Changes in Physiological Characteristics of Trapezius, Multifidus and Hamstring Muscles According to Tilt Angle of Exercise Bed in Elderly (운동침대 틸트 각도 변화에 따른 노인의 승모근, 다열근 및 슬괵근 근육의 생리학적 특성 변화 연속 증례 연구)

  • Purpose: This study investigated the effects of changing the tilt angle of the exercise bed on physiological characteristics of trapezius, multifidus, and hamstring muscle function. Methods: Twenty elderly who complains of back and lumbar pain or discomfort participated in this study (average age: $64.70{\pm}4.55$ years). The physiological characteristics, including muscle tone (F), dynamic stiffness (S), and elasticity (D) of trapezius, multifidus, and hamstring muscles were measured by a Myoton device at three tilt angles (0, 15, and 30 degrees) during exercise. Results: The muscle tone, dynamic stiffness, and elasticity of the right trapezius and left hamstring muscle showed significant differences (p<0.05) according to exercise bed tilt angle, while the other muscles showed no significant differences. Conclusions: Our results demonstrate that physiological characteristics of the upper and lower extremity muscles are affected ipsilaterally, according to the tilt angles of the exercise bed.

  • Objectives The purpose of this study is to know the effect of acupuncture on brachial plexus palsy due to birth injury. Methods We decided to treat one week of Vojta therapy and occupational therapy, but other were combined, and then add three weeks of acupuncture treatment as well. Results Right clavicle fracture at birth brachial plexus injury due to decreased locomotion of the right upper extremity, muscle weakness in children aged 7 months to Vojta therapy, occupational therapy and acupuncture, when performed in conjunction mobility increase of the shoulder joint, elbow support improvement, recovery of motor function of the hand grip, etc. to obtain a significant motor function recovery improvement. Conclusion Acupuncture was a good treatment of choice for the better shoulder, joint and hands movements.

The Effect of Multimodal Intervention through Virtual Reality-Based Immersion Program on Cognitive Function and Brain activity in Patients with Mild Cognitive Impairment (가상현실 기반 몰입형 복합중재프로그램이 경도인지장애 환자의 인지기능 및 두뇌 활성에 미치는 영향)

  • PURPOSE: The purpose of this study was to investigate the effect of multimodal intervention through VR (virtual reality)-based immersion program on the cognitive function and brain activity of patients with mild cognitive impairment. METHODS: The subjects of the study were 10 people in the experimental group who applied a complex intervention that performed cognitive tasks using the movement of the upper extremities through the VR program, and 10 people in the control group who received traditional occupational therapy. After the study intervention was applied 5 times a week, 30 minutes a day for a total of 8 weeks, LOTCA-G(Lowenstein Occupational Therapy Cognitive Assessment for Geriatric Population) and NIRSIT LITE were used to compare. RESULTS: Significant differences in cognitive function and brain activity were noted between the pre- test and post-test in the experimental group. Brain activity showed statistically significant differences in four channels of the working memory domain and one channel of the metacognitive domain (p < . 05). Comparative analysis of the difference between the two groups revealed statistically significant differences in cognitive function and brain activity. The brain activity showed statistically significant differences in three channels of the working memory domain and one channel of the metacognitive domain (p < .05). CONCLUSION: Through the results of this study, it was found that the complex intervention of performing cognitive tasks using upper extremity movements through the VR program had a positive effect on the cognitive function of patients with mild cognitive.

Effect of Robot-Assisted Hand Rehabilitation on Hand Function in Chronic Stroke Patients (손 재활 로봇의 적용이 만성 뇌졸중 환자의 손 기능 향상에 미치는 영향)

  • The purpose of this study was to investigate effect of robot-assisted hand rehabilitation(Amadeo(R)) on hand motor function in chronic stroke patients. This study used a single-subject experimental design with multiple baselines across individuals. Three chronic stroke survivors with mild to sever motor impairment took part in study. Each participants had 2 weeks interval of starting intervention. Participants received robot-assisted therapy(45min/session. 3session/wk for 6wks). Finger active range of motion(AROM) was assessed by Range of Assessment program in Amadeo(R), and test-retest reliability was verified using Pearson correlation analysis. To investigate effect of Amadeo(R), finger AROM was measured immediately after each sessions and Fugl-Meyer Assessment of Upper extremity, Motor Activity Log, Nine hole peg board test and Jebsen-Taylor hand motor function test were assessed at pre-post intervention. Results were analyzed by visual analysis and comparison of pre-post tests. The test-retest reliability of Range of Assessment was good(r=.99). After robot-assisted therapy, finger AROM of participant 1, 2, and 3 was respectively improved by 18%, 3.6%, and 6% each. Hand motor function of participant 1, 3 was improved on all four tests, but not effect in participant 2. Robot-assisted hand rehabilitation could improve finger AROM and effect on hand motor function in chronic stroke patients.

