제주도 노인에서 고관절 골절 발생률과 사망률 (The Incidence and Mortality for Hip Fracture in the Elderly in Jeju-do)

  • Purpose : The purpose of this study was to evaluate the incidence and mortality for hip fracture in the elderly in Jeju-do. Methods : We enrolled 254 cases among 318 patients older than 50 years of age with a hip fracture during two years period(2003-2004). We investigated the incidence and mortality during follow up 3~4 years period until December 31, 2007. Results : The crude incidences of hip fracture the age group ${\geq}50$ years were 141(11.2/10,000) in 2003, 177(13.6/10,000) in 2004, and 249(17.0/10,000), 69(6.3/10,000) for women and men, respectively. The mean age of them was 78.3 years, male was 69(21.7%), female was 249(78.3%) among 318 patients. Survivor was in the 146(57.5%), death was in the 108(42.5%) among 254 cases period for follow up. In the death group, activity was significantly lower at the time of post-fracture 3(p=0.013), 6(p=0.000), and 12 months(p=0.000). The mortality of hip fracture and crude death rate(CDR) were 108(42.5%, n=254), 11,884(1.7%, n=675,889) in 2003~2007 in Jeju. Conclusion : Developing and applying a variety of activity programs that increase activity in post-fracture may improve activities of daily living, reduce burden of family and society, be useful in improving the quality of life and ultimately lower the mortality.

Incidence and Mortality after Proximal Humerus Fractures Over 50 Years of Age in South Korea: National Claim Data from 2008 to 2012

  • Background: There has been lack of epidemiology of proximal humerus fracture using nationwide database in Asia. The purpose of this study was to investigate the incidence of proximal humerus fracture and its mortality following proximal humerus fracture in Korean over 50 years of age. Methods: The Korean National Health Insurance data were evaluated to determine the incidence and mortality of proximal humerus fracture aged 50 years or older from 2008 through 2012. Results: Proximal humerus fracture increased by 40.5% over 5 year of study. The incidence of fracture increased from 104.7/100,000 in 2008 to 124.7/100,000 in 2012 in women and from 45.3/100,000 in 2008 to 52.0/100,000 in 2012 in men, respectively. One year mortality rate after proximal humerus fracture was 8.0% in 2008 and 7.0% in 2012. One year mortality rate were 10.8% for men and 7.0% for women in 2008 and 8.5% for men and 6.4% for women in 2012. Conclusions: Our study showed that the proximal humerus fracture in elderly was recently increasing and associated with high mortality in Korea. Considering proximal humerus fracture was associated with an increased risk of associated fractures and an increased mortality risk, public health strategy to prevent the proximal humerus fracture in elderly will be mandatory.

소아 안와 골절의 임상 분석 (Clinical analysis of Pediatric Blow out Fracture)

  • Purpose: Because of traffic accidents and many criminal violences, the incidence of facial trauma has been increasing not only in adults but also in children. We planed this study to introduce our experience about pediatric blow out fracture and provide more information. Methods: We made retrospective study in 76 children with blow out fracture from January 2001 to September 2005 by retrospective chart review including detailed preoperative and postoperative evaluations, age, sex, cause, symptom and sign, and their post-operative complications. Results: Among our patients, 69 were male and 7 were female. The ages ranged from 7 to 18 years, which shows the greatest incidence of blow out fracture. Physical violence(46%) was the most common cause in this group and was followed by vehicle accident(28%), and fall down accident(17%). Left side(64%) showed slightly more incidence than right side(36%), but there were no statistical importance. Ecchymosis(88%) was the most common symptom and followed by periorbital swelling(68%) and diplopia(30%). 30 patients was diagnosed with another facial bone fracture and nasal bone(51%) was the most common associated facial bone fracture. Fourty four Patients(60%) got an orbital wall reconstruction in 7 days after trauma. After the operation, only 3 patients(4%) suffered from diplopia postoperative 3 month, and resolved in 4 years. Conclusion: The incidence of blow out fracture in children has been increasing every year, and violence has become more important etiology of pediatric blow out fracture and public and private education institutions were the most common place that blow out fracture originated. Accurate diagnosis and careful treatment plans are important in pediatric blow out fracture.

