Acromioclavicular joint cyst is a extremely rare condition, usually occuring in the presence of a wide communication between glenohumeral and acromioclavicular joint in patients with a full thickness rotator cuff tear. Removal of cyst only is reported with high recurrence rate. There was no case previously reported in Korea. We report a case of recurred acromioclavicular joint cyst with a massive rotator cuff tear after simple excision, which was treated by excision of cyst, acromioplasty and resection of the lateral end of the clavicle.
목적: 견관절 자기 공명 영상에서 전방 및 외측 견봉 갈고리각을 측정하고 기존의 견봉 지표들과 비교하였다. 대상 및 방법: 2004년 6월에서 2005년 12월까지 수술을 통해 진단된 충돌증후군 환자를 대상으로, 동일한 시기에 충돌증후군 관련질환 이외의 원인으로 자기공명영상을 촬영한 대조군으로 선정하였다. 충돌증후군 환자군의 자기공명영상에 전방 견봉 갈고리각 (AAHA)과 측방 견봉 갈고리각 (LAHA), 그리고 견봉 갈고리지수 (AHI)를 측정하고 회전근개 파열환자군에 대해, 견봉형태, 견봉각, 전방덮임정도, 견봉경사, AAHA, 외측견봉각, 견봉염전각, 외측견봉굴곡각, LAHA, 외측덮임정도, 견봉상완거리, 견봉갈고리지수를 측정하여 대조군과 비교하였다. 결과: 충돌증후군 환자에서 단계가 진행할수록 AAHA, AHI는 증가하였다. 회전근개 파열환자에서 AAHA와 LAHA는 증가해 있었다. 단변량 분석에서는 견봉유형, 견봉각과 AAHA, LAHA, AHI가 회전근개 파열환자에서 대조군과 차이를 보였다. 다변량 분석에서는 성별, 연령, AAHA, 견봉각이 유의한 변수로 분석되었고 성별이 가장 큰 연관성이 있으며 연령, AAHA, 견봉각은 유사한 연관성을 보였다. 결론: 견봉의 형태는 사관상면의 특징이 회전근개 파열과 연관성을 가지므로 견봉성형술에서 견봉 측방 성형도 중요하다.
회전근개 파열은 성인 어깨 통증의 주요 원인이다. 사회활동의 증가로 인해 어깨 통증을 호소하는 환자가 증가하고 있으며, 어깨질환에 대한 관심도 높아지고 있다. 초음파 장비의 발달로 진단의 정확도가 높아지면서 근골격계 질환 중에서 회전근개 파열 진단에 주로 이용되면서 초음파검사는 회전근개 파열에서 MRI 검사를 보완할 수 있는 방법으로 인정받고 있다. 따라서 본 연구에서는 어깨 회전근개 파열의 진단에 있어 초음파와 MRI 검사의 진단적 유용성을 알아보고자 하였다. 초음파검사와 MRI검사를 완료한 후 관절내시경검사로 최종 회전근개의 손상을 진단받은 환자 262명을 대상으로 후향적 분석을 하였다. 회전근개중 파열의 빈도가 가장 높은 극상건과 견갑하건 두 힘줄의 초음파검사와 MRI검사의 결과에 대한 민감도, 특이도, 양성예측도, 음성예측도, 정확도를 분석하였다. 또한 회전근개파열의 정도를 5단계로 점수화하여 비교하였다. 초음파 검사는 전층파열과 부분파열 모두 MRI 검사 결과와 유사하였고 통계적으로 유의한 차이는 없었다. 부분파열 검사 결과는 MRI 검사보다 양성예측도와 정확도가 높았다. 결론적으로 초음파는 회전근개 질환을 검사하기 위한 선별검사로 충분히 활용될 수 있으며, 환자의 체질과 상황에 따라 선택하여 임상적으로 활용될 것으로 생각된다.
