are embodiment and the design were also used in order of abstractive-combination abstractive with has offers much surprising. The button industries during the 19th and 20th centuries were not only the determination factors those can judge the value of self-pride of Nation and which were far beyond the in-dustrial arts in those days but also highly refelected and influenced by cultural sense ideology and self-pride of the Nation of those period. The followings are details of the role of the buttons categolized in the order of functional ornamental and symbolical aspects : 1. The functional role : The functional role of the buttons were simply designed for dress how-ever the buttons beyond from this role of function now a days. 2. The ornamental role : The ornamental role of he button beyond from this role of the button were effectuated by : 1) shape materials colors 2) technique locations size and design (incused design) 3) The ramaterials used for buttons shall not be over looked because it is highly depends on the taste sense and combination of harmony with the garment to be wore. 4) The color of the buttons are made well contrasted with the color of garments just as in the case of other artistical area such as matchs with the color of garment of contrast with brigtness of colors contrasted as complementary color and so and so. 5) The technique being adoped are: precision casting press handcraft inlay work etching mosic etc,. Since the buttons are no longer a simple catching devise used to fasten together the different part of the dress but now it has formed own and occupied the independent role in the garment or dresses location can be de-termined and varying depending on the ideas of designers. The size of the buttons has no specific limits, However the variation has widely dependined on the entire circumperence rhythm contrast harmonization of the garments. 3. The symbolical role : Since the button is no longer a just a simple devise for catching and fastening device used fastening together the different part of the garments but now were built a independent area as major part of the Garment and well reflected all kinds of occupations political background cultural as-pect etc. on the buttons. The design of buttons in the western circles are more simplified but they are polished looks and their techniques of manufacturing are comination of both machanis and handcraft. The colors used in the buttons are pretty well harmonized with garment(dress). Almost all kind of materials can be used in the but-tons however materials used in the buttons are : Bone of livestocks ivory, turtle shell are no longer used because the prevention of cruely of animal. On the contraly the level of buttons indus-try of Korea is far to reach and catch up with the level of western circles. It is highly suggested therefore the but-tons industrial field of Republic of Korea shall place and encouragement in producing beter industrial environment of the buttons based on the traditional and cultural aspect of republic of Korea to produce both manufacturing of qulified and best designed and colored buttons.