안면 중앙부 골절에 관한 임상적 연구 (CLINICAL STUDY OF MIDFACE FRACTURE)

  • The purpose of this study was to investigate the epidemiology of midface fractures. We observed clinically 71 patients with midface fracture who visited the Department of oral and maxillofacial-surgery, Chosun University, Dental Hospital, from 1991 to 1994. The results obtained were as follows. 1. There was the highest age incidence in the third decade(29.6%). 2. There was the highest incidence in the summer(36.6%). 3. The most etiologic factor of midface fracture was traffic accidents(43.6%) and next factor was fall down(38.0%). 4. The highest incidence fracture was zygomatic arch(38.7%) and next fracture was ZMC fracture(31.5%). 5. Most midface fracture was treated within 2 weeks(86.7%). 6. Midface fractures was most frequently combined with mandibular fracture(15.5%), head injuries(14.1%), orthhopedic injuries(8.5), thorax & abdomen(5.6%). 7. The highest complication was the nerve injury(8명), and next complications were infection(3명), epiphora(3명) etc.

제 5중족골 근위부 골절과 동반된 족관절, 족부 손상과 유형 (Ankle and Foot Injuries Accompanying 5th Metatarsal Fractures)

  • Purpose: The proximal fifth metatarsal fracture is one of the most common foot fractures. However, few studies have evaluated the associated injuries in patients with a proximal fifth metatarsal fracture. The purpose of this study was to investigate the incidence of foot and ankle joint injuries associated with proximal fifth metatarsal fractures and compare the incidence of these injuries based on the injury mechanisms and location of the fracture. Materials and Methods: This retrospective study included 157 patients with a proximal fifth metatarsal fracture who underwent surgery from January 2014 to August 2021. Their medical records and radiology images were reviewed to classify and analyze the associated injuries. The proximal fifth metatarsal fractures were classified using Lawrence and Botte's classification. Injury mechanisms were divided into direct and indirect injuries. The incidence of injuries was statistically analyzed according to the injury mechanism and classification. Results: Of the 157 patients with proximal fifth metatarsal fractures, 81 (51.6%) were diagnosed with foot and ankle joint injuries. The incidence of foot injuries was 65.4%, that of ankle joint injuries was 19.8%, and of both foot and ankle joint injuries was 14.8%. In patients with direct injuries, the incidence of foot and ankle joint injuries was 82.5% and that of indirect injuries was 41.0%. Statistical differences were observed between the incidence of direct and indirect injuries (p<0.001). The incidence of injuries, according to Lawrence and Botte's classification, was 54.9% (Zone I), 41.2% (Zone II), and 50.0% (Zone III) respectively. However, there were no statistically significant variations in the locations of the proximal fifth metatarsal fractures (p=0.051). Conclusion: In this study, the incidence of foot and ankle joint injuries associated with proximal fifth metatarsal fractures was found to be high. Therefore, a careful physical examination and appropriate radiological evaluation are recommended for patients with such fractures.

  • Purpose: The purpose of this study was to investigate the incidence of osteoporosis and falls and their consequences, and to identify predictors of fracture risk in the postmenopausal women. Methods: A total of 687 postmenopausal women were recruited through a stratified convenience sampling. A structured questionnaire was used to obtain osteoporosis and fall history and details of their most recent fall. To predict fracture risk factors, we collected demographic and physical health variables related osteoporosis and fall. Fracture risk was measured by FRAX$^{(R)}$ to calculate 10-year probability of major osteoporotic and hip fracture. Results: The prevalence of osteoporosis was 22.1%, and 66.4% of them had treatments for osteoporosis. The incidence of falls during the past year was 19.2% and 38.6% of those who fell suffered consequent fractures. Women with history of osteoporosis and falls were significant predictors of 10-year probability of major osteoporotic and hip fracture. Other significant predictors were history of fracture, chronic disease, surgical menopause, lower BMI, poorer perceived health and no job. Conclusion: It appears that history of osteoporosis and falls are main predictors of fracture risk. Nursing assessment should be performed by detail history taking for osteoporosis, fall, chronic disease, and fracture to screen fracture risk group among postmenopausal women.

