경기장 관람석의 효율적인 진동해석 (Efficient Vibration Analysis of Stadium Stands)

  • 고장도 재료의 사용과 시공기술의 발달로 경기장 관람석이 장경간화 유연화 되어 구조물의 고유진동수가 매우 낮아지고 있다. 경기장 관람석구조물은 사람의 율동적인 움직임에 의하여 공진 또는 공진과 유사한 거동이 일어날 수 있으므로 진동에 대한 구조물의 안전성 및 사용성에 대한 검토가 요구된다. 경기장 관람석의 효과적인 설계를 위해서는 경기장 관람석에 가해지는 동적하중에 대한 분석과 경기장 관람석의 거동에 대한 정확한 분석이 있어야 한다. 경기장 관람석에 가해지는 동적하중은 매우 다양하며 수식적으로 표현하기가 쉽지 않기 때문에 계측한 동적하중을 적용하는 것이 바람직하다. 그리고 판요소로 이루어진 경기장관람석의 정확한 진동해석을 위해서는 구조물을 조밀하게 나누어야 하기 때문에 해석시간이 길어지게 된다. 또한 경기장 전체 구조물의 진동해석에 있어서 경기장 전체 구조물을 세분화하여 모델링하는 것은 매우 어려울 뿐만 아니라 절점 수가 매우 많아지므로 절점수가 제한되어 있는 상용프로그램으로는 해석이 불가능할 수도 있다. 본 연구에서는 경기장 관람객의 여러 가지 움직임에 의하여 야기되는 다양한 동적하중을 계측하여 분석하였다. 그리고 판요소로 이루어진 경기장 관람석의 동적특성을 비교적 잘 표현할 수 있으면서도 절점을 줄일 수 있는 효과적인 해석방법을 제안하였다. 제안된 방법의 정확성과 효율성을 살펴보기 위하여 경기장 관람석의 진동해석을 수행하였다.

한국(韓國)의 전통적(傳統的) 민간요법(民間療法)의 이용실태(利用實態) 조사연구(調査硏究) -성인병(成人病)에 이용(利用)되는 식품(食品)을 중심(中心)으로- (A Study on the Actual Utilization Korean Traditional Remedies -About foods used on geriatric disease-)

  • This study was designed under the purpose to analyze the Korean Traditional Folk Remedies on food, to submit the basic statistical data, and to utilize them by examining how much they know about the Korean Traditional Folk Remedies and also by examining how frequently they used them. The sample was consisted of 312 housewives living in Daegu, Kyungsan city and Kyungsan-eup, the survey was made by use of questionaires from July 15 to August 1 1989. The results are as follows 1. Analysis of Folk Remedical Contents 1) The method of the Folk Remedies used in this area was mainly food, which can be easily found around their house and easy to use, it was possible to apply scientific principles in some cases. 2) The major popular method was shown to feed boiled barley or brown rice which are known as the Folk Remedy for Diabetes. 3) The ANOVA of variable shows the significant influence on each object which age, religion, the level of education, job, monthly income, and the area except family formation. 2. The Frequency of Folk Remedies and additional analysis 1) The most frequently used Folk Remedy was Arterio-sclerosis(1.44), which is followed by the loss of Eyesight(1.40), Hang over(1.28), Couth(1.27), Cold(1.26) etc. 2) In the analysis of Pearson Correlation between frequencies of Folk Remedies used, and demographic variables such as age(p<.05), the monthly income(p<.01), in that area, total number of response items showed a positive correlation. 3. The source to learn about Folk Remedy. 1) It is acknoledged that they are mostly instructed by their forefathers, friends, neighbors, professional textbooks, mass communications, herb doctors, other medical sources or education at school etc. 2) The ANOVA of Variables shows the big differences between each group by age. Based on the above findings, the following suggestions are made ; Most of Korean Traditional Folk Remedies are recognized scientific and reasonable which are based on the scientific research and herb medicine therefore it should be made good use of for our life in good health additionally. I hereby insist that the importance of our Folk Remedies should be reviewed and focused for maintenance our health.

