Bone mineral density in type 2 diabetic patients aged 50 years or older in men and postmenopausal women in Korea

  • Relationship between bone mineral density (BMD) and type 2 diabetes is still inconsistent. Recently, many epidemiologic data show that fracture risk is increased in type 2 diabetic patients regardless of BMD status. In this study, we used nation-wide data from 2008 to 2011 Korea National Health and Nutrition Examination Survey (KNHANES) to analyze the BMD status in patients with type 2 diabetes compared to non-diabetics. We included subjects aged 50 years or older in men (N=2,959, 2,430 without diabetes, 529 with type 2 diabetes) and postmenopausal women (N=2,902, 2,479 without diabetes, 423 with type 2 diabetes). Subjects with history of medication for osteoporosis or with illness or malignancy affecting bone metabolism were excluded. Data of anthropometric measurements and demographic characteristics were collected by trained examiner. Serum was separated from peripheral venous blood samples obtained after 8 hours of fasting. BMD was measured at lumbar spine and femur using dual-energy X-ray absorptiometry (DXA). There was a significant positive association between lumbar spine BMD and type 2 diabetes after adjusting age, gender, body mass index, monthly house income, education level, physical activity, daily calcium intake and vitamin D concentration by multiple regression analysis in all subjects. In the subgroup analysis by gender, this association was maintained both in male and female after adjusting those confounding factors. However, femur BMD was not different between type 2 diabetic and non-diabetic subjects. In conclusion, lumbar spine BMD was significantly higher in type 2 diabetic patients aged 50 years or more in men and postmenopausal women compared to non-diabetic subjects.

Congenital Insensitivity to Pain with Anhidrosis: Five-Year-Old Girl with a Neglected Distal Femur Fracture (선천성 무통증과 무한증: 5세 여아에서 발생한 방치된 원위 대퇴골 골절)

  • Congenital insensitivity to pain with anhidrosis (CIPA) is a rare disease that affects the sensory and autonomic nervous system. The patients do not have the ability to sense different sensations, such as pain, which tends to lead to different injuries. In addition, the patients suffer from fluctuations in body temperature due to autonomic involvement. The present case was a five-year-old girl with a neglected distal femur fracture. X-rays taken during the follow-up showed marked callus formation and pseudarthrosis of the distal femur. She had biting injuries of the tongue, auto-amputation of the fingers, some developmental delay and a history of recurrent fever with an unknown origin. The electrodiagnostic study was normal. The quantitative sudomotor axon reflex test revealed markedly reduced postganglionic sudomotor axonal responses at all sites recorded on the left. She was diagnosed with CIPA. As the initial presentation of CIPA involves the musculoskeletal system, orthopedic surgeons should have a high index of suspicion.

Geotechnical Properties of Pelagic Red Clay in Northeast Equatorial Pacific (북동태평양 원양성 적점토의 지질공학적 특성에 관한 연구)

  • In order to understand the physical properties of deep-sea sediments, which mainly consist of pelagic red clays, sediment samples were collected at 24 stations using a multiple corer in the Clarion-Clipperton fracture zone of the northeast equatorial Pacific. The sampled sediment cores were examined for the mass physical properties(i.e. grain size distribution, mean grain size, water content, specific grain density, wet bulk density, void ratio, and porosity) and the geotechnical properties(i.e. shear strength and consistency limits) with the content of biogenic opal and mineral composition. Although KR1 and KR2 areas on the same latitude are logitudinally far from each other, the mass physical properties of these areas are not distinctly different except for shear strengths. The maximum shear strength of surface sediments in KR2 area is higher than that in KR1 due to the appearance of a consolidated lower layer(Unit 3) in the sediment core from KR2.

Musculoskeletal Injuries by Weapons in Korean Soldiers: Four-Year Follow-Up (총기 및 폭발물에 의한 군인의 근골격계 손상: 최근 4년간 분석)