한국의 경력내 직업이동에 관한 연구 (A Study on the Career Occupational Mobility in Korea)

  • 이 논문의 목적은 급속한 경제발전과 산업화 초기단계의 한국사회에서 경력내 직업이동구조가 어떻게 구축되었는가를 밝히는 데에 있다. 생애사 자료에 나타난 직업이동의 개인사들을 분석함으로써 이 논문은 산업화 초기단계인 1954년에서 1983년까지 30년 동안의 성인 남녀의 경력내 연간 직업이동들을 모형화하였다. 상위 비육체노동직업, 하위 비육체노동직업, 자영업, 육체노동직업, 농업의 5개 직업범주를 통하여 연원 이동표상에 나타난 이동형태는 비이동성에 의해 일차적으로 특징지워진다. 그러나 이동의 절대량으로 볼 때 비이동성의 정도는 각 직업군에서 달리 나타나, 요약하면 U자형이라고 볼 수 있다. 전체적으로 연간이동의 형태는 직업간의 거리라는 개념으로 가장 잘 설명될 수 있다. 직업이동의 상대적 형태를 연구하기 위해서 일반적으로 사용되고 있는 대수선형모형의 문제점들을 지적하고 비판하면서 필자는 개념적,이론적 작업과 모형추정의 방법론적 작업을 결합시키는 형태의 대수선형모형을 주장하였다. 이러한 모형은 1) 세대간 이동과 세대내(경력간)이동의 구별을 가능하게 하며, 2) 불평등구조 관점, 노동시장적 관점, 생애사적 관점을 동시에 반영할 수 있는 것이어야 한다. 이동표의 분석에서 출발직업과 도달직업의 상대적 규모 외에 크게 6가지의 개념적으로 구별되는 설명요인들(지속성, 천정, 전통부문, 비육체노동, 대안적 이동경로, 직업간 거리)에 바탕을 둔 모형을 통하여 경력이동의 형태를 분석하였다. 우리사회에서 세대내 직업이동의 상대적 형태는 지속성 요인과 천정효과에 의해 좌우되는 것으로 나타나고 있다. 그러나 세대간 이동의 연구결과와는 달리, 우리사회에서 세대내 (경력내) 직업이동에서는 자영업으로의 대안적 이동경로의 요인이 강하게 작용하고 있는 것으로 나타난다. 세대내 직업이동의 상대적 형태는 다음과 같은 몇가지로 특징지울 수 있다. 첫째, 대각선상에서 일어나는 지속성 요인의 효과가 절대적이다. 지속성 요인의 효과는 서구 선진산업사회에서의 U자형과는 달리 역-J자형으로 나타나고 있다. 둘째, 비육체노동직업과 육체노동직업 및 농업간의 직업간 거리에 의한 부의 이동효과가 명백히 발견된다. 셋째 농업으로부터의 이동은 상대적으로 도달직업의 종류와 상관없이 일어나고 있다. 넷째, 비육체노동직업간의 이동은 상대적으로 매우 높고 쌍방향적이며, 다른 직업군에 대해 배타적이다. 다섯째, 유출과 유입의 양 측면에 있어 자영업은 매우 독특한 기능을 수행하고 있다. 여섯째, 우리사회에서 경력내 직업이동의 상대적 형태는 대각선을 중심으로 비대칭적이다.

경관단위 기반 수변환경의 심미적 평가 - 한강 수변을 대상으로 - (Aesthetic Landscape Assessment Based on Landscape Units in the Han River Riparian Area)

  • 본 연구의 목적은 한강 수변을 대상으로 경관단위에 기반한 심미적 경관평가를 통해 수변경관의 관리방향을 제시하는 것이다. 이를 위해서 먼저, 한강 수변을 자연형, 인공형, 농경지형, 혼합형의 수변유형으로 구분한 후, 대표적인 사례지 36개소(약 $1km{\times}1km$)를 선정하였다. 그 후, 지형과 지피의 경관생태적 측면을 고려하여 총 71개의 경관단위를 설정하였다. 경관평가기준은 경관질과 경관통합성으로 구분하였다. 경관질은 자연경관미, 흥미성, 희귀성, 경관기능을 평가기준으로, Matrix기법을 통해 5등급 평가체계로 평가하였다. 경관통합성 평가는 경관단위 내부통합성과 경관단위간 외부통합성으로 구분된다. 현장조사를 통해 평가한 결과, 전체 사례대상지의 경관단위수는 4,288개이며, 총면적은 약 $42.8km^2$이었다. 경관단위별로 분석한 결과, 산림지가 전체 면적의 약 27.1%($11,580,905m^2$)로 가장 높은 분포를 보였으며, 습지가 전체 면적의 0.1%($52,348m^2$)로 가장 낮았다. 경관질 평가에서는 전체 면적의 약 30.5%가 '상'으로 평가받아 높은 자연경관미를 가지고 있었다. 경관통합성 평가에서, 전체 면적의 39.3%가 인접 경관단위간의 통합성이 '상'으로 나타나, 경관단위간의 시각적 연계성과 조화성의 수준이 높은 것으로 평가되었다. 수변유형에 따라 경관질 등급의 차이가 있는지를 Oneway ANOVA를 통해 검정한 결과, 자연유형의 경관질 등급이 뚜렷하게 높게 나타났다. 본 연구 결과는 한강 수변을 생태적으로 건강하고 경관적으로 아름다운 공간으로 조성하는데 기여할 수 있을 것이다. 향후 연구에서는 각 경관단위가 가지고 있는 심미적 가치와 생태적인 가치를 동시에 평가해볼 필요가 있다.