The objective of this study is to report the improved case of Korean medicine treatment with Aconitum ciliare decaisne pharmacopuncture for supraspinatus tendon full-thickness tear. We used Aconitum ciliare Decaisne pharmacopuncture with other Korean medicine treatments (acupuncture, electroacupuncture, and cupping) for this patient. This case is measured and assessed by the numerical rating scale (NRS), shoulder range of motion (ROM), shoulder pain and disability index (SPADI) and shoulder physical examination. As the result, we found the improvement of NRS, SPADI, ROM, and shoulder physical examination. This study suggested that Korean medicine treatment with Aconitum ciliare Decaisne pharmacopuncture is proved to be helpful to relieve pain and recover function for supraspinatus tendon full-thickness tear. But further clinical studies are needed to clarify the effect of Aconitum ciliare Decaisne pharmacopuncture therapy for patients with supraspinatus tendon full-thickness tear.
Background: The purpose of this study was to evaluate and compare deltoid origin status following large rotator cuff repair carried out using either an open or an arthroscopic method with a propensity score matching technique. Methods: A retrospective review of 112 patients treated for full-thickness, large rotator cuff tear via either a classic open repair (open group) or an arthroscopic repair (arthroscopic group) was conducted. All patients included in the study had undergone postoperative magnetic resonance imaging (MRI) and clinical follow-up for at least 12 and 18 months after surgery, respectively. Propensity score matching was used to select controls matched for age, sex, body mass index, and affected site. There were 56 patients in each group, with a mean age of 63.3 years (range, 50-77 years). The postoperative functional and radiologic outcomes for both groups were compared. Radiologic evaluation for postoperative rotator cuff integrity and deltoid origin status was performed with 3-Tesla MRI. Results: The deltoid origin thickness was significantly greater in the arthroscopic group when measured at the anterior acromion (P=0.006), anterior third (P=0.005), and middle third of the lateral border of the acromion level (P=0.005). The deltoid origin thickness at the posterior third of the lateral acromion was not significantly different between the arthroscopic and open groups. The arthroscopic group had significantly higher intact deltoid integrity with less scarring (P=0.04). There were no full-thickness deltoid tears in either the open or arthroscopic group. Conclusions: Open rotator cuff repair resulted in a thinner deltoid origin, especially from the anterior acromion to the middle third of the lateral border of the acromion, at the 1-year postoperative MRI evaluation. Meticulous reattachment of the deltoid origin is as essential as rotator cuff repair when an open approach is selected.
Objectives : The aim of this study is to observe clinical characteristics and treatment outcomes of inpatients admitted to a Korean medicine hospital for shoulder pain based on magnetic resonance imaging(MRI) findings. Methods : 28 patients with MRI data were included. Data on MRI findings, duration of pain, and treatment effects were collected. To measure treatment outcomes, verbal numerical rating scale(VNRS), range of motion(ROM), and a clinical condition grading system were used. Results : 1. The patient population was 75.0 % female and 39.3 %(n=11) belonged to the 51~60-year-old age group. In the under 40 population, 1 or less MRI findings were found. 2. As for the type of disorder, 82.1 % of the patients were diagnosed with 'Supraspinatus lesion', 39.3 % with 'subacromial-subdeltoid(SA-SA) bursitis', and 28.6 % with 'Adhesive capsulitis' 3. Numerous correlations could be made between duration of pain and MRI findings. 'rotator cuff full thickness tear' was more prevalent in acute shoulder pain patients, and 'Adhesive capsulitis' in chronic patients. 4. VNRS and ROM at the time of discharge had significantly improved as compared to VNRS and ROM at the time of admission. 5. As for correlation between MRI findings and clinical condition grade, patients diagnosed with 'rotator cuff partial tear' reported to be in 'good' condition or better whereas those with 'rotator cuff full thickness tear' reported to be in 'fair' condition or worse. Conclusions : Patients admitted to a Korean Medicine hospital for shoulder pain had varying degrees of severity as shown on MRI. Almost all lesions responded well to Korean medicine treatment.
