Validation of Urine Volume Evaluation by Hydraulic Pressure Measurement (압력 측정에 의한 요량 계측의 유용성 분석)

  • Uroflowmetry is non-invasive and easily performed to diagnose benign prostatic hypertrophy(BPH) frequent in aged men. Weight change during urination is usually measured to estimate the urinary flow rate by a load cell, but sensitive to any impacts against the bottom of the container, leading to unnecessary noise generation. Moreover, load cells are relatively expensive raising the production cost. The present study proposed a new technique, measuring hydraulic pressure on the bottom of the urine container to evaluate the urinary volume. Low cost pressure transducer enabled almost perfectly linear relationship between the urine volume and the hydraulic pressure. During both the simulated and human urination experiments, variance of the pressure signal was more than 50% smaller than the weight signal acquired by a load cell, which demonstrated that the impact noise was decreased to a great degree by pressure compared to weight measurement.

  • Purpose: This paper aims to report the effects of Oryeong-san on lower urinary tract infection. Methods: Four women were admitted to Dunsan Oriental Hospital with chief complaint that was not supposed to be a symptom of urinary tract infection. During the admission, urine test showed pyuria with white blood cell and their chief complain also accompanied. We prescribed Oryeong-san three times a day, and then took follow-up 4 days later. Results: After treatment, symptoms of lower urinary tract infection were decreased and the results of urine test improved also. Conclusions: These cases indicate that Oryeong-san is effective in treatment of the lower urinary tract infection.

Three yin disease should be treated by warming the spleen and drying dampness, requiring urgent warming, using formulae such as Center-rectifying dedoction, Aconite dedoction and Counterflow cold dedoction.

Prevalence of Insomnia and Its Related Factors in Korean Women

  • Purpose: The purposes of this study were to investigate the prevalence of insomnia and to compare sleep patterns, demographic characteristics, and obstacles for sleep between women with and without insomnia. Methods: This was a descriptive study. Study participants were 1,679 Korean women aged over 20 years. Information on symptoms of insomnia, sleep patterns, and related factors was assessed by questionnaire. Symptoms of insomnia included difficulties in initiating and maintaining sleep and early morning awakening. Results: This study found that 32.0% of the study participants had insomnia, subjects with insomnia had bad sleep patterns compared to comparison group, lower educational level and menopause were closely related to insomnia, and noise, temperature, lighting, presence of bed partner, intake of caffeine, frequent urination, and pain or itching were associated with insomnia. Conclusion: This study suggests that insomnia is prevalent in women and closely associated with education level and menopausal status.

Polysomnographic Assessment of Nocturnal Enuresis in Adults: A Case Study of Parasomnia Overlap Syndrome With Obstructive Sleep Apnea

  • Enuresis nocturia is more prevalent in children than in adults. Using polysomnography (PSG), we investigated the causes of adult enuresis nocturia in a 20-year-old female patient with nighttime bedwetting. In spite of normal urological examinations, her detailed medical history disclosed frequent sleep paralysis and urination during dreams. During PSG, two electromyograms were attached to her anus to assess the tone of her bladder's sphincter while she slept. During REM sleep, the EMG tone of the mandible decreased, but the anal and bladder sphincter tones did not. The polysomnogram revealed moderate obstructive sleep apnea. Consequently, she was diagnosed with adult parasomnia (nocturnal enuresis) overlap syndrome with OSA. This study demonstrates the value of PSG with simultaneous anal tone EMG for diagnosing NREM parasomnia and nocturnal enuresis.

Three Cases Report of the Female Chronic Relapsing Cystitis Treated with Takchabojoongikkitang-gamibang (택차보중익기탕가미방(澤車補中益氣湯加味方)으로 호전된 여성 만성 재발성 방광염 환자 치험 3례)

  • Purpose: This paper is aim to report the effects of Takchabojoongikkitang-gamibang (澤車補中益氣湯加味方) on the chronic relapsing cystitis. Methods: Three women who had different age 27 years old, 28 years old and 51 years old women, who diagnosed chronic relapsing cystitis. were enrolled in this study. They received oriental treatments such as herbal medicine, acupuncture for several months. The 27 years old women we gave her Takchabojoongikkitang-gamibang 3 times for 3 months. The 28 years old women we gave her Takchabojoongikkitang-gamibang 2 times for 2 months. The 51 years old women we gave her Takchabojoongikkitang-gamibang 5 times for 3 months. During the treatments, we requested them to visit hospital every two weeks. If they could not come to hospital, we called them to take follow-up. Results: After the treatments, symptoms of chronic relapsing cystitis were decreased or disappeared. Conclusion: This clinical cases indicate that Takchabojoongikkitang-gamibang is effective in treatment of the chronic relapsing cystitis and can prevent chronic progress and relapse of symptoms. More clinical data and studies are requested for the treatment of chronic relapsing cystitis.