  • This article is the reserch on actual condition of crime of arson which occurs in Korea and its countermeasures. The the presented problem in this article are that (1) we have generally very low rate concern about the crime of arson contrary to realistic problems of rapid increase of crime of arson (2) as such criminal motives became so diverse as to the economic or criminal purpose unlike characteristic and mental deficiency of old days, and to countermeasure these problems effectively it presentation the necessity of systemantic research. Based on analysis of reality of arson, the tendency of this arson in Korea in the ratio of increase is said to be higher than those in violence crime or general fire rate. and further its rate is far more greater than those of the U.S.A. and Japan. Arson is considered to be a method of using fire as crime and in case of presently residence to be the abject, it is a public offense crime which aqccompany fatality in human life. This is the well It now fact to all of us. And further in order to presentation to the crime of arson, strictness of criminal law (criminal law No, 164 and 169, and fire protection law No. 110 and 111) and classification of arsonist as felony are institutionary reinforced to punish with certainty of possibility, Therefore, as tendency of arson has been increased compared to other nations, it is necessary to supplement strategical policy to bring out overall concerns of the seriousness of risk and damage of arson, which have been resulted from the lack of understanding. In characteristics analysis of crime of arson, (1) It is now reveald that, in the past such crime rate appeared far more within the boundary of town or city areas in the past, presently increased rate of arsons in rural areas are far more than in the town or small city areas, thereby showing characteristics of crime of arson extending nation wide. (2) general timetable of arson shows that night more than day time rate, and reveald that is trait behavior in secrecy.(3) arsonists are usually arrested at site or by victim or report of third person(82,9%).Investigation activities or self surrenders rate only 11.2%. The time span of arrest is normally the same day of arson and at times it takes more than one year to arrest. This reveals its necessity to prepare for long period of time for arrest, (4) age rate of arson is in their thirties mostly as compared to homicide, robbery and adultery, and considerable numbers of arsons are in old age of over fifties. It reveals age rate is increased (5) Over half of the arsonists are below the junior high school (6) the rate of convicts by thier records is based on first offenders primarily and secondly more than 4 time convicts. This apparently shows necessity of effective correctional education policy for their social assimilation together with re-investigation of human education at the primary and secondary education system in thier life. The examples of motivation for arosnits, such as personal animosity, fury, monetary swindle, luscious purpose and other aims of destroying of proof, and other social resistance, violence including ways of threatening, beside the motives of individual defects, are diverse and arsonic suicide and specifically suicidal accompany together keenly manifested. When we take this fact with the criminal theory, it really reveals arsons of crime are increasing and its casualities are serious and a point as a way of suicide is the anomie theory of Durkheim and comensurate with the theory of that of Merton, Specifically in the arson of industrial complex, it is revealed that one with revolutionary motive or revolting motive would do the arsonic act. For the policy of prevention of arsons, professional research work in organizational cooperation for preventive activities is conducted in municipal or city wise functions in the name of Parson Taskforces and beside a variety of research institutes in federal government have been operating effectively to countermeasure in many fields of research. Franch and Sweden beside the U.S. set up a overall operation of fire prevention research funtions and have obtained very successful result. Japan also put their research likewise for countermeasure. In this research as a way of preventive fire policy, first, it is necessary to accomodate the legal preventitive activities for fire prevention in judicial side and as an administrative side, (1) precise statistic management of crime of arson (2) establishment of professional research functions or a corporate (3) improvement of system for cooperative structural team for investigation of fires and menpower organization of professional members. Secondly, social mentality in individual prospect, recognition of fires by arson and youth education of such effect, educational program for development and practical promotion. Thirdly, in view of environmental side, the ways of actual performance by programming with the establishment of cooperative advancement in local social function elements with administrative office, habitants, school facilities and newspapers measures (2) establishment of personal protection where weak menpowers are displayed in special fire prevention measures. These measures are presented for prevention of crime of arson. The control of crime and prevention shall be prepared as a means of self defence by the principle of self responsibility Specifically arsonists usually aims at the comparatively weak control of fire prevention is prevalent and it is therefore necessary to prepare individual facilities with their spontaneous management of fire prevention instead of public municipal funtures of local geverment. As Clifford L. Karchmer asserted instead of concerns about who would commit arson, what portion of area would be the target of the arson. It is effective to minister spontaveously the fire prevention measure in his facility with the consideration of characteristics of arson. On the other hand, it is necessary for the concerned personnel of local goverment and groups to distribute to the local society in timely manner for new information about the fire prevention, thus contribute to effective result of fire prevention result. In consideration of these factors, it is inevitable to never let coincide with the phemonemon of arsons in similar or mimic features as recognized that these could prevail just an epedemic as a strong imitational attitude. In processing of policy to encounter these problems, it is necessary to place priority of city policy to enhancement of overall concerns toward the definitive essense of crime of arson.