  • Purpose: To date, studies of firearm and explosive injuries in the Korean military have been limited compared to its importance. To overcome this, this study examined the characteristics of musculoskeletal damages in soldiers who have suffered firearm and explosive injuries over the past four years. Materials and Methods: From January 2015 to July 2019, military forces who had suffered musculoskeletal injuries from firearms or explosive substances were included. The medical records and radiographs were reviewed retrospectively, and telephone surveys about Short Musculoskeletal Functional Assessment (SMFA) for this group were conducted. To compare the functional outcomes, statistical analysis was performed using a t-test for the types of weapons, and ANOVA for others. Results: Of the 61 patients treated for firearms and explosives injuries, 30 patients (49.2%) were included after undergoing orthopedic treatment due to musculoskeletal injury. The average age at injury was 26.4 years old (21-52 years old). The number of officers and soldiers was similar. Eleven were injured by gunshot and 19 by an explosive device. Sixteen were treated in the Armed Forces Capital Hospital and 10 at private hospitals. More than half of the 16 patients (53.3%) with a fracture had multiple fractures. The most common injury site was the hand (33.3%), followed by the lower leg (30.0%). There were 14 patients (46.7%) with Gustilo-Anderson classification 3B or higher who required a soft tissue reconstruction. Fifteen patients agreed to join the SMFA survey for the functional outcomes. Between officers and soldiers, officers had better scores in the Bother Index compared to soldiers (p=0.0045). Patients treated in the Armed Forces Capital Hospital had better scores in both the Dysfunction and Bother Index compared to private hospitals (p=0.0008, p=0.0149). Conclusion: This is the first study to analyze of weapons injuries in the Korean military. As a result of the study, the orthopedic burden was high in the treating patients with military weapon injuries. In addition, it is necessary to build a military trauma registry, including firearm and explosive injuries, for trauma treatment evaluation and development of military trauma system.

Gemological Characterization of B. C. Jade (비씨 제이드의 보석학적 연구)

  • The Gemological characteristics of B.C. jade from Cassiar Mine, British Colombia, Canada, have been investigated, using polarizing microscopy, Mohs' hardness, refractive index and density measurements, X-ray powder diffraction, X-ray fluorescence spectrometry, ICP-MS, Infrared absorption spectrometry, and DTA/TGA. The B.C. jade is deeply green (spinach peen or olive green) in color and is translucent. It shows a resinous or waxy luster. The principal mineral of the material is tremolite-actinolite solid solution and minor amount of Cr-garnet and unidentified opaque minerals are accompanied. Mohs' hardness value ($5.5{\sim}6$). refractive index (1.62), and specific gravity (3.01) are measured. It is very highly tough and shows hackly fracture. The high Fe content ($Fe_2O_3\;4.14{\sim}4.66\;wt%$) in B.C. jade is attributable to a deepening of green color of the material. The B.C. jade starts to dehydrate at v and dehydration is completed at $1000.8^{\circ}C$, transforming tremolite-actinolite solid solution to enstatite, diopside, quartz, and water in its place. This possible reaction is supported by the weight loss of B.C. jade (1.93 wt%) at $1000.8^{\circ}C$ indicated by TGA curve.

Efficacy and Safety of Micro Current Tape on Chronic Low Back Pain: One Group Pre-Post Test Design, Multi Center Pilot Study (만성 요통 통증 완화에 대한 미세전류 테이프의 안전성 및 유효성 평가)

  • Objectives The aim of this study was to evaluate the efficacy and safety of micro current taping therapy (MTT) on patients with chronic low back pain (CLBP). Methods We included 50 participants who met the inclusion criteria and 5 participants dropped out during the sessions due to the following reasons: 3 participants were personal reasons, 1 participant was taking medication, 1 participant was fore arm fracture. We attached "I" shaped 40 cm, 2 tapes along the erector muscles of the spine starting from both iliac crest and another "I" shaped 30 cm, 1 tape on the painful site horizontally. This procedure was done 8 times and participants visited a total of 9 times including a final visit for evaluation. We measured visual analog scale (VAS), range of motion (ROM) and schober's test on every visit. Participants completed a questionnaire of oswestry disability index (ODI) and Beck's depression inventory (BDI) on the first and last visits. Results In VAS for pain intensity and bothersomeness, there were significant decreases after 1st, 6th treatments. In range of extension, there was significant increase after 3rd treatment at first. In range of flexion, there was significant increase after 2nd treatment at first. In range of left flexion, there was significant increase after 4th treatment at first. In range of right flexion, there was significant increase after 3rd treatment at first. in schober's test, there was significant increase after 2nd treatment at first. In VAS, ROM, schober's test, BDI, ODI, there were significant improvement after all treatments had done. Conclusions There was a significant effect of MTT on CLBP. And there was no adverse effect.