목적: 견관절 회전근 개 전층 파열시 관절경적 복원술 후 대결절에 부착되어진 회전근 개의 건이 상완골에 어느 정도의 비율로 건-골(tendon to bone)에 치유되는지에 대하여 알아보고자 하였다. 또한 저자들은 극상건 전층 파열된 례에서 관절경적 복원술 후의 기능적 평가를 시행하였다. 대상 및 방법: 연속적 인 30례의 극상건 전층 파열을 19례에서 one row of anchor를, 11례에서는 two rows of anchor를 사용해 관절경적 복원술을 시행하였다. 환자들의 연령은 51세부터 79세로 평균 63세였으며, 평균 추시 기간은 술후 12개월부터 28개월로 평균 16개월이었다. 술전과 마지막 추시에 주관적 및 객관적인 기능적인 평가를 위해 Constant, ASES와 HCLA score를 측정하였다. 해부학적인 건 치유를 확인하기 위해 30례 전례에서 술후 6개월에서 20개월, 평균 10개월째 자기 공명 관절 조영술(25례)이나 자기 공명 술(5례)을 시행하였다. 결과: 관절경적 복원술후 회전근 개의 대결절에 대한 완전 치유는 30례중 21례(70.0%)였고, 부분적인 파열은 3례(10%)였으며, 극상건이 전혀 치유되지 않아 대결절에 부착되지 못한 경우는 6례 (20%)였으나, 그 중 5례에서는 술후 남아있는 결손의 크기가 처음 술전 보다는 작았다. 객관적인 평가로 Constant score는 술전에 평균 $55.7{\pm}7.1$점에서 술후 마지막 추시 관찰시에 $77.7{\pm}9.7$점으로 향상되었고(p<0.001), ASES xcore는 술전 $39.2{\pm}7.4$점에서 $72.4{\pm}12.6$점으로 향상되었고(p<0.001), UCLA score는 술전 $17.9{\pm}2.2$점에서 $26.5{\pm}5.0$점으로 향상되었다(p<0.001). 견관절의 전방 거상시 근력 이 대결절에 치유된 례 $(7.1kgs{\pm}2.4)$에서 전혀 치유되지 않아 완전히 재 파열되었거나, 부분 파열을 보인 례 $(4.5kgs{\pm}1.0)$에서 보다 통계학적으로 의미 있는 향상을 보였다(p<0.05). 건 치유에 결정적인 영향을 미치는 인자는 환자의 나이였다. 65세 이상의 환자에서는 단지 41.7%만이 대결절에 극상건의 완전한 치유율을 보였다(p<0.05). 결론: 회전근 개 중 극상건 전층 파열된 례에서 관절경적 복원술 후 약 70%에서 완전한 치 유를 보였다. 술후 극상건의 부분 파열 및 재파열은 근력 저하와 직접적인 연관을 가졌다. 특히, 노령 환자에서의 회전근 개 상태가 술후 재파열과 밀접한 관련이 있다.