한정된 O-D조사자료를 이용한 주 전체의 트럭교통예측방법 개발 (DEVELOPMENT OF STATEWIDE TRUCK TRAFFIC FORECASTING METHOD BY USING LIMITED O-D SURVEY DATA)

  • The objective of this research is to test the feasibility of developing a statewide truck traffic forecasting methodology for Wisconsin by using Origin-Destination surveys, traffic counts, classification counts, and other data that are routinely collected by the Wisconsin Department of Transportation (WisDOT). Development of a feasible model will permit estimation of future truck traffic for every major link in the network. This will provide the basis for improved estimation of future pavement deterioration. Pavement damage rises exponentially as axle weight increases, and trucks are responsible for most of the traffic-induced damage to pavement. Consequently, forecasts of truck traffic are critical to pavement management systems. The pavement Management Decision Supporting System (PMDSS) prepared by WisDOT in May 1990 combines pavement inventory and performance data with a knowledge base consisting of rules for evaluation, problem identification and rehabilitation recommendation. Without a r.easonable truck traffic forecasting methodology, PMDSS is not able to project pavement performance trends in order to make assessment and recommendations in the future years. However, none of WisDOT's existing forecasting methodologies has been designed specifically for predicting truck movements on a statewide highway network. For this research, the Origin-Destination survey data avaiiable from WisDOT, including two stateline areas, one county, and five cities, are analyzed and the zone-to'||'&'||'not;zone truck trip tables are developed. The resulting Origin-Destination Trip Length Frequency (00 TLF) distributions by trip type are applied to the Gravity Model (GM) for comparison with comparable TLFs from the GM. The gravity model is calibrated to obtain friction factor curves for the three trip types, Internal-Internal (I-I), Internal-External (I-E), and External-External (E-E). ~oth "macro-scale" calibration and "micro-scale" calibration are performed. The comparison of the statewide GM TLF with the 00 TLF for the macro-scale calibration does not provide suitable results because the available 00 survey data do not represent an unbiased sample of statewide truck trips. For the "micro-scale" calibration, "partial" GM trip tables that correspond to the 00 survey trip tables are extracted from the full statewide GM trip table. These "partial" GM trip tables are then merged and a partial GM TLF is created. The GM friction factor curves are adjusted until the partial GM TLF matches the 00 TLF. Three friction factor curves, one for each trip type, resulting from the micro-scale calibration produce a reasonable GM truck trip model. A key methodological issue for GM. calibration involves the use of multiple friction factor curves versus a single friction factor curve for each trip type in order to estimate truck trips with reasonable accuracy. A single friction factor curve for each of the three trip types was found to reproduce the 00 TLFs from the calibration data base. Given the very limited trip generation data available for this research, additional refinement of the gravity model using multiple mction factor curves for each trip type was not warranted. In the traditional urban transportation planning studies, the zonal trip productions and attractions and region-wide OD TLFs are