완전무치악 환자에서 임플란트 하이브리드보철물을 이용한 전악 수복 증례 (Full mouth rehabilitation of fully edentulous patient using implant hybrid prosthesis)

  • 치아의 상실은 저작 및 발음 등의 구강 기능의 저하와 치조골 소실을 가져오며 또한 자신감 결여로 심리적 문제까지 초래하는데, 이는 완전 무치악 환자에서 더 큰 문제를 야기한다. 본 증례의 환자는 다수치아의 상실 및 잔존치 동요도 때문에 임플란트를 이용한 고정성 보철을 원하다는 주소로 내원하였다. 치주적으로 예후가 불량한 모든 잔존치를 발치 하였으며 상하악 완전 무치악에서 임플란트를 이용한 하이브리드보철물을 이용하여 전악 수복 시행하였다. 최종치료 후 심미적, 기능적으로 만족스러운 결과를 얻었기에 이를 보고하고자 한다.

Intraoral scanning of the edentulous jaw without additional markers: An in vivo validation study on scanning precision and registration of an intraoral scan with a cone-beam computed tomography scan

  • Purpose: A fully digital approach to oral prosthodontic rehabilitation requires the possibility of combining (i.e., registering) digital documentation from different sources. This becomes more complex in an edentulous jaw, as fixed dental markers to perform reliable registration are lacking. This validation study aimed to evaluate the reproducibility of 1) intraoral scanning and 2) soft tissue-based registration of an intraoral scan with a cone-beam computed tomography (CBCT) scan for a fully edentulous upper jaw. Materials and Methods: Two observers independently performed intraoral scans of the upper jaw in 14 fully edentulous patients. The palatal vault of both surface models was aligned, and the inter-observer variability was assessed by calculating the mean inter-surface distance at the level of the alveolar crest. Additionally, a CBCT scan of all patients was obtained and a soft tissue surface model was generated using patient-specific gray values. This CBCT soft tissue model was registered with the intraoral scans of both observers, and the intraclass correlation coefficient(ICC) was calculated to evaluate the reproducibility of the registration method. Results: The mean inter-observer deviation when performing an intraoral scan of the fully edentulous upper jaw was 0.10±0.09 mm. The inter-observer agreement for the soft tissue-based registration method was excellent(ICC=0.94; 95% confidence interval, 0.81-0.98). Conclusion: Even when teeth are lacking, intraoral scanning of the jaw and soft tissue-based registration of an intraoral scan with a CBCT scan can be performed with a high degree of precision.

골유착성 임플랜트와 자연치의 치은연하 세균조성에 대한 비교연구 (A COMPARATIVE STUDY ON SUBGINGIVAL MICROBIAL COMPOSITION BETWEEN NATURAL TEETH AND OSSEOINTEGRATED IMPLANTS)

  • Osseointegrated dental implants are now an accepted part of the prosthetic treatment of fully and partially edentulous patients. Clinical studies have shown that osseointegrated dental implants may successfully be used as abutments for prosthetic rehabilitations, and that the supporting tissue can be kept in a stable and healthy clinical state for prolonged periods of time. The purpose of this study was to look for differences in the distribution in the bacterial morphotypes around natural teeth and osseointegrated dental implants. The subgingival plaque around natural teeth and implants was analysed by means of differential phase-contrast microscopy. In addition, clinical indices used to analyse natural teeth condition were used to compare the condition of natural teeth and implants. The results were as follows : 1. In implants of partially edentulous patients, the plaque index, sulcus bleeding index(P<0.05), pocket depth(P<0.01) were greater than natural teeth. 2. Gingival crevicular fluid volume was not significantly different between natural teeth and implants. 3. In implants of fully edentulous patients, the plaque index was greater than natural teeth(P<0.01) and implants of partially edentulous patients(P<0.05), but sulcus bleeding index was not significantly different. 4. In partially edentulous patient, coccoid cell proportion was greater in implants than natural teeth (P<0.05). 5. In implants of fully edentulous patients, coccid cell proportion was even greater than implants of partially edentulous patients(P<0.05) and natural teeth(P<0.01). 6. Among three groups, the proportions of non-motile rods, motile rods, spirochetes and others were not significantly different. 7. The proportion of motile bacteria was not significantly different among three group.