지반조건과 지진파를 고려한 해저철도 터널의 동적 수치 모델링 (Dynamic Numerical Modeling of Subsea Railway Tunnel Based on Geotechnical Conditions and Seismic Waves)

  • 철도는 정시성과 수송능력을 갖춘 여객 및 물류 운송 수단으로서 널리 사용되고 있다. 근래의 급격한 건설기술의 발전으로 철도는 대륙과 대륙, 대륙과 섬을 잇는 다양한 해저철도의 건설이 계획되고 있으며 국내에는 보령 해저터널(2021), 가덕 해저터널(2010) 등 유사 설계 및 시공 경험이 축적되고 있다. 그러나 최근 국내외적으로 다양한 규모의 지진이 빈번하게 발생함에 따라 지진위험에 대한 인식이 증대되고 있으며, 국내 지진의 발생빈도 또한 증가하고 있다. 해저터널에 대한 지진의 영향은 매우 복잡하나 주로 지반조건, 지진하중 등에 의하여 큰 차이를 보일 수 있다. 본 연구에서는 외부수압이 작용하는 가상의 해저철도 쉴드터널에 대하여 지반구성과 지진파 의한 영향을 파악하기 위하여 토사, 암반, 복합지반 및 파쇄대로 구성된 4 가지 지반조건을 반영한 3차원 수치해석 모델을 구축하였다. 각 해석 모델에 대하여 장주기, 단주기, 그리고 장단주기 특성을 모두 가지는 인공지진파를 각각 적용하여 유한차분해석법에 의한 동적해석을 수행하여 변위 및 터널 부재력 응답을 검토하였다. 그 결과 터널 변위는 토사지반에서 터널 진행방향에 연직방향 지진하중이 작용 시 장주기파의 영향이 우세하고 암반지반에서는 단주기파의 영향이 우세한 결과를 얻었다. 또한 국부적 취약구간인 파쇄대에서는 인공지진파의 영향이 우세하였다. 터널 세그먼트의 부재력 검토 결과 지반 및 세그먼트의 구속효과로 인하여 지진하중이 변위보다 부재력에 미치는 영향이 크게 나타남을 확인하였다. 

The Survey of Dentists: Updated Knowledge about Basic Life support and Experiences of Dental Emergency in Korea

  • Background: Various medical emergency situations can occur during dental practices. Cardiac arrest is known to comprise approximately 1% of emergency situation. Thus, it is necessary for dentists to be able to perform cardiopulmonary resuscitation (CPR) to increase the chance of saving patient's life in emergency situation. In this paper, we conducted a survey study to evaluate to what extent dentists actually understood CPR practice and if they had experience in handling emergency situations in practice. Method: The survey was done for members of the Korean Dental Society of Anesthesiology (KDSA), who had great interest in CPR and for whom survey-by-mail was convenient. We had selected 472 members of the KDSA with a dental license and whose office address and contact information were appropriate, and sent them a survey questionnaire by mail asking about the degree of their CPR understanding and if they had experience of handling emergency questions before. Statistical analyses -frequency analysis, chi-square test, ANOVA, and so on- were performed by use of IBM SPSS Statistics 19 for each question. Result: Among 472 people, 181 responded (38.4% response rate). Among the respondents were 134 male and 47 female dentists. Their average age was $40.4{\pm}8.4$. In terms of practice type, there were 123 private practitioners (68.0%), 20 professors (11.0%), 16 dentists-in-service (8.8%), 13 residents (specialist training) (7.2%) and 9 military doctors (5%). There were 125 dentists (69.1%) who were specialists or receiving training to be specialist, most of whom were oral surgeon (57, 31.5%) and pediatric dentists (56, 30.9%). There were 153 people (85.0%) who received CPR training before, and 65 of them (35.9%) were receiving regular training. When asked about the ratio of chest pressure vs mouth-to-mouth respiration when conducting CPR, 107 people (59.1%) answered 30:2. However, only 27.1% of them answered correctly for a question regarding CPR stages, C(Circulation)- A(Airway)- B(Breathing)- D(Defibrillation), which was defined in revised 2010 CPR practice guideline. Dentists who had experience of handling emergency situations in their practice were 119 (65.6%). The kinds of emergency situations they experienced were syncope (68, 37.6%), allergic reactions to local anesthetic (44, 24.3%), hyperventilation (43, 23.8%), seizure (25, 13.8%), hypoglycemia (15, 8.3%), breathing difficulty (14, 7.8%), cardiac arrest (11, 6.1%), airway obstruction (6, 3.3%), intake of foreign material and angina pectoris (4, 2.2%), in order of frequency. Most respondents answered that they handled the situation appropriately under the given emergency situation. In terms of emergency equipment they had blood pressure device (70.2%), pulse oximetry (69.6%), Bag-Valve-Mask (56.9%), emergency medicine (41.4%), intubation kit (29.8%), automated external defibrillator (23.2%), suction kit (19.3%) and 12 people (6.6%) did not have any equipment. In terms of confidence in handling emergency situation, with 1-10 point scale, their response was $4.86{\pm}2.41$ points. The average point of those who received regular training was $5.92{\pm}2.20$, while those who did not was $4.29{\pm}2.29$ points (P<0.001) Conclusion: The result showed they had good knowledge of CPR but the information they had was not up-to-date. Also, they were frequently exposed to the risk of emergency situation during their dental practice but the level of confidence in handling the emergency situation was intermediate. Therefore, regular training of CPR to prepare them for handling emergency situation is deemed necessary.

