Biepicondylar Fracture of the Humerus in an Adult Female: A Case Report

  • iepicondylar fracture of the humerus is very rare in adults. To date, there have been limited evidence of this injury in the English literature. We report a case of a 65-year-old female with a biepicondylar fracture of the left distal humerus without dislocation. Open reduction and internal fixation with K-wires, cannulated screw, and suture anchor were performed. We obtained stability of the elbow and a satisfactory functional outcome. Because this type of injury is associated with varus and valgus instability, operative reduction and fixation are essential in order to gain stability and early recovery of normal function.

  • There has been no report of bilateral pedicle stress fractures involving two vertebrae. The authors describe a unique case of spondylolisthesis accompanying a bilateral pedicle stress fracture involving two vertebrae. De novo development of spondylolisthesis at the L5-S1 vertebrae accompanying a bilateral pedicle stress fracture at L4 and L5 was observed in a 70-year-old woman. The patient's medical history was unremarkable and she did not have any predisposing factors except severe osteoporosis. Interbody fusion with bone cement augmented screw fixation was performed. Surgical treatment resulted in good pain management and improved functional recovery.

치관부 파절편 재부착술식을 이용한 상악전치부 치관파절의 치료에 대한 2건의 증례보고 (Reattachment procedure for treatment of fractured maxillary anterior teeth: two case reports)

  • Crown fractures are a common type of dental injuries and very frequently occurred on maxillary anterior teeth, area of requiring a rapid aesthetic recovery. Crown fragment reattachment is a simple and conservative method to restore the fractured teeth. The technique promotes esthetic outcomes as utilizing natural contour, shade, surface texture of teeth and gives mechanical similarity in terms of wear-resistance. Also it gives emotional positive responses to patients and requires less of chair time and costs. This case report presents two cases of crown fragment reattachments on maxillrary anterior teeth, including one complicated crown fracture and one uncomplicated crown fracture. If the fragment is available in cases of crown fractures, reattachment of fragment can be regarded as a predictable alternative. However, it is all the time important that a close conversation informing the patients about the limitations and prognoses of this treatment option.

Lag Screw Fixation of a Slab Fracture in the Third Carpal Bone in a Korean Racehorse

  • A 3-year-old male Thoroughbred serving as a Korean racehorse named RAON PORTEOUS presented with acute lameness concomitant with edema and fever in the right carpal joint after completing a horse race. Through radiological examination using lateromedial oblique, flexed lateromedial oblique and dorsopromixal-dorsodistal oblique views, the horse was diagnosed with a slab fracture in the 3rd carpal bone of right forelimb. The fracture was surgically approached and corrected by applying 4.5 mm leg screw fixation during surgery. At eight months post-surgery, the horse was able to return to racing and has had successful racing performances with two wins and several prizes in the last 8 months. This is the first reported case of the successful use of lag screw fixation surgery in Korea with the horse showing complete recovery from a severe injury that is often considered fatal in a racehorse.

Sternal Fracture Fixation with a Steel Wire: The New "Timala" Technique

  • Purpose: Traumatic sternal fractures are rare but quite disabling injuries. Timely fixation of sternal fractures reduces pain and prevents respiratory complications. However, the fixation technique should be simple, effective, and readily available in local circumstances. Methods: From January 2014 to March 2020, seven patients with sternal fracture/dislocation underwent steel wire fixation with the new "Timala" technique. In this technique, adjacent ribs are anchored with two steel wires to form an "X" in front of the fractured segment of the sternum. Patients were followed up clinically and radiologically. Results: Six of the patients were men and one was a female. Five of them had injuries due to falls and two were injured in road traffic accidents. Their age ranged from 18 years to 76 years, with a median age of 41 years. All seven patients experienced immediate recovery from pain and showed evidence of fracture healing on postoperative chest X-rays and clinical examinations. Conclusions: Anchoring ribs to fix the sternum with steel wire is a safe, effective, easily available, and reproducible method to fix sternal fractures or dislocations.