available. However, for this research, the information available for the development .of the GM model is limited to Ground Counts (GC) and a limited set ofOD TLFs. The GM is calibrated using the limited OD data, but the OD data are not adequate to obtain good estimates of truck trip productions and attractions .. Consequently, zonal productions and attractions are estimated using zonal population as a first approximation. Then, Selected Link based (SELINK) analyses are used to adjust the productions and attractions and possibly recalibrate the GM. The SELINK adjustment process involves identifying the origins and destinations of all truck trips that are assigned to a specified "selected link" as the result of a standard traffic assignment. A link adjustment factor is computed as the ratio of the actual volume for the link (ground count) to the total assigned volume. This link adjustment factor is then applied to all of the origin and destination zones of the trips using that "selected link". Selected link based analyses are conducted by using both 16 selected links and 32 selected links. The result of SELINK analysis by u~ing 32 selected links provides the least %RMSE in the screenline volume analysis. In addition, the stability of the GM truck estimating model is preserved by using 32 selected links with three SELINK adjustments, that is, the GM remains calibrated despite substantial changes in the input productions and attractions. The coverage of zones provided by 32 selected links is satisfactory. Increasing the number of repetitions beyond four is not reasonable because the stability of GM model in reproducing the OD TLF reaches its limits. The total volume of truck traffic captured by 32 selected links is 107% of total trip productions. But more importantly, ~ELINK adjustment factors for all of the zones can be computed. Evaluation of the travel demand model resulting from the SELINK adjustments is conducted by using screenline volume analysis, functional class and route specific volume analysis, area specific volume analysis, production and attraction analysis, and Vehicle Miles of Travel (VMT) analysis. Screenline volume analysis by using four screenlines with 28 check points are used for evaluation of the adequacy of the overall model. The total trucks crossing the screenlines are compared to the ground count totals. L V/GC ratios of 0.958 by using 32 selected links and 1.001 by using 16 selected links are obtained. The %RM:SE for the four screenlines is inversely proportional to the average ground count totals by screenline .. The magnitude of %RM:SE for the four screenlines resulting from the fourth and last GM run by using 32 and 16 selected links is 22% and 31 % respectively. These results are similar to the overall %RMSE achieved for the 32 and 16 selected links themselves of 19% and 33% respectively. This implies that the SELINICanalysis results are reasonable for all sections of the state.Functional class and route specific volume analysis is possible by using the available 154 classification count check points. The truck traffic crossing the Interstate highways (ISH) with 37 check points, the US highways (USH) with 50 check points, and the State highways (STH) with 67 check points is compared to the actual ground count totals. The magnitude of the overall link volume to ground count ratio by route does not provide any specific pattern of over or underestimate. However, the %R11SE for