표준화된 KoFlux 에디 공분산 자료 처리 방법의 변화와 개선 (Changes and Improvements of the Standardized Eddy Covariance Data Processing in KoFlux)

  • KoFlux의 표준화된 에디 공분산 플럭스 자료 처리과정이 갱신되는 과정에서 그 처리 방법에 따른 결과도 조금씩 달라져 왔다. 대부분의 자료 사용자들은 자료 처리 결과의 차이와 이러한 차이가 자신들의 분석결과에 미칠 수 있는 영향에 대해 명확히 인지하지 못하고 자료를 사용하고 있는 실정이다. 본 총설에서는 KoFlux 데이터베이스를 사용하는 연구자들에게 자료처리 과정을 투명하게 정리하여 자료에 대한 신뢰성과 활용성을 확보하기 위해, 과거의 자료 처리 방법이 어떻게 변화되고 개선되었는지를 평탄하고 균질한 해남 논 관측지(HPK)와 복잡하고 비균질한 광릉 활엽수림 관측지(GDK) 자료를 처리하고 그 차이를 확인하여 문서화하였다. 관측 대상지와 관측 장비의 다양화로 인해, 기존에 무시되거나 간소화 되었던 자료 처리 과정(예, 주파수 반응 보정, 정상성 검정 등)을 다시 적용하였고, 메탄 플럭스 결측 메우기와 이산화탄소 플럭스 보정 및 배분 방법을 새롭게 개선하였다. 본 연구결과로부터 에디 공분산 플럭스 관측 자료의 품질에 주파수 반응 보정(HPK: 연적산값의 11~18%의 편향 발생, GDK: 6~10%)과 정상성 점검(HPK: 연적산값의 4~19%의 편향 발생, GDK: 9~23%)이 매우 중요하고, 결측 메우기 및 배분 과정에 있어서 우선적으로 결측을 최소화하는 것이 최선이며, 대상 플럭스의 변동을 설명할 수 있는 적절한 조절 인자의 선택이 처리방법의 선택보다 중요함을 확인 하였다. 장기 KoFlux 관측 자료의 정확성, 투명성 및 연속성 확보를 위해 위의 결과를 반영하는 자료 처리 기술 개발과 문서화를 지속적으로 추진해 나갈 것이다.