The Effect of Korean Medical Treatment on the Overactive Bladder Syndrome with Coughing: Case Report (기침을 동반한 과민성 방광 증후군에 대한 한방 치료 1례)

  • Objective: Overactive bladder syndrome refers to the symptom of frequent urination, as the function of the bladder is so sensitive that the bladder muscles contract regardless of the patient's intent and the patient feels the need to urinate quickly. Overactive bladder syndrome is common among the elderly, especially in women, and the prevalence increases with increasing age. Overactive bladder can decrease social activity, increase isolation, and decrease quality of life due to sleep disorders caused by night urination; therefore, active treatment is required. In this study, we report the effects of Korean medical treatment on a Korean hospital patient with overactive bladder symptoms, including urine frequency, urine incontinence, and night urination that began at a similar time to an onset of coughing. Methods: We treated the patient with herbal medicines and acupuncture therapy. The International Prostate Symptom Score (IPSS), Overactive Bladder Symptom Score (OABSS), and a numeric rating scale (NRS) were used to assess symptom changes. Results: The patient with urine frequency, urine incontinence, and nocturia was hospitalized for 8 days, and the overall symptoms of overactive bladder syndrome were improved. Conclusion: The results indicate that Korean medical treatment is effective in patients with overactive bladder syndrome.

Drug Use Evaluation of Anticholinergic Drugs Prescribed to Elderly Patients in the Ambulatory Setting Based on Beers and STOPP Criteria (Beers Criteria 및 STOPP 근거에 의거한 한국 노인 환자의 항콜린성제 약물요법의 적절성 평가)

  • Anticholinergic drugs are included in the efficacy group of various antidepressants, antihistamines, antispasmodics like skeletal muscle relaxants. Elderly patients are often taking anticholinergic drugs due to various diseases such as sleep disorders and dysuria. But the use of anticholinergic drugs is restricted in guidelines such as Beers Criteria or STOPP due to the anticholinergic adverse effects including dry mouth, constipation, difficult urination, delirium, hallucinations and especially cognitive impairment. In this study, we investigated the usage of anticholinergic drugs in out-of-hospital prescription of 4,442 elderly patients. Results of the study were obtained that 32% (n=1,421) of overall patients were prescribed with 1~6 products (average 1.37) of anticholinergic drugs. 70.9% of the 1,421 patients (n=1,007) were prescribed with one drug, 22.7% (n=323) were two drugs and 4.9% (n=70) were three drugs. 27.1% of the 1,421 patients (n=430) were 70~74 years old patients who were the most commonly prescribed with anticholinergic drugs. Amitriptyline, chlorpheniramine, dimenhydrinate and quetiapine were most frequent component of ACB Score 3 drugs and amantadine, baclofen, carbamazepine, cyproheptadine and oxcarbazepine were most frequent of ACB Score 2 drugs. Anticholinergic Drug Scale (ADS) of individual patients presented one point (48.5%, n=689), 2 points (15.4%, n=219), more than 3 points (36.1%, n=513), and up to maximum 12 point (n=1). More than 2 points were more than half (51.5%, n=732). Therefore, additional prospective study in the use and adverse effects of anticholinergic drugs for elderly patients will be required. And national management such as DUR program will be required for elderly drug administration from now on.

A Study on the Liu Wansu's "Theory on the three wasting-thirst" (유완소(劉完素)의 "삼소론(三消論)"에 대한 연구(硏究))

  • Wasting-thirst(消渴) occurs due mainly to dryness-heat(燥熱) which by depleting 'Fluid and Humor'(津液) make symptoms of thirst(口渴) swift digestion with rapid hungering(消穀善饑) frequent urination(小便頻數) according to "Internal Classic"(內經). However, the doctors of those days had a tendency to manage wasting-thirst with herbal drugs of dryness-heat or with drugs that tonifies the kidney. Liu Wansu(劉完素) thought that those kinds of drugs intensify gatro-intestinal dryness-heat and consequently could not be suitable for the patients with wasting-thirst. He said that they had mistaken about the relation between five viscera(五臟) and six climate condition(六氣) and that they did not know thoroughly on the theory of branch and root(標本). For preventing the disease of wasting-thirst, Liu Wansu(劉完素) recommended avoiding gatro-intestinal drying caused by the intake of dryness-heat diet, and warned not to deplete 'Fluid and Humor' due to stress-induced fire(火), not to damage yin qi(陰氣) through severe diseases.