the ISH shows the least value while that for the STH shows the largest value. This pattern is consistent with the screenline analysis and the overall relationship between %RMSE and ground count volume groups. Area specific volume analysis provides another broad statewide measure of the performance of the overall model. The truck traffic in the North area with 26 check points, the West area with 36 check points, the East area with 29 check points, and the South area with 64 check points are compared to the actual ground count totals. The four areas show similar results. No specific patterns in the L V/GC ratio by area are found. In addition, the %RMSE is computed for each of the four areas. The %RMSEs for the North, West, East, and South areas are 92%, 49%, 27%, and 35% respectively, whereas, the average ground counts are 481, 1383, 1532, and 3154 respectively. As for the screenline and volume range analyses, the %RMSE is inversely related to average link volume. 'The SELINK adjustments of productions and attractions resulted in a very substantial reduction in the total in-state zonal productions and attractions. The initial in-state zonal trip generation model can now be revised with a new trip production's trip rate (total adjusted productions/total population) and a new trip attraction's trip rate. Revised zonal production and attraction adjustment factors can then be developed that only reflect the impact of the SELINK adjustments that cause mcreases or , decreases from the revised zonal estimate of productions and attractions. Analysis of the revised production adjustment factors is conducted by plotting the factors on the state map. The east area of the state including the counties of Brown, Outagamie, Shawano, Wmnebago, Fond du Lac, Marathon shows comparatively large values of the revised adjustment factors. Overall, both small and large values of the revised adjustment factors are scattered around Wisconsin. This suggests that more independent variables beyond just 226; population are needed for the development of the heavy truck trip generation model. More independent variables including zonal employment data (office employees and manufacturing employees) by industry type, zonal private trucks 226; owned and zonal income data which are not available currently should be considered. A plot of frequency distribution of the in-state zones as a function of the revised production and attraction adjustment factors shows the overall " adjustment resulting from the SELINK analysis process. Overall, the revised SELINK adjustments show that the productions for many zones are reduced by, a factor of 0.5 to 0.8 while the productions for ~ relatively few zones are increased by factors from 1.1 to 4 with most of the factors in the 3.0 range. No obvious explanation for the frequency distribution could be found. The revised SELINK adjustments overall appear to be reasonable. The heavy truck VMT analysis is conducted by comparing the 1990 heavy truck VMT that is forecasted by the GM truck forecasting model, 2.975 billions, with the WisDOT computed data. This gives an estimate that is 18.3% less than the WisDOT computation of 3.642 billions of VMT. The WisDOT estimates are based on the sampling the link volumes for USH, 8TH, and CTH. This implies potential error in sampling the average link volume. The WisDOT estimate of heavy truck VMT cannot be tabulated by the three trip types, I-I, I-E ('||'&'||'pound;-I), and