충남 일부지역 청소년의 체질량지수에 따른 채소류 섭취상태 (Evaluation of Vegetable Intakes according to Body Mass Index of Adolescents in Chungnam)

  • 본 연구는 청소년을 대상으로 BMI에 따라 채소류의 섭취상태를 조사함으로써 체형에 따른 적절한 채소 섭취지도와 바람직한 식생활 및 식습관 형성에 필요한 기초자료를 제시하고자 하였다. 따라서 충남 부여군에 거주하는 중 고등학교 청소년 400명을 대상으로 BMI에 따라 저체중군, 건강체중군, 과체중 이상 군으로 분류하여 식습관, 채소류의 1일 섭취수준, 채소류에 대한 인식, 채소류 관련 식습관을 비교 분석하였다. 조사대상자의 가족 수는 4명(49.0%)이 가장 많았고, 아버지 학력과 어머니 학력은 '고등학교 졸업'이 각각 39.0%, 59.0%로 가장 많았으며, 월평균소득은 '300~400만원(32.3)%'이 가장 많았다. 아침식사를 거르는 이유는 '시간이 부족해서(44.3%)'가 가장 많았으며, 편식하는 음식의 종류는 저체중과 과체중의 경우 '곡류'라는 응답이 가장 높게 나타난 반면, 건강체중의 경우에는 '채소류'라는 응답이 가장 높아 유의한 차이를 보였다(P<0.001). 채소류의 1회 섭취량을 고려한 1일 섭취횟수는 저체중군 3.3회, 건강체중군 3.8회, 과체중군 3.8회로 체형군별 유의한 차이가 없었다. 그러나 점심식사를 통한 채소류의 섭취횟수는 저체중군이 1회로 건강체중군 1.3회와 과체중군 1.4회보다 유의하게 낮았다(P<0.05). 채소류에 대한 인식도 평가에서 채소류는 '먹으면 피부가 예뻐진다'(P<0.01)와 '조리방법에 따라 먹을 때도 있고 먹지 않을 때도 있다'(P<0.05)라는 인식이 저체중군에서 유의하게 높았다. 채소류 관련 식습관 평가에서 저체중군의 채소류 기호도는 건강체중과 과체중군에 비해 낮았으며, 채소가 좋은 이유는 '맛이 있어서', 채소가 싫은 이유는 '그냥'이, 채소의 섭취량은 '급식과 집에서 먹는 양이 비슷하다'가, 싫어하는 채소 색은 '흰색', 싫어하는 채소 조리법은 '삶아서 무침', 채소의 싫어하는 느낌은 '쌉쌀하다', 선호하는 김치는 '나박김치'라는 응답이 가장 많았다. 이상의 결과를 종합하면, 본 조사 청소년의 채소류 섭취는 전반적으로 낮았으며, 특히 건강체중군은 채소류에 대한 편식이 높았고 저체중군은 점심식사를 통한 채소류의 섭취횟수와 채소류에 대한 기호가 유의하게 낮았다. 앞으로 청소년의 체형과 채소류 섭취와의 관계를 보다 정확하게 규명하기 위한 연구와 함께 본 연구 결과를 고려한 적절한 채소류 섭취지도가 지속적으로 이루어져야 할 것이다.

  • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70$\pm$1.32mmHg/min)compared to CF dialyzers(4.32$\pm$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The strip loader and cassette were made to obtain more accurate reflectance parameters. The strip was illuminated at 45˚c through optical fiber and the intensity of reflected light was determined at rectanguLat angle using a photodiode. The kubelka-munk coefficient and reflection optical density were determined ar four different wavelengths(500, 550, 570 and 610nm) for blood glucose strip. For higher concentration than 300mg/41 about glucose, a saturation state of abforbance was observed at 500, 550 and 570nm. The mice were divided into two groups. One group was injected with Photofrin II, intravenously (2.5mg/kg body weight). The other group received no Photofrin II. Mice from both groups were irradialed for approximately 15 minutes at 100, 300, or 500 mW/cm2 with the argon (488nm/514.5 nm), dye(628nm) and gold vapor (pulsed 628 nm) laser light. A photosensitizer behaved as an added absorber. Under our experimental conditions, the presence of Photolfrin II increased surface temperature by at least 40% and the temperature rise due to 300 mW/cm2 irradiation exceeded values for hyperthermia. Light and temperature distributions with depth were estimated by a computer model. The model demonstrated the influence of wavelength on the thermal process and proved to be a valuable tool to investigate internal temperature rise. 30536 T00401030536 ^x We investigated the structural geometry of thirty-eight Korean femurs. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. The results suggested that the femoral component suitable for Asians should be different from the currently-used components designed and manufactured mostly by European or American companies. 30537 T00401030537 ^x It is well known that nonlinear propagation characteristics of the wave in the tissue may give very useful information for the medical diagnoisis. In this paper, a new method to detect nonlinear propagation characteristics of the internal vibration in the tissue for the low frequency mechanical vibration by using bispectral analysis is proposed. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Our department has developed an information system for routine hematology using down-sized computer system. We have used an IBM 486 compatible PC with 16MB main memory, 210 MB hard disk drive, 9 RS-232C port and 24 pin dot printer. The operating system and database management system were SCO UNIX and SCO foxbase, respectively. For program development, we used Xbase language provided by SCO foxbase. The C language was used for interface purpose. To make the system use friendly, pull-down menu was used. The system connected to our hospital information system via application program interface (API), so the information related to patient and request details is automatically transmitted to our computer. Our system interfaced with fwd complete blood count analyzers(Sysmex NE-8000 and Coulter STKS) for unidirectional data tansmission from analyzer to computer.   • The wall shear stress in the vicinity of end-to end anastomoses under steady flow conditions was measured using a flush-mounted hot-film anemometer(FMHFA) probe. The experimental measurements were in good agreement with numerical results except in flow with low Reynolds numbers. The wall shear stress increased proximal to the anastomosis in flow from the Penrose tubing (simulating an artery) to the PTFE: graft. In flow from the PTFE graft to the Penrose tubing, low wall shear stress was observed distal to the anastomosis. Abnormal distributions of wall shear stress in the vicinity of the anastomosis, resulting from the compliance mismatch between the graft and the host artery, might be an important factor of ANFH formation and the graft failure. The present study suggests a correlation between regions of the low wall shear stress and the development of anastomotic neointimal fibrous hyperplasia(ANPH) in end-to-end anastomoses. 30523 T00401030523 ^x Air pressure decay(APD) rate and ultrafiltration rate(UFR) tests were performed on new and saline rinsed dialyzers as well as those roused in patients several times. C-DAK 4000 (Cordis Dow) and CF IS-11 (Baxter Travenol) reused dialyzers obtained from the dialysis clinic were used in the present study. The new dialyzers exhibited a relatively flat APD, whereas saline rinsed and reused dialyzers showed considerable amount of decay. C-DAH dialyzers had a larger APD(11.70${\pm}$1.32mmHg/min)compared to CF dialyzers(4.32${\pm}$0.55mmHg/min)(p<0.05). However, there was no observable difference in the UFR between the two dialyzers. Neither APD nor UFR showed any significant increase with an increasing number of reuses for up to more than 20reuses. A substantial number of failures observed in APD(larger than 20mmHe/min)on the reused dialyzers(2 out of 40 CP and S out 26 C-DAK) were attributed to the Possible damage on the fibers. The CF 15-11 HFDs which failed APD test did not show changes in the UFR compared to normal dialyzers indicating that APD is a more sensitive test than UFR test to evaluate the integrity of the fibers. 30527 T00401030527 ^x For quantitative measurement of reflected light from a clinical diagnostic strip, a prototype old reflectance photometer was designed. The purpose of this study is to identify major geometrical differences between Korean femurs 3nd others that we believe belong to Caucasians so that we would be able to get insights into the femoral component design that fits Asians including Koreans. We utilized computerized tomography (CT) images of femurs extracted from cadavers. The CT images were transformed into bitmap data by using a film scanner, and then analyzed by using a commercially available software called Image v.1.0 and a Macintosh IIci computer.The resulting data were compared with already published data. The major results show that the geometry of the Korean femurs is significantly different from that of Caucasians: (1) the anteversion angle and the canal flare index are greater by the amount of approximately 8˚ and 0.5, respectively, (2) the shape of the isthmus cross section is more round, and (3) the distance between the teaser trochanter and the proximal border of the isthmus is shelter by about 15 mm. In the method, low frequency vibration of f0( = 100Hz) is applied on the surface of the object, and the waveform of the internal vibration x (t) is measured from Doppler frequency modulation of silmultaneously transmitted probing ultrasonic waves. Then, the bispectra of the signal x (t) at the frequencies (f0, f0) and (f0, 2f0) are calculated to estimate the nonlinear propagation characteristics as their magnitude ratio, w here since bispectrum is free from the gaussian additive noise we can get the value with high S/N. Basic experimental system is constructed by using 3.0 MHz probing ultrasonic waves and the several experiments are carried out for some phantoms. Optimal stimulus parameters, which can minimize muscle fatigue and can achieve effective muscle contraction were determined in slow and fast muscles in Sprague-Dawley rats. Stimulus patterns of our stimulator were designed to simulate electromyographic activity monitored during locomotion of normal subjects. Muscle types of the lower extremity were classified according to their mechanical property of contraction, which are slow muscle (msoleus m.) and fast muscle (medial gastrocneminus m., rectus femoris m., vastus lateralis m.). Optimal parameters of electrical stimulation for slow muscles were 20 Hz, 0.2 ms square pulse. For fast muscle, 40 Hz, 0.3 ms square pulse was optimal to produce repeated contraction. Higher stimulus intensity was required when synergistic muscles were stimulated simultaneously than when they were stimulated individually. Electrical stimulation for each muscle was designed to generate bipedal locomotion, so that individual muscles alternate contraction and relaxation to simulate stance and swing phases. Portable electrical stimulator with 16 channels built in microprocessor was constructed and applied to paraplegic patients due to lumbar cord injury. The main unit includes a 12.1" TFT color LCD, a main CPU board, and peripherals such as a module controller, Ethernet LAN card, video card, rotate/push button controller, etc. The main unit can connect at maximum three module cases each of which can accommodate up to 7 parameter modules. They include the modules for electrocardiograph, respiration, invasive blood pressure, noninvasive blood pressure, temperature, and SpO2 with Plethysmograph.