목적: 회전근 개 건 전층 파열 시 관절경적 교량형 봉합술 시행 후 단기 추시 관찰을 통해 이 술식의 임상적 결과에 대해 알아보고자 하였다. 대상 및 방법: 회전근 개 건 전층 파열로 관절경적 교량형 봉합술을 시행 받고 조기 재활을 시행한 29명 (남자:17명, 여자:12명)의 환자를 대상으로 시행하였다. 평균 나이는 56.4세 (34~73세)였으며 추시 관찰 기간은 평균 13개월 (12~15개월)이었다. 임상적 및 기능적 평가로 The University of California at Los Angeles (UCLA) score, The Korean Shoulder Scoring system (KSS) 및 Visual Analogue Scale (VAS) score를 이용하였다. 15예에서 최종 추시시 자기 공명 영상을 촬영하여 Sugaya 분류를 통해 회전근 개의 구조적 상태를 분석하였다. 결과: 수술 후의 UCLA score는 수술 전과 수술 후 각각 16.4에서 31.6으로 측정되어 유의할만한 호전을 보였고 (p< 0.05), KSS는 수술 후 6개월에 88이었고, 최종 추시 시 92로 측정되었다. VAS score는 수술 전 8.6에서 수술 후 3개월에 2.1로, 수술 후 6개월에는 1.4로 나타났다. 28예 (96.5%)에서 수술 후 관절 운동 범위가 증가되었다. 추시 시 자기 공명 영상을 촬영한 15예에서 Sugaya 분류에 의해 I형이 6예, II형이 7예, III형이 1예, V형이 1예이었다. 결론: 회전근 개 건 전층 파열에 대한 관절경적 교량형 봉합술을 이용한 회전근 개 봉합술은 임상적으로 96.5%에서 양호 이상의 만족할 만한 결과를 보였으므로 믿을 만한 수술적 방법 중 하나라고 생각한다.
Background: The prognostic factors for patients with full-thickness rotator cuff tears (RCTs) include tear size, muscle atrophy and fatty infiltration. However, the influence of early coexisting degenerative changes on RCT outcomes is unappreciated. The purpose of this study was to calculate the impact that pre-existing partial glenohumeral cartilaginous changes have on patients undergoing arthroscopic RCT repair. Methods: A study of 54 patients undergoing arthroscopic RCT repair was undertaken. The presence of co-existing patches of glenohumeral degenerative cartilaginous changes and RCT size was recorded at surgery. Pre- and postoperative outcomes were assessed using traditional (Oxford Shoulder Score [OSS], 5-level EuroQol-5D [EQ-5D-5L] questionnaire and EuroQol visual analog scale [EQ-VAS]) and patient-centric re-formatted prisms. Outcomes were assessed as an entire dataset, and sub-group analysis was performed according to the grade of co-existing arthritis and tear size. Results: Significant improvements (p<0.05) in clinical outcomes were recognized when assessed using either the traditional or reformatted prisms (average % improvements in OSS, EQ-5D-5L and EQ-VAS were 47%, 33% and 43%, respectively; average improvements in pain, function, and psychological well-being were 48%, 33% and, 29%, respectively). Positive gain was noted in all sub-groups of arthritic grading and tear size. Conclusions: Good clinical outcomes can be achieved following RCT repair even in the presence of local partial degenerative cartilage changes and advancing tear size. These benefits are patient-centered but require RCT repairability.
Background: Assessment of the clinical outcomes after rotator cuff repair is essential for their effectiveness on treatment. The Korean Shoulder and Elbow Society devised the Korean Shoulder Scoring System (KSS) for patients with rotator cuff disorder. The purpose of this study was to evaluate the availability of the KSS for assessment of clinical outcomes in patients after arthroscopic rotator cuff repair, and for comparison with other appraisal scoring systems. Methods: A total of 130 patients with partial-thickness or full-thickness rotator cuff tear who underwent arthroscopic repair using a single row or double row suture bridge technique were enrolled. The average follow-up period was 25.9 months. All patients were classified according to various factors. Comparison within corresponding categories was performed, and the correlation between the KSS and other shoulder assessment methods including University of California Los Angeles (UCLA), Constant and American Shoulder and Elbow Surgeons (ASES) score was analyzed. Results: Total score of the KSS response had increased from 59.6 preoperatively to 88.96 at last follow-up. All KSS domains, including function, pain, satisfaction, range of motion, and muscle power had improved up to 24 months postoperatively. Statistical significance was observed mainly in preoperative measurements with number and size of torn tendons, and greater than or equal to grade 3 of fatty infiltration. The KSS was best correlated with the UCLA scoring system in both preoperative (r=0.785) and postoperative (r=0.951) measurements. Conclusions: The KSS was highly reliable and valid as a discriminative instrument, and it showed strong correlation with ASES and UCLA scoring systems.