E-E. In contrast, the GM forecasting model shows that the proportion ofE-E VMT out of total VMT is 21.24%. In addition, tabulation of heavy truck VMT by route functional class shows that the proportion of truck traffic traversing the freeways and expressways is 76.5%. Only 14.1% of total freeway truck traffic is I-I trips, while 80% of total collector truck traffic is I-I trips. This implies that freeways are traversed mainly by I-E and E-E truck traffic while collectors are used mainly by I-I truck traffic. Other tabulations such as average heavy truck speed by trip type, average travel distance by trip type and the VMT distribution by trip type, route functional class and travel speed are useful information for highway planners to understand the characteristics of statewide heavy truck trip patternS. Heavy truck volumes for the target year 2010 are forecasted by using the GM truck forecasting model. Four scenarios are used. Fo~ better forecasting, ground count- based segment adjustment factors are developed and applied. ISH 90 '||'&'||' 94 and USH 41 are used as example routes. The forecasting results by using the ground count-based segment adjustment factors are satisfactory for long range planning purposes, but additional ground counts would be useful for USH 41. Sensitivity analysis provides estimates of the impacts of the alternative growth rates including information about changes in the trip types using key routes. 원발성 간암의 국소 방사선치료 시 선량반응 관계 (Dose Response Relationship in Local Radiotherapy for Hepatocellular Carcinoma)

  • 목적 : 본 연구에서는 원발성 간암의 국소 방사선치료 시 조사선량을 높여준 것이 종양의 반응의 향상을 유도하였는지 분석하고자 하였다. 또한, 저선량을 조사하였던 환자들과 고선량을 조사하였던 환자들의 종양의 반응 및 부작용과 이에 영향을 미치는 인자들을 분석함으로써 향후 적절한 조사선량 범위를 결정하는데 유용한 기준을 제시하고자 하였다. 대상 및 방법 : 대상 환자의 선정 기준은 방사선치료의 과거력이 없는 경우, 간외전이가 없는 경우, 간경변증의 정도가 Child's class A 또는 B군인 경우, 종양이 전체 간 용적의 2/3를 넘지 않는 경우, 전신수행도가 ECOG 3기 이상으로 악화되지 않은 경우로 하였다. 1992년 1월부터 2000년 3월까지 원발성 간암에 국소 방사선치료를 시행 받은 환자들 중 158명이 연구 대상에 포함되었다. 조사영역은 종양과 주변 부위 $2\~3\;cm$를 포함하였고 일일 1.8 Gy씩 주 5회 조사되었다. 방사선치료 전과 종료 후 $4\~6$주째의 컴퓨터 단층 촬영 영상 및 자기 공명 영상, 간동맥 혈관 촬영 영상 등을 이용하여 조사영역 내의 종양의 반응을 평가하였다. 주 1회 혈액 검사를 시행하여 말초 혈액 혈구수 및 간기능을 관찰하였다. 치료의 종료 후 체중 증가, 복수, 간종대 등의 소견을 관찰하였고 4주와 8주에 간기능 검사를 시행하였다. 위장관계의 부작용은 상부위장관 내시경을 시행하여 확인하였다. 종양의 관해 정도에 따라 반응군과 비반응군으로 구분한 후 조사선량의 분포를 통계적으로 분석하였다. 또한, 연구대상을 조사선량에 따라 세 군으로 분류하고 종양의 반응과 임상적 특성을 분석하였다. 결과 : 종양의 반응은 106명이 부분 관해를 보여서 반응군의 비율은 $67.1\%$이었다. 완전 관해는 없었다. 종양이 $50\%$ 이하로 감소된 경우가 41명$(25.9\%)$, 종양의 진행을 보인 경우가 11명$(7\%)$이었다. 반응군의 평균 조사선량은 $50.1{\pm}6.6\;Gy$이었고 비반응군의 평균 조사선량은 $44.3{\pm}9.0\;Gy$로 통계적으로 의미 있는 차이를 보였다. 다변량 분석에서도 조사선량이 p-value 0.001로 유일하게 의미 있는 인자로 나타났다. 종양의 반응률은 조사선량이 40 Gy 미만인 군에서 $29.2\%$, 40 Gy에서 50 Gy사이인 군에서 $68.6\%$, 50 Gy를 넘는 환자 군에서 $77.1\%$로 나타났다. 종양의 크기는 40 Gy 미만인 군에서 $9.8{\pm}2.6\;cm$, 40 Gy에서 50 Gy사이인 군에서 $9.8{\pm}2.6\;cm$, 50 Gy를 넘는 환자 군에서 $8.3{\pm}3.2\;cm$로 고선량을 조사한 군에서 작았다. 간문맥 혈전증은 각각 $66.7\%,\;64.7\%,\;37.3\%$에서 양성으로 50 Gy를 넘는 선량이 조사된 군에서 상대적으로 간문맥 혈전증의 양성률이 낮았다. 방사선 간염, 십이지장의 궤양은 조사선량을 증가시킬수록 증가하는 경향을 나타내었다. 경미한 정도의 복수가 발생하거나 전형적인 방사선 간염의 발생, 위장관계의 염증 등이 치료 전 간경변증이 심할수록 현저히 많이 발생하는 경향을 보였다. 결론 : 원발성 간암에서 국소 방사선치료를 시행하는 경우 고선량이 조사될수록 반응군의 비율이 증가하는 선량반응 관계가 성립함을 확인할 수 있었다. 고선량이 조사될수록 대상 환자의 종양의 크기가 작았고 간문맥 혈전증의 비율도 적었는데 이는 병기가 낮은 경우 완치를 목표로 오히려 고선량을 투여하는 경향 때문으로 생각되었다. 조사선량이 50 Gy를 넘는 경우 조사선량의 증가에 따른 반응률의 향상이 현저하지 않은 반면 방사선 간염의 발생은 간경변증의 정도에 크게 영향을 받는다는 점을 고려할 때, 조사선량의 증가는 반드시 치료를 받는 대상 환자의 간기능과 간경변증의 정도 등을 고려하여 조심스럽게 진행되어야 할 것으로 생각된다.