  • Type II collagen (CII), major component of hyaline cartilage, has been considered as an auto-antigen in rheumatoid arthritis (RA). However, the clinical and biological significances with regard to the CII autoimmunity need to be clarified in human RA. The presence of antibodies to CII has been identified in sera, synovial fluid, and cartilage of patients with RA. In our study, the increased titer of IgG anti-CII in sera was well correlated with C-reactive protein, suggesting that this antibody may reflect the inflammatory status of RA. The titer of anti-CII antibodies (anti-CII Abs) tended to be higher in early stages of diseases. In our extending study, among 997 patients with RA, 269 (27.0%) were positive for circulatory IgG antibody to CII, those levels were fluctuated over time. It is hard to assess the significant amount of T cell responses to CII and CII (255~274) in RA. By using a sensitive method of antigen specific mixed lymphocyte culture, we can detect the presence of CII-reactive T cells in peripheral blood mononuclear cells of RA patients. Sixty seven (46.9%) of 143 patients showed positive CII reactive T cell responses to CII or CII (255~274). The frequencies of CII reactive T cells were more prominent in inflamed synovial fluid (SF) than in peripheral blood. These T cells could be clonally expanded after consecutive stimulation of CII with feeding of autologous irradiated antigen presenting cells (APC). Moreover, the production of Th1-related cytokine, such as IFN-${\gamma}$, was strongly up-regulated by CII reactive T cells. These data suggest that T cells responding to CII, which are probably presenting the IFN-${\gamma}$ producing cells, may play an important role in the perpetuation of inflammatory process in RA. To evaluate the effector function of CII reactive T cells, we investigated the effect of CII reactive T cells and fibroblasts-like synoviocytes (FLS) interaction on the production of pro-inflammatory cytokines. When the CII reactive T cells were co-cultured with FLS, the production of IL-15 and TNF-${\alpha}$ from FLS were significantly increased (2 to 3 fold increase) and this increase was clearly presented in accord to the expansion of CII reactive T cells. In addition, the production of IFN-${\gamma}$ and IL-17, T cell derived cytokines, were also increased by the co-incubation of CII reactive T cells with FLS. We also examined the impact of CII reactive T cells on chemokines production. When FLS were co-cultured with CII stimulated T cells, the production of IL-8, MCP-1, and MIP-1${\alpha}$ were significantly enhanced. The increased production of these chemokines was strongly correlated with increase the frequency of CII reactive T cells. Conclusively, immune response to CII was frequently found in RA. Activated T cells in response to CII contributed to increase the production of proinflammatory cytokines and chemokines, which were critical for inflammatory responses in RA. The interaction of CII-reactive T cells with FLS further augmented this phenomenon. Taken together, our recent studies have suggested that autoimmunity to CII could play a crucial role not only in the initiation but amplification/perpetuation of inflammatory process in human RA.