Two Years and Four Month's Complex Exercise for Upper Extremity Function and Balance Proficiency of a Older Woman With Stroke : A Case-Study (2년 4개월 동안 수행한 복합운동프로그램이 뇌졸중 환자의 상지 기능 및 균형 능력에 미치는 영향: 사례연구)

  • 본 연구는 관절가동범위 및 근력강화운동, 반복적인 과제-지향적 활동으로 구성된 복합운동프로그램을 뇌졸중 환자에게 2년 4개월 동안 적용하여 상지 기능 및 균형 능력에 미치는 효과를 알아보고자 하였다. 연구대상자는 뇌졸중으로 인한 좌측 편마비 증상을 보이는 68세 여성으로 매 회기 1시간씩, 주 3회, 2년 4개월 동안 복합운동프로그램을 수행하였다. 대상자의 상지 기능은 브론스트롬의 손 회복 단계와 상지 회복단계, Fugl-Meyer Assessment of Motor Function, 뇌졸중 상지기능검사로, 균형 능력은 Tinetti Gait & Balance Scale로 평가하였다. 연구 결과 대상자의 상지 기능과 균형 능력이 유지 및 향상 되었다. 이를 통하여 만성 뇌졸중 환자에서 상지 및 균형 능력의 유지 및 증진을 위한 지속적인 복합 운동프로그램 수행의 유효성을 확인할 수 있었다.

Steindler Flexoplasty of Elbow in Brachial Plexus Injuries (상완 신경총 손상에서 Steindler 주관절 굴곡 성형술)

  • Purpose: The purpose of this study was to analyze the clinical results after Steindler flexoplasty. Materials and Methods: We analyzed 6 cases who had nearly normal finger and wrist joint flexion function, but could not flex elbow joint actively because of upper arm type brachial plexus injury. We performed operation during the period from February 1997 to July 2003. There were 5 males and 1 female with mean age of 28 years (range: $19{\sim}51$ years) when Steindler flexoplasty was done. The average follow-up period was 3 years 11 months (range: 12 months${\sim}$7 years 4 months). We assessed active range of motion of elbow joint, muscle power and elbow function by Mayer & Green grade scale at last follow-up. We assessed how much they were favorable for Steindler flexoplasty and had improvement of upper extremity function and correction of deformity. Results: Postoperative, flexion range of elbow joint improved to average $111.7^{\circ}$ (range: $90{\sim}130^{\circ}$). $25.8^{\circ}$ (range: $15{\sim}45^{\circ}$) in flexion contracture and $16.6^{\circ}$ (range: $10{\sim}35^{\circ}$) in pronation contracture were remained. Range of motion of elbow joint improved to average $85.3^{\circ}$ (range: $45{\sim}105^{\circ}$). Flexion power of elbow joint improved to Grade 5 in all cases. Postoperatively on Mayer & Green grade scale, there were excellent in 3 cases(50%), good in 2 cases(33.3%), fair in 1 case(16.7%). On patient's own assessment of functional improvement, there were excellent in 4 cases(66.6%), good in 1 case(16.7%) and fair in 1 case(l6.7%). Conclusion: Steindler flexoplasty can reserve good clinical results with being improved to active flexion of elbow joint in cases who have functional hand and wrist, but paralysis upper arm muscle in brachial plexus injury.

  • This study was conducted to assess the changes in the functional levels of affected upper extremities after treating hemiplegic patients by applying constraint-induced movement therapy(CIMT). The subjects were selected from 20 hemiplegic patients with scores of 25 or more in Mini Mental State Examination(MMSE-k), transferred to the departments of physical therapy in two university hospitals in Busan from December, 2001 to march, 2002, and were divided into two groups. Eleven subjects with fixing unaffected arms by CIMT were assigned to the experimental patient group and the other 9 patients to control group without fixing unaffected arms. The function of upper arms for both groups were evaluated by using Actual Amount of Use Test(AAUT) and Motor Active Log(MAL) before and after physical therapy. The malts were as follows: The recovery rates of upper extremity by AOU(Activity of Use) and QOM(Quality of Movement) were 23.9% and 27.3% for CIMT treated group, and by 8.3% and 4.6% for the control group on the average, respectively, in AAUT after physical therapy, showing statistically significant differences between two groups. And in MAL, the average recovery rates were 27.3% by AOU and 22.6% by QOM for CIMT treated group while 3.1% by both AOU and QOM for the control group, and were significantly different between twogroups.