고관절 골절로 입원한 노인 환자의 욕창 발생 위험요인 (Factors Influencing on Pressure Ulcer Incidence among Older Patients with Hip Fracture in a Hospital)

  • Purpose: This study aimed to identify the incidence and risks for pressure ulcer among older patients with hip fracture. Methods: The subject were 215 older patients suffering from hip fracture who were admitted for surgical operation from January 1, 2012 to April 30, 2016 in a university-affiliated hospital. The incidence of pressure ulcer was collected retrospectively through medical record review and the risk factors were analyzed using Cox's proportional hazard model. Results: Out of the total, 32 patients (14.9%) developed pressure ulcer with the average occurrence period being 4.72 (${\pm}3.81$) days. Stage II pressure ulcer was the most common at 72.0%. Risk factors included ambulation status before injury (p= .039), spinal anesthesia (p= .029), and stay at intensive care unit after operation (p= .009). Conclusion: Despite pressure ulcer prevention efforts, the incidence remained relatively high. Considering the identified risk factors, more efforts is needed for early detection and prevention of pressure ulcers in such patients.

  • The purpose of this study was to assess the influence of the presence and impacted state of the mandibular third molars on the incidence of mandibular condyle fracture. A retrospective study was designed for patients presenting to the Department of Oral and Maxillofacial Surgery, Kyungpook National University Hospital and Tae-gu Fatima Hospital for treatment of mandibular fractures from January 2003 to January 2006. The independent variables in this study were the presence, degree of impaction of third molars, and the outcome variables were the incidence of mandibular condyle fractures. Hospital charts and panoramic radiographs were used to determine and classify these variables. The demographic data included age, sex, mechanisms of injuries and number of mandibular condyle fractures. The study sample comprised 136 mandibular condyle fractures in 105 patients. Result of this study demonstrated a statistically significant difference in ipsilateral condyle fractures and mandibular third molar absence(P=0.032) and bilateral condyle fractures without another fracture and mandibular third molar absence(P=0.028).

Stranc 분류법에 따른 비골골절 정복술 후 합병증 (Complications of the Nasal Bone Fractures according to the Stranc Classification)

  • Purpose: Although nasal fractures are often discussed as minor injuries but the incidence of post-traumatic nasal deformity remains high. For decrease the incidence of post-traumatic nasal deformity which require the guideline to optimize the management of acute nasal bone fracture. The aim of this study is analysis of post-traumatic nasal deformity according to Stranc classification. Method: We reviewed 310 patients with nasal bone fracture treated at our hospital from January of 2005 to December of 2006. Result: Post-traumatic complication were divided septal deviation, nasal bone deformity, temporary hyposmia and synechia. Nasal bone deformity include nasal bone deviation, hump, flat nose and minimal nasal bone irregularity. The incidence of total complication rate was 36.4%. The most common complication was nasal deformity(22.9%) followed by septal deviation(19.7%). The most common complication was septal deviation(20%) in frontal impact. The most common complication was nasal deformity(19.5%) in lateral impact. In frontal impact, the incidence of complication rate was plane II(68.8%) followed by plane I(29.9%) and plane III(16.7%). In lateral impact, the incidence of complication rate was plane II (78.8%) followed by plane III(61.5%) and plane I(42.7%). Conclusion: This result can be used to improve longterm results and to reduce the incidence of post-traumatic nasal deformity by predict complication of nasal bone fracture according to Stranc classification.

골절 발생에 따른 돈육 반막모양근의 품질변화 (Influence of bone fracture incidence on the quality of pork semimembranous muscle)

  • This study was conducted to investigate the incidence of bone-fracture from pig carcass slaughtered in eight processing plants located in Daejeon and Chungnam area, Korea, during the year 2013 and 2014, and to evaluate the quality of semimembranous muscle from bone-fractured carcasses. Twenty semimembranous muscles were collected from bone-fractured carcasses and none bone-fractured (control) ones, respectively, after storage of pig carcass at $5^{\circ}C$ for 24 h. The pH, cooking loss, and color of semimembranous muscle were measured as quality parameters. In total 4,865,502 of pig carcasses, the occurrence of bone fracture was 0.328% (15,975 heads) and scored the highest defect (26.31%) in total abnormal carcasses. The pH and cooking loss of semimembranous muscle from bone-fractured carcasses were significantly lower than those of control (p<0.05). $L^*$ and $a^*$ values of semimembranous muscle were not significantly different between bone-fractured carcass and control whereas that of $b^*$ values was significantly higher in bone-fractured carcass than control (p<0.05). Eight out of twenty semimembranous muscle collected from bone-fractured carcasses were confirmed as PSE whereas only one in control. In conclusion, the incidence of bone-fracture pre- and during slaughter of pig may cause serious defects in final meat quality. Therefore, the proper handling and treatment should be implicated to avoid and/or decrease the incidence of bone-fracture of pigs.