신생아집중간호단위 환경과 저체중출생아의 반응에 대한 연구 (A Study on the Environment for Lowbirth Weight Infants in Neonatal Intensive Care Unit in the United States)

  • In effort to conduct comparative study on the caregiving environment of Neonatal Intensive Care Unit(NICU) in both U. S and Korea, this study was been conducted first in the U.S. Purpose : The purpose of this study is to identify the physical environment and direct caregiving practices to lowbirth weight infants in NICU in the US. It also aims to examine the NICU outcome status and behavioral reponses of lowbirth weight infants. Methods : A study design using descriptive and inferential statistics was been conducted through an observational, field method. A sample of 15 preform infants admitted to NICU were recruited for the study. The subjects were those with birth weight between 1,000 gm to 1,500 gm, born at the gestation period of 27 to 33 weeks, and without any chromosomal or other genetic anomalies, major congenital infections, or maternal illness. Thirty minutes observation(three times of ten minutes of continuous observation)of the infant's behavior and physiological status, and an four-hour observation of the physical environment and direct care giving procedures were been conducted on the 3rd and 10th day after birth, and on the day of discharge from the NICU or at 34weeks postconception. The data to be collected were in four areas : the demograghic characteristics of the infants, the physical environment and care giving procedures, the frequency of the infant's designated behavior and physiological response, and NICU outcome variables. A descriptive analysis and Kruskal-Wallis, Pearson r were been applied according to variable characteristics. Results : 1. Mother's mean age was 29.47. The sample consisted of 6 males and 9 females. Mean gestational ages were 29.17 weeks. Mean birth weight was 1236.33g. Mean Apgar scores at one minute were 6.6, and 7.8 at five minutes. 2. The location for the incubator was in the distance from the light, X-ray screens and nursing station, in proximity to side-lamp, telephone and faucet on the third day after birth. The location for the incubator was in the distance from the light and radio on the tenth day and in proximity to nursing station on the day of dischage from the NICU or at 34weeks postconception. 3. Nesting was the most applying aids to the infants. And foot roll, shielding and plastic frame were frequently using by nurses for facilitating well modulated restful posture. 4. There were statistically significant changes in the patterns of physical environment included locating the infant's incubator and bedding, specific aids to self regulation on the 3rd and 10th day after birth, and on the day of discharge from the NICU or at 34weeks postconception. 5. Statistically significant changes were not appeared in the patterns of direct caregiving procedure to the infants included stress inducing or reducing manipulations on the 3rd and 10th day after birth, and on the day of discharge from the NICU or at 34weeks postconception. 6. The stress response of the infants in NICU were significantly reduced as the infants grow older. 7. There were not statistically significant correlation between the physical envronment and the stress responses of the infants in NICU. 8. There were statistically significant correlation between the direct caregiving procedure to the infants and the stress response of the infants in NICU in the second and third observation on the day three. 9. Average weight gain per day from birth to discharge was 38.73g, number of days in the hospital was 42.60, number of days before bottle feeding was 3.6. Postconception age starting bottle feed ing was 31/sup +5/ weeks. Number of days on mechanical ventilator was average 7.64, 11.42 was an average number of days of oxygen need. Conclusion : It, thus, appears that to minimize the sensorymotor stimulation for the low birthweight preterm infant in NICU, manipulation of care giving practices to the babies whatever the stress inducing or reducing procedures, have to be limited in the immediate early stage after birth. And it needed to be reexamine to identify the appropriate and specific physical environment and the patterns of direct caregiving to the low birthweight preform infant as the infants grow older in NICU.