Effects of Progressive Scapular Stabilization Exercise on Neck, Muscle Strength, Upper Extremity Function in Patients with Acute Whiplash Injury

  • Objective: The purpose of this study was to investigate the effects of a combination of progressive scapular stabilization exercises, neck and thoracic spine mobilization, and thermoelectric physical therapy on pain, range of motion, muscle strength, and function in patients with acute whiplash injury. Design: A randomized controlled trial design. Methods: A total of 24 subjects were included in this study. They were randomly assigned to an experimental group (n=12) that performed scapular stabilization exercises, neck and thoracic spine mobilization, and physiotherapy, and a control group (n=12) that only performed neck and thoracic spine mobilization and physiotherapy. The pre-test was conducted before the intervention and consisted of a total of 12 treatment sessions of 60 minutes each, three times a week. A posttest was conducted 4 weeks later. Pain, range of motion, muscle strength, and function were assessed before and after intervention. Results: The results of the study showed that there was a significant difference in pain reduction and range of motion and muscle strength improvement in the experimental group, and a significant difference was also found between the experimental group and the control group in terms of functional evaluation. Conclusions: The combination of exercise therapy and joint mobilization technique and physical therapy resulted in greater improvements in pain, range of motion, strength, and function assessment, contributing to improved overall function.

A Case Study of a Taeyangin Patient with Amyotrophic Lateral Sclerosis (태양인 해역증으로 진단한 근위축성 측삭경화증 환자 치험 1례)

  • Objectives This case study was about a Taeyangin patient with Amyotrophic Lateral Sclerosis identified as Hae-Yeok pattern. In this study, we report the significant improvement of extremity weakness of this patient after Sasang Constitutional medicine treatment.Methods The patient was identified as Taeyangin Hae-Yeok pattern and treated with Ogapijangchuk-tang and acupuncture. Korean-Amyotrophic Lateral Sclerosis Functional Rating Scale-Revised (K-ALSFRS-R) was used to assess the overall function of the patient. And the Global Assessment Scale (GAS) was used to assess the change of upper and lower extremities weakness after the treatment.Result and Conclusion The K-ALSFRS-R total score decreased from 30 points to 26 points and extremity weakness showed significant improvement in GAS after 12 days treatment. Furthermore, walking distance was much longer after treatment. In conclusion, this study shows that Sasang constitutional medicine can be effective treatment for Taeyangin patient with Amyotrophic Lateral Sclerosis.

Effect of Home Activity Support Program on Daily Living Performance Skills in Children with Cerebral Palsy and their Parents

  • Objective: This study attempted to find out if it changes the child's ability to perform daily life activities when visiting the familiar environment and daily living space of children with cerebral palsy and conducting a home activity support program for children and parents. Design: Randomized Controlled Trial Methods: Among 22 children aged 3 to 12 years old, they were assigned to the intervention group and control group. Of these, 12 boys and 10 girls participated in the study. Gross motor function measure and upper extremity function evaluation were used to measure the physical function of children with cerebral palsy, and self-care skills, mobility and social functions were evaluated in the pediatric evaluation of disability inventory. In addition, a parenting sense of competence was used to find out the efficacy of parents in raising children. After the pre-evaluation, basic rehabilitation treatment and intervention programs were applied to the intervention group, and only basic rehabilitation treatment was performed to the control group, and post-evaluation was performed 8 weeks later. Results: As a result of the study, among the items that measured the gross motor function, upper limb function, and daily life performance ability of the intervention group in the difference between the intervention group and the control group, statistically improved in personal processing and movement (p<0.05). In addition, the parenting sense of competence children in the intervention group was statistically significant (p<0.05). Conclusions: The home activity support program will help strengthen the ability of cerebral palsy children to perform daily life as a way to set mutually agreed goals with their families or children and achieve them in a familiar environment.