Assessment of the Correlation for Geometry Transition using Bone Mineral Density in Proximal Femur (골밀도를 이용한 대퇴근위부 형태 변화의 상관관계 평가)

  • Osteoporosis is increasing in Korea as it becomes an aging society with the rapid economic growth and the development of medical technology. Osteoporosis also develops due to chemo and radiation therapy of cancer which also increases owing to Westernized diet. Osteoporosis is caused by reduced bone density, has close relationship with the change of geometry of proximal femur, which is a factor of hip fracture risk. The purpose of this study was the analysis of the correlations of osteoporosis and the change of geometry of proximal femur, which was observed according to T-score variance. The 350 male and female patients are chosen from D hospital in Busan, who were classified by age, sex and T-score values (normal, osteopenia, and osteo porosis). The results show that the age and gender have significant difference in the incidence of osteoporosis; the disease classification according to T-score value has significant difference in the geometry of the proximal femur such as Cortical ratio calcar, Cortical ratio shaft, Hip/shaft Angle, Strength index, Section modulus, CSMI, and CSA, and is highly correlated with the incidence of osteoporosis. Therefore, the findings of this research is that the change of the geometry of the proximal femur could be used as an indicator in the diagnosis of osteoporosis, could enhance the accuracy of the diagnosis in the future, and could be used as a clinical predictive factors through the analysis of the correlations of T-score variance and the geometry changes of the proximal femur.

Material Characteristics and Deterioration Assessment for Multi-storied Round shape Stone Pagoda of Unjusa Temple, Hwasun, Korea (화순 운주사 원형다층석탑의 재질특성과 훼손도 평가)

  • The constituting rocks of Multi-storied Round shape stone Pagoda of Unjusa Temple are lithic tuff and rhyolite tuff breaccia which show green or grey and also rock fragment with poor roundness are present in the structure. lithic tuff is composed of feldspar and quartz which are glassy texture and cryptocrystalline and also micro crystalline are scattered. phenocryst quartz and feldspar in the substrate composed of feldspar and opaque minerals are found in rhyolite tuff breaccia. dust, exfoliation, cavity, fracture and crack are observed in all the stone of the pagoda and the result of Infrared Thermography shows partial inter cavities have developed severely which may cause further exfoliation. In addition, a great deal of various grey, green, and yellow brown lichen as well as bryophyte are present at the upper part of eastern and western roof stone located above the third floor. Discolors remarkably shown at stereobate and roof stone are identified as inorganic pollutants such as manganese oxide, iron oxide and iron hydroxide. The stone of the pagoda of the Chemical Index of Alteration (CIA) and the Weathering Potential Index (WPI) are 55.69 and 1.12 respectively and this corresponds to a highly weathered stage. The measured values, average ultrasonic velocity 2,892m/s, coefficient of weathering 0.4k and compressive strength $1,096kg/cm^3$, suggest that the rock strength and durability are weakened.

The Application of Genetic Algorithm for the Identification of Discontinuity Sets (불연속면 군 분류를 위한 유전자알고리즘의 응용)

  • One of the standard procedures of discontinuity survey is the joint set identification from the population of field orientation data. Discontinuity set identification is fundamental to rock engineering tasks such as rock mass classification, discrete element analysis, key block analysis. and discrete fracture network modeling. Conventionally, manual method using contour plot had been widely used for this task, but this method has some short-comings such as yielding subjective identification results, manual operations, and so on. In this study, the method of discontinuity set identification using genetic algorithm was introduced, but slightly modified to handle the orientation data. Finally, based on the genetic algorithm, we developed a FORTRAN program, Genetic Algorithm based Clustering(GAC) and applied it to two different discontinuity data sets. Genetic Algorithm based Clustering(GAC) was proved to be a fast and efficient method for the discontinuity set identification task. In addition, fitness function based on variance showed more efficient performance in finding the optimal number of clusters when compared with Davis - Bouldin index.

Therapeutic Efficacy of Pulsed Radiofrequency Treatment in Lumbar Radicular Pain (요추부 신경근통 환자에서의 박동성 고주파술의 치료 효과)

  • Background: Lumbar radicular pain is a frequent and often debilitating event. Although many treatment methods have been described in several studies, the available evidences regarding efficacy is not sufficient enough to draw definitive conclusions on an optimal therapy regime. Pulsed radiofrequency (RF) treatment was found to exert a beneficial effect on intractable radicular pain in individuals. The purpose of this study was to assess the efficacy of pulsed RF of the dorsal root ganglion for chronic lumbar radicular pain. Methods: Twenty five patients with chronic lumbar radicular pain that was refractory to selective nerve root blockage met the inclusion criteria of our study and received pulsed RF treatment. The average numeric rating scale (NRS) for leg pain during usual activities and the Oswestry disability index (ODI) were measured at 1 and 3 months after the procedure. Results: Of the 25 patients accepted for pulsed RF treatment, one dropped out due to a vertebral compression fracture during this study. ODI and NRS showed a positive trend in favor of the pulsed RF treatment. No significant complications were observed during the study period. Conclusions: It appears that pulsed RF treatment of the lumbar spinal dorsal root ganglion may be an effective treatment method for patients suffering from lumbar radicular pain, and who were not responsive to selective nerve root blockage.