치조골 흡수가 심한 하악 완전 무치악 환자에서 짧은 임플란트를 이용한 고정성 보철 수복 증례 (Rehabilitation using short implants in the mandibular fully edentulous patient with severe alveolar bone loss: a case report)

  • A short dental implant is considered as possible solution in difficult clinical situations for the placement of a regular length implant. Using a short implant avoiding more invasive surgical procedures simplifies the treatment plan and shortenes the duration of treatment. In this case, 71-year-old female came up with discomfort from her old mandibular denture. As she had fully edentulous mandible and got a negative feedback from removable denture, implant-supported fixed prosthesis was planned. Six short implants were placed on her mandible with severe alveolar bone loss. After 1 year follow up period, implants were well retained with any other abnormal findings. The patient was satisfied with her prostheses and satisfactory outcomes were attained in terms of both esthetic and functional clinical results.

Randomized controlled clinical trial of 2 types of hydroxyapatite-coated implants on moderate periodontitis patients

  • Purpose: The aim of this study was to compare and analyze the peri-implant tissue conditions and prospective clinical outcomes associated with 2 types of hydroxyapatite (HA)-coated implants: (1) fully HA-coated implants and (2) partially HA-coated implants with resorbable blast medium on the coronal portion of the threads. Methods: Forty-four partially edentulous patients were randomly assigned to undergo the placement of 62 HA-coated implants, and were classified as the control group (partially HA-coated, n=30) and the test group (fully HA-coated, n=32). All patients had chronic periodontitis with moderate crestal bone loss around the edentulous area. The stability and clinical outcomes of the implants were evaluated using the primary and secondary implant stability quotient (ISQ), as well as radiographic, mobility, and peri-implant soft tissue assessments around the implants. The Wilcoxon signed-rank test and the Mann-Whitney test were used to evaluate differences between and within the 2 groups, with P values <0.05 considered to indicate statistical significance. Results: The fully HA-coated implants displayed good retention of crestal bone, and insignificant differences were found in annual marginal bone loss between the 2 types of HA-coated implants (P>0.05). No significant differences were found in the survival rate (group I, 100%; group II, 100%) or the success rate (group I, 93.3%; group II, 93.8%). The fully HA-coated implants also did not significantly increase the risk of peri-implantitis (P>0.05). Conclusions: The fully HA-coated implants did not lead to an increased risk of peri-implantitis and showed good retention of the crestal bone, as well as good survival and success rates. Our study suggests that fully HA-coated implants could become a reliable treatment alternative for edentulous posterior sites and are capable of providing good retention of the crestal bone.

완전 무치악 환자에서 디지털 시스템을 이용한 임플란트 즉시 보철수복 증례 (Immediate restorations in a fully edentulous patient utilizing digital system: A case report)

  • 본 증례는 하악 완전 무치악 환자에서 인상채득과 석고모형 제작과정 없이 CBCT와 구강스캐너를 이용하여 수술 전에 한 번의 치과방문으로 임플란트 식립과 동시에 즉시 보철수복을 시행하는 방법을 제시하였다. 무치악 부위 연조직 형태, 수직교합고경, 상하악 악궁관계에 대한 정보는 환자가 사용하고 있는 의치를 스캔하여 얻었다. 이 방법은 무치악 환자에서 임플란트를 이용한 보철수복 과정을 기존의 방법보다 내원 횟수를 단축시켰다.

Influence of dental implantation on bone mineral density distribution: a pilot study

  • PURPOSE. Masticatory loading triggers active bone remodeling, altering alveolar bone mineral density (BMD). While dental implants are placed to bear masticatory loading, their influence on changing bone properties has not been fully investigated. Objective of this pilot study was to examine whether the dental implantation has an effect on BMD distribution of bone by comparing dentate, edentulous, and edentulous patients with implants. MATERIALS AND METHODS. Cone beam computed tomography (CBCT) images of 19 partially edentulous patients (Dent), 19 edentulous patients (Edent), and 16 edentulous patients who received implants in the mandible (Edent+Im), were obtained. CBCT images were also obtained from 5 patients within Edent+Im group, before implant placement and after implant loading. Basal cortical bone region of the mandible was digitally isolated. A histogram of gray levels proportional to BMD was obtained to assess mean, histogram standard deviation (HSD), fifth percentile of low and high values (Low5 and High5) of the BMD distribution. Multivariate analysis of variance and paired t-test were used to compare the BMD parameters among the 3 dental status groups and between pre- and post-implantation, respectively. RESULTS. Edentulous patients with implants had significantly greater HSD and High5 values compared to edentulous patients (P < .013). All other comparisons were not significant (P > .097). Mean, HSD, and High5 values significantly increased after receiving implants (P < .022). CONCLUSION. The current findings suggested that receiving dental implants promoted oral bone mineralization for edentulous patients. The longitudinal investigation could provide valuable information on understanding the effects of implantation on the behavior of oral bone quality.