  • 저자등은 하악 과두 골절에 있어 골절의 양상, 환자의 요구, 교합의 상태, 한자의 나이등을 고려하여 기능적 처지가 필요하다고 판단된 환자들에 있어 악간 고정기간 없이 activator를 이용한 기능적 치료를 한 결과, 빠른 악관절 기능의 회복을 관찰하고 정상적인 하악의 전, 측방 기능운동을 유도할 수 있었기에 문헌 고찰과 함께 보고하는 바이다.

상완골 골절과 동반된 요골 신경 손상에서 자가 비복 신경 이식술의 결과 (Results of the Autogenous Sural Nerve Graft for Ruptured Radial Nerve in the Closed Humerus Shaft Fracture)

  • In the high radial nerve palsy caused by displaced humeral shaft fracture, radial nerve have to be explored in the fracture site. 5 cases of the ruptured radial nerve at the fracture site of the humerus from January 1993 through January 2005 were treated at first by open reduction and internal fixation with plates and screws fixation and then defective radial nerves were grafted with autogenous sural nerves by microsurgical epineurial and or perineurial neurorrhaphy. At average 30.4 months follow-up, 5 cases were recovered from motor and sensory deficit with solid bony union of the humerus shaft fracture. Authors have confirmed that ruptured radial nerve in the humerus shaft fracture grafted with autogenous sural nerve with microsurgical epineurial and or perineurial neurorrhaphy would be expected good motor and sensory recovery.

  • Objectives : The purpose of this study is to show the clinical effects of integrative oriental medical treatments for femur pain and disfunction after hip fracture surgery. Methods : Two patients were treated using acupuncture, pharmacopuncture, herbal medication and other oriental medical treatments include moxibustion, cupping therapy, physical rehabilitation for 2~3 months. And the effects of these integrative treatments were measured with numeric rating scale(NRS) and functional recovery score(FRS). Results : NRS of femur pain was decreased from 8 to 4~5 in case 1 and from 6 to 4 in case 2. FRS was improved in both cases. Conclusions : Integrative oriental medical treatments maybe effective to the patients complaining pain and functional limitations after hip fracture surgery.

Fracture patterns and causes in the craniofacial region: an 8-year review of 2076 patients

  • Background: For proper recovery from craniofacial fracture, it is necessary to establish guidelines based on trends. This study aimed to analyze the patterns and causes of craniofacial fractures. Methods: This retrospective study analyzed patients who underwent surgery for craniofacial fractures between 2010 and 2017 at a single center. Several parameters, including time of injury, region and cause of fracture, alcohol intoxication, time from injury to surgery, hospitalization period, and postoperative complications, were evaluated. Results: This study analyzed 2708 fracture lesions of 2076 patients, among whom males aged 10 to 39 years were the most numerous. The number of patients was significantly higher in the middle of a month. The most common fractures were a nasal bone fracture. The most common causes of fracture were ground accidents and personal assault, which tended to frequently cause more nasal bone fracture than other fractures. Traffic accidents and high falls tended to cause zygomatic arch and maxillary wall fractures more frequently. Postoperative complications-observed in 126 patients-had a significant relationship with the end of a month, mandible or panfacial fracture, and traffic accidents. Conclusions: The present findings on long-term craniofacial fracture trends should be considered by clinicians dealing with fractures and could be useful for policy decisions.

대전자부 압박궤양을 가지고 있는 하지마비 환자에서 대퇴골 골절부위에 발생한 비정상적 골증식의 치험례 (A Case of Atypical Bone Growth after Femur Neck Fracture in the Paraplegic Patient with Trochanteric Sore)

  • Purpose: Heterotopic ossification in pressure sore patients is reported to rarely develop, but once it occurs, it frequently causes joint stiffness and mobilization restriction. The aim of this article is to report our experience of atypical bone growing at femur neck fracture site with chronic, extensive pressure sore in patient with paraplegia secondary to spinal injury. Methods: A 28-year-old male patient presented with atypical bone growth at femur neck fracture site with pressure sore. He had undergone atypical growth bone removal and separation of united iliac bone and femur, and then pressure sore was covered by advanced rotation flap. Results: The patient mobilized hip joint and rode in a wheelchair. Complications such as dehiscence, infection, hematoma and flap necrosis did not occur. Conclusion: We experienced successful correction of atypical bone growth removal and recovery of pressure sore. We report our experience of atypical bone growth of fracture site and the related literature was reviewed.
